Survival and Lt50 after exposures at constant low temperature were compared to the values obtained at alternating temperatures in two active (summer acclimated) temperate terrestrial arthropods. The experimental regimes used interruptions - daily transfers from the lower temperature to various higher temperatures for two hours or to one high temperature for Various durations. In both species the alternating conditions improved survival, implying reparation of the chill injury. In the collembolan Orchesella cincta, there was a maximum Lt50 when the higher exposure temperature was equal to the temperature of rearing (19°C). In the bug Pyrrhocoris apterus, Lt50 increased strongly with increasing higher temperature from 0 to 15°C, and was subsequently constant over the entire physiological range suitable for development (to 35°C). Úvod: Odběr ledvin z dárce s nezvratnou zástavou oběhu (DCD – donor after circulatory death) je důležitou součástí transplantačních programů v celosvětovém měřítku. V České republice byla první transplantace ledvin z DCD úspěšně provedena v roce 2002. Autoři diskutují vlastní a literární zkušenosti s tímto programem. Snahou je zvýšit potenciál odběru ledvin z DCD v České republice. Metoda: Od roku 2002 do roku 2015 bylo v plzeňském Transplantačním centru odebráno celkem 44 ledvin z DCD. Technikou byl „in situ“ odběr pomocí „double baloon triple lumen“ katetru při zachování 5–10 min. „no-touch“ intervalu. Metoda hypotermické pulzatilní perfuze byla použita k testování viability odebraných ledvin. Transplantováno bylo celkem 28 ledvin příjemcům o průměrném věku 51,1 roku (26–73 let). Dle maastrichtských kritérií bylo 16 (57,1 %) ledvin ve skupině II, 8 (28,6 %) ve skupině III a 4 (14,3 %) ve skupině IV. Výsledky: 30denní pooperační mortalita byla 0 a morbidita 10,7 tj. 14,3 % (N=4). Primární afunkce ledviny byla přítomna u 2 (7,1 %), opožděný nástup funkce pak u 5 (17,9 %) nemocných. Jeden rok, 5 a 10 let po transplantaci žije 100 %; resp. 86,4 % a 76,7% nemocných a ve stejných intervalech je funkčních 92,9; 69,6 a 61,9 % transplantovaných ledvin. Dlouhodobé výsledky jsou plně srovnatelné s transplantacemi ledvin od dárců se smrtí mozku. Závěr: DCD jsou významným zdrojem ledvin pro transplantace. Transplantace ledvin z DCD je metodou logisticky, ekonomicky a personálně náročnou s velmi dobrými dlouhodobými výsledky., Introduction: Kidney procurement from donors after circulatory death (DCD) is an important part of worldwide transplantation programmes. The first kidney transplantation from DCD was successfully performed in the Czech Republic in 2002. Method: Forty four kidneys from DCD were procured in the Transplant Centre of Pilsen between 2002 and 2015. We used the technique of “in situ“ procurement with the double balloon triple lumen catheter and 5−10 minutes of the no-touch interval. The method of pulsatile hypothermic perfusion was used to test the viability of the kidneys. Twenty eight recipients with mean age 51.1 (26−73) years were transplanted. Sixteen (57.1%) kidneys were from the 2nd, 8 (28.6%) from the 3rd and 4 (14.3%) from the 4th category according to the Maastricht criteria. Results: 30-day mortality and morbidity rates were 0 and 10.7% i. e.14.3% respectively (N=4). Primary non-function was presented in 2 (7.1%), and delayed graft function in 5 (17.9%) cases. One, five and ten years of recipient and graft survival rates were 100%, 86.4% and 76.7%; and 92.9%, 69.6% and 61.9%, respectively. The long-term results are fully comparable with kidneys transplanted from donors after brain death. Conclusion: DCD are an important source for kidney transplantation. Female hybrids of the cross Chironomus t. thummi female × Ch. t. piger male which are largely affected by the sterility inducing Rud syndrome were backcrossed with males of both parental strains. The aim of the study was to provide information about those egg volumes that are insufficient for a normal embryogenesis and to ascertain whether in the hybrids the lethally small egg size represents a new abnormal trait of the Rud syndrome. The egg masses obtained contain eggs of very different sizes with volumes ranging from 0.5 nl to 3.49 nl. Embryo mortality is unusually frequent in those eggs of the backcrosses and of the parental strains that have volumes smaller than 1.5 nl. An egg volume of 1.5 nl represents in Ch. thummi the lower limit for those volumes that are sufficient for a normal embryogenesis. Mortality increases with decreasing egg size, reaching 100% in backcross eggs with volumes of 0.99 nl and smaller. Small egg size is a new trait of the Rud syndrome affected thummi female × piger male hybrids. This trait is part of a postzygotic reproductive isolation barrier between thummi and piger and manifests first in the backcrosses. Most backcross eggs show volumes between 1.5 nl and 2.99 nl. Within this volume range the amount of mortality does not depend upon egg volume. Here, embryo death is great in the backcrosses but normal in the parental strains. The high frequency of embryo death in the backcrosses must be predominantly due to the action of the Rud syndrome and a second hybrid syndrome, called HLE syndrome. Since further characteristic traits of these syndromes could be detected in surviving backcross individuals, the study demonstrates the occurrence of the syndromes in this generation also. Úvod: Každoročně jsou zveřejňována oficiální data, která popisují péči o pacienty s onemocněním diabetes mellitus v České republice. Je vždy uváděn celkový počet osob s diabetem, počet nově zachycených onemocnění a počet zemřelých. Z těchto dat však nelze zjistit, jaké jsou diference v mortalitě pro jednotlivé skupiny diabetiků ve vztahu k terapii. Cíl: Porovnat vývoj mortality v letech 2002 až 2006 s vývojem v letech 2010 až 2013 v reprezentativním vzorku populace pacientů s diabetes mellitus 2. typu léčených perorálními antidiabetiky v ČR vedených v databázi Všeobecné zdravotní pojišťovny České republiky, u níž bylo v roce 2013 pojištěno 63 % populace ČR. Metodika: Retrospektivní epidemiologická analýza. V databázi VZP jsme identifikovali všechny osoby, které měly v letech 2002–2008 a 2009–2013 záznam o diagnóze DM (E10-E16 podle MKN 10), nebo měly předepsanou jakoukoliv antidiabetickou terapii (ATC skupina A10). Pro vlastní analýzu jsme vybrali pouze pacienty, kteří byli léčeni perorálními antidiabetiky (v daném roce nebo v roce předcházejícím měli záznam o léčbě alespoň jedním preparátem ze skupiny A10B a současně neměli v obou letech záznam o léčbě preparáty ze skupiny A10A). Zvoleným kritériím odpovídalo v roce 2003 celkem 237 665 osob, a v roce 2013 celkem 315 418 osob. Výsledky: Mortalita se snížila napříč všemi věkovými skupinami (rok 2003–2013): věk 50–59 let 1,2 %-0,7 %; věk 60–69 let 2,6 %-1,6 %; věk 70–79 let 5,8 %-3,5 %. V roce 2013 se mortalita blížila obecné populaci, v níž byla pro tytéž věkové skupiny 0,6 %, 1,5 % a 3,4 %. Mortalita se snížila v relativním vyjádření oproti roku 2003 ve věkové skupině 50–59 let o 42 % (ČR o 25 %), ve věkové skupině 60–69 let o 39 % (ČR o 17 %) a ve věkové skupině 70–79 let o 40 % (ČR o 28 %). Pokles mortality u diabetiků léčených perorálními antidiabetiky byl v abslolutním i relativním vyjádření větší mezi roky 2003–2013, než v obecné populaci ČR. Závěr: Analýza mortality pacientů léčených perorálními antidiabetiky, kteří jsou v databázi VZP, prokázala jednoznačně příznivý trend snižování, který je rychlejší než v obecné populaci. Kriticky důležitým zjištěním je skutečnost, že se mortalita v tomto souboru přibližuje populaci obecné odpovídajícího věku. Současně lze oprávněně předpokládat, že při extenzivnějším využití dosavadních terapeutických postupů bude mortalita dále klesat. An overall number of individuals with diabetes, the number of newly reported cases and the number of patient deaths is always specified. However this data does not allow us to identify the differences in mortality between the individual cohorts of diabetic patients in relation to therapy. Goal: Comparison of the mortality development in the periods of 2002–2006 and 2010–2013 in a representative sample of the patient population with type 2 diabetes mellitus using oral antidiabetic drugs, kept in the database of the General Health Insurance Company of the Czech Republic (VZP) which provided health care coverage for 63% of Czech population in 2013. Methodology: A retrospective epidemiologic analysis. We identified all individuals in the VZP database who had a record of DM diagnosis (E10 – E16 based on ICD 10) or who had any antidiabetic therapy prescribed (ATC group A10) in the periods of 2002–2008 and 2009–2013. We only selected those patients for the analysis who were treated with oral antidiabetic medicines (in the given year or the preceding years they had a record of treatment with at least one medicine from A10B group, while having no record of treatment with medicines from A10A group within both years). 237 665 individuals met the selected criteria in 2003 and 315 418 individuals in 2013. Results: Mortality rates dropped for all age groups (from 2003–2013): for 50–59 year olds by 1.2%-0.7%; in 60–69 year olds by 2.6%-1.6%; for 70–79 year olds by 5.8%-3.5%. In 2013 mortality rates came close to the general population where for the same age groups they reached 0.6%, 1.5% and 3.4% respectively. When expressed in relative terms, the mortality among 50–59 year olds declined by 42% (Czechia by 25%), among 60–69 year olds by 39% (Czechia by 17%) and among 70–79 year olds by 40% (Czechia by 28%) from the year 2003. The decline in mortality among the patients with DM treated with oral antidiabetic medicines was greater in both absolute and relative terms in the period of 2003–2013 than among the general population in the Czech Republic. Conclusion: The analysis of mortality among the patients treated with oral antidiabetic medicines, registered in the VZP database, has shown a clearly favourable trend of mortality decline which is faster than among the general population. The fact that mortality among this cohort is getting closer to that among the general population of the corresponding age is a finding of critical importance. There is a justified expectation that mortality, with increasingly extensive utilization of the present therapeutic procedures, will continue to decrease. Léčba diabetes mellitus 2. typu je v současnosti účinná, ale bohužel ještě není ideální. Novou skupinu perorálních antidiabetik představují DPP4 inhibitory, tzv. gliptiny. V roce 2015 byla ukončena již 3. velká mortalitní klinická studie s gliptiny u pacientů s diabetes mellitus 2. typu a kardiovaskulárním onemocněním. Jedná se o studie TECOS, SAVOR a EXAMINE s léky sitagliptin, saxagliptin a alogliptin. Studie zahrnuly téměř 37 000 nemocných. Tyto studie prokázaly kardiovaskulární bezpečnost DPP4 inhibitorů, přesto ale zůstává otevřená otázka výskytu srdečního selhání. Prokázaná je i účinnost na snižování glykemie, resp. glykovaného hemoglobinu a bezpečnost z pohledu onemocnění pankreatu. DPP4 inhibitors (gliptins) are a new group of peroral antidiabetic drugs. The third clinical mortality study with gliptins in patients with diabetes mellitus type 2 was finished in 2015. The studies are known under acronym TECOS, SAVOR and EXAMINE and the tested drugs are sitagliptin, saxagliptin and alogliptin. The studies included about 37 000 patients. The studies confirmed the cardiovascular safety of the DPP4 inhibitors, but the question about increased heart failure remains open. The effectiveness of lowering glycaemia (glycated haemoglobin) was confirmed and also the pancreatic safety is confirmed.
