Cíl studie: zhodnocení efektivity cerkláže dle McDonalda ve Fakultní nemocnici Ostrava v období 1/2007 – 3/2014. Typ studie: retrospektivní studie Název a sídlo pracoviště: Porodnicko–gynekologická klinika, Fakultní nemocnice Ostrava Metodika: retrospektivní analýza provedených cerkláží ve Fakultní nemocnici Ostrava a rozbor výsledků takto ošetřených gravidit. Kritériem úspěšnosti byl stanoven porod po ukončeném 34. týdnu gravidity. Závěr: ve sledovaném období bylo indikováno devět cerkláží – 7 terapeutických a 2 záchranné. Výkony byly provedeny mezi 18. a 25. týdnem gravidity. Tři pacientky již měly v minulosti cerkláž provedenu. 57 % takto ošetřených těhotných porodilo po ukončeném 34. týdnu gravidity. Pouze v jednom případě (11,1 %) byla cerkláž neúspěšná a těhotenství skončilo potratem. Průměrná doba mezi provedením cerkláže a porodem činila 69 dnů., Objective: To evaluate the efficacy of cerclage according to McDonald at the University Hospital Ostrava in the period 1/2007 - 3/2014. Design: A retrospective study Obstetrics and Gynecology: Obstetrics - Gynecology, University Hospital Ostrava Methods: A retrospective analysis of cerclage performed at the University Hospital Ostrava and analysis of the results thus treated pregnancies. The criterion for success was determined after childbirth completed 34 weeks of preg - nancy. Conclusion: In the reporting period, nine indicated cerclage - 7 of therapeutic and 2 emergency. The procedures were performed between the 18th and 25th week of pregnancy. Three patients have had in the past cerclage performed. 57% of the treated pregnant women gave birth after 34 completed weeks of gestation. Only in one case (11.1 %) cerclage was unsuccessful and the pregnancy ended in a miscarriage. Práce prezentuje prenatální diagnostiku, možnosti terapie a následnou péči u plodů a novorozenců s kongenitálním chylothoraxem na podkladě zhodnocení 14 případů na naší klinice v letech 1993 až 2012. Povšechný hydrops plodu byl popsán ve 4 případech (29 %), v 10 případech (71 %) byla provedena odlehčovací amniodrenáž a ve 3 případech (21 %) prenatální odlehčovací punkce hrudníku. Spontánně bylo porozeno 11 plodů (78 %), hrudní drenáž byla nezbytná v průměru 9 dnů po porodu. Mortalita dosáhla v souboru 29 % (4 případy). V článku popisujeme vyšetřovací algoritmy, terapeutické prenatální i postnatální postupy a je zdůrazněna nutnost mezioborové spolupráce., The work presents prenatal diagnosis, treatment and care options in fetuses and neonates with congenital chylothorax based on a detailed evaluation of 14 cases recorded in our department between years 1993 and 2012. Generalized fetal hydrops was described in 29% of cases, therapeutic amnio drainage performed in 71% and prenatal thoracentesis in 21% of cases. 78% of fetuses were delivered spontaneously; thoracic drainage was necessary nine days after birth in average. Mortality reached 29%. Motivation Until recently it was considered that 65 years is cutoff for defining patients as elderly, but newer reports indicate that this age limit shift to 70 years of age. Elderly patients with advanced non small cell lung cancer, associated comorbidities and poor performance status represent a specific population and a challenge for use of chemotherapy. Primary aim was to evaluate the impact of mono therapy with oral etoposide on overall survival in elderly patients (≥ 70 years of age) with advanced non small cell lung cancer and poor performance status (PS) ≥ 2 (clinical stage IIIb and IV ), and as well to evaluate tolerability of this therapy. Secondary aim was to evaluate response rate. Methods Retrospectively, medical records of 79 female and male patients with advanced non small cell lung cancer and poor performance status treated with oral etoposide (2x25 mg 20 days/10 days pause) in period from 2007 till 2010 were checked for relevant data. Data regarding demographics, performance status, overall survival, response rates and drug toxicity were collected. For statistical analysis we used Pearson chi-square test, T-test, Kaplan-Meier product limited method and Cox regression. Results Median overall survival (OS) was 31 weeks, in patients with PS 2 overall survival was 34 weeks, and in group with PS 3 was only 24 weeks. Partial response was registered in 20.2% of patients, stable disease in 41.85 % and disease progression in 38% of patients. Treatment was well tolerated, febrile neutropenia and toxic deaths were not registered. Toxic effects didn't have statistically significant influence on OS. Východiska: Impulzem k realizaci retrospektivní analýzy historických dokumentů byla práce britské autorky Susan McGannové o organizaci mezinárodních ošetřovatelských kurzů v Londýně ve dvacátých a třicátých letech 20. století, jichž se pravidelně účastnily diplomované sestry z Československa. Cíl: Doložit na příkladu zahraničních zkušeností československých diplomovaných sester z let 1920–1938 genezi některých dnešních aktuálních problémů v ošetřovatelství (transformace vzdělávání nelékařských zdravotnických pracovníků, otázka kompetencí všeobecných sester). Metody: Práce je založena na obsahové analýze článků a drobných zpráv publikovaných na stránkách dvou pro rozvoj ošetřovatelské profese v období první republiky klíčových odborných časopisů. Záměrem bylo projít všech 17 ročníků, tj. 204 čísel Zpráv Československého červeného kříže vydávaných v letech 1920–1936 a 8 ročníků, tj. 96 čísel Československé nemocnice z let 1931–1938. Výsledky: Výzkum prokázal intenzivní zahraniční styky československých diplomovaných sester na několika úrovních (mezinárodní kurzy, konference, jednání mezinárodních ošetřovatelských organizací, působení zahraničních sester v Československu, překlady odborných publikací), jež byly finančně podpořeny Československým červeným křížem, Ligou Červených křížů, Americkým červeným křížem a Rockefellerovou nadací. I když první ošetřovatelská škola v Praze byla původně budována podle zásad uplatňovaných na ošetřovatelské škole ve Vídni a československé sestry udržovaly po celé sledované období osobní i korespondenční styky s kolegyněmi z různých zemí světa, ukázalo se, že klíčová pro získávání nových zkušeností v oblasti ošetřovatelství byla orientace na angloamerické prostředí. Mimořádný význam měly zejména mezinárodní odborné kurzy pořádané na Bedford College a Royal College of Nursing v Londýně. Závěry: Obsahová analýza vybraných návrhů na prosazení změn, jež byly projednávány na mezinárodních ošetřovatelských konferencích v meziválečném období, se ukazuje jako inspirativní i pro současné diskuse o transformaci vzdělávání a vymezení kompetencí všeobecných sester., Background: e study has been inspired by the paper of Susan McGann who discusses the international courses organized in London in the 1920s and 1930s, in which nurses from Czechoslovakia regularly participated. Aim: Using the experiences of Czechoslovak nurses abroad from 1920–1938, to give evidence of the origins of some topical problems in present nursing (the transformation of education for paramedical staff, the question of nurse competencies). Methods: e paper is based on the content analysis of articles and short reports published in two main Czechoslovak professional journals of the period: Reports of Czechoslovak Red Cross/Zprávy Československého červeného kříže (in total 204 issues from 1920–1936) and Czechoslovak Hospital/Československá nemocnice (in total 96 issues from 1931–1938). Results: Research has proved intensive foreign contacts of Czechoslovak nurses on several levels (international courses, conferences, meetings of the international nursing organizations, activities of foreign nurses in Czechoslovakia, translations of professional publications), supported by the Czechoslovak Red Cross, the League of Red Cross Societies, the American Red Cross and the Rockefeller Foundation. Even when the first nursing school in Prague was established according to principles used at the school in Vienna and even when Czechoslovak nurses kept personal and correspondence contacts with colleagues from different countries of the world during the whole searched period, the orientation to the Anglo-American milieu proved to be crucial for gaining new experience in the field of nursing. Particularly the courses organized at Bedford College and Royal College of Nursing in London were of special importance. Autori v retrospektívnej štúdii analyzovali 28 prenatálne diagnostikovaných prípadov CDH u plodov. Cieľom práce bola analýza 28 prenatálne diagnostikovaných prípadov CDH u plodov za uvedené obdobie k počtu terminácií a pôrodov. Poukazujú na vplyv lokalizácie, veľkosti defektu, stanovenia týždňa diagnózy, stupňa hypoplázie pľúcneho tkaniva, výsledku LHR (lung-to-head ratio) u plodov s CDH na prognózu a výsledné prežívanie takto postihnutých novorodencov, pričom LHR je pomer násobku dĺžky a šírky hypoplastických pľúc k obvodu hlavičky plodu. Autori sa pokúsili stanoviť zásady manažmentu gravidity a pôrodu u plodov s CDH, ktorý vychádza z analýzy nášho klinického materiálu z rokov 2004–2013. Najhoršiu prognózu mali plody, u ktorých bola diagnóza stanovená pred 25. týždňom gravidity, plody s herniáciou laloka pečene do dutiny hrudnej, plody s ťažkou hypopláziou pľúcneho parenchýmu, plody s veľkým diafragmatickým defektom a nízkym indexom LHR. Prognóza a prežívanie novorodencov v našom prípade priamoúmerne závisela od stupňa hypoplázie pľúc, stupňa perzistujúcej pľúcnej hypertenzie, závažnosti pridružených vrodených vývojových chýb a úspešnosti neonatálnej intenzívnej starostlivosti v prevencii a liečbe pľúcnej hypertenzie. Prežívanie novorodencov v našom súbore bolo 21,43 %., The authors in a retrospective study analyzed 28 prenatal diagnosed cases of CDH fetuses. The aim of the study was to analyze the number of diagnosed cases of CDH fetuses for the given period compared to the number of births and terminations. They show how the location, defect size, the week of diagnosis, degree of hypoplasia of the pulmonary tissue, the result of the LHR (lung-to-head ratio) in fetuses with CDH influence the prognosis and survival rates of affected newborns. LHR is the ratio between the length and width of the hypoplastic lung to the periphery of the fetal head. They tried to establish the principles of management of pregnancy and the birth of fetuses with CDH, based on our analysis of clinical material collected between 2004 -2013. The worst prognosis fetuses were in diagnosed before 25th week of gestation, fetuses with liver lobe herniation into the thoracic cavity, fetuses with severe hypoplasia of the pulmonary parenchyma, fetuses with large diaphragmatic defects and low index LHR. The prognosis and survival of newborns in our study depended directly on the degree of pulmonary hypoplasia, the degree of persistent pulmonary hypertension, severity of associated birth defects and the success of neonatal intensive care in the prevention and treatment of pulmonary hypertension. Východiska: Příspěvek předkládá výsledky retrospektivní studie zaměřené na screening otoakustických emisí u novorozenců. Cíl: Hlavním cílem výzkumného šetření bylo zjistit incidenci sluchových vad u fyziologických a rizikových novorozenců ve vybraném období. Metodika: V rámci kvantitativního výzkumu byla použita metoda retrospektivní studie prostřednictvím záznamového archu, které autorům poskytlo novorozenecké oddělení. Pro sledování průběhu těhotenství, porodu, osobní a rodinné anamnézy byl využit informační portál vybraného zdravotnického zařízení. Výběrový soubor zahrnoval novorozence, u nichž bylo ve stanovém období provedeno vyšetření sluchových vad. Výsledná data byla vyhodnocena deskriptivní statistikou a prostřednictvím Pearsonova chí-kvadrátového testu. Výsledky: V roce 2014 bylo ve zdravotnickém zařízení hospitalizováno 1 516 novorozenců, screening otoakustických emisí byl proveden u 1 510 novorozenců. Vyšetřeno bylo 1 258 fyziologických novorozenců, přičemž sluchová vada byla odhalena u 5 (0,4 %) novorozenců. Ze skupiny novorozenců rizikových byla vada diagnostikována u 7 (2,78 %) novorozenců z 252. Byl statisticky potvrzen častější výskyt sluchových vad u této skupiny respondentů. Nejčastějším společným faktorem uváděným v anamnéze matky u novorozence se sluchovou vadou bylo onemocnění diabetes mellitus. 7 (58,33 %) novorozenců s poruchou sluchu mělo rodinnou anamnézu nezatíženou. Závěr: Incidence sluchových vad je u novorozenců rizikových i fyziologických. Pro správný vývoj dítěte je žá- doucí tyto vady odhalit v časném stádiu. V České republice však nadále chybí legislativní podpora ukládající zdravotnickým zařízením povinnost provádět novorozenecký screening sluchu celoplošně., Background: The article presents the results of retrospective study which is focused on otoacoustic emission screening of newborns. Aim: To analyse and compare physiological and risk newborns examinated of otoacoustic emission. Method: There was used the method of retrospective study through the record sheet and information portal in a chosen medical institution within quantitative research. File selection contained all newborns hospitalized in the medical institution in the year of 2014 and babies who took an examination of hearing impairments. Results: In the year of 2014 in the medical institution was born 1 516 newborns. The examination of otoacoustic emission was done of 1 510 newborns. From the number of 1 258 physiological newborns there were 5 (0,4 %) of them with hearing impairment. From the number of 252 risk newborns there were diagnosed 7 (2,78 %) newborns with hearing impairment.The conclusion is that the hearing impairment is more often in risk newborns. Diabetes mellitus was the risk factor in the hearing impairment. Conclusion: The incidence of hearing impairments occurs in risk newborns as well as physiological. It is necessary to discover hearing impairment in early stage for right child evolution. To study defects and chromosomal abnormalities of the fetus, we retrospectively analyzed results of comprehensive dynamic survey of 26,404 pregnant women aged 1850 years old at 6 to 40 weeks of pregnancy. Of them, 25,956 (98.3%) women had physiological course of pregnancy, 448 (1.7%) women had abnormal pregnancy. For the diagnosis of fetal defects, we carried out ultrasound, biochemical, invasive and cytogenetic studies. The results of study showed that the majority of fetal defects and pathological course of pregnancy was noted in women at the age of 2125 years old, since at this age period women have the highest number of pregnancies. At the older age, we noted a gradual decrease in the number of pregnant women, as well as the number of abnormalities of the fetus. Based on the analysis of the results obtained, we have developed an algorithm for early fetal ultrasound examination. In order to exclude nondeveloping pregnancy and intrauterine fetal death, as well as for early diagnosis of fetal defects, we recommend screening women in the first trimester of pregnancy., Shahnoza Kamalidinova, and Literatura
Cieľ: Cieľom prieskumu bolo zistiť prevalenciu a rizikové faktory hypertenzie bieleho plášťa (WCH) u detí. Metodika: Formou retrospektívnej štúdie sme analyzovali zdravotné záznamy 140 detí (36 dievčat, 104 chlapcov) s vysokým krvným tlakom nameraným v kardiologickej ambulancii, u ktorých bolo vykonané 24-hodinové ambulantné monitorovanie krvného tlaku (ABPM) a určený typ hypertenzie. Hypertenzia bola diagnostikovaná, keď bol systolický a/alebo diastolický krvný tlak meraný auskultačnou technikou ≥ 95. percentil pre pohlavie, vek a výšku. WCH bola definovaná ako priemerná hodnota krvného tlaku v ambulancii nachádzajúca sa v percentilovom pásme hypertenzie a hodnota tlaku krvi počas 24-hodinového monitorovania ambulantného tlaku nachádzajúca sa v pásme normálnych hodnôt krvného tlaku. Výsledky: V skupine 140 detí bola potvrdená prítomnosť WCH u 51 detí (36,4 %) – 13 dievčat a 38 chlapcov. Z celkového súboru sa WCH vyskytovala u 36,1 % zaradených dievčat a 36,5 % zaradených chlapcov. Systolický TK sa v kardiologickej ambulancii nachádzal nad 95. percentilom, diastolický TK pod 90. percentilom. Väčšina pacientov s WCH bola vo veku 15 – 19 rokov (80,4 %) a v pásme od 3. do 90. percentilu BMI (72,5 %). U pacientov prevládala negatívna rodinná anamnéza hypertenzie (54,9 %) a zriedkavé návštevy kardiologickej ambulancie (2-4x). Krvný tlak v ambulancii merala deťom výlučne sestra. Záver: Zistenie prevalencie WCH a jej rizikových faktorov je u detí významné, pretože prevažuje názor, že v detskom veku je WCH predispozíciou k trvalej hypertenzii. Na základe zistení rizikových faktorov, môžeme vykonávať prevenciu WCH., Aim: The aim of this study was to find out the prevalence and risk factors of white coat hypertension (WCH) in children. Methods: We realized the retrospective study and analyzed records of 140 children (36 girls, 104 boys) with high blood pressure measured in cardiology outpatients' clinic, to whom a 24-hour ambulatory blood pressure monitoring (ABPM) was made and the type of hypertension was specified. Hypertension was diagnosed when systolic and/or diastolic blood pressure measurements with auscultatory technique were ≥ 95th percentile for sex, age and height. White coat hypertension (WCH) has been defined as mean blood pressure (BP) readings in outpatients' clinic being located in the hypertensive range while 24-hour ambulatory blood pressure monitoring is in the normal range. Results: In the group of 140 children the presence of WCH was confirmed in 51 children (36,4 %) - 13 girls and 38 boys. WCH occurred in 36,1 % of girls and 36,5 % of boys in the total sample of respondents. Systolic BP in cardiology outpatients' clinic reached the value above the 95th percentile, diastolic BP below the 90th percentile. The most patients with WCH were 15 – 19 years old (80,4 %) and in 3rd - 90th percentile of BMI (72,5 %). Patients prevailed negative family history of hypertension (54,9 %) and fewer visit (2-4 times) of cardiology outpatients' clinic. Blood pressure of children was measured exclusively by a nurse in this outpatients' clinic. Conclusion: Determination of the prevalence of WCH and its risk factors in children is important because of prevailing opinion that WCH in children is predisposition in permanent hypertension.
