This article presents results of using proton therapy after operation of patient with brain meningioma. After operation brain meningiomas is known to not rarely recur due to certain localization and growth of tumor into main cerebral vessels, nerves and other main brain structures. This creates certain difficulties in operative access and surgical intervention, i.e. tumor resection. We report the case of a 52year old man with severe acute pancreatitis. In this case report we discuss the undesirable effects of parenteral nutrition and the importance of nutritional support at patients with severe necrotizing pancreatitis. Severe acute pancreatitis is usually accompanied by systemic inflammatory response syndrome, which results in hypermetabolism with prominent protein catabolism. Providing nutrition to these patients is of paramount importance. An adequate nutritional support is crucial in patients with severe and complicated pancreatitis. Autoři předkládají kazuistiku „antibiomanie“ u 55leté pacientky s emočně nestabilní poruchou osobnosti a mentální anorexií v anamnéze, nyní dlouhodobě ambulantně léčenou pro úzkostnou a depresivní symptomatologii, související se závažnou somatickou nemocí manžela. Přesmyk do mánie nastal po druhé 500mg dávce klarithromycinu. Nicméně ten byl již třetím podaným antibiotikem v intervalu posledních čtyř měsíců. Manické symptomy dosáhly takové intenzity, že byla nutná nedobrovolná hospitalizace v psychiatrické nemocnici. Zde příznaky odezněly do týdne. Autoři nabízejí hypotézu patogeneze manického zvratu. Předpokládají, že přesmyk mohl nastat primárně v důsledku alterace střevního mikrobiomu antibiotiky. Další patogenetická cesta může vést buď přes přímé vagové dráždění, nebo přes zvýšenou propustnost střevní bariéry pro falešné neurotransmitery., The authors present a case of antibiomania in 55years woman with borderline personality, anorexia nervosa in personal history, and long lasting anxious and depressive symptomatology associated with husband´s serious illness. Switch to mania occured after two 500mg doses of clarithromycin. However, clarithromycin was the third antibiotic administered in the last four months. Manic symptoms were so severe that involuntary hospitalization was required. Signs of mania disappeared within a week on psychiatric ward. Pancreatic schwannoma is a rare neoplasm. By 2012, there were only 47 cases reported on pancreatic schwannoma the English literature to the best of our knowledge. Hereby, we report a 37-year-old Caucasian male with a cystic schwannoma in the head of the pancreas. The patient, complaining of vague abdominal pain and pruritus underwent a computed tomography (CT) scan of the abdomen. The CT scan of the abdomen revealed a cystic lesion in the head of the pancreas. Initially, a cystic mucinous tumor of the pancreas was suspected. Whipple’s procedure was undertaken. The histopathologic evaluation of the specimen revealed a pancreatic schwannoma. The patient did well postoperatively. V tomto textu se zabývám formami zavádění PBSP do individuální práce s klientem v rámci běžné klinicko-psychologické praxe. Reflektuji v něm svou zkušenost s metodou, kterou od roku 2001 praktikuji v její základní podobě, tedy formou pravidelných, dlouhodobých, malých skupin. Vývoj terapeutické technologie PBSP posledních let však umožnil její širší využití také v individuální terapii. Zatímco v prvních desetiletích vývoje metody postupoval terapeutický proces v průběhu sezení rychle a často dramaticky „od těla ke slovu“ (bezprostřední prací s pocity ve speciálních cvičeních), což vyžadovalo téměř bezpodmínečně velmi specificky pojaté prostředí skupiny, dnešní přístup sleduje pozvolnější, jemnější i bezpečnější směr – „od slova k tělu“ (od rozhovoru k tělovým projevům, které význam slova evokuje). K možnostem i atmosféře individuálního psychoterapeutického uspořádání se tak PBSP dostala mnohem blíže. Naléhavá otázka však zůstala: jak vložit do dyády terapeut – klient METODU? Metodu, která není jen funkcí terapeutova myšlení, o něž se klient nemusí v posledku mnoho zajímat, ale která naopak vyžaduje jeho aktivní spoluúčast a nemalou kompetenci, která má velmi nezvyklou vnější podobu, navíc patinu zvláštního slangu?, The topic of the paper is: how to set up PBSP into one to one session format within an ordinary clinical psychology practice. I reflect my own professional experience with the method I have been using for years in its basic group form. However, recent developents of PBSP technology have facilitated it‘s use also in individual psychoteprapy. In the first decades of PBSP developement, the process of therapy was running on ahaed fast, often dramatically, due to body work that had been carried out immediately in special exercises. The current approach has become much slower, subtle and safe and therefore it fits better for the individual setting conditions. However, the basic question stayed remained: how to introduce A METHOD into dyad therapist – client??
