Úvod: PARADIGM HF srovnávala inhibitor angiotenzinového receptoru a inhibitor neprilysinu LCZ696 s enalaprilem u nemocných se srdečním selháním a sníženou ejekční frakcí. Metodika: Jednalo se o dvojitě slepou studii u 8 442 nemocných se srdečním selháním NYHA klasifikace II, III a IV a ejekční frakcí pod 40 %, kteří dostávali LCZ696 (2 × 200 mg) nebo enalapril (2 × 10 mg) přidané ke standardní medikaci. Primární cíl byl smíšený – kardiovaskulární mortalita a hospitalizace pro první srdeční selhání. Studie byla naplánována tak, aby odhalila rozdíl v kardiovaskulární mortalitě. Výsledky: Studie byla ukončena předčasně podle předepsaných pravidel při průměrné době sledování 27 měsíců pro jasný prospěch z léčby LCZ696. V době ukončení se primární cíl vyskytl u 914 nemocných (21,8 %) ve LCZ696 skupině a u 1 117 nemocných (26,5 %) ve skupině léčené enalaprilem (HR LCZ696 skupina, 0,80; 95% CI, 0,73–0,87; p < 0,001). Celkem 711 nemocných (17,0 %) léčených LCZ696 a 835 nemocných (19,8 %) léčených enalaprilem zemřelo (HR pro úmrtí z jakékoli příčiny 0,84; 95% CI, 0,76–0,93; p < 0,001); z těchto nemocných 558 (13,3 %) a 693 (16,5 %) zemřelo z kardiovaskulární příčiny (HR 0,80; 95% CI, 0,71–0,89; p < 0,001). LCZ696 ve srovnání s enalaprilem snižoval riziko hospitalizace pro srdeční selhání o 21 % (p < 0,001) a snižoval symptomy srdečního selhání (p = 0,001). Ve skupině léčené LCZ696 bylo více hypotenzí a nezávažných angioedémů, ale menší výskyt renálního selhání, hyperkalemie a kašle než ve skupině léčené enalaprilem. Závěr: LCZ696 byl účinnější než enalapril ve snížení rizika kardiovaskulárního úmrtí a hospitalizací pro srdeční selhání. Methods: In this double‑blind trial, 8442 patients with class II, III, or IV heart failure and an ejection fraction of 40% or less were assigned to receive either LCZ696 (at a dose of 200 mg twice daily) or enalapril (at a dose of 10 mg twice daily), in addition to the standard therapy. There were two primary outcomes – mortality from cardiovascular causes and hospitalisation for heart failure, but the trial was designed to detect the differences in the rates of mortality from cardiovascular causes. Results: In line with the pre‑specified rules, the trial was stopped early, after a median follow‑up of 27 months, because the boundary indicating a clear benefit achieved with LCZ696 had been exceeded. At the time of study closure, the primary outcome had occurred in 914 patients (21.8%) in the LCZ696 group and 1117 patients (26.5%) in the enalapril group (hazard ratio in the LCZ696 group, 0.80; 95% confidence interval [CI], 0.73 to 0.87; P < 0.001). A total of 711 patients (17.0%) receiving LCZ696 and 835 patients (19.8%) receiving enalapril died (hazard ratio for death from any cause, 0.84; 95% CI, 0.76 to 0.93; P < 0.001); of these patients, 558 (13.3%) and 693 (16.5%), respectively, died from cardiovascular causes (hazard ratio, 0.80; 95% CI, 0.71 to 0.89; P < 0.001). As compared with enalapril, LCZ696 also reduced the risk of hospitalisation for heart failure by 21% (P < 0.001) and decreased the symptoms and physical limitations of heart failure (P = 0.001). The LCZ696 group showed higher proportions of patients with hypotension and non‑serious angioedema but lower proportions of patients with renal impairment, hyperkalemia, and cough when compared to the enalapril group. Conclusions: LCZ696 was more effective than enalapril in reducing the risk of death and of hospitalisation for heart failure. OBJECTIVES: The main aim of the study was to investigate the physiological and social needs of patients hospitalized with schizophrenia to uncover potential issues in these areas. METHODS: The relevant self-evaluating CANSAS questionnaire for physiological and social needs was used by nurses in a cohort of hospitalized schizophrenic patients undergoing rehabilitation before discharge from the mental hospital. RESULTS: Two hundred and forty-four patients (women N = 115) aged 18-58 years were involved in the study. Intimate relations, financial matters, treatment of psychotic symptoms, and sexual life were among the most pressing physiological and social needs in our study subjects. CONCLUSION: The results of our study should stimulate psychiatric nurses in their effort not only to detect but also address the problems of schizophrenic patients concerning unfulfilled needs. and J. Cíl: Popsat profil nemocných s akutním srdečním selháním (ASS) z pohledu přednemocniční péče v České republice. Metodika: Data pochází z registru Zdravotnické záchranné služby (ZZS) Jihomoravského kraje z období leden 2010–prosinec 2011. Byl proveden konsekutivní sběr nemocných s podezřením na ASS. Vyloučeni byli nemocní po kardiopulmonální resuscitaci nebo zemřelí před příjezdem ZZS nebo pacienti s dominantně extrakardiální příčinou dušnosti. Srovnáni jsou nemocní transportovaní do nemocnice oproti pacientům léčeným ambulantně. Cílovým parametrem je 30denní celková mortalita. Výsledky: Z celkového počtu 131 798 nemocných bylo podezření na ASS vysloveno ve 1 964 (1,5 %) případech. Konečná diagnóza ASS byla nakonec stanovena u 1 310 pacientů, kteří byli transportováni do nemocnice s předpokladem hospitalizace. Ambulantně bylo ošetřeno 35 nemocných. Průměrný věk celé populace byl 78 let, nejčastější komorbiditou byla hypertenze (67,0 %), u téměř poloviny jsou do doby předání do nemocnice přítomny známky plicního edému (46,4 %). Ambulantně léčení nemocní jsou častěji muži (71,4 % vs 49,1 %), mají menší výskyt komorbidit a nevykazují známky těžší respirační insuficience (kyslíková saturace 96 % oproti 87 %, žádné známky plicního edému). Nejčastěji aplikovaným lékem v přednemocniční péči u pacientů s podezřením na ASS je furosemid (74,0 %). Jednoměsíční mortalita souboru dosahovala 20,7 %. Závěr: ASS představuje 1,5 % všech volání ZZS pro akutní potíže, většina nemocných je transportována do nemocnice s předpokladem hospitalizace. Časná mortalita nemocných je vysoká. Klíčové slova: akutní – srdeční selhání – úmrtnost – zdravotnická záchranná služba, Purpose: To describe the profile and management of patients with acute heart failure (AHF) in pre‑hospital care in the Czech Republic. Methods: Registry data of the Emergency Medical System (EMS) of the South Moravian region of Czech Republic were used. Patients with suspected AHF were enrolled consecutively during the period of January 2010–December 2011. Excluded were patients who died or had cardiac arrest before the arrival of EMS and those who presented with dyspnea of dominantly extra-cardiac etiology. We compared patients who were transported to the hospital with those treated in an outpatient setting. Overall mortality within the first 30 days after the initial EMS contact was assessed as a main endpoint. Results: From the 131,798 of patients who called EMS, 1,964 (1.5%) of cases were suspected of having AHF. The diagnosis of AHF was determined in 1,310 patients who were transported to the hospital; 35 were treated in Outpatients. The mean age of the study population was 78 years, the most common comorbidity was hypertension (67.0%) and almost one half of the study population had signs of pulmonary edema before being admitted to hospital (46.4%). Patients not transported to hospital were more frequently of male gender (71.4% vs. 49.1%), with lower incidence of comorbid conditions and did not have signs of respiratory distress (oxygen saturation 96% vs. 87% in those transported to the hospital; no signs of pulmonary edema). The most common drug used in pre‑hospital care in patients with suspected AHF was furosemide (74.0% patients). The 30-day mortality rate of the whole study population reached 20.7%. Conclusion: AHF represents 1.5% of all emergency calls due to acute symptoms. The vast majority of the patients are transported to the hospital with the assumption of hospitalization. Early mortality rates of these patients are high. Léčba diabetes mellitus 2. typu je v současnosti účinná, ale bohužel ještě není ideální. Novou skupinu perorálních antidiabetik představují DPP4 inhibitory, tzv. gliptiny. V roce 2015 byla ukončena již 3. velká mortalitní klinická studie s gliptiny u pacientů s diabetes mellitus 2. typu a kardiovaskulárním onemocněním. Jedná se o studie TECOS, SAVOR a EXAMINE s léky sitagliptin, saxagliptin a alogliptin. Studie zahrnuly téměř 37 000 nemocných. Tyto studie prokázaly kardiovaskulární bezpečnost DPP4 inhibitorů, přesto ale zůstává otevřená otázka výskytu srdečního selhání. Prokázaná je i účinnost na snižování glykemie, resp. glykovaného hemoglobinu a bezpečnost z pohledu onemocnění pankreatu. DPP4 inhibitors (gliptins) are a new group of peroral antidiabetic drugs. The third clinical mortality study with gliptins in patients with diabetes mellitus type 2 was finished in 2015. The studies are known under acronym TECOS, SAVOR and EXAMINE and the tested drugs are sitagliptin, saxagliptin and alogliptin. The studies included about 37 000 patients. The studies confirmed the cardiovascular safety of the DPP4 inhibitors, but the question about increased heart failure remains open. The effectiveness of lowering glycaemia (glycated haemoglobin) was confirmed and also the pancreatic safety is confirmed. Studie TOPCAT byla studií sledující vliv spironolaktonu u chronického srdečního selhání se zachovalou ejekční frakcí levé komory. Studie nezjistila ovlivnění primárního cíle studie, tj. kombinace kardiovaskulární mortality, zvládnuté srdeční zástavy nebo hospitalizace pro léčbu srdečního selhání. Jediným nálezem studie byl pokles hospitalizací pro srdeční selhání. Post-hoc analýza však zjistila nápadný rozdíl mezi pacienty z Amerik (USA, Kanada, Argentina, Brazílie) a pacienty z Ruska/Gruzínska. Rozdíly byly veskrze nápadné. Zatímco u pacientů z Amerik došlo k významnému snížení výskytu primárního cíle, kardiovaskulární mortality a hospitalizace pro srdeční selhání léčbou spironolaktonem, výskyt všech klinických příhod byl výrazně nižší v Rusku/Gruzii, a u pacientů z Ruska/Gruzie nebyl zjistitelný žádný zjistitelný dopad spironolaktonu na jakýkoliv cíl. Tato post-hoc analýza tedy ukázala na možný léčebný účinek spironolaktonu v Amerikách, jakož i v populacích vykazujících obdobné vlastnosti jako populace v Amerikách. Většina prací se spokojuje s definicí opírající se jen o srdeční selhání se zachovalou ejekční frakcí levé komory. Práce upozorňuje na nutnost zpřesnění této definice, která se musí také opírat o změny diastolické funkce levé komory při definici diastolického srdečního selhání., The TOPCAT study followed the effect of spironolactone on a chronic heart failure with a preserved left ventricular ejection fraction. The study did not find any impact on the primary goal of the study, i.e. a combination of cardiovascular mortality, managed cardiac arrest or hospitalization rate for heart failure treatment. The only finding of the study was the decrease in hospitalization rates for a heart failure. A post hoc analysis, however, identified a significant difference between patients from the Americas (USA, Canada, Argentina, Brazil) on the one hand and those from Russia/Georgia on the other. The differences were rather striking. Whereas the former manifested a significant reduction in the incidence of the primary goal, i.e. cardiovascular mortality and hospitalization with heart failure through treatment with spironolactone, the incidence of all clinical events was considerably lower in Russia/Georgia and no impact of spironolactone at all on any of the goals was identified regarding the patients from Russia/Georgia. This post hoc analysis therefore suggested a possible therapeutic effect of spironolactone in the Americas, as well as in the populations of similar characteristics. Most studies accept a definition only based on the heart failure with a preserved left ventricular ejection fraction. The study points to the need to further elaborate this definition which also has to consider changes of the left ventricular diastolic function when defining a diastolic heart failure., and Jiří Widimský Sr
