A hybrid approach to elephant trunk technique for treatment of thoracic aortic aneurysms combines a conventional surgical and endovascular therapy. Compared to surgery alone, there is a presumption that mortality and morbidity is reduced. We present a case report of a 42-year-old man with a giant aneurysm of the entire thoracic aorta, significant aortic and tricuspid regurgitation and ventricular septum defect. Úvod: U 60 % nemocných s kolorektálním karcinomem jsou diagnostikovány jaterní metastázy (JMKRK), ať už v době stanovení diagnózy, nebo v průběhu léčby. Chirurgická léčba je jedinou potenciálně kurabilní metodou s pětiletým přežíváním dosahujícím cca 50 %. Jaterní resekci však podstoupí méně než 20 % pacientů s JMKRK. Společnou snahou onkologů a chirurgů je navýšit počet resekovaných pacientů. Jedním z postupů, jak navýšit resekabilitu JMKRK, je „liver first approach“ (LFA). Autoři prezentují vlastní předběžné výsledky této metody. Metody: Do multicentrického grantu IGA MZ NT 13660 – Hodnocení kvality multimodální péče u nemocných s jaterními metastázami kolorektálního karcinomu – bylo od září 2012 do ledna 2015 zařazeno v ÚVN 102 pacientů. LFA jsme použili u 12 pacientů (12 %). Po příznivé léčebné odpovědi neoadjuvantní systémové léčby jsme pacienty indikovali k resekci jater. U 11 pacientů se jednalo o mnohočetné (>4 meta) bilobární postižení. V jednom případě se jednalo o objemnou solitární metastázu pravého jaterního laloku. Primární tumor byl u 9 nemocných v rektu, u 3 v oblasti rektosigmatu, 3 pacienti měli derivační kolostomii, ostatní byli bez poruchy pasáže. Výsledky: U 11 pacientů jsme provedli R0 resekci jaterních metastáz, 3x jsme indikovali dvoufázovou hepatektomii s portální embolizací (PVE), 1x jsme pro progresi po PVE ustoupili od 2. fáze). Velkou resekci jsme provedli 7x, neanatomickou resekci včetně RFA 11x (6x v kombinaci s velkou resekcí). Perioperační mortalita byla nulová, morbidita 33 % (Dindo-Clavien >2). 10 pacientů absolvovalo adjuvantní chemoterapii, 7 z nich včetně radioterapie malé pánve pro lokálně pokročilý tumor rekta. U 7 pacientů (58 %) jsme doplnili resekci primárního tumoru. V současné době zemřeli na progresi onemocnění 2 pacienti (17 %), progresi onemocnění jsme zaznamenali u 6 pacientů (50 %). Závěr: LFA považujeme za vhodný především u pacientů s generalizovaným tumorem rekta, kdy lze po resekci jater adjuvantní systémovou léčbu kombinovat s radioterapií malé pánve. Otázkou zůstává načasování operace primárního tumoru, kdy je nutné vyloučit recidivu jaterních metastáz, která se objevuje u více než 50 % pacientů. Profit LFA musí být prokázán randomizovanými studiemi., Introduction: Liver metastases are diagnosed in 60% of patients with colorectal cancer, both at the time of diagnosis or later in the course of their management. Surgical treatment is the sole potentially curable method with 5–year overall survival of approximately 50%. However, only less than 20% of patients underwent liver resection. A joint effort of medical oncologists and surgeons is to increase the numbers of resected patients. The “liver first approach” (LFA) is one of approaches aimed at increasing resecability. The authors present their preliminary results using this method. Methods: 102 patients were included in the multicentre study supported by the grant IGA NT 13660 − Evaluation of quality of multimodal treatment for patients with colorectal cancer liver metastases – conducted at the Central Military Hospital between September 2012 and January 2015. We used LFA in 12 patients (12%). Patients were indicated for liver resection based on good response to neoadjuvant systemic therapy. Multiple bilobar liver involvement (>4 metastases) was present in 11 cases and one large solitary metastasis in the right liver lobe in one case. The primary tumor was located in the rectum in 9 patients, in the rectosigmoid in 3 patients; 3 patients had a colostomy. Others showed no signs of bowel obstruction. Results: We have performed R0 resections in 11 cases, and two-stage hepatectomy with portal vein embolisation was indicated 3 times (in one case we did not finish the second stage due to quick progression after PVE). We performed major resections 7 times, along with sever extraanatomic resections, incl. 11 RFA (6 times in combination with major resections). Perioperative mortality was 0%, morbidity 33% (Dindo-Clavien >2). Ten patients underwent adjuvant chemotherapy, in 7 cases including radiotherapy of the small pelvis due to a local advanced primary tumor. Resection of the primary tumor was done in 7 patients (58%). Two patients died recently because of disease progression (17%); progression was observed in 6 patients (50%). Conclusion: We deem the LFA suitable especially for patients with metastatic rectal tumors where adjuvant systemic therapy can be combined with radiotherapy. The timing of the resection of the primary tumor still remains a question: it is necessary to rule out potential recurrence of liver metastases, which affected more than 50% of the patients. The aim of this study was to compare the isolation systems OptraDam® Plus and OptiDam™ with the conventional rubber dam in terms of objective and subjective parameters. The isolation systems were applied during the dental treatment of the patients. The time of preparation, placement, presence and removal were measured and the quality of isolation was evaluated. The median time of rubber dam placement was 76 s (Q1=62 s; Q3=111.25 s). The application time of OptraDam® Plus was significantly longer compared to the other systems (P ® plus. Mnohočetný myelom (MM) a solitární plazmocytom (SP) jsou nejčastější primární nádory páteře. Léčba MM a SP páteře je komplexní proces, který zahrnuje léčbu kauzální s cílem potlačit nádorový klon a dále léčbu podpůrnou, jejíž součástí je i chirurgická terapie. Operace přispívají k lepší kvalitě života a nabývají na významu a smysluplnosti zejm. díky zlepšené prognóze onemocnění. Navrhli jsme algoritmus operační léčby MM a SP a ten jsme dále prospektivně ověřovali a jeho vyhodnocení bylo cílem této studie. Pacienti byli operováni v průběhu 10 let, minimální doba sledování byl jeden rok. Soubor tvořilo 32 pacientů (20 mužů a 12 žen) s průměrným věkem 61 let. Operováno bylo celkem 61 obratlů. Důvodem k operaci byly projevy útlaku míchy a progredující neurologický deficit (hodnoceno Frankelovou škálou) nebo na konzervativní léčbu nereagující axiální bolest (hodnoceno VAS; Visual Analogue Scale) páteře. Provedena byla některá z těchto operací: vertebroplastika (VP), laminektomie, transpedikulární fixace, somatektomie a jejich kombinace, nebo okcipito‑cervikální fixace. Pooperační VAS a Frankelova škála byly vyhodnoceny za jeden rok, skiagramy a MR jsme provedli a zhodnotili každý jeden rok. U žádného pacienta nenastala lokální recidiva tumoru ani selhání stabilizace na operovaném úseku v celém sledovaném období. Průměrný předoperační VAS byl 6,8 a jeden rok po operaci se průměrný VAS snížil na 1,1. Všechny operační výkony vedly nejen ke kontrole bolesti, ale také k prevenci nebo zlepšení neurologického postižení po celou dobu sledování. Hodnotili jsme náš postup – algoritmus operací, míru radikality a jejich načasování jako optimální pro MM i SP., Multiple myeloma (MM) and solitary plasmacytoma (SP) are the most frequent primary tumors of the spine. Management of the spinal MM and SP is a complex process involving causal treatment to suppress the tumor clone, as well as supportive therapy, including surgery and radiotherapy. Surgery should be considered because of its favourable effect on disease prognosis. We proposed a surgical treatment algorithm in patients with spinal MM and SP and the aim of this article is to present prospective evaluation of this algorithm. Patients undergoing surgical treatment during the past 10 years were included in this study, with the minimum follow-up of one year. A total of 32 patients (20 males and 12 females) with the mean age of 61 years were included. Surgery of 61 vertebrae was performed. The procedures were indicated for progressing neurological deficit (Frankel score) and for axial spinal pain (VAS classification). We performed the following procedures: vertebroplasty, laminectomy, transpedicular fixation, somatectomy, and their combination, or occipito-cervical fixation. Frankel score and VAS was assessed one year after the surgery and X-ray and spinal MRI was performed every year. No local relapses of the tumor or stabilization failure were detected. The mean preoperative VAS was 6.8 and improved to 1.1 one year after the surgery. During the follow up period, we observed positive effect of surgery on pain control and on prevention or improvement of neurological dysfunction. Psoriasis patients are often susceptible to cardiovascular diseases (CVD), including atherosclerosis. Traditional markers (biochemical and inflammatory) and diagnostic tools could detect occlusive but not subclinical atherosclerosis. Carotid intima-media thickness (CIMT), has recently been recognised as a non invasive diagnostic tool for identification of premature atherosclerosis. Therefore we evaluated 80 psoriasis patients and 80 age sex matched healthy controls for serum leptin levels and apolipoprotein B/apolipoprotein A-I ratio (apoB/apoA-I ratio) in relation with CIMT of carotid artery. Carotid intima-media thickness and carotid plaques were simultaneously measured by carotid sonography. Serum concentration of leptin and apolipoprotein were measured using enzyme-linked immuno sorbent assay (ELISA) and nephelometry respectively. Raised CIMT correlated to age of onset of the disease, serum leptin and apoB/apoA-I ratio in psoriasis patients. Taking into account, values that were above the 75 percentile of the three markers (leptin, apoB/apoA-I ratio and CIMT) the odds ratio was 4.26 (2.06-8.80 CI). Leptin and apoB/apoA-I ratio showed significant cumulative association with CIMT. Results of predictive analysis supports measurement of CIMT along with estimation of serum leptin and apoB/apoA-I ratio for prediction of premature atherosclerosis in psoriasis patients. and K. Úvod: Existuje mnoho studií věnujících se problematice biliárních komplikací po jaterních resekcích. Přes zlepšení operační techniky a perioperační péče má incidence této komplikace spíše vzrůstající charakter. Byly analyzovány četné prediktivní faktory. Na jejich vliv na vznik biliárního leaku doposud nepanuje shoda. Za cíl analýzy jsme si stanovili zhodnotit incidenci biliárního leaku, jeho vliv na mortalitu a na dobu hospitalizace na našem pracovišti. Současně jsme provedli rozbor známých prediktivních faktorů. Metoda: Autoři retrospektivně analyzují soubor 146 nemocných, u kterých byla na Chirurgické klinice 2. LF Univerzity Karlovy a ÚVN, Praha v období 2010–2013 provedena jaterní resekce. K vyhodnocení biliárního leaku jsme užili současnou uznávanou klasifikaci ISGLS (International Study Group of Liver Surgery). Závažnost komplikace byla stanovena dle Clavien-Dindo. Statistickou významnost prediktivních faktorů jsme určili pomocí Fisherova exaktního testu a Studentova t-testu. Výsledky: Incidence biliárního leaku byla 21 %. Dle ISGLS bylo 6,5 % pacientů zařazeno do skupiny typu A, v 61,2 % se jednalo o leak B a v 32,3 % o leak typu C. Stupeň závažnosti dle Clavien-Dindo I-II, IIIa, IIIb, IV a V byl v poměru 19,3 %, 42 %, 9,7 %, 9,7 %, resp. 19,3 %. Jako statisticky významné jsme stanovili tyto faktory: operaci pro malignitu (p<0,001), velkou jaterní resekci (p=0,001), dobu operace (p<0,001), vyšší peroperační krevní ztrátu (p=0,02), konstrukci HJA (p=0,005), portální venózní embolizaci/two-stage chirurgii (p=0,009) a ASA skóre (p=0,02). Biliární leak významně prodloužil dobu hospitalizace (p<0,001). V souboru pacientů s biliárním leakem byla 23krát vyšší perioperační mortalita (p<0,001) než v souboru bez leaku. Závěr: Biliární leak je jednou z nejzávažnějších komplikací jaterních resekcí. Většina rizikových faktorů je těžko ovlivnitelných a na jejich vliv nepanuje jednoznačná shoda. Incidenci by mohly redukovat diskutované peroperační leak testy. V budoucnu bude třeba dále pracovat na zlepšení perioperačního managementu a techniky k prevenci vzniku této závažné komplikace. V léčbě se uplatňuje multidisciplinární přístup., Introduction: Many previous reports have focused on bile leakage after liver resection. Despite the improvements in surgical techniques and perioperative care the incidence of this complication rather keeps increasing. A number of predictive factors have been analyzed. There is still no consensus regarding their influence on the formation of bile leakage. The objective of our analysis was to evaluate the incidence of bile leakage, its impact on mortality and duration of hospitalization at our department. At the same time, we conducted an analysis of known predictive factors. Method: The authors present a retrospective review of the set of 146 patients who underwent liver resection at the Department of Surgery of the 2nd Faculty of Medicine of the Charles University and Central Military Hospital Prague, performed between 2010−2013. We used the current ISGLS (International Study Group of Liver Surgery) classification to evaluate the bile leakage. The severity of this complication was determined according to the Clavien-Dindo classification system. Statistical significance of the predictive factors was determined using Fisher‘s exact test and Student‘s t-test. Results: The incidence of bile leakage was 21%. According to ISGLS classification the A, B, and C rates were 6.5%, 61.2%, and 32.3%, respectively. The severity of bile leakage according to the Clavien-Dindo classification system – I-II, IIIa, IIIb, IV and V rates were 19.3%, 42%, 9.7%, 9.7%, and 19.3%, respectively. We determined the following predictive factors as statistically significant: surgery for malignancy (p<0.001), major hepatic resection (p=0.001), operative time (p<0.001), high intraoperative blood loss (p=0.02), construction of HJA (p=0.005), portal venous embolization/two-stage surgery (p=0.009) and ASA score (p=0.02). Bile leakage significantly prolonged hospitalization time (p<0.001). In the group of patients with bile leakage the perioperative mortality was 23 times higher (p<0.001) than in the group with no leakage. Conclusion: Bile leakage is one of the most serious complications of liver surgery. Most of the risk factors are not easily controllable and there is no clear consensus on their influence. Intraoperative leak tests could probably reduce the incidence of bile leakage. In the future, further studies will be required to improve the perioperative management and techniques to prevent such serious complications. Cíl: Zjistit prevalenci celiakie u dospělých pacientů s diabetes mellitus 1. typu (DM1T). Vliv nově zahájené léčby celiakie na kompenzaci diabetu a body mass index (BMI) pacientů. Chování protilátek proti tkáňové transglutamináze (atTG) po roce od zahájení léčby. Metodika: Retrospektivní hodnocení cíleného screeningu na celiakii v období 1. 1. 2007–1. 7. 2011 u 465 dospělých pacientů s DM1T v Diabetologickém centru I. interní kliniky LF UK a FN Plzeň (80 % všech pacientů s DM1T). Při pozitivitě atTG-A (popř. atTG-G) byla indikována enterobiopsie. U pacientů s nově indikovanou bezlepkovou dietou byl porovnán HbA1c a BMI v průběhu roku před stanovením diagnózy celiakie vs období 1 roku po 6 měsících od zahájení léčby (3–4 kontroly). Po roce od zahájení léčby byla kontrolována hladina atTG. Ke statistice byl použit párový t-test. Výsledky: Prevalence všech forem celiakie v souboru byla 10,5 %. 1,1 % pacientů (5 ze 465) mělo diagnostikovanou celiakii v dětství. Nově byla pozitivita atTG zachycena u 9,5 % (44 ze 465) pacientů – indikováni k enterobiopsii. Z toho 3 pacienti s atTG > 300 kIU/l odmítli enterobiopsii, u těchto pacientů je celiakie vysoce pravděpodobná a pro malkomplianci nešlo hodnotit vliv bezlepkové diety na BMI a HbA1c. 22 pacientů mělo potenciální formu celiakie (negativní histologie). Pozitivní nález z enterobiopsie byl zjištěn u 19 pacientů (4,1 %). Z podsouboru 22 pacientů (nově indikována bezlepková dieta), bylo nutné vyřadit další 3 pacienty (vývoj HbA1c a BMI byl ovlivněn prvozáchytem DM1T). Charakteristika podsouboru: 9 žen a 7 mužů, průměrný věk 38 ? 12 let, trvání diabetu 21 ? 13 let, celiakie diagnostikována 20,7 ? 13 let od záchytu DM1T. Během sledovaného období nedošlo ke statisticky významné změně bazální hodnoty HbA1c 67 ? 11,4 vs 69 ? 13,9 mmol/mol po sledování a významně se zvýšil BMI z 25,4 ? 4,2 na 25,9 ? 4,3 (p < 0,01). Po roce přetrvávala pozitivita atTG u 47 % (9 z 19) pacientů. Závěr: V souboru dospělých pacientů s DM1T byla zjištěna celková prevalence celiakie 10,5 %. Po zahájení léčby nedošlo k významné změně HbA1c, došlo k významnému vzestupu BMI. Pozitivita atTG přetrvávala po roce u 47 % pacientů., Objective: To assess the prevalence of celiac disease in adult patients with type 1 diabetes mellitus (T1DM). Influence the new started treatment of celiac disease on glycemic control and body mass index (BMI) of the patients. Prevail the anti-transglutaminase antibody (atTG) positivity one year after commencement of the therapy. Methods: A retrospective assessment of celiac disease targeted screening in 465 adult T1DM patients at Diabetes Center, 1st Medical Department, University Hospital in Pilsen (80 % of all T1DM patients) from 1. 1. 2007 until 1. 7. 2011. Enterobiopsy was indicated in case of atTG-A (or atTG-G) positivity. In patients with newly started gluten-free diet, HbA1c and BMI within a year after diagnosis of celiac disease were compared to a year period six months after treatment commencement (3–4 visits), atTG was evaluated one year after treatment beginning. Paired T-test was used for statistical evaluation. Results: The prevalence of all forms of celiac disease in the studied group was 10.5 %. Celiac disease diagnosed in childhood was found in 1.1 % patients (5/465). Positivity of atTG was newly observed in 9.5 % (44/465) patients. Three patients with atTG > 300 kIU/l refused the enterobiopsy examination. Celiac disease is highly plausible. The influence of gluten-free diet on BMI and HbA1c could not be evaluated due to the lack of compliance. 22 patients had a potential form of celiac disease (negative histology). Positive enterobiopsy was found in 19 patients (4.1 %). Another 3 patients had to be excluded from the subgroup of 22 patients (newly indicated gluten-free diet) as the HbA1c values and BMI were affected by the primary diagnosis of T1DM. Subgroup characteristics: 9 women and 7 men, mean age 38 ? 12 years, diabetes duration 21 ? 13 years, celiac disease diagnosed 20.7 ? 13 years since first diagnosis of T1DM. No statistically significant change in HbA1c (67 ? 11.4 vs 69 ? 13.9 mmol/mol) was observed in the studied period, however and a significant change of BMI from 25.4 ? 4.2 to 25.9 ? 4.3 (p < 0.01) was found. The atTG positivity prevailed in 47 % (9/19) of patients after one year. Conclusion: A total prevalence of the celiac disease in the group of adult T1DM patients was 10.5 %. No significant change in HbA1c occurred following treatment, a significant change of BMI was observed. Úvod: Tumory thalamu jsou relativně vzácné léze rostoucí ve vysoce funkčně důležité části mozku. Jsou častější v pediatrické populaci. Jejich chirurgické odstranění je náročné s rizikem značné morbidity. Relativně benigní charakter mnoha těchto lézí vybízí k pokusu o jejich radikální odstranění. Chirurgický přístup musí být velice pečlivě plánován, obvykle s použitím moderních diagnostických metod, jako je DTI (Diffusion Tensor Imaging). Lokalizace a projekce tumoru v thalamu hraje při výběru přístupu důležitou roli. Materiál a metodika: Od roku 2005 do roku 2014 bylo na Neurochirurgické klinice FN Brno operováno celkem 13 pacientů s tumorem thalamu, průměrný věk 20,6 roku (1–64 let). Jednalo se o 10 pacientů mužského a tři ženského pohlaví. Další 15letý pacient s pilocytárním astrocytomem je observován. Transkortikální přístup byl použit 7×, transkalózní přístup 4×, transsylvijský přístup 2× a supracerebellární infratentoriální 1×. Výsledky: Totální resekce (GTR) byla provedena v šesti případech, subtotální resekce ve čtyřech případech a parciální resekce ve třech případech. Jednalo se o osm pilocytárních astrocytomů, jeden subependymální obrovskobuněčný astrocytom, jeden difúzní astrocytom GII a tři glioblastomy. Dva starší pacienti s glioblastomem zemřeli šest a devět měsíců po operaci. Ostatní pacienti přežívají se střední délkou sledování 5 let. Neurologický stav po operaci byl dobrý kromě jednoho staršího pacienta s glioblastomem, u kterého se rozvinula těžká hemiparéza a fatická porucha. Závěr: Pokud je zvolen správný chirurgický přístup, thalamické tumory mohou být celkem bezpečně radikálně odstraněny. Volba přístupu je dána projekcí tumoru. Menší tumory, které nedosahují povrchu thalamu, mohou být observovány nebo bioptovány, pokud máme podezření na maligní povahu léze. Následná onkologická léčba je vyhrazena pro vysokostupňové gliomy., Introduction: Thalamic tumours are relatively rare lesions growing in a highly functional part of the brain. They are more frequent in paediatric population. Their surgery is challenging and associated with a risk of significant morbidity. Relatively benign nature of many of these tumours suggests that radical resection should be attempted. The approach has to be very carefully planned, sometimes with help of modern diagnostic methods such as DTI. Location and projection of the tumour in the thalamus plays an important role in selecting an approach. Material and methods: During 2005–2014 we operated on 13 patients with thalamic tumours, mean age 20.6 years (1–64 years). These were 10 males and three females. Another 15-year-old male patient is being observed. Transcortical approach was used 7×, transcallosal 4×, transsylvian 2× and supracerebellar infratentorial 1×. Results: Gross total resection was achieved in six cases, subtotal in four and partial in three. There were eight pilocytic astrocytomas, one subependymal giant cell astrocytoma, one diffuse astrocytoma G II and three glioblastomas. Two older patients with glioblastomas died six and nine month after operation. All other patiens are still alive with the mean follow-up 5 years. Neurological status after the operation was good except for one older patient with glioblastoma who developed severe hemiparesis and aphasia. Conclusion: Thalamic tumours might be safely radically resected if correct approach is used. The choice of approach is based on the projection of the tumour. Smaller tumours that are not reach thalamic surface might be followed up or biopsied if there is a likelihood of their malignant nature. Oncological treatment should be reserved to malignant tumours. Úvod: V roce 2016 to bude 60 let od doby, kdy Nissen poprvé publikoval svoji operační metodu 360° fundoplikace kolem distálního jícnu a její výsledný efekt. V současnosti je Nissenova fundoplikace pravděpodobně nejčastěji používanou metodou k chirurgické léčbě gastroezofageálního refluxu (GER) a hiátové hernie (HH). Incidence HH je v západní populaci mezi 25–50 %. Současná prevalence GER je v Evropě kolem 4–10 %, v Severní Americe 5–7 %, ale v některých studiích je tato prevalence udávána dokonce kolem 10–20 %. Metody: Na Chirurgické klinice 2. LF UK a ÚVN bylo v období 2010 až 2015 provedeno 157 laparoskopických operací pro hiátovou hernii nebo refluxní nemoc jícnu (GERD). Většina nemocných byla indikována pro symptomatickou HH s/bez GERD, jen minimum nemocných bylo indikováno pro GERD bez HH. V souboru bylo 73 mužů a 84 žen s průměrným věkem 53,9 roku v rozmezí 20–88 let. Z tohoto počtu mělo 11 nemocných syndrom hrudního žaludku. U 109 nemocných (69,4 %) byla provedena 360° fundoplikace dle Nissena (NFP) a u 48 nemocných (30,6 %) byla provedena 270° fundoplikace dle Toupeta (TFP). Po operaci byli nemocní ambulantně kontrolováni v intervalu 3 měsíce, 6 měsíců a 12 měsíců a dále po roce. Prospektivně byly sledovány příznaky pyrózy, dysfagie, možnosti či nemožnosti odříhnutí, meteorismu nebo výskyt recidivy, získaná data byla zkoumána retrospektivně. Pro vyhodnocení získaných dat byla použita metoda deskriptivní statistiky a explorační jednosměrné analýzy. Výsledky: V období 2010 až 2015 bylo provedeno 157 laparoskopických operací pro hiátovou hernii nebo GERD. V souboru se nevyskytlo úmrtí ve vztahu k provedené operaci. Tři měsíce po operaci došlo jak u pacientů po NFP, tak u pacientů po TFP k výraznému poklesu až vymizení symptomů refluxu, příznaků pyrózy a epigastrické bolesti. Pooperační dysfagické obtíže vymizely ve skupině po NFP do 4–7 týdnů, u nemocných po TFP do 3–5 týdnů. V intervalu 2–5 let po operaci došlo celkem u 36 (22,9 %) nemocných k recidivě obtíží. U 22 nemocných (14 po NFP, 8 po TFP), u kterých byla prokázána endoskopicky nebo RTG vyšetřením recidiva refluxu bez recidivy kýly, byla zahájena léčba inhibitory protonové pumpy s velmi dobrým efektem. U 14 nemocných s anatomickou recidivou hiátové hernie s/bez refluxu (8 po NFP, 6 po TFP) byla provedena laparoskopicky rehiátoplastika a refundoplikace dle Nissena. Závěr: Laparoskopická chirurgická antirefluxní léčba GERD a HH je bezpečná metoda s velmi nízkou perioperační morbiditou a s minimální mortalitou. NFP nebo TFP ve stejné míře dlouhodobě obnovují mechanickou bariéru proti regurgitaci žaludečního obsahu do jícnu se signifikantním efektem na vymizení příznaků GERD a HH a s výrazným zlepšením kvality života nemocných. U obézních nemocných je před operací nutná redukce hmotnosti. U morbidně obézních nemocných s HH indikovaných k bariatricko-metabolické operaci je doporučováno provést hiátoplastiku současně s bariatrickým výkonem., Introduction: In 2016 60 years will have elapsed from the first publication by Nissen about his surgical method of 360° fundoplication around distal esophagus and its final effect. Nissen’s fundoplication is currently the most common surgical method for the gastroesophageal reflux (GER) and hiatal hernia (HH) probably. Incidence of HH ranges between 25–50% in the western population. Current prevalence of GER ranges between 4–10% in Europe and 5–7% in North America, but some studies report even 10–20%. Methods: From 2010 to 2015, 157 laparoscopic procedures for HH or gastroesophageal reflux disease (GERD) were done at the Department of Surgery, the Second Faculty of Medicine, Charles University and Central Military Hospital in Prague. Most of the patients were operated because of symptomatic HH with/without GERD. Only very few patients were operated only for GERD without HH. The group of the patients included 73 men and 84 women with the mean age 53.9 years in the range 20–88 years. 11 patients of the group were operated because of the upside-down stomach syndrome. 109 (69.4%) patients underwent Nissen’s 360° fundoplication (NFP) and 48 (30.6%) patients underwent Toupet’s 270° fundoplication (TFP). The patients were followed after 3 month, 6 month, 12 month and then once a year after the surgery. Symptoms such as heartburn, dysphagia, belching, bloating or recurrence of HH or GERD were collected prospectively and analyzed retrospectively. Methods of descriptive statistics and explorative unidirectional analysis were used. Results: From 2010 to 2015, 157 laparoscopic procedures were done for HH or GERD. No death occurred in the group of the patients in connection with the surgery. Similar rates of considerable improvement or resolution of the symptoms such as the reflux, heartburn, and epigastric pain were observed after three months from the surgery among patients after NFP or TFP. Postoperative dysphagia resolved in 4–7 weeks in the group after NFP, and in 3–5 weeks in the group after TFP. Disease recurrence was observed in 36 (22.9%) patients in 2 to 5 years after the surgery. Recurrence of only the gastroesophageal reflux without HH was observed in 22 patients (14 after NFP; 8 after TFP), shown by endoscopy or x-ray assessment, with a satisfactory response to PPI medication. Laparoscopic rehiatoplasty and refundoplication according to Nissen were performed in 14 patients with HH recurrence with/without the reflux (8 after NFP; 6 after TFP). Conclusion: Laparoscopic antireflux surgery of GERD and HH is safe and is associated with very low perioperative morbidity and minimal morbidity. Both NFP and TFP have a similar effect on restoration of the mechanical gastroesophageal barrier against gastric reflux, with significant effect on resolution of GERD and HH symptoms and with a marked improvement of the patient’s quality of life. As regards morbid obese patients, weight reduction is needed before the surgery. Hiatoplasty done at the same time with the bariatric procedure is recommended in morbid obese patients with HH indicated for the bariatric-metabolic surgery., and M. Kasalický, E. Koblihová
Úvod: Česká republika zaujímá v incidenci kolorektálního karcinomu (KRK) čelní místo ve světě. Radikální odstranění nádoru představuje nejúčinnější část v rámci současné multimodální terapie. Autoři prezentují průběžné výsledky roboticky asistované léčby u nemocných s karcinomem rekta (KR). Metoda: Vlastní soubor obsahuje 61 roboticky operovaných pacientů pro KR. U posledních 31 pacientů byla data získávána prospektivně. Analyzovány byly epidemiologické údaje, perioperační ukazatele, komplikace a onkologické parametry. Výsledky: Roboticky asistovanou operační léčbu pro karcinom rekta na našem pracovišti podstoupilo 61 nemocných: 34 mužů a 27 žen průměrného věku 62 let (33–80 let). Neoadjuvantní onkologická léčba byla indikována u 46 % pacientů. Průměrná krevní ztráta činila 187 ml, u tří nemocných byly podány transfuzní přípravky. Šest operací jsme byli nuceni konvertovat, u 16 pacientů jsme zaznamenali komplikace. Insuficience anastomózy se vyskytla u 10 % pacientů, u 4 nemocných si stav vyžádal operační léčbu. Žádný nemocný nezemřel. Rekurenci maligního onemocnění jsme diagnostikovali u 3 (5 %) pacientů. U 27 nemocných byla (od roku 2013) stanovena kvalita mezorektální excize (ME) a metrické stanovení cirkumferentního resekčního okraje – (y)pCRO. Ve dvou případech byla potvrzena pozitivita (y)pCRO a přibližně u čtvrtiny operantů nebyla ME kompletní. Závěr: Chirurgická léčba je v interdisciplinárním terapeutickém procesu u KR klíčová. Naše průběžné výsledky jsou srovnatelné se závěry některých literárně uveřejněných sdělení. Da Vinci systém je bezpečným manipulátorem v léčbě KR a operatérovi přináší nesporné výhody při preparaci ve stísněném pánevním prostoru. V současnosti chybí zhodnocení přínosu robotické operativy pro pacienta v oblasti břišní chirurgie z dlouhodobého hlediska a s dostatečným počtem operantů, resp. vysokým stupněm evidence based medicine (EBM). Vysoká pořizovací cena přístroje, jednotlivých instrumentů s vybavením a nesystémová úhrada představují významnou překážku v širším využití robotického systému v léčbě KR i jiných abdominálních malignit v ČR., Introduction: The incidence of colorectal cancer (CRC) in the Czech Republic is reported to be one of the highest on the global scale. Radical tumor removal has been observed to be the most effective part in the context of current multimodal therapy. The authors present their preliminary results of robotic assisted treatment of rectal carcinoma (RC). Method: The observed group includes 61 patients who underwent robotic assisted treatment for rectal cancer. The data were collected prospectively in the last 31 patients. Analyses were conducted on epidemiological data, perioperative outcomes, complications and oncological results. Results: Robotic assisted treatment of RC was performed in 61 patients: 34 men and 27 women, mean age of 62 years (33–80). Neoadjuvant oncological treatment was indicated in 46% of the patients. Average blood loss was 187 ml, transfusions were administered in three cases. Conversion to open procedure was performed 6 times, and 16 patients had postoperative complications. Anastomotic leak was observed in 10% of the patients, and 4 patients undewent surgical treatment. No patient died. Local recurrence of the cancer was diagnosed in 3 (5%) patients. The quality of mesorectal excision (ME) and the circumferential resection margin [(y)pCRM] have been determined in 27 patients since 2013. Positive (y)pCRM was recorded in two cases and incomplete ME was observed in 25.8% of the patients. Conclusion: Surgical treatment for RC is pivotal in multimodal therapy. Our preliminary results are similar to the conclusions in other published studies. The da Vinci robotic system is a safe manipulator in the treatment of RC and provides indisputable benefits to the surgeon when operating in the narrow pelvic space. However, the benefits of robotic treatment in abdominal surgery are yet to be evaluated in patients (with respect to long-term results, sufficient number of patients or a high EBM level of evidence). The high purchase price of the robotic device, individual instruments with equipment and non-systemic compensation constitute a significant hindrance that prevents wider use of the robotic system in the treatment of RC and other abdominal malignancies in the Czech Republic., and D.
