Léčba karcinomu prostaty se v posledních letech stává stále složitější, převážně kvůli nástupu nových cytostatik, antiandrogenů a radiofarmak. Problém sekvenční léčby nebude pravděpodobně nikdy vyřešen, neboť se do léčebných schémat budou neustále zapojovat další preparáty. Vždy je nezbytné brát do úvahy preference pacienta, jeho výkonnostní stav a komorbidity a bohužel také i možnosti jednotlivých pracovišť. Skupiny pacientů vstupujících do klinických studií jsou pečlivě vybrané a neodpovídají realitě denní praxe. Multidisciplinární týmy za účasti urologa, klinického a radiačního onkologa a dalších odborností tak nabývají na jasném významu. V rámci konzilií je možné vybrat vhodnou léčebnou strategii individuálně pro každého pacienta, včetně možnosti účastnit se klinických zkoušek na pracovištích konzultantů. Abirateron a enzalutamid budou brzy schváleny i pro použití pro metastatický karcinom před aplikací chemoterapie s nespornou výhodou příznivého profilu nežádoucích účinků. Účinná cytotoxická léčba bude vždy k dispozici v případě selhání těchto antiandrogenů., Treatment of prostate cancer has become more complex in recent years, mainly due to the onset of new cytotoxic agents, antiandrogens and radiopharmaceuticals. The problem of sequential therapy will probably never be resolved, because there will always be new drugs involved in the therapeutic algorithms. It is always necessary to take into account the patient’s preferences, his performance status and comorbidities, and unfortunately also the possibilities of different departments. Groups of patients entering clinical trials are highly selected and do not reflect the reality of daily practice. Multidisciplinary teams with participation of urologist, clinical and radiation oncologists and other specialities are thus of great importance. The consultation can select an appropriate treatment strategy for each patient individually, including the possibility to participate in clinical trials at the respective departments. Abiraterone and enzalutamide will soon be approved for use in metastatic carcinoma before chemotherapy with indisputable advantage of a favourable side effects profile. Vzduchová embolie byla popisována při neurochirurgických a kardiochirurgických operacích, při potápění nebo při porodu. Může k ní dojít v důsledku úrazu hrudníku nebo u laparoskopických operací při porušení žíly většího kalibru. V posledních letech jsme svědky exponenciálního nárůstu používání přístupů do centrálního žilního řečiště. V současné době se se vzduchovou embolií nejčastěji setkáváme při manipulaci s centrálními žilními katétry. Jde o vzácnou, ale potenciálně letální komplikaci. Na kazuistice 55leté pacientky léčené pro akutní divertikulitidu chceme seznámit s naší zkušeností s tímto stavem, který se klinicky projevil jako iktus. Autoři uvádějí preventivní kroky, jak tomuto stavu předejít., Air embolism was described in neurosurgery and cardiac surgery, diving or childbirth. Recently, we have witnessed exponential increase in the use of central venous accesses. At present, air embolism is most frequently seen when central venous catheters are being handled. It is a rare but potentially lethal complication. We wish to present our experience with air embolism clinically manifested as a stroke on a case of a 55-year-old patient treated for acute diverticulitis. The authors propose preventive steps to avoid air embolism. Zánětlivý pseudotumor je vzácná patologie neznámé etiologie vyskytující se nejčastěji v oblasti orbity a plic. Zcela raritně můžeme identifikovat ložisko zánětlivého pseudotumoru intrakraniálně. Predilekční lokalizace v nitrolebí je baze lební a kavernózní sinus. V naší kazuistice prezentujeme případ 43leté pacientky, u které jsme diagnostikovali konvexitární, extraaxiální, kontrastem se sytící expanzi. Pacientka podstoupila operační zákrok s extirpací procesu. Histologické vyšetření stanovilo diagnózu zánětlivého pseudotumoru. Nemocná je půl roku po operačním výkonu bez obtíží., Inflammatory pseudotumour is a rare pathology of unknown aetiology, located predominantly in the orbita and lungs. Intracranial localisation is infrequent. The most frequent intracranial localisations include the cranial base and cavernous sinus. In the present paper, we present a case report of a 43-year-old woman diagnosed with a contrast-enhancing extra-axial lesion of the convexity. The patient underwent a surgery and the lesion was extirpated. The diagnosis of inflammatory pseudotumour was verified by histology. The patient is free of any symptoms one year after surgery.
