Periprosthetic fractures are the third most common reason for revision total hip arthroplasty. Surgical treatment of periprosthetic fractures belongs to the most difficult procedures due to the extensive surgery, elderly polymorbid patients and the high frequency of other complications. The aim of this study was to evaluate the results of operatively treated periprosthetic femoral fractures after total hip arthroplasty. We evaluated 47 periprosthetic fractures in 40 patients (18 men and 22 women) operated on between January 2004 and December 2010. The mean follow-up period was 27 months (within a range of 12-45 months). For the clinical evaluation, we used modified Merle d'Aubigné scoring system. In group of Vancouver A fractures, 3 patients were treated with a mean score of 15.7 points (good result). We recorded a mean score of 14.2 points (fair result) in 6 patients with Vancouver B1 fractures, 12.4 points (fair result) in 24 patients with Vancouver B2 fractures and 12.7 points (fair result) in 7 patients with Vancouver B3 fractures. In group of Vancouver C fractures, we found a mean score of 16.2 points (good result) in 7 patients. Therapeutic algorithm based on the Vancouver classification system is, in our opinion, satisfactory. Accurate differentiation of B1 and B2 type of fractures is essential. Tromboembolická nemoc (TEN) je relativně častým onemocněním s akutním rizikem úmrtí a potenciálními dlouhodobými následky v podobě posttrombotického syndromu nebo chronické plicní hypertenze. Základním léčebným postupem zůstává antikoagulační léčba; trombolytická léčba a zavedení kaválního filtru jsou určeny pro vhodně indikované individuální stavy. V posledních letech se v léčbě TEN objevila nová přímá perorální antikoagulancia (NOAC) – inhibitory faktoru Xa nebo trombinu, která prokázala stejnou účinnost a lepší bezpečnost léčby ve srovnání s konvenčním postupem. V polovině roku 2014 jsou registrovány v České republice 3 z těchto léků – rivaroxaban (Xarelto?), dabigatran (Pradaxa?) a apixaban (Eliquis?). Mají srovnatelnou účinnost a bezpečnost, rozdílná jsou však schémata podávání. Rivaroxabanem a apixabanem lze léčit perorálně ihned od stanovení diagnózy žilní trombózy nebo hemodynamicky stabilní plicní embolie, dabigatranem po úvodní několikadenní aplikaci LMWH. Omezením nových léků je ekonomická náročnost léčby. Nedostupnost antidota je dočasná, vyvíjejí se jak pro dabigatran, tak pro oba xabany. Délka antikoagulační medikace po akutní fázi se odvíjí od přítomnosti řady faktorů vzniku trombózy, v kontextu individuálního rizika krvácení. Minimální délka antikoagulační léčby je 3 měsíce, častěji však léčíme 6–12 měsíců, u nejrizikovějších nemocných tzv. dlouhodobě. Dobré výsledky extenze antikoagulační medikace na zábranu rekurence TEN ve studiích s novými antikoagulancii mohou změnit zavedené zvyklosti dlouhodobé medikace nemocných s TEN., Thromboembolic disease (TD) is a relatively common disease with acute risk of death and potential long-term consequences in term of postthrombotic syndrome or chronic pulmonary hypertension. Anticoagulant therapy is the basic therapeutic procedure; thrombolytic therapy and the introduction cava filter are appropriately indicated for individual cases. In past few years, new direct oral anticoagulant drugs (NOAC) have occurred – Xa factor or thrombin inhibitors which have demonstrated the same efficacy and even higher safety in comparison to conventional treatment. In mid 2014, 3 drugs of this group are registered in Czech Republic – rivaroxaban (Xarelto?), dabigatran (Pradaxa?) and apixaban (Eliquis?). These drugs have comparable efficacy and safety but they differ in schedule of dose administration. Rivaroxaban and apixaban can be administered immediately after diagnosis of venous thrombosis or hemodynamically stable pulmonary embolism. LMWH application has to precede few days the administration of dabigatran. Limitation of new drugs is their price. Unavailability of antidotes is temporary because current researches continue to find one for dabigatran and another for both of xabans. Duration of anticoagulant treatment after acute phase depends on the presence of thrombosis risk factors and the individual bleeding risk. Minimal duration of anticoagulant therapy is 3 months, commonly 6–12 months and in high risk patients it is “long term” treatment. Úvod: Kardiochirurgické výkony jsou zatíženy signifikantním rizikem vzniku ischemické cévní mozkové příhody (CMP), které se dle jednotlivých studií pohybuje mezi 1,3 a 3,6 %. Asymptomatické ischemické infarkty lze detekovat po kardiochirurgické operaci dokonce až u 32 % pacientů. Cílem studie je prokázat efekt sonolýzy (kontinuálního transkraniálního dopplerovského (TCD) monitoringu a. cerebri media (ACM)) u pacientů podstupujících kardiochirurgický výkon na snížení rizika vzniku nových ischemických lézí detekovaných pomocí magnetické rezonance (MR). Soubor a metodika: Do studie jsou zařazováni pacienti indikovaní k izolovanému aortokoronárnímu bypassu nebo k izolovanému výkonu na jedné srdeční chlopni. Pacienti jsou randomizováni do skupiny se sonolýzou (monitorování pomocí TCD) a kontrolní skupiny bez sonolýzy. U všech pacientů byla provedena MR mozku před výkonem a 24 hod po něm a byla hodnocena četnost výskytu, lokalizace a velikost nových ischemických změn po výkonu. Výsledky: Během 16 měsíců bylo zařazeno 78 pacientů (48 mužů, průměrný věk 63,7 ? 15,8 let), 35 pacientů bylo randomizováno do skupiny se sonolýzou, 43 pacientů do kontrolní skupiny. Ve skupině sonolýzy byly detekovány nové ischemické změny u osmi pacientů (23 %) – u pěti (14 %) v povodí pravé (stimulované) ACM, ale jen u dvou (6 %) větší než 0,5 cm3. V kontrolní skupině byly detekovány nové ischemické změny u 10 pacientů (23 %) – u osmi (19 %) v povodí pravé ACM, u pěti (12 %) větší než 0,5 cm3 (p > 0,05 ve všech případech). Závěr: Pilotní data studie prokázala trend k redukci počtu i objemu ischemických lézí mozku u pacientů se sonolýzou v průběhu kardiochirurgické operace., Background: Heart surgery is burdened with a significant risk of ischemic stroke. Asymptomatic cerebral infarctions can be detected in as many as 32% of patients after heart surgery. The aim of this study is to prove the efficacy of sonolysis (continual transcranial Doppler (TCD) monitoring) during open-heart surgery to decrease the risk of new brain infarctions detected by magnetic resonance imaging (MRI). Material and methods: Patients indicated to isolated coronary artery bypass or isolated one heart valve surgery were included in the study. Patients were randomized to a sonolysis group (TCD monitoring), and to a control group without sonolysis. All patients underwent brain MRI before and 24 hours after the surgery and the presence of new ischemic lesions was evaluated. Results: During 16 months, 78 patients (48 males, mean age 63.7 ? 15.8 years) were enrolled to the study. Thirty five patients were randomized to the sonolysis group and 43 to the control group. In the sonolysis group, new brain infarctions were found in eight (23%) patients – in the right (TCD-monitored) MCA territory in five patients (14%) but > 0.5 cm3 in two (6%) patients only. In the control group, new infarctions were found in 10 patients (23%) – in the right (TCD-monitored) MCA in eight (19%) patients and > 0.5 cm3 in five (12 %) (p > 0.05 in all cases). Conclusion: Pilot study results showed a trend towards reduction in a number and volume of new brain ischemic lesions in patients treated with sonolysis during heart surgery. Spinální arteriovenózní malformace jsou skupina vzácných onemocnění, jejichž klinické projevy mohou být velmi rozmanité. Optimální léčebný postup je nutno vždy volit individuálně a pacienty směřovat do specializovaných center. Cílem sdělení je uvést přehlednou klasifikaci SAVM, jejich diferenciální diagnostiku a terapii a dále prezentovat dvě kazuistiky. První kazuistika se zmiňuje o případu 27letého muže, u kterého se rozvinuly silné bolesti bederní páteře s dominující meningeální symptomatikou v klinickém obraze. Na MR hrudního úseku páteře (Th11/12) byla nalezena AVM zásobovaná hypertrofickou radikulopiální tepnou. Na specializovaném pracovišti byla provedena úspěšná embolizace AV zkratu. Druhá kazuistika uvádí případ 41leté ženy, které vznikla při lyžování náhlá prudká bolest za krkem. V neurologickém obraze došlo k rozvoji inkompletní léze míšní s kvadruparézou s levostrannou akcentací do těžkého stupně s hranicí čití ve výši C1. Na provedeném MR a MRAG krční míchy a následně na DSA vertebrálních tepen byla detekována AVM zásobovaná levou arteria spinalis posterior. Terapeuticky byla indikována elektivní endovaskulární embolizace a pacientka byla ošetřena na specializovaném pracovišti ve Francii., Spinal arteriovenous malformations are a group of rare diseases with highly diverse clinical manifestations. The optimal therapeutic practice should always be selected individually and patients should be sent to specialized centres. The objective of this paper is to provide a structured classification of spinal AVMs, their differential diagnosis and therapy, and to present two case reports. The first report is concerned with a case of a 27-year-old man who developed severe pain in the lumbar spine with predominantly meningeal symptomatology. An AVM fed by a hypertrophic radiculopial artery was found on the thoracic spine (Th11/12) MR. A specialized centre performed successful embolization of an arteriovenous shunt. The second report presents a case of a 41-year-old woman, who felt sudden sharp pain in the neck while skiing. Neurologically, she developed an incomplete spinal cord lesion with quadriparesis with left-sided accentuation of severe intensity and sensation threshold at C1. The MR and MRAG of the cervical spinal cord and the following DSA of vertebral artery detected AVM fed by the left posterior spinal artery. Účel studie: Retrospektivně jsme zpracovali výsledky léčby mozkových nádorů stereotaktickou radioterapií a radiochirurgií. Použité metody: Pacienti podstoupili plánovací CT a MR vyšetření. Poté bylo provedeno plánování cílového objemu a ozařovací techniky v BrainLab systému a pacienti byli ozářeni na X-noži Varian s vícelamelovým kolimátorem BrainLab. Radiochirurgie byla aplikována jednorázově s fixací ve stereotaktickém rámu. Pacienti, kteří podstoupili frakcionovanou stereotaktickou radioterapii, byli fixováni pomocí speciální stereotaktické masky. Statistické zpracování bylo provedeno ve spolupráci s Ústavem matematiky a statistiky PřF MU Brno. Výsledky: Celkem bylo ozářeno 101 pacientů s primárními mozkovými tumory. Nejčastěji se jednalo o meningeomy a high-grade gliomy. Medián sledování byl 22,4 měsíců. Pacienti, kteří podstoupili radiochirurgii, měli medián dávky 18 Gy. Pacienti, ozařovaní frakcionovaně, měli medián dávky 25 Gy. Toxicita léčby byla nízká. Stabilizace onemocnění byla zaznamenána u 68 % pacientů, parciální remisi dosáhlo 10 % a jeden pacient měl kompletní remisi. Závěr: Výsledky naší léčby jsou srovnatelné s publikovanými studiemi. Stereotaktické ozařovací metody mají své důležité místo v léčbě primárních tumorů mozku., We analysed retrospectively our treatment of primary brain tumors using stereotactic radiotherapy and radiosurgery. Methods: Patients underwent CT and MR examinations. Target volume and technique were planned in BrainLab system. Patients were irradiated on X knife. Radiosurgery was apllied with fixation using stereotactic frame. Stereotactic mask was used in cases of stereotactic radiotherapy. Statistical processing was performed at Department of mathematics and statistics, Faculty of Science, Masaryk university Brno. Results: 101 paients were included in our study. In the most of cases there were diagnosis of meningeoma and high-grade glioma. Median follow up was 22,4 months. Median of dose of radiosurgery was 18 Gy. Patients, whose underwent stereotactic radiotherapy, were median of doses 25 Gy. Treatment toxicity was low. Stable disease was recorded in 68 % of cases, partial remission achieved in 10 % patients and one patient had the complete remission. Conclusion: Results of our study are comparable with results other publicated studies. Introduction: Ankylosing spondylitis (AS) is an inflammatory rheumatic disease characterized by the development of osteoproductive changes in the spine which could possibly result in ankylosis. Treatment with tumour necrosis factor alpha (TNFα) inhibitors has proved to be an important step forward in the treatment of this disease, but for the time being it is not clear whether it favourably influences radiographic progression of the disease. Vascular endothelial growth factor most probably plays a role in the development of osteoproductive changes and recently its predictive influence on radiographic progression has been demonstrated. Bone morphogenic protein 2 (BMP-2) participates in the regulation of bone proliferation and its increased serum level has been demonstrated in patients with advanced AS and correlated with the degree of radiographic changes. Aim: The study aims to evaluate the VEGF and BMP-2 levels in patients with ankylosing spondylitis and how these levels relate to the concurrent treatment with TNFα inhibitors. Methods: Sera were evaluated from patients at the Rheumatologic Clinic of the Hradec Králové Faculty Hospital who fulfilled the modified New York Criteria for AS (n = 55). In these patients, the parameters of the activity of the disease (BASDAI = Bath Ankylosing Spondylitis Disease Activity Index, CRP = C-reactive protein) and the concurrent therapy (TNFα inhibitors, n = 21, vs. non-anti TNFα, n = 34) were recorded. The levels of VEGF and BMP-2 were analyzed using the ELISA method. Results: In patients treated with TNFα inhibitors, a significantly lower VEGF level was found when compared to untreated patients (140.3 (109.4; 262.2) vs. 261 (172.4; 396.6) pg/ml; p = 0.02). No difference was found between BMP-2 levels in both groups (treated vs. untreated patients) (254.8 (2301; 267.3) vs. 261.1 (248.6; 273.5) pg/ml; p = 0.24). A correlation analysis did not reveal any relationship between VEG F and BMP-2 (r = 0.057; p = 0.68). Serum levels of VEGF correlated with serum levels of CRP (r = 0.56; p = 0.00001) and the BASDAI value (r = 0.33; p = 0.015). Conclusion: Significantly lower VEGF levels were found in patients treated with TNFα inhibitors versus the untreated patients. These findings are in harmony with some hitherto published analyses and may give evidence of a favourable effect of TNFα inhibitors on radiographic progression. Kontinuální tvorba výpotků jakékoliv etiologie zásadním způsobem ovlivňuje kvalitu života nemocných. Akumulace tekutiny v pleurální dutině postupně komprimuje plicní parenchym. Prvním příznakem bývá progrese námahové dušnosti, později se přidává i klidová dušnost. S nárůstem výpotku dochází ke kompresi plicního parenchymu s rozvojem plicní atelektázy. Nemocný je ohrožen vznikem zánětového onemocnění plic v nevzdušném plicním parenchymu. U diagnózy maligního onemocnění je riziko zánětových komplikací výrazně vyšší vzhledem k předchozí chemoterapii či radioterapii, která zásadním způsobem snižuje imunitu. Opakovaná tvorba výpotku se výrazně podílí na rozvoji terminálního stavu nádorové kachexie. Efektivní metodou léčby je chemická pleurodéza, která se užívá u vybraného spektra nemocných. Principem je navození aseptické pleuritidy. Nejefektivnější cestou aplikace léčiva je videotorakoskopie. V souboru bylo 47 nemocných, 29 (61,7 %) mužů a 18 (38,3 %) žen, s cytologicky prokázaným recidivujícím maligním pleurálním výpotkem. Věkové rozmezí 42–80 let, Ø věk 65 let. Cílem práce bylo objektivizovat průběh celkových a lokálních zánětových změn po talkáži. Efekt léčby nebyl ovlivněn základním typem nádorového onemocnění. U obou skupin nebyly významně odlišné pooperační komplikace, což dokládá bezpečnost zvolené metody a podporuje správně zvolený algoritmus léčby., Continuous pleural effusion production of any aetiology can significantly affect the quality of patients life. Chronic effusion accumulation in the pleural cavity can lead to lung parenchyma compression. The first symptom of this entity is a progressive exertional dyspnea, which can later turn in dyspnea at rest. With the increase of the pleural effusion volume, pulmonary atelectasis can develop due to chronic parenchycha compression. The patient is at risk of inflamatory complications from the territory of non-ventilated parenchyma. Patients with a diagnosis of malignant disease have this risk considerably higher due to their previous chemotherapy or radiation, which substantially affect the immunity system. Repeated pleural effusion formation can significantly take part in the development of the terminal status of cancer cachexia. An effective method of palliative treatment is a chemical pleurodesis, which is used in selected patients. The principle is inducing aseptic inflammation. The most effective way of application is via videothoracoscopy. In a group of 47 patients, there were 29 (61.7 %) men and 18 (38.3 %) women with cytologically diagnosed recurrent malignant pleural. The age ranged between 42 and 80 with average age of 65 years. The aim was to assess the course of local and systemic inflammatory changes after talc application. The effect of treatment was not influenced by the type of malignancy . In both groups there was no significant difference in postoperative complications. The aim of this study is to evaluate the results of total hip arthroplasty in patients with Parkinson's disease during a period of five years, focusing on the assessment of the risks and benefits of surgery. During this period we performed total hip arthroplasty in 14 patients (15 hips) with Parkinson's disease. Patients were evaluated by subjective symptoms and objective findings, with a focus on the use of support while walking and walking distance, severity of Parkinson's disease before surgery and at the time of the last follow-up. During the postoperative period, the following parameters were assessed: length of ICU stay, mobilization, complications, the total duration of hospitalization and follow-up care after discharge. Of the 11 patients (12 hips) followed-up 1-5 years with an average of 3 years after operation 8 cases showed progression of neurological disability. 5 patients (6 hips) showed an increased dependence on the use of support when walking and reduced distance that the patient was able to walk. Subjectively, 10 hip joints were completely painless and 2 patients complained of only occasional mild pain in the operated hip. Complications that were encountered were urinary tract infection (5 patients), cognitive impairment (3 patients) and pressure ulcer (2 patients). We did not observe any infection or dislocation of the prosthesis. Three patients fell and fractured the femur and 3 patients in our cohort died during follow up. Implantation of total replacement is possible with judicious indication after careful evaluation of neurological finding in patients with minimal or mild functional impairment of the locomotor system. Toxická epidermální nekrolýza je vzácné, akutní, život ohrožující onemocnění projevující se rozsáhlým odlučováním epidermis a slizničních povrchů. Jde o závažnou nežádoucí reakci na podávané léky, nejčastěji antibiotika, antikonvulziva či nesteroidní antirevmatika. Jako první ji popsal skotský dermatolog Alan Lyell v roce 1956, proto je též nazývána Lyellův syndrom. Mortalita tohoto onemocnění je vysoká, proto je klíčová včasná diagnostika a neodkladné zahájení adekvátní péče o pacienta. Jedná se o vzácné onemocnění s velmi malou incidencí a závažnou prognózou, což značně ztěžuje provedení velkých randomizovaných klinických studií. Je tedy obtížné hodnotit efektivitu jednotlivých terapeutických možností. Jako nejperspektivnější se prozatím jeví nitrožilní podávání imunoglobulinů se zřejmým zlepšením klinického stavu pacienta, dobrou tolerancí a minimálními vedlejšími nežádoucími účinky. Nové experimentální postupy se snaží hledat cestu terapie cíleným ovlivněním granulyzinu jakožto hlavního cytotoxického mediátoru. It is a serious adverse response to administered drugs, mostly antibiotics, anticonvulsants or NSAIDs. First described by Scottish dermatologist Alan Lyell in 1956, it is also known as Lyell’s syndrome. Mortality of the disease is high, therefore its early diagnosis is crucial and immediate initiation of appropriate patient care necessary. It is a rare disease with a very low incidence and serious prognosis, which is a considerable hindrance to undertaking large randomized clinical studies. It is therefore difficult to evaluate the effectiveness of various therapeutic options. As the most promising so far appears the administration of intravenous immunoglobulins, apparently reaching improvement in the clinical condition of the patient, with a good tolerances and minimal side adverse effects. New experimental techniques endeavour to seek a way to the therapy through targeted influencing of granulysin as the major cytotoxic mediator. Úvod: Tato práce referuje první zkušenosti s laparoskopicky asistovanou transanální totální excizí mezorekta u pacientů s karcinomem středního a dolního rekta. Metody: V období od října 2013 do května 2014 bylo na dvou pracovištích (CHK FN Brno, CHO KOC Nový Jičín) operováno 17 pacientů technikou transanální totální mezorektální excize. V souboru bylo 11 mužů a 6 žen ve věku 49–81 let. Vzdálenost dolního okraje tumoru od análního okraje byla 30–80 mm, medián 60 mm. U všech pacientů byla konstruována ručně šitá kolo-anální anastomóza. Výsledek: U všech operovaných byl dosažen negativní distální resekční okraj. Cirkumferentní resekční okraj byl pozitivní ve dvou případech. Kvalita totální mezorektální excize byla hodnocena v 8 případech jako kompletní a v 9 případech jako téměř kompletní. Dehiscence kolo-anální anastomózy byla zaznamenána ve dvou případech (12 %). Operační čas se pohyboval v rozsahu 212–375 minut (medián 280 minut). Medián pooperační hospitalizace byl 9 dnů v rozsahu 6 až 30 dnů, v případě řešení anastomotické a rané komplikace. Závěr: Na základě prvních získaných zkušeností hodnotíme výkon jako realizovatelný a bezpečný v podmínkách našich chirurgických pracovišť s uspokojivými patologickými výsledky. Přesné indikace zatím nelze jednoznačně doporučit. Methods: During a 9 month period (between October 2013 and May 2014), 17 laparoscopically assisted rectal resections with transanal total mesorectal excision, hand-sewn colo-anal anastomosis and protective ileostomy were performed at two institutions (CHK FN Brno, CHO KOC Nový Jičín). There were 11 males and 6 females, with a mean age 68 years (range 49–81 years) in our cohort. The tumor was located 30–80mm from the anal verge (median 60 mm). Results: A negative distal resection margin was achieved in all patients. The circumferential resection margin was positive in two cases. The TME quality was described as complete in 8 cases and nearly complete in 9 cases. Mean operative time was 280 min (range 212–375 min) with no intra-operative complications. Anastomotic leakage was observed in two patients (12%). Median length of postoperative hospital stay was 9 days with a range of 6–30 days, in case of anastomotic and wound complications. Conclusion: According to our first experience we evaluated this method as feasible and safe with satisfactory pathological outcomes. We cannot recommend exact indications yet.
