Východiska: Systematické review (SR) představuje v kontextu s přístupem a filozofií Evidence-Based Healthcare (EBHC) nejvýznamnější a nejhodnotnější typ studie v rámci úrovně vědeckých důkazů. Hallux valgus (HV) patří mezi nejčastější deformity v oblasti přednoží. SR, zabývající se vlivem operace HV na chůzi, kvalitu života, spokojenost a současně i bolest, ve světovém písemnictví chybí. Cíle: Hlavním cílem tohoto propedeuticky zaměřeného článku bylo poukázat na význam SR v rámci přístupu EBHC a přiblížit rigorózní a systematickou metodiku jeho tvorby příkladem dílčích výsledků SR zaměřeného na hodnocení efektu operační korekce deformity HV. Metodika: Nejprve jsme formulovali review otázku a specifikovali kritéria pro zařazení a vyloučení primárních studií do SR. Protokol SR jsme vypracovali pomocí softwaru, vyvinutého Joanna Briggs Institute pro tvorbu SR. Systematické vyhledávání publikovaných i nepublikovaných primárních studií zahrnovalo 3 kroky: 1. Iniciální vyhledávání v Medline, Embase a Cinahl; 2. sekundární vyhledávání v 13 databázích; 3. prostudování referenčních seznamů relevantních studií. Vyhledávali jsme randomizované kontrolované studie (RCT) bez časového omezení a ve všech jazycích. Výsledky: Celkem jsme nalezli 2412 studií a odstranili 767 duplikátů. Na základě dvoufázového hodnocení relevance a kritického hodnocení metodologické kvality studií pomocí standardizovaného hodnotícího nástroje JBI-MAStARI bylo zařazeno 27 RCT. V současné době probíhá extrakce výsledků pomocí standardizovaného nástroje JBI-MAStARI a příprava dat na meta-analýzu. Dílčí výsledky ukázaly nejednotnost použitých metod k hodnocení bolesti a spokojenosti pacientů po operaci HV v zahrnutých RCT. Závěry: SR představují velmi významné pojítko mezi vědeckými důkazy a jejich implementací do klinické praxe. Hallux valgus (HV) is a common forefoot deformity. In the current literature, there is no SR which has reviewed the effect of HV surgery on gait, pain or function in the global evidence. Aim: Main aim of this propedeutically focused paper was to highlight the importance of SR within EBHC approach and explain rigorous and systematic methods of its development by example of partial results of the SR focusing on evaluation of the effect of the hallux valgus surgery. Medhods: Firstly, we formulated an answerable review question and specified inclusion and exclusion criteria. Protocol of the SR was prepared using soware developed by Joanna Briggs Institute for SR preparation. Systematic search of published and unpublished studies included three steps: 1. Initial search in Medline, Embase and Cinahl; 2. secondary search in 13 databases; 3. hand-search in the reference lists of relevant papers. We have searched randomized controlled trials (RCT) in any date and language. Results: We found 2412 papers and deleted 767 duplicates. en a paper retrieval according to inclusion and exclusion criteria and critical appraisal of methodological quality were done using standardized critical appraisal instruments JBI-MAStARI. 27 RCT were included into SR. Currently, we are extracting data using JBI MAStARI and preparing them for meta-analysis. Partial results showed heterogeneity of used method for evaluation of pain and patient satisfaction aer HV surgery in included RCT. Conclusions: SR is a very important link between scientific evidence and their implementation into clinical practice. Průlomová bolest (PB) je definována jako přechodné vzplanutí bolesti silné intenzity při dobře kontrolované základní bolesti analgetickou léčbou. Vyskytuje se u bolesti nádorového i nenádorového původu a dotaz na ni by měl být běžnou součástí každého kontaktu s pacientem. Ideálním řešením léčby je užití analgetika s rychlým nástupem účinku, snadno aplikovatelného a současně bezpečného pro pacienta. Článek uvádí přehled dostupných analgetik z každého stupně analgetického žebříčku WHO vhodných k léčbě PB a poukazuje na současné trendy léčby PB transmukózními formami fentanylu u onkologických pacientů., Breakthrough pain (BP) is defined as transitory pain that occurs on a background of relatively well controlled baseline pain. This pain character can join pain of tumour disease origin but not only that one. Question to specify the character of pain should be common in daily contact with a patient. To resolve this problem it seems necessary to provide patients wit rescue mediation with rapid 30 minute onset. The article comes with analgetics review according to WHO classification and suitable for BP and available in the Czech Republic. From the point of fast onset view we are looking for new approaches and application routes practical also for outpatient departments. Intranasal fentanyl offers such option. Termín „psychomotorické“ má svůj původ v Německu. Wilhelm Griesinger, jeden ze zakladatelů neuropsychiatrie, použil tento termín poprvé roku 1844. V průběhu let se v jednotlivých zemích vyvinuly odlišné koncepce psychomotoriky ve výchově a psychomotoriky jako terapeutického prostředku. Základ obou zaměření je stejný. Pohybem se snažíme vyvolat předem definovanou změnu prožívání a tomu odpovídajícího chování. Předem definovanou změnu je možno navodit pomocí znalostí funkcí mozku. Je však nutno počítat s různou úrovní motivace k této změně a různou úrovní předchozí zkušenosti. Cílem této studie je upozornit na myšlenky, z nichž současná psychomotorická terapie čerpá., The term „psychomotor“ has its origin in Germany. Východiska: Východiskem výzkumného šetření byla Psychologická studie motivů pozdního rodičovství autorky Ireny Bímové. Cíl: Hlavním cílem výzkumného šetření bylo zjistit důvody oddálení narození prvního dítěte u matek po 35. roce. Metody: Výběrovým kritériem pro zařazení do výběrového souboru byl věk primipary 35 let a více. Jako technika sběru dat byl použit polostrukturovaný dotazník vlastní konstrukce. Počet respondentů byl 92. Data byla získávána na gynekologicko-porodnických a novorozeneckých odděleních v období od ledna 2013 do ledna 2014. K analýze získaných dat byl použit Fischerův přesný test. Výsledky: Nejčastějším důvodem oddálení narození prvního dítěte u matek po 35. roce byl výběr vhodného partnera. Tuto variantu uvádí 42,4 % matek. Druhým nejčastějším důvodem byly problémy s otěhotněním u 32,6 % matek. Součástí výzkumu bylo zpětné pozitivní a negativní hodnocení věku matek při narození prvního dítěte po 35. roce. Zpětně hodnotilo pozitivně věk porodu 45,7 % žen, negativně hodnotilo věk 54,3 % žen. Ženy, které zpětně hodnotily věk porodu 1. dítěte negativně, uvedly signifikantně častěji jako důvod oddálení porodu „Problémy s otěhotněním“ (p = 0,014). Ženy, které zpětně hodnotily věk porodu 1. dítěte pozitivně, uvedly signifikantně častěji jako důvod oddálení porodu „Preferenci nezávislého způsobu života“ (p = 0,039). Závěry: Důvody oddálení narození prvního dítěte v pozdějším věku spadají zejména do oblasti sociální, psychologické a zdravotní. Výzkum může vést k zamyšlení nad problematikou trendu pozdního rodičovství. Aim: e aim of the research was to determine the reasons for postponing the first birth for mothers aer 35 years of age. Methods: e group selection criterion was a primipara age 35 and more. A semi-structured questionnaire of an original design was used to collect the research data. e number of respondents was 92. Data were obtained on obstetric and neonatal departments from January 2013 to January 2014. To analyze the data was used Fisher's exact test. Results: e most frequent reason for birth postponement amongst mothers aged 35 and more was the search for a suitable partner. is option was selected by 42.4% mothers. e second most frequent answer involved issues with conception, this being reported by 32.6% mothers. Additionally, the survey research included a retrospective positive or negative evaluation of the mothers’ age when they had their first child aer the age of 35. In retrospect, the age for the first birth was seen positively by 45.7% women and negatively by 54.3% women. Women evaluating the age for the first birth negatively significantly more oen reported the reason „Problems with conception“ (p = 0.014). The purpose of study was to analyze clinical and genetic polymorphism of Duchenne/Becker progressive muscular dystrophies among patients with neuromuscular diseases in Uzbekistan. 106 male patients with progressive pseudohypertrophic forms of muscular dystrophy were retrospectively and prospectively analyzed in the period from 2004 till 2014: 93 patients with Duchenne PMD aged from 3 years to 18 years and 13 patients with Becker PMD aged from 10 years to 25 years, who had been examined in the medico-genetic consulting department of the Republican Center “Mother and Child Screening” of Tashkent city. Příspěvek nabízí pohled na tradiční pojem „psychosomatických rodin“ z hlediska jeho současné platnosti a užitečnosti. Poukazuje na výzkumné a klinické zkušenosti s častějším výskytem psychosomatických onemocnění u rodin s určitými poruchovými vlastnostmi struktury a interakce, v nichž tělesný projev nemoci plní účelnou, krátkodobě takovou rodinu ochraňující, dlouhodobě ji v její porušenosti ustalující a vývojově ji poškozující úlohu. Výsledkem má být připomenutí si vlastností takto rizikovějších rodin s výhledem na diagnostické rozpoznávání a porozumění funkci takového psychosomatického dění a zacílení terapeutické pomoci ke zdravějším způsobům zvládání rodinného stresu, než poskytuje takto probíhající patogenní „vyrovnávání účtů“ vzájemných potřeb v rodinném soužití., The paper presents a review of the traditional concept of „psychosomatic families“ from the viewpoint of its present validity and usefulness. It refers to research and clinical pieces of experience with more frequent occurrence of psychosomatic diseases in families that exhibit certain dysfunctional traits of structure and interaction, in which the somatic symptom fulfils an effective function in momentarily protecting the family from breakdown, however, in the long run, solidifying its ongoing malfunction and developmental impairment. Czech health and social services have undergone significant changes related to the reform of public administration including the transfer of certain social and health services to the competence of municipalities and regions. Regions also became legally responsible for social services ́ planning process including their registration and since 2015 financing. The findings in the article are based on a qualitative methodological approach, which was used as a combination of public policy documents analyses and primary data analysis. The empirical research was based on focus groups when respondents represented persons caring for the elderly and disabled people as well as providers of health-social services in the Central Bohemian Region. Based on the survey we can state that some types of services are underdeveloped, particularly the domiciliary services, social services for older children and adults with disabilities, respite care and other support services for caregivers. The great problem is the absence of interconnection between health and social services in the region. From the viewpoint of service providers and caregivers the level of municipalities is perceived as closer to the clients than regional level, thus service providers and caregivers aim their demands to municipalities more frequently. and Kotrusová Miriam, Dobiášová Karolína, Angelovská Olga
