Cíl: Cílem studie bylo zjistit, jak rodinní pečovatelé pečující o blízkou osobu v České republice hodnotí definující charakteristiky ošetřovatelské diagnózy Přetížení pečovatele. Metodika: Na klinickou validizaci ošetřovatelské diagnózy Přetížení pečovatele byl použit „Na pacienta zaměřený Fehringův model klinické diagnostické validity“. Výzkum u rodinných pečovatelů probíhal v měsících srpen a září 2010 v Agentuře domácí péče Victoria na území města Šumperk. Do výzkumného souboru bylo vybráno 50 rodinných pečovatelů, kteří pečovali o své blízké v domácím prostředí déle než 2 roky. Souhlas se zařazením do výzkumu vyjádřilo 23 rodinných pečovatelů, co je 46 %. Výsledky: Rodinní příslušníci za hlavní definující charakteristiky Přetížení pečovatele označili celkem 8 charakteristik: obavy z možné institucionalizace péče příjemce; obavy, jak vyřešit péči o člena rodiny, když nebudu schopen tuto péči vykonávat; obavy o zdraví příjemce péče v budoucnosti; obavy související se schopností pečovatele poskytovat péči v budoucnosti; konflikt v rodině; nejistota ze změny vztahu s příjemcem péče; smutek ze změny vztahu s příjemcem péče; starost o členy rodiny. Za vedlejší bylo označeno 10 definujících charakteristik: obtíže v dokončování požadovaných úkolů, obtíže se sledováním příjemce péče v průběhu nemoci, obtíže ve vykonávání požadovaných úkolů, vysoký krevní tlak, kardiovaskulární onemocnění, somatizace, frustrace, depresivní nálada, porušený spánek, stres. Závěr: Pro skupinu vybraných pečovatelů nebyly všechny definující charakteristiky uvedeny v klasifikačním systému NANDA International významné., Aim: The aim of the study was to find out how the family caregivers caring for their close relatives in the Czech Republic evaluate defining characteristics of the nursing diagnosis Caregiver Role Strain. Methods: Fehring´s patient – focused clinical diagnostic validity model was used to clinical validation of the nursing diagnosis Caregiver Role Strain. The research was ongoing during the months August and September 2010 at Home Care Agency Victoria in region of city Šumperk. Fifty family caregivers, who take care for their family members more than 2 years, were chosen to the research sample. Twenty three caregivers (46 %) agreed with their including to the research. Results: Caregivers classified in total the following eight defining characteristics as major ones: apprehension about possible institutionalization of care receiver, apprehension about care receiver´s care if caregiver unable to provide care, apprehension about the future regarding care receiver´s health, apprehension about the future regarding caregiver´s ability to provide care, family conflict, uncertainty regarding changes relationship with care receiver, grief regarding changed relationship with care receiver, concerns about family members. Caregivers classified ten defining characteristics as minor ones: difficulty completing required tasks, difficulty watching care receiver go through the illness, difficulty performing required tasks, hypertension, cardiovascular disease, somatization, frustration, feeling depressed, disturbed sleep, stress. Cieľ: Obsahová validácia ošetrovateľskej diagnózy Chronická zmätenosť a identifi kácia defi nujúcich charakteristík v podmienkach slovenskej a českej ošetrovateľskej praxe. Metódy: Na validáciu bol použitý retrospektívny Diagnostic Content Validity Model (DCV Model) podľa Fehringa. Za expertov boli považované sestry, ktoré získali minimálne 4 body podľa modifi kovaných Fehringových kritérií. Súbor expertov tvorilo 144 slovenských a 107 českých sestier. Výsledky: Za hlavné defi nujúce charakteristiky (vážené skóre VS viac ako 0,75) považovali zhodne slovenskí aj českí experti: narušená interpretácia podnetov a reakcia na ne, progresívna kognitívna porucha, klinicky podložená organická porucha mozgu, zmeny v časopriestorovej orientácii a dlhotrvajúca kognitívna porucha, ktoré sú totožné s charakteristikou syndrómu demencie. Závery: Experti nepovažovali všetky defi nujúce charakteristiky za rovnako významné. Určitý podiel zohralo zloženie samotného súboru expertov, ktorý naplnil iba spodnú hranicu kritérií na experta (6 bodov), nekompatibilita NANDA-I Taxonómie II so Zoznamom sesterských diagnóz platným v SR a absencia štandardizovanej ošetrovateľskej terminológie v Českej republike., Aim: Content validation of the nursing diagnosis Chronic confusion and identification of the defining characteristics in Slovak and Czech nursing practices. Method: Fehring’s retrospective Diagnostic Content Validity Model (DCV Model) was used. Experts were nurses who obtained minimum four points according to modified Fehring’s criteria. The group of respondents comprised of 144 Slovak and 107 Czech nurses. Results: Slovak and Czech experts alike considered the following characteristics to be defining (weighted score higher than 0.75): altered interpretation and altered response to stimuli, progressive cognitive impairment, clinical evidence of organic impairment, changes in spatiotemporal orientation and long-standing cognitive impairment, which are congruent with the characteristic features of dementia syndrome. Conclusion: Experts did not regard all the defining characteristics to be of the same importance. This was due to the composite of respondents, who only reached bottom level of criteria for an expert (6 points), incompatibility of NANDA-I Taxonomy II with the List of nursing diagnosis used in the Slovak Republic and lastly the absence of standardised nursing terminology in the Czech Republic., Martina Tomagová, Ivana Bóriková, and Literatura 28
