Psychosociální faktory významně participují na patogenezi komplexních chorob. Jejich působení na mnoha úrovních organismu koresponduje s bio-psycho-sociálním konceptem zdraví a nemoci. Mozek, chování a imunita jsou úzce propojeny a reagují na environmentální kontext. Účinky chronického psychosociálního stresu popisuje model alostatické zátěže, jenž ozřejmuje dynamičnost nastavení biologických procesů a význam mozku pro zpětnovazebnou regulaci v adaptaci na prostředí. Osobnost jedince, jeho kognitivní a emoční charakteristiky, kvalita sociálních vztahů a socioekonomické faktory modifikují stresovou zátěž a mají přímý i nepřímý vliv na zdraví. Řada účinků psychosociálních faktorů je zprostředkována epigeneticky; v období raného vývoje tento mechanismus formuje vulnerabilitu vůči rozvoji patologií souvisejících se stresem. Psychické, genetické, behaviorální a environmentální vlivy spolu průběžně interagují. Nepřímé účinky psychosociálních faktorů spočívají v modifikaci chování a životního stylu. Toto působení je obousměrné, neboť životní styl zpětně ovlivňuje psychické a fyzické zdraví. Přístupy, zaměřené na podporu odolnosti vůči stresu, mají důležité místo v prevenci., Psychosocial factors significantly participate in the pathogenesis of complex diseases. Their effects correspond to the bio-psycho-social concept of health and disease on many levels. Brain functions, behaviour and immunity are closely linked and responsive to the environmental context. The effects of chronic psychosocial stress are described by the allostatic load model which highlights dynamic adjustment of biological processes and the importance of the brain for feedback regulation in adapting to the environment. The personality of an individual, his cognitive and emotional characteristics, quality of social relationships and socioeconomic factors modify stress load and have a direct and indirect impact on health. Many of the effects of psychosocial factors are mediated epigenetically. In the early development epigenetic mechanisms form the vulnerability to stress-related pathologies. Psychological, genetic, behavioural and environmental factors continuously interact. The indirect effects of psychosocial factors are contained in the modification of behaviour and lifestyle. These effects are bidirectional, because lifestyle in turn has an impact on mental and physical health. Podmínkou zdraví, nejenom reprodukčního, je zdravý způsob života vycházející z kvalitní informovanosti. Výsledky současných výzkumů ukazují, že nezdravý způsob života matky/rodičů již před početím dítěte a následně prenatálně hraje roli v etiologii různých poruch zdraví a abnormalit vývoje jedince, které se mohou manifestovat okamžitě, ale také teprve s dlouhou latencí. Znalosti a návyky žen mohou svým poradenstvím významně ovlivnit porodní asistentky prenatálně v kurzech, v optimálním případě již prekoncepčním poradenstvím. Záměrem práce bylo zjistit, jaká je současná situace v oblasti přípravy žen na těhotenství a mateřství, které faktory ji determinují a jaký je její případný benefit. Užito bylo dotazníkového šetření a hodnocení vybraných antropometrických parametrů. Práce zjistila, že příprava žen na těhotenství a mateřství vykazuje značné rezervy projevené nedostatečnou informovaností i výskytem nevhodného či rizikového chování. Výsledky ve většině sledovaných parametrů korelovaly s dosaženým stupněm vzdělání a byly lepší u žen, které se na své těhotenství a mateřství připravovaly. Prenatální kurzy navštěvuje ale celkově malý počet žen, nejméně ženy s nižšími stupni vzdělání, pro které ale paradoxně kurzy přinášejí vyšší benefit., Healthy lifestyle resulting from well-informed parental knowledge is a condition of both reproductive and general health. Results of current studies show that both pre-conceptual and prenatal unhealthy lifestyles of mothers/parents play an important role in the etiology of different health malfunctions and abnormalities in ontogeny. These malfunctions and abnormalities can be manifested either immediately or with a long latency. The knowledge and habits of women can be significantly influenced by midwives in prenatal courses or, optimally, in a pre-conception advisory capacity. The goal of our work was to ascertain the current situation of pregnanacy and maternity preparedness and associated determining factors. A questionnaire survey and evaluation of chosen anthropometrical parameters were used. The study revealed that existing pregnancy and maternity preparation is insufficient, with associated inadequacies or risks in behaviour. In most cases the results correlated with the educational capacity of mothers and were better in cases where the subjects had prepared for their pregnancy and maternity. Ve 21. století jsou podmínky v oblasti zdraví charakterizovány vzrůstajícím množstvím vzájemně propojených faktorů, které ovlivňují zdraví a životní pohodu, a vzájemnou provázaností na globální, regionální i místní úrovni. V současné době není možné zhostit se úspěšně náročných úkolů v oblasti veřejného zdraví (např. nepřenosné choroby, antimikrobiální rezistence, nerovný přístup ke zdravotní péči a zdravotní důsledky vynucených opatření ke snížení nákladů na zdravotní péči v některých zemích) bez spolupráce mezi jednotlivými sektory a koordinace kroků na mezinárodní, národní i místní úrovni. 53 zemí z evropského regionu WHO vytvořily a přijaly evropskou strategii pro 21. století (Zdraví 2020) jako společný, výzkumem podložený souhrn opatření pro rozvoj veřejného zdraví, podporu spolupráce ve všech sektorech a na všech úrovních a koordinaci kroků vedoucích ke zlepšení zdraví a životní pohody obyvatel evropského regionu. Tento článek se věnuje vzniku a vývoji strategie Zdraví 2020, hlavním strategickým cílům, úkolům i náplni. Dále uvádí kroky nezbytné k úspěšnému zavedení strategie Zdraví 2020 v jednotlivých zemích a pomoc poskytovanou WHO zemím, které začaly v oblasti zdraví uplatňovat postupy v souladu se strategií Zdraví 2020., The 21st-century health landscape is shaped by growing global, regional and local interdependence and an increasingly complex array of interlinking factors that influence health and well-being. Most of today’s major public health challenges, including noncommunicable diseases, antimicrobial resistance, health inequalities and the health effects of austerity measures in some countries, cannot be addressed effectively without intersectoral and coordinated action at supranational, national and local levels. The 53 countries of the WHO European Region developed and adopted a European policy framework and strategy for the 21st century (Health 2020) as a common, evidence-informed policy framework to support and encourage coordinated action by policy-makers at all levels and in all sectors to improve population health and well-being. This article presents the development process of Health 2020 and its main strategic goals, objectives and content.
