Úvod: Cílem tohoto článku je prezentovat naše zkušenosti s transanální endoskopickou mikrochirurgií (TEM) jako jedním ze základních operačních přístupů k selektovaným pacientům s nádory konečníku. Metody: Retrospektivně jsme vyhodnocovali soubor 80 pacientů, u kterých byla pro nádor konečníku provedena operační léčba pomocí TEM. U těchto pacientů jsme sledovali a statisticky vyhodnocovali konkrétní předoperační a pooperační ukazatele, které bychom chtěli prezentovat: předoperační bioptické vyšetření a pooperační výsledek histologického vyšetření, velikost nádoru a jeho uložení v konečníku, hloubku resekce a hodnocení základních demografických ukazatelů. Výsledky: Z našich zkušeností u zmíněných pacientů vyplývá, že není významný rozdíl mezi věkem operovaných mužů a žen ani při rozlišení na benigní a maligní nádory. Nejčastěji operovanými pacienty byli muži, průměrně ve věku 64 let, kteří byli primárně vyšetřeni pro krvácení z konečníku. Nejčastějším nálezem byl low-grade adenom, který byl potvrzen i v definitivním resekátu. Celkem u 17 pacientů byl nalezen v definitivním histologickém vyšetření maligní tumor, který nebyl zachycen předoperačně. Závěr: V rukách zkušeného chirurga představuje TEM reálnou možnost provedení radikální resekce i maligního, lokálně nepokročilého nádoru konečníku, pokud je v dosahu operačního rektoskopu. TEM je metodou bezpečnou, bez negativního vlivu na funkce análních svěračů. Vzhledem k možnosti falešně negativního předoperačního histologického vyšetření preferujeme radikální odstranění nádoru s celou šířkou stěny konečníku. I když optimální lokalizace pro použití operačního rektoskopu je na zadní straně konečníku, téměř u poloviny našich pacientů se tyto nádory vyskytovaly na stěně přední. Vzhledem k technické proveditelnosti sutury stěny rekta je vhodné, aby báze tumoru nepřesahovala polovinu obvodu konečníku. Technika TEM je vhodná zejména pro nízce uložené, nestenotizující, lokálně nepokročilé nádory konečníku. Naše dobré výsledky jsou dány především přísnou selekcí pacientů, technickým zvládnutím operačního postupu a letitými zkušenostmi., Introduction: The purpose of this paper is to present our experience with transanal endoscopic microsurgery (TEM) as an operative approach in a selected group of patients with rectal tumours. Methods: We present a retrospective study of 80 patients operated for rectal tumour using TEM. Specific preoperative and postoperative indicators were gathered and statistically evaluated. We assessed histological examination before the surgery and definitive histology of the resected tumour, resection depth and basic demographic data of our patients. Results: Our experience suggests that there is no significant difference between the age of patients according to sex, even if malignant and benign tumours are assessed separately. The majority of our operated patients were male patients, mean age of 64 years who were primarily referred to our department because of bleeding low-grade adenomas of the rectum. In 17 patients a malignant tumour was diagnosed based on final histopathologic examination. None of them had signs of malignancy before the surgery. Conclusion: In the hands of an experienced surgeon, TEM is a feasible option of radical removal of rectal, locally not advanced malignancies if within reach of TEM rectoscope. TEM is a safe procedure that does not result in any alteration of rectal sphincter functions. Possible false negative preoperative benign diagnosis leads us to approach radical removal of the tumour with full wall thickness resection. Although the posterior aspect of the rectum is an optimal location for the rectoscope, anterior tumours were found in almost one half of our patients. Considering technical feasibility of the rectal wall suture, the base of the tumour should not extend beyond one half of the rectal circumference. Low non-stenotising noninvasive tumours of the rectum are suitable for TEM. Úvod: Tato práce referuje první zkušenosti s laparoskopicky asistovanou transanální totální excizí mezorekta u pacientů s karcinomem středního a dolního rekta. Metody: V období od října 2013 do května 2014 bylo na dvou pracovištích (CHK FN Brno, CHO KOC Nový Jičín) operováno 17 pacientů technikou transanální totální mezorektální excize. V souboru bylo 11 mužů a 6 žen ve věku 49–81 let. Vzdálenost dolního okraje tumoru od análního okraje byla 30–80 mm, medián 60 mm. U všech pacientů byla konstruována ručně šitá kolo-anální anastomóza. Výsledek: U všech operovaných byl dosažen negativní distální resekční okraj. Cirkumferentní resekční okraj byl pozitivní ve dvou případech. Kvalita totální mezorektální excize byla hodnocena v 8 případech jako kompletní a v 9 případech jako téměř kompletní. Dehiscence kolo-anální anastomózy byla zaznamenána ve dvou případech (12 %). Operační čas se pohyboval v rozsahu 212–375 minut (medián 280 minut). Medián pooperační hospitalizace byl 9 dnů v rozsahu 6 až 30 dnů, v případě řešení anastomotické a rané komplikace. Závěr: Na základě prvních získaných zkušeností hodnotíme výkon jako realizovatelný a bezpečný v podmínkách našich chirurgických pracovišť s uspokojivými patologickými výsledky. Přesné indikace zatím nelze jednoznačně doporučit. Předpoklad je, že k transanální totální mezorektální excizi by měli být indikováni pacienti s nízce uloženým karcinomem rekta, event. s úzkou pánví, objemným mezorektem a objemnou prostatou., Introduction: First experiences with laparoscopic low anterior resection with transanal mesorectal excision in patients with low and mid rectal cancer are referred. Methods: During a 9 month period (between October 2013 and May 2014), 17 laparoscopically assisted rectal resections with transanal total mesorectal excision, hand-sewn colo-anal anastomosis and protective ileostomy were performed at two institutions (CHK FN Brno, CHO KOC Nový Jičín). There were 11 males and 6 females, with a mean age 68 years (range 49–81 years) in our cohort. The tumor was located 30–80mm from the anal verge (median 60 mm). Results: A negative distal resection margin was achieved in all patients. The circumferential resection margin was positive in two cases. The TME quality was described as complete in 8 cases and nearly complete in 9 cases. Mean operative time was 280 min (range 212–375 min) with no intra-operative complications. Anastomotic leakage was observed in two patients (12%). Median length of postoperative hospital stay was 9 days with a range of 6–30 days, in case of anastomotic and wound complications. Conclusion: According to our first experience we evaluated this method as feasible and safe with satisfactory pathological outcomes. We cannot recommend exact indications yet. Transarteriální chemoembolizace (TACE) má v současnosti důležitou roli v managementu vybraných nádorů jater. Technikou provedení TÁCE zásadně nevybočuje od jiných embolizačních výkonů, nicméně kromě obecných se zde můžeme setkat s některými specifickými technickými problémy a komplikacemi. Mezi technické problémy patří technické obtíže při selektivní katetri-zaci (např. při přítomnosti odstupové stenózy či anatomických variant větvení truncus coeliacus nebo vinutosti a ostrých odstupů cílové tepny), vznik disekcí a spasmů, přítomnost extrahepatálního zásobení lézí či přítomnost arterioportálních nebo arteriovenózních zkratů. K nejčastějším specifickým komplikacím řadíme nežádoucí únik chemoembolizačního materiálu do necílových tepen zásobujících okolní orgány (žlučník, gastrodudoenum), vznik jaterních abscesů nebo rozvoj jaterního selhání. Znalost možných technických problémů a komplikací TACE a možností jejich předcházení může podstatně zlepšit prognózu pacientů indikovaných k tomuto výkonu., Transcathetral arterial chemoembolization of the liver (TACE) plays important role in management of selected liver tumours nowdays. The technique is similar to other embolization procedures, nevertheless besides general it is also associated with specific technical problems and complications. Technical problems are represented by technical difficulties of catheterization (stenosis, anatomical variants, etc.), occurence of dissections or spasms, presence of extra hepatic collateral supply and occurence of arterioportal or arterivenous shunts. Specific complications include nontarget embolization of arteries supplying surrounding organs (gallbladder, gastroduodenum), development of liver abscess or liver failure. The high prevalence of obesity and related metabolic complications has inspired research on adipose tissues. Three kinds of adipose tissues are identified in mammals: brown adipose tissue (BAT), beige or brite adipose tissue and white adipose tissue (WAT). Beige adipocytes share some characteristics with brown adipocytes such as the expression of UCP1. Beige adipocytes can be activated by environmental stimuli or pharmacological treatment, and this change is accompanied by an increase in energy consumption. This process is called white browning, and it facilitates the maintenance of a lean and healthy phenotype. Thus, promoting beige adipocyte development in WAT shows promise as a new strategy in treating obesity and related metabolic consequences. High salt concentration is a major abiotic stress limiting plant growth and productivity in many areas of the world. Elaeagnus angustifolia L. adapts to adverse environments and is widely planted in the western region of China as a windbreaker and for landscape and soil stabilization. High salt concentrations inhibited photosynthesis of E. angustifolia, but the mechanism is not known. In this paper, RNA-sequencing was used to investigate effects of salt stress on the photosynthetic characteristics of the species. In total, 584 genes were identified and involved in photosynthetic pathways. The downregulation of genes that encode key enzymes involved in photosynthesis and genes correlated to important structures in photosystem and light-harvesting complexes might be the main reason, particularly, the downregulation of the gene that encodes magnesium chelatase. This would decrease the activity of enzymes involved in chlorophyll synthesis and the downregulation of the key gene that encodes Rubisco, and thereby decreases enzyme activity and the protein content of Rubisco., J. In order to obtain basic information on the transport properties of differentiating embryonic nephrons, we examined the 7-day-old chick mesonephros by measuring the transtubular epithelial potential difference (TPD) and by histochemical detection of Na,K-ATPase activity. TPD as an indicator of the electrogenic transport was measured in individual segments of superficial nephrons in vivo. Their electric polarity was always lumen-negative. TPD was reduced by addition of 10 mM KCN applied to the mesonephric nephrons from the outside. In the proximal tubules, TPD was significantly lower (mean±SD: -1.0±0.5 mV) than in the distal and collecting tubules (-2.2±1.0 mV, pŁ0.05). Activity of the sodium pump was evaluated histochemically by detection of ouabain-sensitive potassium-dependent p-nitrophenyl phosphatase in cryostat sections of the mesonephros. The enzyme activity was demonstrated only in distal tubules and in the collecting ducts, but not in the proximal tubules. These findings have revealed significant differences between embryonic nephron segments: the distal tubule, in contrast to the proximal one, is supplied by the sodium pump and is able to generate higher TPD. Studie shrnuje současné poznatky o teorii transformačního vedení. Zabývá se tématy, která jsou důležitá pro další výzkum transformačního vedení a pro využití teorie transformačního vedení v psychologické praxi v České republice a na Slovensku. Zařazuje transformační teorii do kontextu dalších teorií vůdcovství, představuje pozitivní důsledky transformačního vedení na úrovni organizací, týmů i jednotlivců, věnuje se také kritice teorie a jejím slabým stránkám. Všímá si metody měření transformačního vedení a její validity a možností rozvoje vedoucích v organizacích. Závěr je věnován aktuálním trendům ve výzkumu transformačního vedení., The overview study sums up the contemporary knowledge concerning the transformational leadership theory. It deals with topics important for further study of transformational leadership and for utilization the theory of transformation leadership in psychological praxis in the Czech Republic and Slovakia. Článek se zabývá některými důsledky otevřené vědy z perspektivy komunikace vědy a filosofie komunikace. Kromě čistě komunikačních a filosofických témat se text věnuje i otázkám tykajícím se procesu popularizace vědy prostřednictvím sociálních médií (zejména Twitteru a blogů). Článek se sestává ze tří oddílů: první navrhuje definici komunikace vědy a sociálních médií; druhý zkoumá proměnu komunikace vědy v éře internetu a zabývá se vlivem sociálních médií na komunikaci vědy; třetí a závěrečný oddíl přináší několik případových studií a filosofických postřehů. Nejdůležitějším, zde dosaženým závěrem je tvrzení, že sociální media vědu a vědeckou komunikaci proměnila. Twitter a blogy jakožto nové nástroje vědecké komunikace mohou být užitečné a smysluplné pro vědu i společnost. Sociální média mohou být navíc použita k usnadnění širšího zapojení občanů do diskusí o vědě., The aim of the present article is to discuss several consequences of the Open Science from a perspective of science communication and philosophy of communication. Apart from the purely communicative and philosophical issues, the paper deals with the questions that concern the science popularization process through social media (especially Twitter and blogs). The article consists of three sections: the first one suggests a definition of science communication and social media, the second examines the transformation of science in the Age of the Internet and considers the influence of social media on science communication, the third and final one presents some case studies and philosophical observations. The most important conclusion to be reached here is that the social media have changed science and science communication. Twitter and blogs as novelty tools of science communication can be useful and meaningful for both science and society.
