Takotsubo kardiomyopatie je náhle vzniklé onemocnění charakterizované reverzibilní poruchou kinetiky levé srdeční komory. V typickém případě se rozvíjí následkem silného stresového podnětu a prezentuje se symptomy, EKG a laboratorním nálezem, které připomínají akutní koronární syndrom. Od toho se liší nepřítomností onemocnění koronárních tepen a ve většině případů nekomplikovaným průběhem s následnou velmi dobrou dlouhodobou prognózou. V kazuistice popisujeme případ 63leté ženy, u které se počátek onemocnění projevil fibrilací komor. Díky okamžité kardiopulmonální resuscitaci a následné odborné péči ve specializovaném kardiocentru, která mimo jiné zahrnovala implantaci implantabilního kardioverteru-defibrilátoru v rámci sekundární prevence náhlé srdeční smrti, se pacientka zotavila bez jakýchkoliv následků. Takotsubo kardiomyopatie je důležitou jednotkou v diferenciální diagnóze akutního koronárního syndromu. I přes dobrou prognózu, kterou se vyznačuje ve většině případů, ji nelze podcenit a zaslouží si náležitou pozornost a léčbu, která umožní předcházet případným komplikacím., Takotsubo cardiomyopathy (TTC) is an acute syndrome characterized by rapid onset of transient systolic dysfunction of the left ventricle. Symptoms, ECG and laboratory findings resemble acute coronary syndrome, from which TTC differs by the absence of coronary artery disease. In typical cases, TTC is triggered by exposure to unexpected stress and the clinical course and prognosis are very good. We present a case of a 63-year-old woman, in whom the onset of the disease was complicated by ventricular fibrillation. The patient recovered without sequelae thanks to immediate cardiopulmonary resuscitation and following care in a specialized cardiocenter, which involved also implantation of implantable cardioverter defibrillator to prevent sudden cardiac death. Takotsubo cardiomyopathy is an important entity in the differential diagnosis of acute coronary syndrome. Tuberkulózní postižení střev je v rozvinutých zemích vzácné onemocnění, které postihuje zejména imigranty a imunokompromitované pacienty. Klinický obraz může imitovat nitrobřišní infekce, nádorová onemocnění či Crohnovu chorobu. Diferenciální diagnostika Crohnovy choroby a břišní tuberkulózy může být pro klinika a patologa svízelná, v obou případech se jedná o chronický granulomatózní zánět s podobnou symptomatologií. Prezentujeme případ 53letého pacienta s perforační peritonitidou při tuberkulóze střeva, u kterého byla původně mylně diagnostikována Crohnova choroba. Průběh choroby, rozsah a povaha nálezu byly v každé fázi léčby příčinou kritických komplikací. S touto zkušeností předáváme i doporučení: břišní tuberkulózu zvažovat v diferenciálně diagnostické rozvaze každého chronického střevního onemocnění. Clinical presentation may mimic many other abdominal diseases such as intra-abdominal infections, tumors, and Crohn´s disease. Differential diagnosis of Crohn´s disease and intestinal tuberculosis poses a dilemma to clinicians and pathologists as both are chronic granulomatous disorders with similar clinical features. We report the case of a 53 year-old man presenting with perforated intestinal tuberculosis in which the initial diagnostic work-up suggested Crohn´s disease. The severity of the findings resulted in critical complications during the course of treatment. This case points out the need for awareness of intestinal tuberculosis in the differential diagnosis of chronic intestinal disease. Angiofibromas are rare vascular tumors which originate predominantly in the nasopharynx and occur typically in male adolescents. Extranasopharyngeal sites such as nasal cavity and paranasal sinuses are less frequent. This review article was undertaken to evaluate the incidence, clinical features and management of extranasopharyngeal angiofibromas originating exclusivelly from nasal cavity structures. Our focus of interest was to evaluate the significance of immunohistochemical analysis in diagnosis of such extremely rare neoplasms. In the PubMed and Google Search, we found only 39 cases of nasal angifibroma, 27 males and 12 females from 1980 to 2012. The most prevalent site of origin was nasal septum, followed by inferior and middle turbinate. The commonest symptoms were nasal obstruction and epistaxis. Nasal angiofibromas are clinically distinct from nasopharyneal angiofibromas and can therefore be misdiagnosed. The differential diagnosis includes other vascular lesions, such as lobular capillary hemangioma and sinonasal-type hemangiopericytoma. Although immunohistochemistry is not necessary for differentiation between angiofibroma and capillary hemangioma, that diagnostic procedure may be helpful in distinction from sinonasal hemangiopericytoma. As an ilustration for immunohistochemical analysis, we presented a case of an elderly woman with tumor arising from the middle turbinate, diagnosed as angiofibroma. Pneumatizace okcipitální kosti a krčních obratlů je vzácný nález a při jeho náhodném zobrazení radiologickými metodami může vyvolat rozpaky. Nejčastěji je tento stav spojován s dysfunkcí Eustachovy trubice a často se opakujícím Valsalvovým manévrem. V této práci popisujeme náhodný nález kraniocervikální pneumatizace u pacienta s úrazem na kole a následnými polykacími problémy a potížemi s mluvením. Pro vznik kraniocervikální pneumatizace u tohoto pacienta se jeví klíčové opakované rýmy a záněty paranazálních dutin a rovněž záliba v potápění. Neurologické postižení je poměrně vzácná komplikace hepatitidy E. Uvedená kazuistika popisuje 52letého imunokompetentního muže s aseptickou meningitidou a mnohočetným postižením periferních nervů na horních končetinách typu mononeuropatia multiplex, u kterého byla sérologickým vyšetřením prokázána akutní virová hepatitida E. Navíc byl virus hepatitidy E zjištěn polymerázovou řetězovou reakcí v séru i likvoru (genotyp 3f). Pacient byl léčen pouze symptomaticky, po půl roce přetrvával mírný neurologický deficit. Infekci virem hepatitidy E je vhodné zvažovat u pacientů se současným neurologickým a jaterním postižením., Neurological complications of acute hepatitis E are relatively rare. The case report describes mononeuropatia multiplex with aseptic meningitis in a 52‑year‑old immunocompetent man. Viral hepatitis E was confirmed serologically and by polymerase chain reaction from the blood and cerebrospinal fluid (genotype 3f). Symptomatic treatment was given. Residual neurological deficit persisted six months after the onset of the disease. Diagnostic of hepatitis E should be considered in patients with concurrent neurological symptoms and liver cytolysis. Oncocytic Schneiderian papilloma (OSP) is one of the three morphologically distinct tumors that arise from Schneiderian membrane (the others include exophytic papilloma and inverted papilloma). OSP almost always occurs unilaterally in the paranasal sinuses, usually in the maxillary sinus, ethmoid cells or sphenoid sinus. We report a case of a 64-year-old woman with OSP arising from the left frontal sinus. In the report herein, we describe an OSP originating in the region of frontal sinus, which, to the best of our knowledge, represents the first documented example in English literature of OSP developing in this anatomical site. and D. Von Meyenburgovy komplexy jsou benigní jaterní léze složené obvykle z dilatovaných žlučových kanálků obklopených fibrózním stromatem. Jejich nález je většinou incidentální v rámci nečekaného intraoperačního nálezu. Předoperačním vyšetřením nebývají tyto drobné hamartomy většinou zachyceny. Suverénní diagnostickou metodou je peroperační kryohistologické vyšetření. Von Meyenburgovy komplexy nejsou ve většině případů patologicky či funkčně klinicky významné, ale u pacientů s ložiskovým jaterním postižením mohou představovat důležitou možnost, kterou je třeba mít na zřeteli v rámci diferenciální diagnostiky metastatických jaterních patologií., Von Meyenburg complexes are benign liver lesions usually consisting of dilated bile ducts surrounded by fibrous stroma. Their discovery is usually incidental and unsuspected during the early phase of the operative procedure. The sovereign diagnostic method is intraoperative frozen section examination. The complexes are not important as regards their clinical or functional significance.
