Cíl: Nepravidelné dávkování inzulinu je jedním z hlavních problémů spojených s inzulinovou terapií u pacientů s diabetem 2. typu; jeho skutečný rozsah však není přesně znám. Cílem průzkumu provedeného v ČR v rámci mezinárodního projektu GAPP2TM – Globální přehled přístupů pacientů a lékařů (Global Attitudes of Patients and Physicians) proto bylo ověřit, jaký je výskyt a dopad nepravidelného užívání bazálních inzulinových analog u pacientů s diabetem 2. typu, jakým způsobem a z jakých důvodů k těmto nepravidelnostem dochází, a jakým způsobem zdravotníci o nepravidelné aplikaci inzulinu s pacienty diskutují. Metodika: Projekt GAPP2TM je mezinárodní průřezovou studií provedenou on-line formou dotazníku prostřednictvím internetu u pacientů s diabetem 2. typu léčených inzulinovými analogy a u lékařů a zdravotníků, kteří tyto pacienty ošetřují. Průzkum proběhl ve dvou vlnách v celkem 17 zemích světa; do první vlny, která byla dokončena začátkem roku 2012, bylo zahrnuto 6 zemí, do druhé, dokončené v roce 2014, pak dalších 11 zemí včetně České republiky. Průzkum měl za cíl získat data o pohledech pacientů a lékařů na některé aspekty léčby diabetu 2. typu inzulinem a o přetrvávajících problémech v této oblasti v reálné každodenní praxi. Zabýval se zejména incidencí a zvládáním lehkých hypoglykemií a nepravidelnostmi v aplikaci inzulinu. V části zaměřené na adherenci k aplikaci bazálního inzulinu byly sledovány 3 typy nepravidelností v léčbě inzulinem: vynechání dávky, časový posun dávky (? 2 hodiny oproti předepsanému času) a snížení dávky, ve všech v případech během uplynulých posledních 30 dnů před vyplněním dotazníku. Dále bylo zkoumáno chování pacientů ve vztahu k této problematice a jejich vnímání těchto problémů. Výsledky: Výsledky průzkumu ukázaly, že nepravidelné dávkování inzulinu je v České republice méně časté než v ostatních zemích, v nichž probíhal výzkum GAPP2TM. I tak se ovšem týká cca pětiny všech nemocných s DM 2. typu léčených inzulinovými analogy v režimu bazál-bolus nebo pouze bazál. Při poslední nepravidelné aplikaci bazálního inzulínu se jednalo o záměr pacienta při úplném vynechání dávky ve 13 % případů, posun aplikace byl záměrný ve 23 % a snížení dávky inzulínu v 61 % případů. Nejčastěji hlášenými důvody bylo snížení rizika hypoglykemie a dodržování doporučení zdravotníků. Obavu z vynechání dávky bazálního inzulinu má 40 % českých pacientů a 35 % by mělo pocit viny, pokud by svou dávku bazálního inzulinu vynechali (u pacientů s intenzivním inzulinovým režimem je to 47 %). Pouze 60 % pacientů si uvědomuje, že vynechávání dávek bazálního inzulinu může mít negativní dopad na jejich dlouhodobý zdravotní stav. Dotazovaní lékaři měli podezření, že během pravidelných kontrol hlásí nižší počet vynechaných dávek bazálního inzulínu nebo vynechávání zcela zatajuje přibližně třetina pacientů. Tuto skutečnost však připustilo jen 11 % pacientů užívajících bazální inzulin a 15 % pacientů léčených intenzifikovaným režimem. Čtvrtina předepisujících lékařů navíc uvádí, že se svými pacienty nepravidelné užívání bazálního inzulinu během kontrol běžně neprobírá. Závěry: Ačkoli pacienti s diabetem 2. typu dodržují v České republice předepsané schéma léčby bazálním inzulinem častěji než v jiných zemích účastnících se průzkumu GAPP2TM, vynechání, časový posun a snížení dávky inzulinu je poměrně časté a ze strany zdravotníků si zasluhuje během pravidelných kontrol více pozornosti a cílenou edukaci., Objective: Irregular insulin dose is one of the main problems associated with insulin therapy in patients with type 2 diabetes; its extent is not known precisely. The aim of survey conducted in the Czech Republic in the international project GAPP2™ – Global Attitudes of Patients and Physicians was to determine the incidence and the impact of irregular use of basal insulin analogues in patients with type 2 diabetes, to point out the reasons for these irregularities and to focus on how physicians discuss irregular application of insulin with patients. Methods: The project GAPP2™ is an international cross-sectional study performed on-line via the Internet using a questionnaire filled by diabetic patients treated with insulin analogues and physicians who treat these patients. The survey was conducted in two steps in 17 countries; the first step included 6 countries and was completed in the beginning of 2012, the second step involved 11 other countries including the Czech Republic with termination in 2014. The survey was designed to obtain the views of patients and physicians on certain aspects of insulin treatment and persistent issues in this field in the real daily practice. Special focus was on the incidence and management of hypoglycaemia as well as on irregularities of insulin application. In the part dedicated to adherence to basal insulin application were observed three types of irregular insulin therapy: missed dose, time imprecision of dose (? 2 hours vs. the prescribed time) and dose reduction in all cases in the past 30 days before completing the questionnaire. In addition, it was investigated the attitude and relation of patients to these issues. Results: The results have shown that irregular insulin dose in the Czech Republic is less frequent than in other countries involved in the GAPP2™ research. Nevertheless, approximately one fifth of diabetic patients using insulin analogues in basal-bolus or only basal therapy regimen is related to this problem. The last irregular insulin application was due to missed dose in 13% of cases, time imprecision in 23% and reduction of dose in 61% of cases. The most commonly reported reason was risk reduction of hypoglycaemia and the recommendations of health professionals. Fear of missed dose is present in 40% Czech patients and 35% would feel guilty if their insulin dose is missed (up to 47% in patients with intensified insulin regimen). Only 60% patients are aware of negative impact on their long-term health after missed dose of basal insulin. Questioned doctors have suspected that the patients report lower number of missed doses during regular medical check because one third of patients doesn´t admit missed dose. However, this fact conceded only 11% of patients on basal insulin and 15% of patients on intensified insulin therapy. Quarter of prescribing doctors admit that they usually don´t discuss with patients irregularities in basal insulin treatment. Conclusion: Although, type 2 diabetes patients in the Czech Republic follow prescribed basal insulin therapy scheme more often than patients in other countries participating in the survey GAPP2 ™, missed dose, time imprecision and reduction of dose is quite common and it deserves more attention from medical side during regular medical check together with appropriate education of patients., and Martin Prázný
Úvod: V posledních letech prošla více než 20 let sloužící Atlantská klasifikace akutní pankreatitidy (AP) revizí. Přesto se prognostické ukazatele AP neustále hledají. Naše studie se pokouší zhodnotit odůvodnění 3 nově zavedených stupňů tíže revidované klasifikace akutní pankreatitidy a přínos 2 jednoduchých krevních laboratorních testů jako markerů rizika úmrtí a těžkého průběhu AP. Metody: Do studie bylo zahrnuto a sledováno celkem 159 pacientů s AP. Pacienti byli rozděleni do skupin podle Revidované Atlantské klasifikace a jednotlivé skupiny byly hodnoceny podle počtu úmrtí, délky hospitalizace a počtu stavem vyžadovaných intervencí včetně operací pankreatu. Počet leukocytů a distribuční šíře červených krvinek (RDW – red blood cell distribution width) v první den hospitalizace byly hodnoceny z hlediska prognózy AP. Výsledky: Podíl úmrtí, délka hospitalizace a počty operací pankreatu se statisticky významně lišily mezi všemi jednotlivými skupinami tíže podle Revidované Atlantské klasifikace. Počty bílých krvinek a RDW v 1. den hospitalizace byly identifikovány jako významné prediktory těžké AP a RDW také jako významný prediktor úmrtí. Souhrn: Nově zavedené kategorie lehké, středně těžké a těžké AP podle pravidel Revidované Atlantské klasifikace dobře vystihují prognózu, délku hospitalizace a riziko intervencí u nemocných. Počet bílých krvinek a RDW v první den hospitalizace se jeví jako možný adept mezi ukazatele k predikci úmrtí a rozvoje těžké formy AP., Introduction: In the last few years the Atlanta classification of acute pancreatitis (AP) have been revised. However prognostic markers of AP are still being searched for. The aim of this study is to validate the 3 severity cathegories proposed by the revised Atlanta classification. We also tried to reevaluate the association between two laboratory markers (leucocyte count and RDW – red cell distribution width) on admission and prognosis of the patients with AP. Methods: 159 patients were included into the study. The patients were classified according to revised Atlanta criteria and the subgroups evaluated according to mortality, lenght of hospital stay and need for interventions. Leucocyte count and RDW on admission was evaluated in the patients. Results: All the subgroups of patients were associated with significantly relevant differences in mortality, lenght of hospital stay and need for operations on pancreas. Leucocyte count and RDW were identified as significant predictors for severe AP and RDW was also identified as significant predictor for mortality in patients with AP. Conclusion: New categories of severity as defined by Revised Atlanta classification are describing well the mortality, lenght of hospital stay and need for interventions in the patients with AP. Cíl: Převod Testu paměti pro záměry (Memory for Intentions (Screening) Test; MIST, 2010) do české verze zahrnoval standardizaci pomůcek, převod administrace a skórování, validační studii na české populaci, aby MIST bylo možné použít na české populaci. Úvod: Konstrukt prospektivní paměti (PP), tj. schopnosti si zapamatovat a realizovat záměry, je nezbytný pro uchování soběstačnosti, vykonávání aktivit denního života. V posledních letech hraje čím dál významnější roli pro diagnostiku poruch paměti. Standardizovaná a validovaná měřítka PP však v české diagnostice chybí. Metodika: Český převod MIST jsme administrovali 30 zdravým osobám. Výsledky: Celkový skór PP v testu MIST koreloval na střední úrovni s měřítky z neuropsychologické baterie zejména s retencí v paměti, mentální flexibilitou, odolností k interferenci a premorbidní inteligenční úrovní (všechna rho = 0,37–0,42, všechna p < 0,05). Reliabilita testu MIST ve smyslu vnitřní konzistence byla u základních osmi subškál nedostatečná (α = 0,50) i ve smyslu reliability obou polovin testu (split‑half reliabilita = 0,56), avšak reliabilita šesti subškál tříděných dle typu (oddálení, vodítka a modality odpovědi) byla vysoká (α = 0,88; split‑half = 0,95). Introduction: Prospective memory (PM), i.e., the ability to remember and implement intentions after a delay, is essential as a subsystem of episodic memory for the maintenance of independence and execution of activities of daily living. PM assessment thus plays an important role in the diagnosis of episodic memory disorders. However, there are currently no standardized and validated PM tools in Czech language. Methods: The Czech version of MIST was administered to 30 healthy persons. Results: The MIST Summary score correlated at a medium level with a range of neuropsychological measures including memory retention, mental flexibility, and resistance to interference (all rho = 0.37–0.42; all p < 0.05). The reliability of MIST in terms of internal consistency was insufficient when analyzing the eight individual MIST trials (α = 0.50), as was split‑half reliability (split‑half reliability = 0.56). In contrast, there was a high degree of reliability between six subscales classified by type (delay, cue and mode of response; α = 0.88, split‑half = 0.95). Conclusion: The reliability and validity of the Czech version of MIST is comparable to the original English version. Psoriasis patients are often susceptible to cardiovascular diseases (CVD), including atherosclerosis. Traditional markers (biochemical and inflammatory) and diagnostic tools could detect occlusive but not subclinical atherosclerosis. Carotid intima-media thickness (CIMT), has recently been recognised as a non invasive diagnostic tool for identification of premature atherosclerosis. Therefore we evaluated 80 psoriasis patients and 80 age sex matched healthy controls for serum leptin levels and apolipoprotein B/apolipoprotein A-I ratio (apoB/apoA-I ratio) in relation with CIMT of carotid artery. Carotid intima-media thickness and carotid plaques were simultaneously measured by carotid sonography. Serum concentration of leptin and apolipoprotein were measured using enzyme-linked immuno sorbent assay (ELISA) and nephelometry respectively. Raised CIMT correlated to age of onset of the disease, serum leptin and apoB/apoA-I ratio in psoriasis patients. Taking into account, values that were above the 75 percentile of the three markers (leptin, apoB/apoA-I ratio and CIMT) the odds ratio was 4.26 (2.06-8.80 CI). Leptin and apoB/apoA-I ratio showed significant cumulative association with CIMT. Results of predictive analysis supports measurement of CIMT along with estimation of serum leptin and apoB/apoA-I ratio for prediction of premature atherosclerosis in psoriasis patients. and K. Cíl: Zjistit prevalenci celiakie u dospělých pacientů s diabetes mellitus 1. typu (DM1T). Vliv nově zahájené léčby celiakie na kompenzaci diabetu a body mass index (BMI) pacientů. Chování protilátek proti tkáňové transglutamináze (atTG) po roce od zahájení léčby. Metodika: Retrospektivní hodnocení cíleného screeningu na celiakii v období 1. 1. 2007–1. 7. 2011 u 465 dospělých pacientů s DM1T v Diabetologickém centru I. interní kliniky LF UK a FN Plzeň (80 % všech pacientů s DM1T). Při pozitivitě atTG-A (popř. atTG-G) byla indikována enterobiopsie. U pacientů s nově indikovanou bezlepkovou dietou byl porovnán HbA1c a BMI v průběhu roku před stanovením diagnózy celiakie vs období 1 roku po 6 měsících od zahájení léčby (3–4 kontroly). Po roce od zahájení léčby byla kontrolována hladina atTG. Ke statistice byl použit párový t-test. Výsledky: Prevalence všech forem celiakie v souboru byla 10,5 %. 1,1 % pacientů (5 ze 465) mělo diagnostikovanou celiakii v dětství. Nově byla pozitivita atTG zachycena u 9,5 % (44 ze 465) pacientů – indikováni k enterobiopsii. Z toho 3 pacienti s atTG > 300 kIU/l odmítli enterobiopsii, u těchto pacientů je celiakie vysoce pravděpodobná a pro malkomplianci nešlo hodnotit vliv bezlepkové diety na BMI a HbA1c. 22 pacientů mělo potenciální formu celiakie (negativní histologie). Pozitivní nález z enterobiopsie byl zjištěn u 19 pacientů (4,1 %). Z podsouboru 22 pacientů (nově indikována bezlepková dieta), bylo nutné vyřadit další 3 pacienty (vývoj HbA1c a BMI byl ovlivněn prvozáchytem DM1T). Charakteristika podsouboru: 9 žen a 7 mužů, průměrný věk 38 ? 12 let, trvání diabetu 21 ? 13 let, celiakie diagnostikována 20,7 ? 13 let od záchytu DM1T. Během sledovaného období nedošlo ke statisticky významné změně bazální hodnoty HbA1c 67 ? 11,4 vs 69 ? 13,9 mmol/mol po sledování a významně se zvýšil BMI z 25,4 ? 4,2 na 25,9 ? 4,3 (p < 0,01). Po roce přetrvávala pozitivita atTG u 47 % (9 z 19) pacientů. Závěr: V souboru dospělých pacientů s DM1T byla zjištěna celková prevalence celiakie 10,5 %. Po zahájení léčby nedošlo k významné změně HbA1c, došlo k významnému vzestupu BMI. Pozitivita atTG přetrvávala po roce u 47 % pacientů., Objective: To assess the prevalence of celiac disease in adult patients with type 1 diabetes mellitus (T1DM). Influence the new started treatment of celiac disease on glycemic control and body mass index (BMI) of the patients. Prevail the anti-transglutaminase antibody (atTG) positivity one year after commencement of the therapy. Methods: A retrospective assessment of celiac disease targeted screening in 465 adult T1DM patients at Diabetes Center, 1st Medical Department, University Hospital in Pilsen (80 % of all T1DM patients) from 1. 1. 2007 until 1. 7. 2011. Enterobiopsy was indicated in case of atTG-A (or atTG-G) positivity. In patients with newly started gluten-free diet, HbA1c and BMI within a year after diagnosis of celiac disease were compared to a year period six months after treatment commencement (3–4 visits), atTG was evaluated one year after treatment beginning. Paired T-test was used for statistical evaluation. Results: The prevalence of all forms of celiac disease in the studied group was 10.5 %. Celiac disease diagnosed in childhood was found in 1.1 % patients (5/465). Positivity of atTG was newly observed in 9.5 % (44/465) patients. Three patients with atTG > 300 kIU/l refused the enterobiopsy examination. Celiac disease is highly plausible. The influence of gluten-free diet on BMI and HbA1c could not be evaluated due to the lack of compliance. 22 patients had a potential form of celiac disease (negative histology). Positive enterobiopsy was found in 19 patients (4.1 %). Another 3 patients had to be excluded from the subgroup of 22 patients (newly indicated gluten-free diet) as the HbA1c values and BMI were affected by the primary diagnosis of T1DM. Subgroup characteristics: 9 women and 7 men, mean age 38 ? 12 years, diabetes duration 21 ? 13 years, celiac disease diagnosed 20.7 ? 13 years since first diagnosis of T1DM. No statistically significant change in HbA1c (67 ? 11.4 vs 69 ? 13.9 mmol/mol) was observed in the studied period, however and a significant change of BMI from 25.4 ? 4.2 to 25.9 ? 4.3 (p < 0.01) was found. The atTG positivity prevailed in 47 % (9/19) of patients after one year. Conclusion: A total prevalence of the celiac disease in the group of adult T1DM patients was 10.5 %. No significant change in HbA1c occurred following treatment, a significant change of BMI was observed. Úvod: PARADIGM HF srovnávala inhibitor angiotenzinového receptoru a inhibitor neprilysinu LCZ696 s enalaprilem u nemocných se srdečním selháním a sníženou ejekční frakcí. Metodika: Jednalo se o dvojitě slepou studii u 8 442 nemocných se srdečním selháním NYHA klasifikace II, III a IV a ejekční frakcí pod 40 %, kteří dostávali LCZ696 (2 × 200 mg) nebo enalapril (2 × 10 mg) přidané ke standardní medikaci. Primární cíl byl smíšený – kardiovaskulární mortalita a hospitalizace pro první srdeční selhání. Studie byla naplánována tak, aby odhalila rozdíl v kardiovaskulární mortalitě. Výsledky: Studie byla ukončena předčasně podle předepsaných pravidel při průměrné době sledování 27 měsíců pro jasný prospěch z léčby LCZ696. V době ukončení se primární cíl vyskytl u 914 nemocných (21,8 %) ve LCZ696 skupině a u 1 117 nemocných (26,5 %) ve skupině léčené enalaprilem (HR LCZ696 skupina, 0,80; 95% CI, 0,73–0,87; p < 0,001). Celkem 711 nemocných (17,0 %) léčených LCZ696 a 835 nemocných (19,8 %) léčených enalaprilem zemřelo (HR pro úmrtí z jakékoli příčiny 0,84; 95% CI, 0,76–0,93; p < 0,001); z těchto nemocných 558 (13,3 %) a 693 (16,5 %) zemřelo z kardiovaskulární příčiny (HR 0,80; 95% CI, 0,71–0,89; p < 0,001). LCZ696 ve srovnání s enalaprilem snižoval riziko hospitalizace pro srdeční selhání o 21 % (p < 0,001) a snižoval symptomy srdečního selhání (p = 0,001). Ve skupině léčené LCZ696 bylo více hypotenzí a nezávažných angioedémů, ale menší výskyt renálního selhání, hyperkalemie a kašle než ve skupině léčené enalaprilem. Závěr: LCZ696 byl účinnější než enalapril ve snížení rizika kardiovaskulárního úmrtí a hospitalizací pro srdeční selhání. Methods: In this double‑blind trial, 8442 patients with class II, III, or IV heart failure and an ejection fraction of 40% or less were assigned to receive either LCZ696 (at a dose of 200 mg twice daily) or enalapril (at a dose of 10 mg twice daily), in addition to the standard therapy. There were two primary outcomes – mortality from cardiovascular causes and hospitalisation for heart failure, but the trial was designed to detect the differences in the rates of mortality from cardiovascular causes. Results: In line with the pre‑specified rules, the trial was stopped early, after a median follow‑up of 27 months, because the boundary indicating a clear benefit achieved with LCZ696 had been exceeded. At the time of study closure, the primary outcome had occurred in 914 patients (21.8%) in the LCZ696 group and 1117 patients (26.5%) in the enalapril group (hazard ratio in the LCZ696 group, 0.80; 95% confidence interval [CI], 0.73 to 0.87; P < 0.001). A total of 711 patients (17.0%) receiving LCZ696 and 835 patients (19.8%) receiving enalapril died (hazard ratio for death from any cause, 0.84; 95% CI, 0.76 to 0.93; P < 0.001); of these patients, 558 (13.3%) and 693 (16.5%), respectively, died from cardiovascular causes (hazard ratio, 0.80; 95% CI, 0.71 to 0.89; P < 0.001). As compared with enalapril, LCZ696 also reduced the risk of hospitalisation for heart failure by 21% (P < 0.001) and decreased the symptoms and physical limitations of heart failure (P = 0.001). The LCZ696 group showed higher proportions of patients with hypotension and non‑serious angioedema but lower proportions of patients with renal impairment, hyperkalemia, and cough when compared to the enalapril group. Conclusions: LCZ696 was more effective than enalapril in reducing the risk of death and of hospitalisation for heart failure. Retrospektivní studie u žen do 49 let analyzovala novotvary následující po primárních nádorech cervixu (CC), dělohy (CU), vaječníků (CO) a primární novotvary před těmito gynekologickými nádory (GC), hlášené v Národním onkologickém registru ČR v letech 1976–2010. Celkem 3 727 pacientek těchto tří diagnóz představovalo 12,1 % z 30 794 nově evidovaných GC, spojených s 4 160 dalšími novotvary. Jejich vývoj se týkal a) 1 339 žen s primárním CC, 715 s CU, 664 s CO (s následnými 1 509 a 831, respektive 758 dalšími novotvary), b) 263 žen s následným CC, 212 s CU, 534 s CO (s předcházejícími 274 a 223, respektive 565 primárními novotvary). Průměrný interval vzniku následných novotvarů byl 14,2 roku po CC, 13,6 roku po CU a 10,2 roku po CO. Nejvíce z 3 098 následných nádorů bylo 21 % trávicího traktu, 16 % prsů, 13 % kůže, 11,7 % rodidel, 9,7 % dýchacích a 7,8 % močových cest; z nich během prvního roku po primárním GC bylo nejvíce nádorů vaječníků, endometria, cervixu a kolorekta. Nejvíce z 1 063 primárních novotvarů před GC bylo 26,7 % rodidel, 24,3 % prsů a 14,8 % trávicího traktu. Během 35 let počet žen s primárními GC klesal, zejména v zastoupení jejich časných klinických stadií, žen s následnými GC rostl, zejména jejich pokročilých stadií. Časná stadia byla evidována u 70,4 % žen s primárními GC a 54 % s následnými GC, pokročilá stadia u 29,5 % žen s následnými GC a 13,3 % s primárními GC, neznámá stadia zahrnovala asi 16,5 %. Ze studie se odhaduje asi 1100 případů pokročilých stadií u žen do 49 let přežívajících s GC v letech 1976–2010. Většina z hodnocených kritérií byla nepříznivá u žen s karcinomy ovarií. Zejména těmto případům v pokročilých stadiích by měly zabránit dispenzární opatření a preventivní intervence. Omezené finanční zdroje a přetrvávající rizika životního stylu neumožňují zpomalit v blízké budoucnosti nárůst primárních a následných novotvarů u onkologicky přežívajících žen nejen v gynekologii., The neoplasms following primary cancers of cervix (CC), uterus (CU), ovary (CO), and primary neoplasms before these gynaecological cancers (GC), were analyzed in a retrospective study in survived females aged 0–49 years, notified in the National Czech Cancer Registry between 1976 and 2010. A total 3,727 patients of these three diagnoses, presented 12.1 % of 30,794 newly registered GC, were associated with 4,160 other neoplasms. Their development was concerned with a) 1,339 females with primary CC, 715 with CU and 664 with CO (followed by 1,509 and 831, respectively 758 other neoplasms) and b) subsequent 263 CC, 212 CU and 534 CO (preceded by 274 and 223, respectively 565 primary neoplasms). The average interval of occurrence of subsequent neoplasms was 14.2 years after CC, 13.6 years after CU and 10.2 years after CO. The most frequent of 3.098 subsequent neoplasms were 21 % of digestive tract, 16 % breast, 13 % skin, 11.7 % female genital, 9.7 % respiratory and 7.8 % urinary organs; the most frequent during the first year after primary GC were those of the ovarial, endometrial, cervical and colorectal cancers. The most frequent of 1,062 primary neoplasms before GC were 26.7 % cancers of female genital organs, 24.3 % breast and 14.8 % digestive tract. During 35 years the number of females with primary GC has decreased, especially of their representation at the early clinical stages, the females with subsequent GC has increased, especially at the advanced stages. There were recorded early stages in 70.4 % females with primary GC and 54 % with subsequent GC, advanced stages in 29.5 % females with subsequent GC and 13.3 % with primary GC, the unknown stages included about 16.5 %. In our study are estimated about 1100 cases of advanced stages in females aged 49 years survived with GC in 1976–2010. Most of the evaluated criteria were unfavorable among females with ovarian cancers. Mainly the cases at advanced stages must be prevented by the dispensary guidelines and preventive interventions. Cíl: Správný pooperační režim je důležitou součástí rekonvalescence po dekompresi karpálního tunelu. Cílem této prospektivní randomizované studie je zjistit, zda pooperační dočasné dlahování má klinicky významný efekt, nebo zda není naopak zbytečné. Výsledky dosud publikovaných prací nedávají jasné, prokazatelné a jednoznačné doporučení. Soubor a metodika: Do souboru bylo zařazeno celkem 72 pacientů s klinicky a elektromyograficky prokázaným syndromem karpálního tunelu, kteří byli randomizováni do dvou skupin. V první skupině byla pacientům po operaci na dva týdny přiložena dorzální dlaha fixující zápěstí v neutrální poloze a v druhé skupině dlaha použita nebyla. Před operací i po operaci byla hodnocena distální motorická latence, síla stisku a míra subjektivních obtíží pomocí specializovaných dotazníků DASH (Disabilities of Arm, Shoulder and Hand Questionnaire) a Bostonského dotazníku (Boston Carpal Tunnel Questionnaire). Výsledky: Po operaci došlo ke zkrácení distální motorické latence a mírnému snížení síly stisku, ale mezi skupinami nebyly nalezeny statisticky významné rozdíly (p > 0,05). Po operaci došlo také k předpokládanému zmírnění subjektivních obtíží, objektivizovanému dotazníkovými koeficienty, ale mezi skupinami nebyl nalezen statisticky významný rozdíl (p > 0,05). Závěr: Podle našich zjištění dočasné dlahování po otevřené dekompresi karpálního tunelu ve srovnání s nedlahovanou skupinou nezlepšuje výsledky chirurgické léčby. Používání dlahy v této indikaci se jeví jako nadbytečné jak z lékařského, tak z ekonomického pohledu. Na druhou stranu dočasné dlahování po operaci výsledky operační léčby nezhoršuje., Aim: Correct post-operative care is an important part of recovery after carpal tunnel decompression. The aim of this prospective randomized study was to determine whether postoperative temporary splinting has a clinically significant effect, or whether it is unnecessary. The results of published data do not give clear, demonstrable and unambiguous recommendations. Material and methods: A total of 72 patients with clinically and electrodiagnostically established diagnosis of carpal tunnel syndrome were randomized into two groups. The first group included patients with a dorsal splint, fixing the wrist in a neutral position for two weeks after surgery, whereas the second group included patients, in whom the splint was not used after surgery. Distal motor latency, grip strength and degree of subjective symptoms were assessed before and after surgery using specific questionnaires, namely the DASH questionnaire (Disabilities of Arm, Shoulder and Hand) and the Boston questionnaire (Boston Carpal Tunnel Questionnaire). Results: After surgery, there was a reduction of distal motor latency and a moderate reduction of grip strength, but there were no statistically significant differences (p > 0.05) between the two groups. After surgery, the anticipated improvement of subjective symptoms was achieved, objectified by questionnaire coefficients. However, there was no statistically significant difference (p > 0.05) between the two groups either. Conclusion: According to our findings, temporary splinting after open carpal tunnel decompression does not improve the results of surgical treatment compared to the non-splinted group. Using the splint in this indication seems to be redundant from the medical and economic point of view. On the other hand, temporary splinting after surgery does not cause worsening of the results of surgical treatment. Background: Few data are available on the specific care giving-related problems of stroke patient’s caregivers and factors that influence the burden of these caregivers. Aim: To study the influences of the active rehabilitation process on anxiety, depression, care burden and perceived social support level of stroke patients caregivers. Design: A prospective clinical trial. Setting: Patients and caregivers entering a rehabilitation program at a university hospital in Turkey. Populations: Ninety patients with a first episode of stroke and 90 caregivers responsible for their care were recruited for our study. Methods: Patients and caregivers were assessed before and after the active rehabilitation process. The functional disability level of the patients was assessed by Functional Independence Measure (FIM). The Beck Anxiety Scale (BAS) and the Beck Depression Scale (BDS) were used for anxiety and depression assessment, the Zarit Care Burden Scale (ZCBS) for care burden assessment and the Multi-Dimensional Scale of Perceived Social Support (MDSPSS) for perceived social support assessment. Results: A statistically significant rise is observed in the special person sub-assessment of MDSPSS in both female and male caregivers. Also, a significant decrease in care burden, anxiety and depression levels of caregivers was noted after the rehabilitation program (p < 0.05). Conclusion: Caregivers accept the rehabilitation period as important social support in addition to the support provided by family and friends. Also, our positive results were associated with an improvement in the patients’ functional level and an increase in the acquisition of knowledge and skill required of caregivers in order to provide care during rehabilitation. Clinical Rehabilitation Impact: The rehabilitation team should be aware of the fact that the perceived care burden may be greater due to the lack of knowledge concerning available resources and due to the inability to cope with stress effectively. and A.
