Významný úspech liečby s inhibítormi tyrozínkinázy (TKI) u chorých s Philadelphia chromozóm (Ph) pozitívnou chronickou myelocytovou leukémiou (CML) dramaticky zmenil prognózu tohto ochorenia. Aj napriek tomu, že prvolíniová liečba CML použitím imatinibu znamená významný benefit pre väčšinu chorých, ukazuje sa, že 25–35 % pacientov potrebuje v priebehu 5 rokov zmenu liečby z dôvodu rezistencie alebo zlyhania liečby. Najčastejším mechanizmom zodpovedným za rezistenciu proti imatinibu u pacientov s CML je rozvoj mutácií v BCR-ABL kinázovej doméne. Tieto mutácie vyvolávajú rôznu mieru rezistencie proti imatinibu a zatiaľ čo niektoré z nich môžu reagovať na zvýšené dávky imatinibu, iné sú rezistentné proti nilotinibu a ďalšie vykazujú zvýšenú mieru rezistencie proti dasatinibu. Práca je prehľad liečebných možností v primárnej liečbe chronickej fázy CML a pri jej zlyhaní. Ukazuje sa, že pokroky v molekulových metódach umožňujú lepšie porozumieť podstatu choroby, zvážiť benefit a riziko terapie, individualizovať terapeutický prístup a prispôsobiť liečbu CML včas tak, aby sa minimalizovalo riziko progresie do akcelerovaných fáz., The phenomenal success of therapy with tyrosine kinase inhibitors (TKI) in Philadelphia chromosome (Ph) – positive chronic myeloid leukemia (CML) has dramatically changed the prognosis of this disease. Despite the fact that first-line therapy of CML using imatinib represents revolutionized the treatment of this disease, it became clear that during 5 years 25–35 % of the patients require change in the therapy due to the development of imatinib resistance or failure. The most frequent mechanism responsible for imatinib resistance is development of mutation in BCR-ABL kinase domain. Mutations cause different level of imatinib resistance and while some of them can be overcome by increased dose of imatinib, others seem to be resistant to nilotinib and others are more resistant to dasatinib. This review is focused on primary treatment and treatment after imatinib failure. Karcinóm prsníka je príkladom rýchleho rozvoja vedeckých poznatkov, začínajúc od molekulárnej diagnostiky a končiac cielenou liečbou na molekulárnej úrovni. Neoadjuvantná chemoterapia je indikovaná u veľmi heterogénnej skupiny nádorov. O type liečby rozhoduje nielen biologická variabilita (stav receptorov, stav HER-2), ale tiež individuálna variabilita (PS, vek, komorbidita, rozsah ochorenia). Primárna systémová liečba (neoadjuvantná, indukčná, iniciálna) je dnes štandardná pre lokálne pokročilé nádory a niektoré vybrané operabilné typy karcinómov prsníka. Pre neuspokojivú liečebnú odpoveď sa hľadajú nové režimy s iným konceptom dávkovej intenzity, denzity, sekvencie. V neoadjuvancii je možné využiť všetky necielené aj cielené liečebné metódy na podklade výsledkov mnohých klinických štúdií., Breast cancer is a model of quickly development scientific knowledges begining from molecular diagnostic and termination with targeted therapies on the molecular level. Neoadjuvant chemotherapy is indicated for very heterogeneous group of tumours. Treatment type is decided not only by biologic variability (receptor status, HER-2 status) but with individual variability (PS, comorbidity, extent of illness). Primary systemic treatment (neoadjuvant, induction, inicials) is standard treatment for locally advanced tumor and for some eligible type of surgery breast cancer, too. If treatment response is not satisfied it is necessary to look for new treatment regimens with different concept of dose intensity, density and sequence of treatment. Cieľ: Klinicky izolovaný syndróm (Clinically Isolated Syndrome, CIS) môže byť prvým prejavom sclerosis multiplex (SM). Za potenciálny prognostický marker na "vytipovanie" pacientov po prekonanom CIS s vysokým rizikom konverzie do klinicky potvrdenej SM (Clinically Definited Multiple Sclerosis, CDMS, Poser 1983) je považovaný aj počet mozgových T2 hypersignálnych lézií pri prvom MR vyšetrení pacienta v čase CIS. Cieľom štúdie bolo overiť význam počtu mozgových T2 hypersignálnych lézii pri prvom MR vyšetrení pacienta v čase CIS ako možného prognostického markera vývoja SM vo vlastnom súbore chorých. Metóda: retrospektívne zhodnotenie MR nálezov 76 pacientov v čase CIS za obdobie od 06/2005 do 12/2012. Všetci pacienti boli po prekonaní CIS klinicky i rádiologický sledovaní po dobu najmenej 2 rokov. Zisťované parametre: pohlavie a vek pacienta v čase CIS, klinický obraz CIS, počet T2 lézií pri prvotnom MR vyšetrení. Výsledky: Priemerný vek 76 pacientov súboru bol v čase CIS 34,12 ? 8,55 roka. Ženy v súbore dominovali (60). CDMS bola za sledované obdobie potvrdená u 31 pacientov (46,27 %). Logranktest (p = 0,0046) porovnaním kriviek prežívania, metóda Coxovho modelu proporcionálnych rizík, ale aj multivariantná regresná analýza (p = 0,0538) v našom súbore potvrdili, že vyšší počet T2 lézií pri vstupnom MR predstavuje vysoké riziko včasnej konverzie CIS do CDMS. Záver: Väčší počet T2 hypersignálnych lézii pri prvotnom MR vyšetrení pacienta po prejavoch CIS znamenal v našom súbore vyššie riziko konverzie CIS do CDMS. Ako ďalší možný prediktívny faktor rizika konverzie CIS do CDMS bol v súlade s poznatkami z literatúry multifokálny klinický charakter CIS. Results: The study comprised 76 patients with a mean age at the time of CIS 34.12 ? 8.55 years. Women dominated in the set (60). To CDMS passed 31 patients (46.27%). Logranktest (p = 0.0046) comparing survival curves, the method of Cox proportional hazards model, as well as multivariate regression analysis (p = 0.0538) confirmed in our study that a higher number of T2 lesions at baseline MRI, poses a high risk of early transition CIS to CDMS. Conclucion: Higher number of hyper-signal T2 lesions on initial MR examination after CIS in our group meant a higher risk of conversion CIS to CDMS. As another possible risk factor predictive of conversion of CIS to CDMS was in accordance with literature multifocal nature of the CIS. Cíl: Otoskleróza je autozomálne dominantné ochorenie s variabilnou penetráciou a expresiou, charakterizované dystrofiou enchondrálnej vrstvy kostného puzdra labyrintu. Ochorenie sa typicky manifestuje prevodovou poruchou sluchu za celistvou blankou bubienka, ale môže sa manifestovať aj percepčnou alebo zmiešanou poruchou sluchu. Metódy a výsledky: Otosklerózu rozdeľujeme na dve základné formy, fenestrálnu a retrofenestrálnu (kochleárnu). Diagnóza otosklerózy sa stanoví na základe anamnézy (rodinná anamnéza), spektra audiologických vyšetrení a zobrazovacími metódami. Dominantné postavenie v zobrazení má HRCT spánkových kostí. HRCT pomáha potvrdiť diagnózu otosklerózy, vylúčiť iné príčiny prevodovej poruchy sluchu za celistvou blankou bubienka a určiť rozsah postihnutia labyrintu. Záver: Predmetom skúmania je vzťah rozsahu postihnutia labyrintu a stavu sluchu pr, Aim: Otosclerosis is an autosomal dominant disease with variable penetrance and expression. It is characterized by dystrophy of enchondral layer of otic capsule. Otosclerosis typically presents with conductive hearing loss and intact tympanic membrane. Sensorineural or mixed hearing loss may be present in some cases. Methods and results: Otosclerosis is categorized into two major groups, fenestral and retrofenestral (cochlear). The diagnosis of otosclerosis is based on history, series of audiologic tests and imaging modalities. HRCT is imaging modality of choice. HRCT helps to establish diagnosis of otosclerosis, excludes other cause of conductive hearing loss with intact tympanic membrane and allows grading of otosclerosis.
