Úvod: Jedním z onemocnění, které je potenciálním rizikovým stavem pro vznik chronické pankreatitidy, je pankreatitida akutní. Materiál: Z celkového počtu 231 osob, které bylo sledovaných s diagnózou chronické pankreatitidy, 56 nemocných bylo iniciálně léčeno s akutní pankreatitidou (24,2 %). V intervalu 12–24 měsíců od prvé ataky akutní pankreatitidy jejich stav postupně progredoval do obrazu pankreatitidy chronické. Osoby zařazené do studie po 1. atace akutní pankreatitidy abstinovaly, během období jejich sledování nebyla recidiva akutní pankreatitidy prokázána. Výsledky: V etiologii akutní pankreatitidy převládal alkohol (55,3 %), biliární etiologie byl prokázána v 35,7 %. Dle revidované Atlantské klasifikace byla u 69,6 % nemocných stanovena pankreatitida těžká, ostatní nemocní splnili kritérium formy intermediární, nemocní s lehkou formou nebyli zařazeni. Závěr: Významnými přítomnými rizikovými faktory bylo kouření a obezita a u 18 %, resp. u 25,8 % nemocných byl zjištěn pankreatogenní diabetes mellitus. Přitom 88,1 % nemocných trpících akutní pankreatitidou byli kuřáci. Největší počet osob s chronickou pankreatitidou po atace pankreatitidy akutní byl v produktivním věku, tj. 25–50 let. Do chronické pankreatitidy nepřechází pouze akutní pankreatitida alkoholická, ale nalezli jsme tento přechod i u pankreatitidy biliární etiologie., Introduction: One of the diseases involving a potential risk of developing chronic pancreatitis is acute pancreatitis. Material: Of the overall number of 231 individuals followed with a diagnosis of chronic pancreatitis, 56 patients were initially treated for acute pancreatitis (24.2 %). Within an interval of 12- 24 months from the first attack of acute pancreatitis, their condition gradually progressed to reached the picture of chronic pancreatitis. The individuals included in the study abstained (from alcohol) following the first attack of acute pancreatitis and no relapse of acute pancreatitis was proven during the period of their monitoring. Results: The etiology of acute pancreatitis identified alcohol as the predominant cause (55.3 %), biliary etiology was proven in 35.7 %. According to the revised Atlanta classification, severe pancreatitis was established in 69.6 % of the patients, the others met the criterion for intermediate form, those with the light form were not included. Conclusion: Significant risk factors present among the patients were smoking, obesity and 18 %, resp. 25.8 % had pancreatogenous diabetes mellitus identified. 88.1 % of the patients with acute pancreatitis were smokers. The majority of individuals with chronic pancreatitis following an attack of acute pancreatitis were of a productive age from 25 to 50 years. Úvod: Cieľom štúdie bolo analyzovať výsledky činnosti transplantačného centra v Košiciach. Metódy: Do transplantačného programu boli zaradení pacienti v kritickom štádiu obličkového zlyhávania. Na transplantáciu obličky bolo indikovaných 457 pacientov. Z metodík bola použitá plastika tepny, našitie tepny samostatne a našitie na spoločnom terčíku. Transplantácia (Tx) obličky vyžaduje predovšetkým techniku cievnej anastomózy - chirurgického spojenia ciev. Výsledky: Transplantácie obličiek sa v Transplantačnom centre Košice realizujú od roku 1988. Plastiku tepny sme vykonali u 22,3 % (102) pacientov. Z toho tepny našité samostatne boli u 5,5 % (25). Na spoločnom terčíku ich bolo 9,4 % (43). V 7,4 % (34) bola plastika realizovaná ako implantácia pólových renálnych artérií do hlavného kmeňa. Záver: Transplantácia obličiek je život zachraňujúca metóda., Introduction: The aim of the study was to analyze own results from Transplant Center Kosice, Slovak Republic. Methods: In total 457 patients in the end-stage renal failure were included to the kidney transplant program. Following techniques were used: arterioplasty, individual renal artery anastomosis and multiple renal arteries anastomosed on a common patch. The kidney transplant (Tx) especially requires surgical technique of a vascular anastomosis. Results: Renal transplantation has been established in Transplant Center Kosice since 1988. The arterioplasty of renal artery was performed in 102 (22.3%) and individual artery anastomosis in 25 (5.5%) patients. Multiple artery anastomosis on the patch was done in 43 (9.4%) and implantation of pole renal arteries to the main trunk performed in 34 (7.4%) cases. Antrochoanal polyps are benign lesions originating from the mucosa of the maxillary sinus. Nasal obstruction and rhinorrhea are their main symptoms. Their endoscopical and radiological appearance makes them relatively easy to diagnose. These polyps are usually presented unilaterally, although bilateral presentation is also possible. We described two cases of atypically giant antrochoanal polyps: in a 15-year-old child and in a 38-year-old man. In both cases, the diagnosis was done by nasal endoscopy and computed tomography (CT) of the paranasal sinuses and supported by histopathological analysis. In the first patient, the excised polyp had the histological characteristic of an angiomatous antrochoanal polyp. Because of their unusual dimension, the combined transoral and endonasal endoscopic approach was performed for complete polyp excision. We discussed the clinical, histopathological and immunohistochemical characteristics of choanal polyps in comparison to inflammatory nasal polyps, and the applicable surgical techniques for treatment of these polyps. and C. Úvod: Česká republika zaujímá v incidenci kolorektálního karcinomu (KRK) čelní místo ve světě. Radikální odstranění nádoru představuje nejúčinnější část v rámci současné multimodální terapie. Autoři prezentují průběžné výsledky roboticky asistované léčby u nemocných s karcinomem rekta (KR). Metoda: Vlastní soubor obsahuje 61 roboticky operovaných pacientů pro KR. U posledních 31 pacientů byla data získávána prospektivně. Analyzovány byly epidemiologické údaje, perioperační ukazatele, komplikace a onkologické parametry. Výsledky: Roboticky asistovanou operační léčbu pro karcinom rekta na našem pracovišti podstoupilo 61 nemocných: 34 mužů a 27 žen průměrného věku 62 let (33–80 let). Neoadjuvantní onkologická léčba byla indikována u 46 % pacientů. Průměrná krevní ztráta činila 187 ml, u tří nemocných byly podány transfuzní přípravky. Šest operací jsme byli nuceni konvertovat, u 16 pacientů jsme zaznamenali komplikace. Insuficience anastomózy se vyskytla u 10 % pacientů, u 4 nemocných si stav vyžádal operační léčbu. Žádný nemocný nezemřel. Rekurenci maligního onemocnění jsme diagnostikovali u 3 (5 %) pacientů. U 27 nemocných byla (od roku 2013) stanovena kvalita mezorektální excize (ME) a metrické stanovení cirkumferentního resekčního okraje – (y)pCRO. Ve dvou případech byla potvrzena pozitivita (y)pCRO a přibližně u čtvrtiny operantů nebyla ME kompletní. Závěr: Chirurgická léčba je v interdisciplinárním terapeutickém procesu u KR klíčová. Naše průběžné výsledky jsou srovnatelné se závěry některých literárně uveřejněných sdělení. Da Vinci systém je bezpečným manipulátorem v léčbě KR a operatérovi přináší nesporné výhody při preparaci ve stísněném pánevním prostoru. V současnosti chybí zhodnocení přínosu robotické operativy pro pacienta v oblasti břišní chirurgie z dlouhodobého hlediska a s dostatečným počtem operantů, resp. vysokým stupněm evidence based medicine (EBM). Vysoká pořizovací cena přístroje, jednotlivých instrumentů s vybavením a nesystémová úhrada představují významnou překážku v širším využití robotického systému v léčbě KR i jiných abdominálních malignit v ČR., Introduction: The incidence of colorectal cancer (CRC) in the Czech Republic is reported to be one of the highest on the global scale. Radical tumor removal has been observed to be the most effective part in the context of current multimodal therapy. The authors present their preliminary results of robotic assisted treatment of rectal carcinoma (RC). Method: The observed group includes 61 patients who underwent robotic assisted treatment for rectal cancer. The data were collected prospectively in the last 31 patients. Analyses were conducted on epidemiological data, perioperative outcomes, complications and oncological results. Results: Robotic assisted treatment of RC was performed in 61 patients: 34 men and 27 women, mean age of 62 years (33–80). Neoadjuvant oncological treatment was indicated in 46% of the patients. Average blood loss was 187 ml, transfusions were administered in three cases. Conversion to open procedure was performed 6 times, and 16 patients had postoperative complications. Anastomotic leak was observed in 10% of the patients, and 4 patients undewent surgical treatment. No patient died. Local recurrence of the cancer was diagnosed in 3 (5%) patients. The quality of mesorectal excision (ME) and the circumferential resection margin [(y)pCRM] have been determined in 27 patients since 2013. Positive (y)pCRM was recorded in two cases and incomplete ME was observed in 25.8% of the patients. Conclusion: Surgical treatment for RC is pivotal in multimodal therapy. Our preliminary results are similar to the conclusions in other published studies. The da Vinci robotic system is a safe manipulator in the treatment of RC and provides indisputable benefits to the surgeon when operating in the narrow pelvic space. However, the benefits of robotic treatment in abdominal surgery are yet to be evaluated in patients (with respect to long-term results, sufficient number of patients or a high EBM level of evidence). The high purchase price of the robotic device, individual instruments with equipment and non-systemic compensation constitute a significant hindrance that prevents wider use of the robotic system in the treatment of RC and other abdominal malignancies in the Czech Republic., and D. INTRODUCTION: Cellulitis remains a very serious disease even today. Mortality, which varied between 10-40%, has been reduced owing to the standard securing of airway patency and use of an appropriate surgical treatment approach. MATERIALS AND METHODS: A total of 195 patients were hospitalised for cellulitis at the University Hospital in Hradec Králové during 2007-2011. The following parameters were evaluated: age, gender, dependence of incidence of the disease on the season of the year, frequency of attacks of the particular areas and their clinical characteristics, aetiology of the inflammation, types of patient complaints, prevalence of current systemic diseases, results of microbiological and selected laboratory analyses, socio-economic status of the patients, and duration of patient stay at the hospital. Statistical analysis was performed by using Pearson's correlation coefficient, the statistical significance level was p < 0.05. RESULTS: The mean age of the patients was 39.8 years. The group of 195 patients included 108 (55%) males and 87 (45%) females. The mean time between the first symptoms of the disease and admission to the Department was 5 days. From among the 195 patients, 116 (59.5%) were working persons, 79 (40.5%) were non-working (children, students, unemployed persons, women on maternity leave, retired people). The odontogenic origin of the disease was verified in 173 (88.7%) patients. In total, 65 (33.3%) patients had no coinciding complicating systemic disease, 22 (11.3%) patients had diabetes mellitus. The most frequent symptom of cellulitis was painful swelling, found in 194 (99.5%) patients, followed by jaw contracture, found in 153 (78.5%) patients. CONCLUSION: The results are largely very similar to those of previous studies performed in other countries, except that we found no correlation between the prevalence of cellulitis and the socio-economic status, nor have we confirmed Klebsiella pneumoniae sp. as the cause of cellulitis in patients with diabetes mellitus. and H. Úvod: Dekubity u paraplegiků představují náročný problém vedoucí k opakovaným hospitalizacím, mnohonásobným operacím a potenciálně devastujícím komplikacím Metody: Přehledový článek. Výsledky: Uvádíme možnosti rekonstrukčních operací ischiadických, sakrálních a trochanterických dekubitů a vlastní zkušenost s lalokovými plastikami takových dekubitů u pacientů operovaných v průběhu tří let. Závěr: Zdůrazňujeme univerzálnost a spolehlivost gluteálního laloku, nezbytnost kvalitní předoperační přípravy a pooperační péče., Introduction: Pressure ulcers in spinal cord injury represent a challenging problem that often leads to recurrent hospitalizations, multiple surgeries, and potentially devastating complications. Denosumab is a human monoclonal antibody representing a novel therapy of osteoporosis. Contrary to always other antiosteoporotic drugs, it is not contraindicated in advanced chronic kidney disease, as its pharmacokinetic does not differ from patients with normal kidney function. However, published case reports in chronic kidney disease (CKD) patients stopped the therapy after single dose because of hypocalcemia. BACKGROUND: Human immunodeficiency virus (HIV) continues to be a serious health issue and one of the world most devastating epidemics. An estimated 1.5 million people died from AIDS-related illnesses in 2013, and an estimated 37 million people with AIDS have died worldwide since the epidemic has begun. HIV infection is known for its oral manifestations which causes discomfort and pain for infected individuals. The objective of this study was to document oral conditions of HIV positive patients and the pattern and frequency of oral and dental lesions. METHODS: All patients with confirmed HIV infection who were treated at the Department of Dentistry, University Hospital in Hradec Králové, were examined. RESULTS: During the study period, 29 HIV positive patients were examined and treated--19 men, 10 women, with mean age of 32.9 years (range 22-58 years). 72.41% patients received ART. In total, all patients underwent 186 visits. The most frequent treatments were associated with teeth and periodontal lesions (71.80%), oral mucosal lesions were diagnosed and treated only in 3.96% cases. CONCLUSION: Since the introduction of ART, the frequency of oral mucosal lesions is minimal in patients with HIV infection. and M. Šembera, V. Radochová, R. Slezák
Diastematomyelie je vzácná vrozená vada patřící mezi spinální dysrafizmy a obvykle je asociována se syndromem míšního ukotvení. Diastematomyelie je tvořena dvěma polovinami rozštěpené míchy s vlastními durálními vaky, které jsou rozděleny osteokartilaginózním septem. Tato vada bývá diagnostikována především v dětském věku. U dospělých je vzácná a dominujícím příznakem je bolest. Autoři prezentují kazuistiku ženy ve věku 44 let s diastematomyelií, která si stěžovala na chronické bolesti zad a progredující slabost dolních končetin. U pacientky jsme provedli laminektomii, deliberaci míchy a plastiku durálního vaku. Po operaci pacientka zůstala bez neurologického deficitu a udávala výraznou úlevu od bolestí., Diastematomyelia is a rare congenital defect, a type of spinal dysraphisms, often associated with the tethered cord syndrome. Diastematomyelia is formed by the two halves of the split spinal cord that each has own dural sac separated by a rigid or fibrotic septum. This defect is mainly diagnosed in children. Pain is the dominant symptom in adult patients. The authors present a case of a woman aged 44 years with diastematomyelia who complained of chronic back pain and progressive weakness in her legs. In this patient, laminectomy was performed, the spinal cord released and the dural sac restored. After the surgery, the patient had no neurological deficits and reported significant pain relief. Úvod: Při neklidu, úzkosti a agitovanosti je někdy nutné přikročit k intramuskulární (i. m.) aplikaci benzodiazepinů. V ČR je často používán diazepam. Cílem práce je zjistit interindividuální variabilitu koncentrací diazepamu v séru po i. m. aplikaci a hodnotit klinický efekt léku. Metodika: Do hodnocení jsme zařadili šest mužů, kterým bylo při ambulantní léčbě aplikováno 10 mg diazepamu i. m. Měření farmakokinetických parametrů a anxiety byla prováděna po 30 min (T1), dále po 4 (T2) a 24 hod (T3). Diagnóza byla stanovena pomocí Mini-International Neuropsychiatric Interview, anxietu jsme kvantifikovali Hamiltonovou škálou úzkosti (HAMA). Výsledky: V čase T1 (po 30 min) byly průměrné (geometrický průměr) hladiny diazepamu v séru 14,6 ng/ml, za 4 hod 48,6 ng/ml a za 24 hod 28,7 ng/ml. Hodnoty v čase T1 se pohybovaly v rozmezí 0,5–148,0 ng/ml. Nenalezli jsme vztah mezi skórem HAMA a koncentracemi diazepamu. U všech pacientů došlo k poklesu úzkosti v čase T1 na normální hodnoty (HAMA < 13). Při dalším sledování už nebyly hodnoty HAMA zvýšené. Závěr: Koncentrace diazepamu byly v průměru u pacientů 30 min po aplikaci výrazně nižší než po 4 i 24 hodinách. Největší variabilitu koncentrací jsme pozorovali za 30 min, kdy byl rozdíl mezi nemocnými 280násobný. Tato zjištění potvrzují zahraniční zkušenosti, podle kterých diazepam v i. m. formě nemá být užíván k ovlivnění akutních stavů. Diskutujeme o rozdílech mezi SPC jednotlivých přípravků a oficiálních doporučujících pokynů, které nutí k nevhodné aplikaci diazepamu i. m. a zároveň neumožňují jiné než off-label užití v guidelines doporučovaného midazolamu., Introduction: There is a need for intramuscular (IM) administration of benzodiazepines for acute restlessness, anxiety and agitation. In the Czech Republic, diazepam is frequently used. The aim of this study was to determine the variability of serum diazepam concentrations after IM administration and evaluate its clinical effect. Methods: We included six men, who were administered 10 mg of diazepam IM in an outpatient setting. Measurements were carried out 30 minutes (T1), 4 (T2) and 24 hours (T3) after diazepam administration. The diagnosis was determined using the Mini-International Neuropsychiatric Interview, anxiety was quantified by the Hamilton Anxiety Rating Scale (HAMA). Results: The mean levels of diazepam in the serum were 14.6 ng/ml at T1 (after 30 minutes), 48.6 ng/ml at 4 hours and 28.7 ng/ml at 24 hours after administration. The values at T1 ranged from 0.5 ng/ml to 148.0 ng/ml. We found no relationship between the scores of HAMA and concentrations of diazepam. Anxiety decreased to normal values (HAMA < 13) at the time T1 in all patients. Conclusions: Concentrations of diazepam were lower 30 minutes after the administration than after 4 and 24 hours, respectively. We observed the largest variability in concentrations 30 minutes after the administration, when the concentrations between patients differed 280-times. This finding confirms international experience that IM diazepam administration should not be used. We discuss the differences between individual SPCs and guidelines that force inappropriate use of diazepam IM and do not permit other than off-label use of midazolam.
