Úvod: V posledních letech prošla více než 20 let sloužící Atlantská klasifikace akutní pankreatitidy (AP) revizí. Přesto se prognostické ukazatele AP neustále hledají. Naše studie se pokouší zhodnotit odůvodnění 3 nově zavedených stupňů tíže revidované klasifikace akutní pankreatitidy a přínos 2 jednoduchých krevních laboratorních testů jako markerů rizika úmrtí a těžkého průběhu AP. Metody: Do studie bylo zahrnuto a sledováno celkem 159 pacientů s AP. Pacienti byli rozděleni do skupin podle Revidované Atlantské klasifikace a jednotlivé skupiny byly hodnoceny podle počtu úmrtí, délky hospitalizace a počtu stavem vyžadovaných intervencí včetně operací pankreatu. Počet leukocytů a distribuční šíře červených krvinek (RDW – red blood cell distribution width) v první den hospitalizace byly hodnoceny z hlediska prognózy AP. Výsledky: Podíl úmrtí, délka hospitalizace a počty operací pankreatu se statisticky významně lišily mezi všemi jednotlivými skupinami tíže podle Revidované Atlantské klasifikace. Počty bílých krvinek a RDW v 1. den hospitalizace byly identifikovány jako významné prediktory těžké AP a RDW také jako významný prediktor úmrtí. Souhrn: Nově zavedené kategorie lehké, středně těžké a těžké AP podle pravidel Revidované Atlantské klasifikace dobře vystihují prognózu, délku hospitalizace a riziko intervencí u nemocných. Počet bílých krvinek a RDW v první den hospitalizace se jeví jako možný adept mezi ukazatele k predikci úmrtí a rozvoje těžké formy AP., Introduction: In the last few years the Atlanta classification of acute pancreatitis (AP) have been revised. However prognostic markers of AP are still being searched for. The aim of this study is to validate the 3 severity cathegories proposed by the revised Atlanta classification. We also tried to reevaluate the association between two laboratory markers (leucocyte count and RDW – red cell distribution width) on admission and prognosis of the patients with AP. Methods: 159 patients were included into the study. The patients were classified according to revised Atlanta criteria and the subgroups evaluated according to mortality, lenght of hospital stay and need for interventions. Leucocyte count and RDW on admission was evaluated in the patients. Results: All the subgroups of patients were associated with significantly relevant differences in mortality, lenght of hospital stay and need for operations on pancreas. Leucocyte count and RDW were identified as significant predictors for severe AP and RDW was also identified as significant predictor for mortality in patients with AP. Conclusion: New categories of severity as defined by Revised Atlanta classification are describing well the mortality, lenght of hospital stay and need for interventions in the patients with AP. Mnohočetný myelom (MM) a solitární plazmocytom (SP) jsou nejčastější primární nádory páteře. Léčba MM a SP páteře je komplexní proces, který zahrnuje léčbu kauzální s cílem potlačit nádorový klon a dále léčbu podpůrnou, jejíž součástí je i chirurgická terapie. Operace přispívají k lepší kvalitě života a nabývají na významu a smysluplnosti zejm. díky zlepšené prognóze onemocnění. Navrhli jsme algoritmus operační léčby MM a SP a ten jsme dále prospektivně ověřovali a jeho vyhodnocení bylo cílem této studie. Pacienti byli operováni v průběhu 10 let, minimální doba sledování byl jeden rok. Soubor tvořilo 32 pacientů (20 mužů a 12 žen) s průměrným věkem 61 let. Operováno bylo celkem 61 obratlů. Důvodem k operaci byly projevy útlaku míchy a progredující neurologický deficit (hodnoceno Frankelovou škálou) nebo na konzervativní léčbu nereagující axiální bolest (hodnoceno VAS; Visual Analogue Scale) páteře. Provedena byla některá z těchto operací: vertebroplastika (VP), laminektomie, transpedikulární fixace, somatektomie a jejich kombinace, nebo okcipito‑cervikální fixace. Pooperační VAS a Frankelova škála byly vyhodnoceny za jeden rok, skiagramy a MR jsme provedli a zhodnotili každý jeden rok. U žádného pacienta nenastala lokální recidiva tumoru ani selhání stabilizace na operovaném úseku v celém sledovaném období. Průměrný předoperační VAS byl 6,8 a jeden rok po operaci se průměrný VAS snížil na 1,1. Všechny operační výkony vedly nejen ke kontrole bolesti, ale také k prevenci nebo zlepšení neurologického postižení po celou dobu sledování. Hodnotili jsme náš postup – algoritmus operací, míru radikality a jejich načasování jako optimální pro MM i SP., Multiple myeloma (MM) and solitary plasmacytoma (SP) are the most frequent primary tumors of the spine. Management of the spinal MM and SP is a complex process involving causal treatment to suppress the tumor clone, as well as supportive therapy, including surgery and radiotherapy. Surgery should be considered because of its favourable effect on disease prognosis. We proposed a surgical treatment algorithm in patients with spinal MM and SP and the aim of this article is to present prospective evaluation of this algorithm. Patients undergoing surgical treatment during the past 10 years were included in this study, with the minimum follow-up of one year. A total of 32 patients (20 males and 12 females) with the mean age of 61 years were included. Surgery of 61 vertebrae was performed. The procedures were indicated for progressing neurological deficit (Frankel score) and for axial spinal pain (VAS classification). We performed the following procedures: vertebroplasty, laminectomy, transpedicular fixation, somatectomy, and their combination, or occipito-cervical fixation. Frankel score and VAS was assessed one year after the surgery and X-ray and spinal MRI was performed every year. No local relapses of the tumor or stabilization failure were detected. The mean preoperative VAS was 6.8 and improved to 1.1 one year after the surgery. During the follow up period, we observed positive effect of surgery on pain control and on prevention or improvement of neurological dysfunction. The authors concluded that all surgical procedures, the extent of resection and timing were adequate in all subjects. Úvod: Alveolární echinokokóza je vzácné a závažné zoonotické parazitární onemocnění. Kazuistika: Autoři demonstrují případ mladého nemocného s postižením jater, bránice a plic touto formou infekce. Diagnóza echinokokové infekce byla stanovena na základě anamnézy, klinické symptomatologie v kombinaci s USG, CT, MRI a sérologickými metodami. Byla provedena radikální bloková resekce 7. jaterního segmentu, dolního plicního laloku vpravo a bránice. Definitivní stanovení diagnózy alveolární echinokokózy bylo provedeno histopatologicky a pomocí PCR metody ze vzorku resekované tkáně. Závěr: Nemocný je 8 měsíců po operaci bez potíž, trvale dispenzarizován s long-life terapií albendazolem., Introduction: Alveolar echinococcosis is a life-threatening zoonotic parasitic disease. Its incidence is rare. In some cases, the correct and timely diagnosis can be difficult. Case report: The authors present the case of a young patient with liver, diaphragm and lung involvement. The suspicion of echinococcus infection was made on the basis of medical history, clinical symptoms, and a combination of ultrasonography, computed tomography, magnetic resonance imaging tests and serological methods. The patient underwent multimodal treatment with albendazole and en-bloc resection of the liver, lung and diaphragm. The definitive diagnosis of alveolar echinococcosis was determined from samples of the resected tissues using histopathology and polymerase chain reaction methods. The patient has been followed regularly and is on life-long treatment with albendazole. Úvod: Výsledky poskytované chirurgické péče v širším přesahu jsou obtížně dosažitelné. Práce testovala systém účtování zdravotní pojišťovně z hlediska využitelnosti dat. Metoda: Analyzováno bylo 6879 operací tříselné kýly v období 2008–2015. Využit byl systém účtování v DRG Vojenské zdravotní pojišťovny České republiky. Výsledky: Operace provedlo 156 pracovišť. V režimu jednodenní chirurgie bylo provedeno 3,8 % (248) výkonů, z toho 77 % (193) bylo operováno laparoskopicky. Za hospitalizace bylo provedeno 96,2 % (6631) výkonů, medián délky hospitalizace byl 4 dny. U hospitalizovaných pacientů byla otevřená plastika bez síťky provedena v 36,54 % (2423), otevřená plastika se síťkou v 19,84 % (1316), laparoskopická plastika v 43,61 % (2892). Reoperace pro recidivu v souboru byla v 1,85 % (127), po otevřené plastice bez síťky v 2,76 % (66), po otevřené plastice se síťkou v 1,66 % (22), po laparoskopické plastice v 1,19 % (37). Predikce recidiv do 5 let byla 2,42 % celkem. Hodnoceny byly i další parametry. Závěr: Studie potvrdila využitelnost systému pojištění k získání relevantních dat o léčbě vybrané diagnózy., Introduction: Information about actual results of provided surgical care is difficult to obtain. We tested the insurance company billing system in terms of data usability. Methods: 6,879 groin hernia surgeries carried out from 2008 to 2015 were analysed using registry data of one health insurance company. Results: Groin hernia surgeries were performed by 156 providers in the Czech Republic. Day surgeries were used in 3.8% cases (248), of which laparoscopic surgeries were used in 77% (193) of patients. The other 96.2% (6,631) surgeries were carried out on inpatients with the median hospital stay of 4 days. Of this number: open procedures without the use of a mesh were done in 36.54% (2,423), open procedures using a mesh in 19.84% (1,316), and laparoscopic procedures in 43.61% (2,892) cases. Reoperations for recurrence after hernia repair were done in 1.85% (127) of all cases, while 2.76% (66) reoperations were done after open procedures without the use of a mesh, 1.66% (22) after open procedures using a mesh, and 1.19% (37) after laparoscopic surgeries. The prediction rate for recurrent hernia incidence within 5 years was 2.42% in all cases. Others parameters were also evaluated. Úvod: Cílem práce je zhodnocení našich zkušeností a výsledků s použitím hybridních rekonstrukcí v terapii multietážového postižení tepenného systému dolních končetin a vyhodnocení vlivu indikace a typu hybridní rekonstrukce na jejich průchodnost. Metoda: Retrospektivní analýza souboru pacientů s prospektivním sběrem dat. Na základě typu hybridní rekonstrukce rozdělení pacientů do 5 skupin. Skupina pacientů „1“ podstoupila transluminální angioplastiku (TA) ? implantaci stentu do pánevního řečiště v kombinaci s plastikou tepen v třísle (35 pacientů; 27,6 %). Skupina pacientů „2“ podstoupila TA ? implantaci stentu do pánevního řečiště v kombinaci s infrainguinálním bypassem (15 pacientů; 11,8 %). Skupina pacientů „3“ podstoupila plastiku tepen v třísle v kombinaci s TA výtokového traktu končetiny: AFS ? AP ? bércové tepny (52 pacientů; 40,9 %). Skupina pacientů „4“ podstoupila provedení infrainguinálního bypassu v kombinaci s TA distálně od místa distální anastomózy rekonstrukce (3 pacienti; 2,4 %). Skupina pacientů „5“ představuje heterogenní skupinu hybridních rekonstrukcí, které nespadaly do některé ze 4 výše uvedených skupin (22 pacientů; 17,3 %). Na základě indikace k výkonu rozdělení souboru do 3 skupin: skupina „ALI“ pacienti s akutní končetinovou ischemií, skupina „CLI“ pacienti s kritickou končetinovou ischemií, skupina „II“ pacienti s klaudikačními obtížemi. Analýza průchodností provedena s použitím Kaplan-Meier life tables. Porovnání průchodnosti mezi skupinami provedeno pomocí log-rank testu. Statistická analýza provedena s použitím softwaru IBM SPSS Statistics 21.0 . Výsledky: Soubor prezentuje 127 pacientů, kteří podstoupili 127 hybridních intervencí. Technický a klinický úspěch hybridní rekonstrukce byl 96,9 % a 98,4 %. 30denní perioperační mortalita byla 3,1 %. Výsledky primární (PP), primární asistované (PAP) a sekundární (SP) průchodnosti ve 2 letech byly nejlepší u pacientů s hybridním výkonem typu „1“ a „3“, tedy kombinací plastiky tepen v třísle a transluminální angioplastiky vtokového nebo výtokového traktu končetiny. Výsledky skupiny „1“ v pořadí PP, PAP a SP ve 2 letech: 80,6 %, 84,3 % a 84,3 %. Výsledky skupiny „3“ ve 2 letech ve stejném pořadí: 81,1 %, 82,8 % a 86,3 %. O poznání horších výsledků bylo dosaženo u pacientů ze skupiny „2“ a „5“. Výsledky PP, PAP a SP ve 2 letech u pacientů ze skupiny „2“ byly: 39,4 %, 59,1 % a 59,1 %. Výsledky průchodnosti pacientů z heterogenní skupiny „5“ byly ve 2 letech: 30,4 %, 49,2 % a 70,7 %. Při zohlednění vlivu indikace na průchodnost rekonstrukce ve 2 letech byly nejlepší výsledky primární asistované a sekundární průchodnosti dosaženy u pacientů ze skupiny „CLI“: 81,6 % a 86,2 %. Jenom primární průchodnost ve 2 letech byla nejlepší u pacientů s klaudikacemi (skupina „II“), a to 71,1 %. Pacienti indikováni k hybridnímu výkonu pro akutní končetinovou ischemií (ALI) dosahovali nejhorších výsledků z uvedených 3 skupin, a to PP, PAP a SP ve 2 letech: 33,7 %, 46,2 % a 45,5 %. Amputation-free survival ve 2 letech měl nejlepší hodnotu pro pacienty ze skupiny „CLI“, a to 89,1 % („II“ 81,9 %; „ALI“ 61,4 %). Overall-survival a amputation-free survival ve 2 letech za celý soubor dosahovaly hodnot 93,2 % a 82,3 %. Závěr: Hybridní revaskularizace je kombinovaný výkon, který umožňuje řešení multietážového postižení tepenného systému dolních končetin v rámci jednoho sezení. Je zatížen nízkým rizikem periprocedurální mortality a morbidity pro pacienta. Lze jím dosáhnout dobrých výsledků průchodnosti tepenného systému a záchrany končetiny bez ohledu na typ chronické ischemie (klaudikace, kritická končetinová ischemie). Výsledky u pacientů s akutní končetinovou ischemií jsou inferiorní. Jako výkon s nejlepšími výsledky nám vychází kombinace plastiky tepen v třísle spojená s transluminální angioplastikou vtokového nebo výtokového traktu končetiny., Introduction: The goal of this study was to evaluate our experience and results of single staged hybrid procedures for revascularization of lower extremities affected by multilevel arterial occlusive disease. We analysed the effect of the indication and type of reconstruction on patency. Method: Patients were retrospectively reviewed. Data collection was conducted prospectively. Patients were divided into 5 groups based on the type of hybrid reconstruction. Group “1” included patients who underwent transluminal angioplasty (TA) ? stenting of iliac arteries with endarterectomy (EA) and patch arterioplasty of the femoral bifurcation (35 patients; 27.6 %). Group “2” included patients who underwent TA ? stenting of iliac arteries with infrainguinal bypass (15 patients; 11.8 %). Group “3” consisted of patients who underwent TA ? stenting of outflow lower extremity arteries: superficial femoral artery ? popliteal artery ? crural arteries in combination with EA and patch arterioplasty of the femoral bifurcation (52 patients; 40.9 %). Group “4” represented patients who underwent infrainguinal bypass surgery in combination with TA distal to the site of open reconstruction (3 patients; 2.4%). Group “5” represented a heterogenous population of patients who underwent a hybrid reconstruction which did not belong to any of the 4 previously mentioned groups (22 patients; 17.3 %). The patients were divided into 3 groups based on the indication criteria of the intervention: Patients with acute limb ischemia (ALI) were put into group “ALI”. Patients with critical limb ischemia (CLI) were put into group “CLI”. Patients with claudications were put into group “II”. Patency analyses were performed using Kaplan-Meier life tables. Differences in patency rates between the different groups of patients were determined using the log-rank test. Statistical analysis was performed using the software IBM SPSS Statistics 21.0. Results: The study included 127 patients who underwent 127 hybrid arterial procedures. Technical and clinical success rates were 96.9% and 98.4%. 30-day perioperative mortality rate was 3.1%. The primary (PP), assisted-primary (APP) and secondary patency (SP) results at 2 years were the best amongst the patients from groups “1” and “3”. These patients underwent patch arterioplasty ? EA of the femoral bifurcation in combination with transluminal angioplasty of inflow or outflow arteries of the lower extremity. The resulting PP, APP and SP rates at 2 years in group “1” were 80.6%, 84.3% and 84.3%. Patency rates in group “3“ in the same order at 2 years were 81.1 %, 82.8% and 86.3%. Significantly lower patency rates were achieved in patients from groups “2“ and “5“. The resulting PP, APP and SP rates in group “2” at 2 years were 39.4%, 59.1% and 59.1%; in group “5“ at 2 years they were 30.4%, 49.2% and 70.7%. Taking into account the effect of the indication on patency rates, we found that patients from group “CLI” had the best APP and SP rates at 2 years: 81.6% and 86.2%. Only the PP rate at 2 years was the best in group “II” (patients with claudications): 71.1%. Patients who underwent their operations because of acute limb ischemia (group “ALI”) achieved the worst results among these 3 groups. Their PP, APP and SP rates at 2 years were 33.7%, 46.2% and 45.5%. Patients from the group „CLI“ had the best amputation-free survival at 2 years: 89.1% amongst the 3 groups subdivided according to the indication for the intervention (“II“ 81.9%; “ALI“ 61.4%). Overall survival and amputation-free survival at 2 years were 93.2% and 82.3%, respectively, for the whole studied population. Conclusion: The hybrid procedure is an intervention which combines endovascular and open reconstruction performed at the same time. Hybrid procedures provide effective therapy for patients with multilevel lower extremity arterial disease. They have low complication rates, periprocedural morbidity and mortality. Using these procedures, we are able to achieve good limb salvage rates and patency rates irrespective of the form of chronic limb ischemia (claudications, critical limb ischemia). Results for patients with acute limb ischemia are inferior. The combination of patch arterioplasty ? endarterectomy of the femoral bifurcation with transluminal angioplasty of inflow or outflow limb arteries is our hybrid procedure of choice with the most favourable results., and E. Úvod: Incidence akutního krvácení do horní části gastrointestinálního traktu (GIT) se udává 85–108/100 000 obyvatel za rok, přičemž nonvariceální krvácení odpovídá za 80–90 %. Antiagregační a antikoagulační terapie patří mezi významné rizikové faktory pro krvácení do horní části GIT. Cíl práce: Zjistit výskyt krvácení do horní části GIT u běžné populace pacientů v podmínkách krajské nemocnice, srovnání podílu antiagregancií, antikoagulancií a dalších rizikových léků na dané problematice. Soubor a metodika: Retrospektivní analýza pacientů, kteří v roce 2013 (leden–červen) podstoupili za hospitalizace gastroskopii pro akutní krvácení do horní části GIT nebo pro anémii (Hb < 100 g/l) s verifikovaným zdrojem ztrát v horní části GIT. Výsledky: Soubor tvořilo 111 pacientů s průměrným věkem 69 ? 15 let, mužů bylo 60 %. Nonvariceální krvácení tvořilo 90 %. Žádný z pacientů s variceálním krvácením (10 % pacientů) neužíval antiagregační či antikoagulační terapii. Pacientů s nonvariceálním krvácením do horní části GIT bylo 100 s průměrným věkem 70 ? 15 let, 61 % mužů. Příznaky akutního krvácení (hemateméza, meléna) mělo 73 % těchto pacientů. Nejčastější příčinou krvácení byla vředová choroba gastroduodena, a to v 54 % případů. Ze všech pacientů s nonvariceálním krvácením do GIT užívalo antiagregancia 32 %, antikoagulancia 19 %, dalších 10 % pacientů užívalo nesteroidní antirevmatika, selektivní inhibitory zpětného vychytávání serotoninu nebo kortikoidy. 30denní mortalita pacientů s nonvariceálním krvácením byla 11 % a roční mortalita 23 %. Nebyl zjištěn významný rozdíl v mortalitě, nutnosti aplikace krevních transfuzí nebo chirurgické intervence u pacientů na antitrombotické terapii a bez ní. 25 % (8 pacientů) s terapií kyselinou acetylsalicylovou (ASA) nebylo k této léčbě indikováno dle guidelines. Závěr: V souboru pacientů ošetřených gastroskopií pro symptomatické nonvariceální krvácení do GIT nebo anémií (Hb < 100 g/l) je signifikantně větší podíl léčených antiagregancii než antikoagulancii. To může reflektovat větší zastoupení této léčby v populaci, ale také její vyšší riziko pro vznik GIT krvácení. V souvislosti s tím vyznívá varovně fakt, že stále existuje nezanedbatelné procento pacientů, kteří užívají kyselinu acetylsalicylovou i přesto, že nesplňují indikaci k této terapii., Background: The incidence of acute upper gastrointestinal bleeding is about 85–108/100,000 inhabitants per year, nonvariceal bleeding accounts for 80–90 %. Antiplatelet and anticoagulation treatment are the significant risk factors for upper gastrointestinal bleeding. Objectives: To evaluate the occurrence of upper gastrointestinal bleeding in the general community of patients in a county hospital. And to compare the role played by antiplatelet and anticoagulation drugs and other risk medication. Design and methods: Retrospective analysis of patients over 18 years of age who underwent endoscopy for acute upper gastrointestinal bleeding or anaemia (haemoglobin < 100 g/l) with proved source of blood losses in upper gastrointestinal tract during a hospital stay in 2013 (from January to June). Results: We included 111 patients of average age 69 ? 15 years, men 60 %. Nonvariceal bleeding accounted for 90 % of the cases. None of the patients with variceal bleeding (10 % of patients) took antiplatelet or anticoagulation therapy. There were 100 patients with nonvariceal bleeding of average age 70 ? 15, 61 % men. With the symptoms of acute bleeding (hematemesis, melena) presented in 73 % of patients. The most frequent cause of bleeding was gastric and duodenal ulcer (54 %). 32 % of patients with nonvariceal bleeding had antiplatelets, 19 % anticoagulants and 10 % used nonsteroidal anti-inflammatory drugs, selective serotonin reuptake inhibitors or corticosteroids. 30-days mortality of patients with nonvariceal bleeding was 11 %, annual mortality was 23 %. There was no significant difference in mortality, blood transfusion requirements or surgical intervention between the patients with antithrombotic agents and without them. 25 % of patients (8 patients) using acetylsalicylic acid did not fulfil the indication for this treatment. Conclusion: Among the patients examined by endoscopy for symptomatic nonvariceal bleeding and/or anaemia (haemoglobin < 100 g/l) significantly higher portions of patients are taking antiplatelet rather than anticoagulation therapy. This may be caused by greater use of these drugs in the population, but on the other hand it may reflect an association with greater risk of gastrointestinal bleeding. INTRODUCTION: The issue of resistance to antiplatelet therapy has raised many questions in the area of neurovascular diseases. The first objective of this work was to determine the prevalence of aspirin resistance in neurovascular patients with clinical non-responsiveness to aspirin treatment and a high-risk of atherothrombotic complications using two interpretable and independent methods (aggregation and PFA 100). The second objective was to find the correlation between both assays and to evaluate the results in groups at risk for various cerebrovascular diseases. MATERIAL AND METHODS: Laboratory tests of aspirin resistance were performed in 79 patients with clinical non-responsiveness to aspirin treatment suffering from neurovascular diseases. Patients were divided into the two groups: expected low risk for aspirin resistance due to the first manifestation of a neurovascular disease (n = 34) and expected high risk due to the second clinical manifestation of a neurovascular disease (n = 45). RESULTS: The prevalence of aspirin resistance in both groups combined as determined by the PFA-100 and CPG techniques were 50.6% and 17.7%, respectively. No correlation was found between the two techniques. CONCLUSIONS: No significant prevalence of aspirin resistance was demonstrated by either method despite the heterogeneous pathophysiological mechanisms. Cíl: Převod Testu paměti pro záměry (Memory for Intentions (Screening) Test; MIST, 2010) do české verze zahrnoval standardizaci pomůcek, převod administrace a skórování, validační studii na české populaci, aby MIST bylo možné použít na české populaci. Úvod: Konstrukt prospektivní paměti (PP), tj. schopnosti si zapamatovat a realizovat záměry, je nezbytný pro uchování soběstačnosti, vykonávání aktivit denního života. V posledních letech hraje čím dál významnější roli pro diagnostiku poruch paměti. Standardizovaná a validovaná měřítka PP však v české diagnostice chybí. Metodika: Český převod MIST jsme administrovali 30 zdravým osobám. Výsledky: Celkový skór PP v testu MIST koreloval na střední úrovni s měřítky z neuropsychologické baterie zejména s retencí v paměti, mentální flexibilitou, odolností k interferenci a premorbidní inteligenční úrovní (všechna rho = 0,37–0,42, všechna p < 0,05). Reliabilita testu MIST ve smyslu vnitřní konzistence byla u základních osmi subškál nedostatečná (α = 0,50) i ve smyslu reliability obou polovin testu (split‑half reliabilita = 0,56), avšak reliabilita šesti subškál tříděných dle typu (oddálení, vodítka a modality odpovědi) byla vysoká (α = 0,88; split‑half = 0,95). Závěr: Výsledky validační studie naznačují, že český převod MIST je srovnatelný s verzí originální. Studie zpřístupňuje standardní vyšetření PP a umožňuje měření PP na klinických populacích. Test na prospektivní paměť (MIST) a jeho administrace</a>, Aim: The goal of the present study was to validate the Czech version of the Memory for Intentions (Screening) Test (MIST, 2010). We included standardized testing material, translation of administration and scoring, and assessment of normative data for the MIST in the Czech population. Introduction: Prospective memory (PM), i.e., the ability to remember and implement intentions after a delay, is essential as a subsystem of episodic memory for the maintenance of independence and execution of activities of daily living. PM assessment thus plays an important role in the diagnosis of episodic memory disorders. However, there are currently no standardized and validated PM tools in Czech language. Methods: The Czech version of MIST was administered to 30 healthy persons. Results: The MIST Summary score correlated at a medium level with a range of neuropsychological measures including memory retention, mental flexibility, and resistance to interference (all rho = 0.37–0.42; all p < 0.05). The reliability of MIST in terms of internal consistency was insufficient when analyzing the eight individual MIST trials (α = 0.50), as was split‑half reliability (split‑half reliability = 0.56). In contrast, there was a high degree of reliability between six subscales classified by type (delay, cue and mode of response; α = 0.88, split‑half = 0.95). Conclusion: The reliability and validity of the Czech version of MIST is comparable to the original English version. Psoriasis patients are often susceptible to cardiovascular diseases (CVD), including atherosclerosis. Traditional markers (biochemical and inflammatory) and diagnostic tools could detect occlusive but not subclinical atherosclerosis. Carotid intima-media thickness (CIMT), has recently been recognised as a non invasive diagnostic tool for identification of premature atherosclerosis. Therefore we evaluated 80 psoriasis patients and 80 age sex matched healthy controls for serum leptin levels and apolipoprotein B/apolipoprotein A-I ratio (apoB/apoA-I ratio) in relation with CIMT of carotid artery. Carotid intima-media thickness and carotid plaques were simultaneously measured by carotid sonography. Serum concentration of leptin and apolipoprotein were measured using enzyme-linked immuno sorbent assay (ELISA) and nephelometry respectively. Raised CIMT correlated to age of onset of the disease, serum leptin and apoB/apoA-I ratio in psoriasis patients. Taking into account, values that were above the 75 percentile of the three markers (leptin, apoB/apoA-I ratio and CIMT) the odds ratio was 4.26 (2.06-8.80 CI). Leptin and apoB/apoA-I ratio showed significant cumulative association with CIMT. Results of predictive analysis supports measurement of CIMT along with estimation of serum leptin and apoB/apoA-I ratio for prediction of premature atherosclerosis in psoriasis patients. and K. Brachial Plexus is formed by the union of the anterior rami of cervical 5, 6, 7, 8 and thoracic 1 nerves. These nerves unite and divide to form the key nerves innervating the upper limb. Variations in the course of these nerves are clinically important to anesthetists, neurologists and orthopedicians. We report bilateral variations in the arterial and neural structures in the upper limb of a 65 year old cadaver. The muscles of the arm on one side were innervated by the median nerve with absence ofmusculocutaneous. While on the other side the musculocutaneous nerve contributed to the formation of the median nerve. There was a presence of high bifurcation of brachial artery on both sides. Knowledge of such variations in the innervations of muscles and the arterial supply of the limbs are important to remember before performing any reconstructive procedures or interventions on the limb. and V.
