Nejdéle užívaná aspirace tenkou jehlou je dnes ve většině screeningových zemí užívána pro biopsii solidních lézí velmi omezeně a její hlavní význam je v aspiraci tekutinových útvarů v prsu. Core cut biopsie (CCB) je standardní metodou pro získání histologické diagnózy u solidních lézí prsní žlázy a její zavedení v devadesátých letech 20. století znamenalo zásadní změnu pro následnou chirurgickou a onkologickou léčbu karcinomu prsu. Jsou referovány nejnovější bioptické metody, které využívají k odběru většího množství tkáně z prsu vakua. Jedná se o vakuovou biopsii, která stanoví velmi přesnou histologickou diagnózu u lézí, kde core biopsie může podhodnotit histologickou diagnózu. Je to zejména v případě histologicky hraničních lézí, jako jsou in situ karcinomy a atypické hyperplazie. Intact BLES biopsie (radiofrekvenční ablace) je vysoce přesná bioptická metoda, která může odebrat ložisko v jednom vzorku tkáně. CD200/CD200R are highly conserved type I paired membrane glycoproteins that belong to the Ig superfamily containing a two immunoglobulin‑like domain (V, C). CD200 is broadly distributed in a variety of cell types, whereas CD200R is primarily expressed in myeloid and lymphoid cells. They fulfill multiple functions in regulating inflammation. The interaction between CD200/CD200R results in activation of the intracellular inhibitory pathway with RasGAP recruitment and thus contributes to effector cell inhibition. It was confirmed that the CD200R activation stimulates the differentiation of T cells to the Treg subset, upregulates indoleamine 2,3‑dioxygenase activity, modulates cytokine environment from a Th1 to a Th2 pattern, and facilitates an antiinflammatory IL‑10 and TGF‑β synthesis. Karcinóm žalúdka je smrteľná choroba a je stále druhá najčastejšia príčina úmrtí na rakovinu na celom svete. Chirurgický zákrok s kompletnou resekciou tumoru, ktorá má kuratívny potenciál, zostáva hlavnou terapiou pre skoré štádiá. Bohužiaľ, väčšina pacientov s rakovinou žalúdka je diagnostikovaná v pokročilom štádiu, čím paliatívna systémová liečba zostáva jedinou liečebnou možnosťou. Najčastejšia kombinácia chemoterapie ako liečby prvej línie na liečbu pokročilého karcinómu žalúdka (PKŽ) zahŕňa zlúčeniny platiny a fluoropyrimidínov. Fluoropyrimidíny sú chrbtovou kosťou chemoterapeutických režimov pre liečbu karcinómu žalúdka. Surgical resection remains the main treatment for early stages with complete resection having the potential for a cure. Unfortunately, most patients with gastric cancer are diagnosed in advanced stages, rendering palliative systemic therapy as the only choice of treatment. The most common chemotherapy combination as a first-line treatment in advanced gastric cancer (AGC) includes a platinum compound and fluoropyrimidine. Fluoropyrimidines have been the backbone in the chemotherapy regimens for the treatment of gastric cancer. Teysuno® (S-1) is a peroral fourth-generation fluoropyrimidine that combines tegafur, which is a prodrug of 5-fluorouracil (5-FU), and two biochemical modulators: 5-chloro-2,4-dihydroxypyridine, a powerful but reversible inhibitor of dihydropyrimidine dehydrogenase that prevents 5-FU degradation, and potassium oxonate, which reduces gastrointestinal (GI) toxicity by inhibition of 5-FU phosphorylation in the GI mucosa. Chronická neinfekční onemocnění představují závažný problém soudobé společnosti. Jejich etiologie má komplexní charakter. Jsou výsledkem v čase se vyvíjející interakce genetických a vnějších faktorů, jejichž podstatná část je vázána na životní styl a chování jedince. Genetická složka může být životním stylem modifikována, do jisté míry však sama chování a životní styl determinuje. Celoživotní vliv prostředí na genom je z velké části zprostředkován epigeneticky. Epigenetické mechanismy řídí chování genů, jsou podkladem fenotypové plasticity. Expozice faktorům modifikujícím epigenom v prenatálním a časně postnatálním období formuje vnímavost ke vzniku onemocnění, v dalším životě pak modifikuje manifestaci genetické vlohy a potenciálně participuje na patogenezi. Prenatálně indukované změny epigenomu mohou být předány na další generace potomků. Významným induktorem těchto alterací jsou nutriční faktory a kouření. V souvislosti s environmentální modifikací genové exprese je studována zejména DNA metylace, post-translační modifikace histonů a regulační aktivita nekódující RNA. Potenciální reverzibilita epigenetických procesů otevírá nové možnosti pro prevenci komplexních chorob. Their etiology is complex as they result from a time-evolving interaction of genetic and environmental factors, a significant part of which are based on individual behaviour and lifestyle. Lifestyle can modify the impact of genetic load; nonetheless, behaviour and lifestyle are themselves determined by genes to a certain extent. Lifelong environmental influences on the genome are largely mediated epigenetically. Epigenetic mechanisms modify the activity of genes and form a base of the phenotype plasticity. Exposure to epigenetic modificators prenatally and in the early post-natal stage form susceptibility to diseases, modifiying manifestation of a genetic predisposition later in life and potentially participating in pathogenesis. Prenatally induced changes of the epigenome can be passed on to subsequent generations of offspring. Nutritional factors and smoking are major inducers of these epigenetic alterations. The most studied epigenetic mechanisms in relation to environmentally induced modification of gene expression are DNA methylation, post-translational histone modifications, and non-coding RNA regulatory activity. The potential reversibility of epigenetic processes opens up new possibilities for the prevention of complex diseases. Psychosociální faktory významně participují na patogenezi komplexních chorob. Jejich působení na mnoha úrovních organismu koresponduje s bio-psycho-sociálním konceptem zdraví a nemoci. Mozek, chování a imunita jsou úzce propojeny a reagují na environmentální kontext. Účinky chronického psychosociálního stresu popisuje model alostatické zátěže, jenž ozřejmuje dynamičnost nastavení biologických procesů a význam mozku pro zpětnovazebnou regulaci v adaptaci na prostředí. Osobnost jedince, jeho kognitivní a emoční charakteristiky, kvalita sociálních vztahů a socioekonomické faktory modifikují stresovou zátěž a mají přímý i nepřímý vliv na zdraví. Řada účinků psychosociálních faktorů je zprostředkována epigeneticky; v období raného vývoje tento mechanismus formuje vulnerabilitu vůči rozvoji patologií souvisejících se stresem.
