Chemodektom, paragangliom glomus caroticum neboli tumor karotického glomu se vyskytuje v populaci s incidencí 1 : 30 000. Tumor je makroskopicky, mikroskopicky i biologicky velmi podobný adrenálnímu feochromocytomu. Významným znakem těchto tumorů je jejich bohatá vaskularizace. Patří mezi nejvíce prokrvené tumory vůbec. Histologicky se popisuje alveolární uspořádání nádorových buněk, na rozdíl od feochromocytomu, ostře od sebe ohraničených jemnými pruhy vaziva se širokými kapilárami a buňkami podobnými buňkám Schwannovým. Cytoplazma nádorových buněk hlavních je slabě eozinofilní a jemně granulovaná. Jádra buněk mohou být pleomorfní a hyperchromní, ale nutno konstatovat, že jaderná atypie není jednoznačnou známkou malignity. Biologicky se jedná o tumory převážně benigní. Klinicky se jedná o pomalu rostoucí, nebolestivý, tuhý a kulovitý útvar za úhlem dolní čelisti. The author reported clinical characteristic features and risk factors for acute intestinal infections in 225 children (140 boys and 85 girls, mean age 1.5 years old). 90 (40%) patients aged up to 12 months, 101 (44.9%) were from 1 to 2 years old, and 34 (15.1%) were more than 2 years old. Most children were hospitalized in the first week of illness and had moderate severe course of disease. Severe forms of illness reliably more often were noted in children being on artificial feeding. Toxicdystrophic condition, severe forms and perinatal encephalopathy were prevailed in boys. Factorial analysis revealed 20 most significant factors with factorization fullness 70.8%. The most significant factors were F1( factor of hyperthermia duration) with 8.5% effect, F2 (factor of pain syndrome) 5.8%, F3 (factor of normal enzymatic activity of colon bacillus) 4.9%, F4 (etiological factor) 4.6%, as well as F5( factor of opportunistic flora of the intestine) 4.4%, respectively., Mukarram Shadjalilova, and Literatura
Hlavní příčiny morbidity a mortality u polycythemie vera (PV) a esenciální trombocytemie (ET) jsou trombóza a krvácení, progrese do myelofibrózy a přechod do akutní leukemie. Myelosupresivní léčba, především pak hydroxyurea, sice může četnost cévních komplikací omezit, ale u tohoto přípravku existují jisté obavy stran zvýšeného výskytu leukemické transformace. Léčba těchto poruch proto představuje náročný problém a abychom se vyhnuli neadekvátní expozici cytotoxickým léčivům na straně jedné či suboptimální léčbě na straně druhé, měli bychom dodržovat „risk-oriented” léčebnou strategii. Mezi známé rizikové faktory kardiovaskulárních příhod patří vyšší věk a předchozí trombóza, zatímco vliv nových biologických faktorů včetně leukocytózy a mutačního statutu, popř. mutační zátěže JAK2V617F je předmětem bádání. Nízkorizikoví pacienti s PV by měli být léčeni pouze flebotomií a aspirinem, zatímco vysoce rizikoví pacienti by měli rovněž podstoupit cytotoxickou léčbu. Co se týká léčby ET, u nízkorizikových pacientů jednoznačné indikace pro intervenci neexistují, nicméně vysoce rizikoví pacienti by měli podstoupit chemoterapii. U vybraných pacientů včetně pacientů s rezistencí nebo intolerancí na hydroxyureu se mohou uplatnit i jiné léčebné možnosti, jako např. interferon alfa či anagrelid., Major causes of morbidity and mortality in polycythemia vera (PV) and essential thrombocythemia (ET) are represented by thrombosis and bleeding, progression to myelofibrosis and transformation to acute leukemia. Myelosuppressive therapy, preferentially with hydroxyurea, can reduce the rate of vascular complications, but there is some concern about an increased rate of leukemic transformation with this agent. Therefore, the management of these disorders poses a significant challenge, and a risk-oriented therapeutic approach should be followed to avoid inappropriate exposure to cytotoxic drugs on one side or suboptimal treatment on the other. Established risk factors for cardiovascular events are represented by older age and previous thrombosis, while the impact of novel biological factors, including leukocytosis and JAK2V617F mutational status and/or mutational burden, is under investigation. Low-risk PV patients should be managed only with phlebotomy and aspirin, while high-risk patients should also receive cytotoxic therapy. Regarding the management of ET, there is no clear indication for intervention in low-risk patients, while high-risk patients should be managed with chemotherapy. Tato práce pojednává o některých klíčových aspektech diagnostických kritérií pro MPN podle klasifikace Světové zdravotnické organizace (WHO) z roku 2008, přičemž většina poznámek se omezuje na onemocnění polycythaemia vera (PV). Při analýze hlavních kritérií poznamenáváme, že existují pacienti s PV, jejichž erytrocytóza může být dokumentována pouze měřením objemu erytrocytární masy a nikoli měřením koncentrace hemoglobinu nebo hematokritu. Probíráme reprodukovatelnost histopatologie pro diagnostiku různých stadií MPN, existenci normálních hladin EPO u pacientů s PV, specifitu růstu endogenních erytroidních kolonií (EEC) pro diagnózu PV a naše současné znalosti týkající se cytogenetiky a molekulární biologie PV a jiných MPN a jejich patofyziologie. Rovněž analyzujeme různé prognostické ukazatele morbidity (trombóza) a mortality u PV – vysokou hladinu hematokritu, leukocytózu a procentuální zastoupení buněk s mutací JAK2V617F. V případě esenciální trombocytemie (ET) komentujeme rozlišení pravé ET a “časné PMF”. V závěru stručně shrnujeme obecně přijímaná doporučení European LeukemiaNet (ELN) pro léčbu Ph-negativní MPN na základě nové stratifikace rizika, na důkazech založené riziko kancerogeneze u hydroxyurey a racionální doporučení pro výběr konkrétních léků., In this paper, we discuss some critical parts of the World Health Organization (WHO) 2008 criteria for classification of MPNs classification, with most of the remarks limited to polycythemia vera (PV). When analyzing the major criteria, we note that there are PV patients whose erythrocytosis can only be documented by red cell mass measurements and not by hemoglobin concentration or hematocrit measurements. We discuss the reproducibility of histopathology for the diagnosis of different stages of MPNs, the existence of normal EPO levels in PV patients, the specificity of Endogenous erythroid colony (EEC) growth for PV diagnosis and our current knowledge of the cytogenetics and molecular biology of PV and other MPNs and their pathophysiology. We also analyze various prognostic morbidity (thrombosis) and mortality markers in PV – high hematocrit level, leukocytosis and percentage of cells with the JAK2V617F mutation. In essential thrombocythemia (ET), we comment on the distinction of true ET and “early PMF”. This article presents results of using proton therapy after operation of patient with brain meningioma. After operation brain meningiomas is known to not rarely recur due to certain localization and growth of tumor into main cerebral vessels, nerves and other main brain structures. This creates certain difficulties in operative access and surgical intervention, i.e. tumor resection. Cíl. Práce je zaměřena na možnosti provedení CT angiografie břišní aorty a dolních končetin na nové generaci dvouzdrojového CT přístroje využívající skenování s vysokým pitch faktorem a ohledem na redukci radiační zátěže a množství podávané kontrastní látky. Metoda. V rámci prospektivní studie bylo vyšetřeno 29 osob s klinickými známkami či již známým postižením tepen dolních končetin. Vyšetření byla provedena na přístroji Somatom Definition FLASH (Siemens, Německo), použita kolimace 128 x 0,6 mm, rychlá rotace rentgenky (0,28 s/otáčku) a vyšším fázovým posunem (pitch faktor 1,55). Kontrastní látka v množství 60-70 ml byla aplikována rychlostí 5 ml/s, načasování akvizice bylo provedeno na základě monitorace testovacího bolusu (10 ml) v popliteálních tepnách. Hodnocení kvality zobrazení bylo provedeno na základě vizuálního posouzení a stanovení denzity v jednotlivých částech tepenného systému. Výsledky. Ve všech případech bylo dosaženo vysoké kvality zobrazení aorty, pánevních, femorálních i popliteálních tepen na základě vizuálního hodnocení i stanovení denzity (aorta - 456,3 HU, třísla - 507,3 HU, podkolenní - 554,3). V případě distální části bérců a nohy došlo u tří osob k suboptimální náplni vlivem rychlejší akvizice vůči postupu bolusu kontrastní látky. Pouze v jednom případě byla distální oblast bérců nehodnotitelná. Průměrná úroveň denzity v úrovni kotníků byla 367,7 HU. Průměrná radiační dávka byla nízká -CTDIvol 2,40, efektivní dávka 1,79 mSv. Závěr. První výsledky naší prospektivní studie prokázaly možnost provedení CT angiografie dolních končetiny pomocí "high-pitch" módu. Přes velmi nízkou hodnotu radiační dávky i aplikovaného množství kontrastní látky bylo dosaženo vysoké diagnostické kvality v oblasti aorty, pánve stehen a proximálních částí bérců. Tento způsob vyšetření je zvlášť vhodný pro osoby s vysokým rizikem kontrastní nefropatie, eventuálně pro mladé osoby v produktivním věku., Aim. Our work was focused on the feasibility of CT angiography of abdominal aorta and lower limbs using new generation of dual-source CT with a high pitch factor, concerning on the reduction of radiation exposure and the amount of administered contrast agent. Methods. In the prospective study were examined 29 patients with clinical signs, or known disability of lower limbs arteries. The examinations were performed on the Somatom Definition Flash (Siemens, Germany) using collimation 128 x 0.6 mm, high tube rotation speed (0.28 s/rotation) and a higher pitch factor of 1.55).Amount of contrast agent (60 to 70 ml) was applied at 5 ml/s, the timing of acquisition was based on results of the test bolus monitoring (10 ml) in the popliteal arteries. Evaluation of image quality was based on visual assessment and determination of density in different parts of the arterial system. Results. In all cases were achieved a high display quality of the aorta, pelvic, femoral and popliteal arteries by visual assessment and also determination of luminal density (aorta - 456.3 HU, groin - 507.3 HU, knee - 554.3). In the region of the distal leg and ankle occurred in 3 people in suboptimal quality due to overrun of bolus. Only in one case, the distal region leg was uninterpretable. The average level density at ankle level was 367.7 HU. The average radiation exposure was low - CTDIvol 2.40,1.79 mSv effective dose. Conclusion. The preliminary results of our prospective study showed the possibility of performing CT angiography of the lower limbs in "high-pitch" mode. Despite the very low radiation dose and the amount of applied contrast agent was achieved a high diagnostic quality of the aorta, pelvic, femoral, popliteal and proximal crural arteries.
