Anomálie urachu jsou vzácné. V literatuře najdeme práce pojednávající spíše o jednotlivých případech než velké soubory nemocných. Optimální léčba zůstává předmětem debat. Uvádíme kazuistiku 13měsíční dívky s infikovanou cystou urachu. Jedná se o onemocnění s dobrou prognózou při včasně zahájené adekvátní léčbě, může se však manifestovat i jako život ohrožující sepse či peritonitis, pokud dojde k ruptuře cysty., Urachal anomalies are rare. Kvalita ţivota je ovlivněna zdravotním stavem, který zahrnuje fyzické, psychické a sociální charakteristiky. Největším zdravotním, ale i společenským problémem současné epochy nejsou nemoci akutní, ale nemoci dlouhodobé, chronické a trvalé. Výzkum se zaměřuje na oblast sexuality u pacientů s hypertenzí, s důrazem na holistické pojetí lidské osobnosti, hlubší vnímání a proţívání sexuálního ţivota, sexuálních potřeb pacientů. Cíl: Cílem kvalitativního šetření bylo zjistit, zda léčba hypertenze ovlivňuje sexuální potřeby pacientů. Metodika: Výzkumný soubor tvořilo 20 respondentů, výběr zkoumaného vzorku byl záměrný, byli to pacienti, kteří se léčili pro hypertenzi minimálně 1 rok ve Fakultní nemocnici Ostrava. Výzkum byl proveden pomocí kvalitativní metody, sběr dat byl proveden rozhovorem, který byl nahráván na diktafon v příjemném, nerušeném prostředí. Ve výzkumu byla pouţita metoda barvení textu s definovanými 4 kategoriemi a podkategoriemi. Výsledky: Z analýzy rozhovorů vyplynulo, ţe u některých pacientů antihypertenzní terapie negativně ovlivnila jejich sexuální potřeby. Byl zjištěn výskyt sexuálních dysfunkcí, u ţen se jednalo o sníţený sexuální apetit, sníţenou lubrikační schopnost, delší interval k vyvrcholení a u muţe byl objeven problém s erekcí. Závěr: V rámci subjektivní spokojenosti s vlastní sexualitou u pacientů, kteří se léčí s arteriální hypertenzí je důleţitá znalost vedlejších účinků a následná edukace s vyuţitím výsledků vědeckých studií domácích i zahraničních odborníků., The quality of life is influenced by health condition, which include physical, psychological and social features. The biggest, not only medical but also social problem nowadays are not acute diseases but long-term diseases, chronical and permanent. The research is concentrated on the sphere of sexuality among patients with hypertension, with stress on holistic concept of human being, deeper perception and experiencing of sexual life and sexual needs of patients. Aim: The aim of qualitative research was to find out how the treatment of hypertension influences sexual needs of patients. Methods: Researched sample was formed by 20 respondents. The choice of researched sample was intentional. They were patients who were treated for hypertension minimally one year in University Hospital Ostrava. The research was done by qualitative method, collection of datas was done by interview, which was recorded on voice recorder in a pleasant, by nothing disturbed place. In the research was used method of coloring the text with 4 defined categories and subcathegories. Results: From the analysis of interviews was clear that at some patients antihypertension therapy influenced their sexual needs in a negative way. There was found out the occurrence of sexual dysfunctions. Among women it was lower sexual appetite, lower lubricative ability, longer time for climax and among men was found the problem with erection. Medicínské technologie dokáží udělat rodiče i z člověka v pozdním seniu. Odborné debaty se soustřeďují nejen na to, jaké metody asistované reprodukce jsou eticky přijatelné, ale i na to, pro jaké skupiny lidí a za jakých okolností jsou přijatelné. Moje analýza je inspirována úmyslem státních orgánů a) zvýšit věkovou hranici pro přístup k léčbě placené ze zdravotního pojištění (ZP), b) zvýšit počty cyklů placených ze ZP; c) zrušit zákonem danou horní věkovou hranici pro samoplátce. Pro rozsáhlost tématu je text rozdělen do dvou samostatných článků. V této první části analyzuji věk pro platbu léčby ze ZP, relevantní argumenty nacházím pouze pro platbu léčby ženám do 40 let., Medical technology can make parents from persons in their late senium. Druhá část do dvou článků rozdělené analýzy věkových limitů v asistované reprodukci (ART). V této části se zaměřuji na hledání věkových limitů pro přístup k ART u samoplátců, zvažuji přitom zájmy čtyř z pěti zúčastněných členů systému: žadatele/lů o léčbu, dárce gamet, budoucího dítěte a společnosti, a rizika s tím spojená. Pro potvrzení současné zákonné věkové hranice pro ženu (den před 50. narozeninami) ani pro její zvýšení či odstranění nenacházím dostatečné důvody. The analysis of data of 97 children with brain tumors of supratentorial localization was carried out. To estimate treatment efficacy was used a rating scale of quality of life. Based on survey of 257 women with nondeveloping pregnancies of various geneses, it was shown that the level of apoptosis marker P53 in the decidual tissue is increased, and antiapoptotic protein Bcl2 simultaneously is reduced, compared with similar data of women with physiological pregnancies. Cíl: Výměnný obrázkový komunikační systém (VOKS) je na podmínky České republiky (ČR) modifi kovaná metoda vycházející z komunikačního systému PECS (The picture exchange communication system), který byl vytvořen v Chicagu v USA. Výzkum měl za cíl zmapovat spokojenost uživatelů s touto metodou, zhodnotit obtížnost osvojení metody VOKS a zmapovat dodržování doporučených postupů. Uživatelem autoři rozumí, pracovníka, který využívá metodu VOKS při komunikaci s klienty s PAS. Ve výzkumné části jsou prezentovány výsledky šetření, které se uskutečnilo v pěti vybraných speciálních zařízení a čtyřech školách na území celé ČR. Metody: Výzkumné šetření proběhlo v období od září 2010 do prosince 2010. Byl vytvořen dotazník vlastní konstrukce, který obsahoval 24 otázek. Kritériem pro zařazení respondentů do výzkumu, byla práce s metodou VOKS v zaměstnání, které respondenti aktuálně vykonávali. Výzkumný soubor tvořil 54 respondentů. Výsledky: Analýzou výsledků bylo zjištěno, že většina respondentů je spokojena s metodou VOKS. Rozdíly jsou patrné v době osvojení metody VOKS v praxi. Na tomto konkrétním vzorku respondentů se nepodařilo určit obecně platný čas potřebný k osvojení metody VOKS. Jako problematické se jeví i přesné dodržování postupů. Hlavními důvody nedostatečného dodržování postupů v praxi je potřeba individuálního přizpůsobení klientovi, nedostatek personálu a nedostatek času na nácvik. Velkým problémem je aplikace metody v době dovolených proškoleného personálu. Závěr: Výzkum naznačil, že metoda VOKS se prozatím, z pohledu individuálního hodnocení respondentů, jeví jako vhodná pro komunikaci s klienty s různorodými poruchami řeči. Metoda VOKS může přispět pracovníkům ve speciálních zařízeních ke zkvalitňování ošetřovatelské péče a k lepšímu uspokojování potřeb klienta., Aims: Czech picture exchange communication system called VOKS is a modifi ed method of communication system PECS (Picture Exchange Communication System) which was created in Chicago in the USA. The research aim was to assess user satisfaction with this method, to evaluate diffi culties with acquiring the VOKS methods and to map compliance with recommended practice. A “user” is understood as somebody who uses VOKS method to communicate with clients suff ering from ASD. The research section presents results of the survey which took place in selected fi ve specialised facilities and four schools all over the Czech Republic. Methods: The survey covered the period from September 2010 to December 2010. We designed our own questionnaire which contained 24 questions. The criterion for inclusion of respondents in the research was current professional working experience with the VOKS method. The research sample consisted of 54 respondents. Results: The results of the analysis revealed that most respondents are satisfi ed with the VOKS methods. Differences are evident in the time when they adopted VOKS methods in practice. With this particular sample of respondents the time necessary to master the VOKS method could not be established. Accurate compliance with recommended procedures proved to be problematic. The main reason for poor compliance with the procedures in practice is the necessity to adapt it for an individual client, staff shortages and insuffi cient time for training. A major problem is the application of the method when the trained staff is on holiday. Conclusion: The research indicates that for the time being the respondents consider the VOKS method to be a suitable method for communicating with clients with a variety of speech disorders.
