We investigate the invariant rings of two classes of finite groups $G\leq{\rm GL}(n,F_q)$ which are generated by a number of generalized transvections with an invariant subspace $H$ over a finite field $F_q$ in the modular case. We name these groups generalized transvection groups. One class is concerned with a given invariant subspace which involves roots of unity. Constructing quotient groups and tensors, we deduce the invariant rings and study their Cohen-Macaulay and Gorenstein properties. The other is concerned with different invariant subspaces which have the same dimension. We assessed the intensive care unit (ICU) patients for Invasive aspergillosis (IA) with culture and non-culture based diagnostic methods from Iran. Thirty-six ICU patients with underlying predisposing conditions for IA were enrolled in the study. Sixty eight Bronchoalveolar lavage (BAL) samples were collected by bronchoscope twice a weekly. BAL samples were analyzed by microscopic examination, fungal culture and galactomannan (GM) detection. The Platelia Aspergillus GM EIA was used to quantify GM indices. Samples with a BAL GM index > or = 1 were considered as positive for GM. Patients were classified as having probable or possible IA. Out of 36 suspected patients to IA, 36.1% of cases showed IA which were categorized as: 4 cases of possible IA and 9 of probable IA. 76.2% of BAL samples were positive for GM. From 13 patients with IA, 11 (84.6%) had at least one positive BAL GM index. Of these patients, 9 (81.8%) showed probable IA. The main underlying predisposing conditions were neutropenia (53.8%) and COPD (30.8%). Our study has indicated that COPD must be considered as one of the main predisposing condition for occurrence of aspergillosis in ICU patients. Karotická endarterektomie byla dlouho považována za zlatý standard v léčbě a prevenci ischemické cévní mozkové příhody u pacientů s významnou stenózou vnitřních karotických arterií. Karotický stenting, i přes počáteční limitace, dosahuje v současné době výsledků srovnatelných s karotickou endarterektomií a u selektovaných pacientů je považován za rovnocennou volbu. Bezpečnost obou výkonů je dána zejména rizikem periprocedurálních komplikací, které jsou významně ovlivněny zkušenostmi operatéra. Transkraniální dopplerovská ultrasonografie je zavedenou diagnostickou a monitorovací metodou, jejíž využití přispívá ke zkvalitnění péče o pacienty podstupující karotické intervence. Despite its initial limitations and slow acceptance, carotid artery stenting has evolved into an elaborate method currently considered to be equivalent and in some patients even preferable to carotid endarterectomy. Given the fact that the long‑term outcomes of carotid artery stenting and carotid endarterectomy are comparable, the major risk of both procedures is derived from the periprocedural complications, which are highly operator dependent. Transcranial Doppler ultrasound is a well‑established diagnostic and monitoring tool allowing optimising the care of patients with carotid artery disease.
