Cíl: Cílem šetření bylo zjistit, jak studenti ošetřovatelství hodnotí svoji klinickou praxi, spolupráci s mentorem, jaké aspekty ovlivňují jejich spokojenost s praxí a jak se hodnocení praxe liší na jednotlivých odděleních. Metodika: Výzkumný soubor zahrnoval 59 studentů druhého a třetího ročníku oboru všeobecná sestra. Dotazníkové šetření bylo realizováno standardizovaným nástrojem pro hodnocení klinické výukové prostředí a supervize CLES (Saarikoski, 2002) po absolvované klinické praxi studentů (2008). Výsledky: Byly potvrzeny rozdíly v hodnocení supervize a klinického výukového prostředí vzhledem k typu oddělení, kdy interní oddělení byla studenty hodnocena kladněji. Studenti hodnotili nejvyššími známkami dimenzi ošetřovatelská péče na oddělení, nejhůře pak vztah mezi mentorem a studentem. Současně byly potvrzeny pozitivní korelace mezi celkovou spokojeností studentů a jejich hodnocením. Statisticky významné vztahy byly identifikovány mezi zátěží sester na oddělení a hodnocením studentů – s vyšší fyzickou zátěží sester byla supervize a klinické výukové pracoviště hodnoceno kladněji. Závěr: Výsledky šetření ukazují na význam supervize a kvalitního klinického výukového prostředí pro ošetřovatelské praxe. Praktická výuka tvoří významnou část vzdělávacího programu sester, a proto je nezbytné sledovat zkušenosti a reflexe studentů., Objective: The aim of this study was to find out how nursing students evaluate their clinical practice, co-operation with the mentor, what aspects affects their satisfaction with the practice and how the evaluation of practice varies from wards. Methods: The sample comprised of 59 nursing students of the second and third year of study. The survey was realized by standardized Clinical Learning Environment and Supervision instrument CLES (Saarikoski, 2002) after the completion of students clinical practice (2008). Results: Differences were confirmed in the evaluation of the supervision and clinical learning environment depending on the type of wards, when the internal wards were evaluated by students more favourably. Students rated the dimension of nursing care at the word the highest marks, the lowest was rated the relationship between mentors and students. At the same time were confirmed positive correlations between students’ overall satisfaction and their evaluation. Statistically significant relationship were identified between the nurses´ workload in the wards and students´ assessment – with the higher workload of nurses was the supervision and clinical learning environment evaluated more favourably. Conclusion: The results of survey show the importance of clinical supervision and quality of clinical learning environment for nursing practice. Existuje řada faktorů, které mohou komplianci ovlivňovat. Tyto faktory bývají povětšinou kategorizovány na charakteristiky pacienta, znaky dané nemoci, charakteristiky léčebného režimu, charakteristiky léčebného uspořádání – settingu, povahu vztahu mezi pacientem a zdravotníkem. Právě vztah mezi pacientem a zdravotníkem je klíčovou záležitostí ve studiu kompliance. „Vztah“ však může být obtížně uchopitelným (a tudíž i ovlivnitelným) pojmem. Více jasnosti může vnést uvažování o tom, čím je povaha vztahu mezi zdravotníkem a pacientem utvářena: o spokojenosti pacienta s léčbou, o rozvržení moci mezi zdravotníkem a pacientem, o otevřené komunikaci mezi nimi, o sdílení informací, o rovnováze aktivity pacienta a zdravotníka i o zájmu zdravotníka o to, co si pacient o léčbě myslí a jak ji prožívá. To vše jsou příležitosti k větší spolupráci pacientů a zdravotníků., There are many specific factors, which can influence compliance. These factors are mostly put into five categories: patient characteristics, disease characteristics, treatment factors, factors of the treatment setting, and factors of patient-provider relationship. Just this patient-provider relationship is a key figure in the study of compliance. However, „relationship“ may be a term difficult to be understood (and influenced, as well). Considering the patients´ satisfaction with treatment, the power between providers and patients, the open communication between them, sharing information, balanced activity of both parties, providers´ interest in patients´ beliefs and experiences may make the term „relationship“ clearer. Tématem této přehledové studie je kompliance – adherence – tedy míra, se kterou se pacientovo jednání (např. ve smyslu braní léků, dodržování diety, uskutečňování životních změn) shoduje s radami získanými od zdravotníků. Míra nekompliance je vysoká. Proto se objevují postupy zvyšující spolupráci mezi pacienty a zdravotníky. Je to postup motivační, který se snaží pacientovi pomoci, aby důvody pro změnu odkryl v sobě; edukativní postup usilující o to, aby pacient rozuměl léčbě, jak on sám potřebuje; behaviorální postup zaměřený na pacientovo učení se dovednostem, které kompliantní chování vyžaduje; rovněž postupy využívající sociální oporu pacienta; organizační strategie, které usilují o zjednodušení terapie, o její zpříjemnění i o zvýšený dohled nad pacientem., This study is focused on the topic of compliance (adherence). These concepts refer to the extent to which patient´s behavior (in terms of taking medication, following diets, or achieving lifestyle changes) coincides with medical or health advice. The extent of non compliance is high. Therefore new interventions increasing cooperation between patients and health-care providers are being looked for. Among the most important strategies there is the motivation method helping the patient himself to find the reasons for compliance. Educative technique is outstanding as well: it enables patient to understand the subjects of the treatment, which he consider to be most important. Behavioral approach helps patients to gain skills necessary for compliance. Kompliance je závažným problémem v současném zdravotnictví. Uvažuje se, jak ji zvýšit, neboť míra nekompliance je vysoká. Současně se kompliance ukazuje i jako etický problém. Nejčastěji se diskutuje o způsobech, jakými může (a zda vůbec může) zdravotník vést pacienta k přijetí hodnot, které on uznává jako prvořadé (zdraví, úleva od bolesti, prodloužení života, kompliance). A co má dělat v případě, že se jeho a pacientovy hodnoty od sebe odlišují. Někteří etici přiznávají zdravotníkovi velkou míru zodpovědnosti za dodržení pacientovy kompliance a podobně velkou míru zodpovědnosti pacientovi v případě, že komplianci nedodrží. Jiní etici zpochybňují komplianci jako nejvyšší hodnotu, což ovšem nezbavuje zdravotníka povinnosti pomoci pacientovi plně pochopit zdravotnické pokyny. Ve středu zájmu etiků je rovněž motiv moci ve vztahu mezi zdravotníkem a pacientem. Zdravotníci mají svou moc danou svým samotným zaměstnáním, svými znalostmi i svou důvěryhodností. Za etické se považuje použití těchto forem moci k získání „moci spolu s pacientem“ („power with“) namísto získání moci nad pacientem („power over“)., Compliance (adherence) is a serious problem in today health-care system. The strategies increasing cooperation between patients and health-care providers are being looked for, because the extent of non compliance is high. Simultaneously, the compliance is an ethical problem. Mostly the ethics considers the ways how to lead patients to adopting of the health-care providers values, such as health, relief of suffering, prolongation of life, compliance. The ethics also considers if it is allowed at all and what to do if the patients´ values and health-care providers´ ones differ. Some moralists consider providers responsible for maintaining compliance and blame patients for discontinuing it. Others think that providers are responsible for explaining the importance of cooperation, but they doubt compliance as the most desirable value. The moralists also debate over the power between patient and health-care provider. The latter derives his power from his formal position, information and credibility. Autori v retrospektívnej štúdii analyzovali 28 prenatálne diagnostikovaných prípadov CDH u plodov. Cieľom práce bola analýza 28 prenatálne diagnostikovaných prípadov CDH u plodov za uvedené obdobie k počtu terminácií a pôrodov. Poukazujú na vplyv lokalizácie, veľkosti defektu, stanovenia týždňa diagnózy, stupňa hypoplázie pľúcneho tkaniva, výsledku LHR (lung-to-head ratio) u plodov s CDH na prognózu a výsledné prežívanie takto postihnutých novorodencov, pričom LHR je pomer násobku dĺžky a šírky hypoplastických pľúc k obvodu hlavičky plodu. Autori sa pokúsili stanoviť zásady manažmentu gravidity a pôrodu u plodov s CDH, ktorý vychádza z analýzy nášho klinického materiálu z rokov 2004–2013. Najhoršiu prognózu mali plody, u ktorých bola diagnóza stanovená pred 25. týždňom gravidity, plody s herniáciou laloka pečene do dutiny hrudnej, plody s ťažkou hypopláziou pľúcneho parenchýmu, plody s veľkým diafragmatickým defektom a nízkym indexom LHR. Prognóza a prežívanie novorodencov v našom prípade priamoúmerne závisela od stupňa hypoplázie pľúc, stupňa perzistujúcej pľúcnej hypertenzie, závažnosti pridružených vrodených vývojových chýb a úspešnosti neonatálnej intenzívnej starostlivosti v prevencii a liečbe pľúcnej hypertenzie. Prežívanie novorodencov v našom súbore bolo 21,43 %., The authors in a retrospective study analyzed 28 prenatal diagnosed cases of CDH fetuses. The aim of the study was to analyze the number of diagnosed cases of CDH fetuses for the given period compared to the number of births and terminations. They show how the location, defect size, the week of diagnosis, degree of hypoplasia of the pulmonary tissue, the result of the LHR (lung-to-head ratio) in fetuses with CDH influence the prognosis and survival rates of affected newborns. LHR is the ratio between the length and width of the hypoplastic lung to the periphery of the fetal head. They tried to establish the principles of management of pregnancy and the birth of fetuses with CDH, based on our analysis of clinical material collected between 2004 -2013. The worst prognosis fetuses were in diagnosed before 25th week of gestation, fetuses with liver lobe herniation into the thoracic cavity, fetuses with severe hypoplasia of the pulmonary parenchyma, fetuses with large diaphragmatic defects and low index LHR. The prognosis and survival of newborns in our study depended directly on the degree of pulmonary hypoplasia, the degree of persistent pulmonary hypertension, severity of associated birth defects and the success of neonatal intensive care in the prevention and treatment of pulmonary hypertension. Tento článek se pokouší zodpovědět na otázky související s problematikou konstrukce pohlavní identity v právním řádu. V této souvislosti se zabývá i nejasnostmi ohledně chápání pohlaví a pohlavní identity v různorodých pozicích, jak je uchopeno biologicky (sex), sociálně (gender) a psychicky. Zároveň tato stať poukazuje i na problematiku možného rozkolu právní identity (nazírané) s identitou subjektu (vnímanou). Jako příčinu možných problémů v konstrukci právního řádu spatřuje článek v zpředmětňování kategorie „identita“. V závěru jsou nastíněny praktické problémy českého právního řádu, které vznikají právě z artefaktů historického vnímání právní identity a poukázáno i na možné praktické řešení., This article attempts to answer questions related to issues of the construction of sexual identity in the legal system. In this context, it deals with the uncertainty regarding the understanding of gender and sexual identity in diverse positions - biologically (sex), socially (gender) and mentally. At the same time, this paper points out the problem of possible disruption betwen the unity of the legal identity and subjective identity. The article sees the objectification of "identity" to be a possible cause for the problems the legal construction of identity faces. Článek podává stručný přehled o možnostech konzervativní léčby endometriózy. Na základě poznané patofyziologie nemoci představuje léčbu hormonální, a to nejen aktuálně používanou, jako je kombinovaná hormonální antikoncepce, progestiny nebo analoga gonadoliberinů, ale i tu, která prošla klinickými studiemi, ale z důvodu ekonomické náročnosti nebo výrazných nežádoucích účinků se dnes používá jen výjimečně. Článek zmiňuje i léčbu analgetickou a alternativní, která rozšiřuje terapeutické možnosti léčby endometriózy a pomáhá odstranit její hlavní symptom, a to bolest., The article gives a brief overview of the possibilities of conservative treatment of endometriosis. Cieľ: Príspevok má design prierezovej štúdie, s parciálnym cieľom testovať psychometrické vlastnosti nástroja kvality ţivota (Caregiver Quality of Life Index Cancer) opatrovateľov v starostlivosti o zomierajúcich z hľadiska formy poskytovanej starostlivosti. Metodika: Výskumný súbor tvorilo 263 neprofesionálnych opatrovateľov, poskytujúcich starostlivosť zomierajúcemu v podmienkach domácej, kombinovanej a paliatívnej/hospicovej starostlivosti v troch krajoch Slovenskej republiky (Prešovský, Ţilinský, Banskobystrický). Psychometrické vlastnosti nástroja pouţitého v našej štúdii boli testované na základe analýzy spoľahlivosti, korelačnej a faktorovej analýzy. Výsledky: Faktorovou analýzou sa v našom výskume podarilo potvrdiť 4-faktorovú štruktúru nástroja CQOLC (záťaţ/narušenie ţivotného štýlu; sociálna opora; pozitívna adaptácia; finančná situácia). Extrahované faktory na základe odpovedí respondentov v našom výskumnom súbore nie sú úplne identické s originálnou verziou škály CQOLC. Najlepšiu konštruktovú validitu a vnútornú konzistenciu škály CQOLC sme potvrdili u respondentov, ktorí poskytujú domácu starostlivosť zomierajúcemu príbuznému. Záver: Psychometrickou analýzou dotazníkov sme zistili, ţe reliabilita a konštruktová validita nami pouţitého nástroja je porovnateľná s originálnou verziou, napriek odchýlkam v lokalizácii niektorých poloţiek. Nástroj CQOLC je pouţiteľný na posúdenie kvality ţivota neprofesionálnych opatrovateľov v kontexte starostlivosti o zomierajúcich v domácom prostredí., Aim: The paper has the design of the cross-sectional study. The aim of the study was to test the psychometric properties of the quality of life (Caregiver Quality of Life Index of Cancer) the caregivers who provide care of the dying patients in terms of forms of care. Methods: The sample of our research consisted of 263 non-professional caregivers, providing home care of the dying, combined care and palliative care in three districts of the Slovak Republic (Prešovský, Ţilinský, Banskobystrický). Psychometric properties of the instrument used in our study were tested on the basis of the analysis of the reliability, of the correlation analysis and factor analysis. Results: Factor analysis of questionnaire CQOLC in our research confirmed the fourfactor structure of the instrument CQOLC (burden/lifestyle disruption; social support; a positive adaptation; the financial concerns). Extracted the factors on the basis of the answers of the respondents in our research are not completely identical with the original version CQOLC. The best construct validity and reliability of the CQOLC we have confirmed with the caregivers, who provide a home care of the dying patients. Conclusion: Psychometric analysis of the questionnaires we found that, the reliability and construct validity is comparable to the original version of CQOLC, despite variations in the location of some items.
