Laryngeal electromyography is used in the evaluation of vocal cord paralysis to confirm the diagnosis, to guide the diagnostic work-up for etiology, to provide prognostic information and to help choose the correct treatment for the patient. Cardiovocal syndrome is characterised by vocal cord paralysis due to a cardiovascular disease. A wide spectrum of conditions can result in this syndrome. Here we present a case of cardiovocal syndrome in association with primary pulmonary hypertension. Aspergillus sp. are ubiquitous mould infections and in most patients, the source is presumed to be air-borne infections during surgical procedures. Prevention of these infections requires special attention of ventilation systems in operating rooms. Post-operative aspergillosis occurs mainly in immunocompromised patients as well as those who receive corticosteroids temporarily. Capsular bag distension syndrome is a very rare complication of cataract surgery. Most cases occur after uneventful phacoemulsification with continuous curvilinear capsulorhexis and implantation of an intraocular lens in the capsular bag. The entity presents with reduction of visual acuity and myopic shift in the early-to-late postoperative period. Characteristic findings include the distension of the capsular bag due to the accumulation of milky material and the forward displacement of the intraocular lens. We present two cases with an unusually delayed presentation of 6 and 8 years respectively following phacoemulsification, and describe their successful management with Nd:YAG laser posterior capsulotomy. and P. Bronchogenní karcinom je jedním z nejčastěji se vyskytujících zhoubných nádorů. Incidence v ČR u mužů sice mírně klesá, v roce 2009 to bylo 87,45 případů na 100 000 obyvatel, u žen dochází k neustálému nárůstu incidence, která v roce 2009 dosáhla 35,75 případů na 100 000 obyvatel. Téměř u 75 % pacientů je diagnóza stanovena ve stadiu lokálně pokročilého nebo generalizovaného onemocnění (st. IIIB, IV). V minulosti byli tito pacienti léčeni systémovou chemoterapií, v posledních letech byla do klinické praxe zavedena biologická léčba, která je zaměřena na buněčné signální cesty a jejich ovlivnění. Tyto léčebné přípravky jsou také uváděny pod názvem „cílená léčba“, protože reagují s přesně specifikovaným buněčným molekulárním cílem. U buněk řady solidních nádorů epiteliálního původu byla popsána zvýšená aktivace receptoru pro epidermální růstový faktor (EGFR1), proti jehož neřízené aktivaci je zaměřená cílená terapie nízkomolekulárními inhibitory tyrozinkináz (gefitinib, erlotinib), které, po navázání na protein, inhibují fosforylaci tyrozinkinázové části receptoru a vedou k následnému blokování signálních drah. V důsledku tohoto procesu dochází ke snížení mitotické aktivity buňky, zvýšení senzitivity k apoptóze a zpomalení, eventuálně zastavení, nádorového růstu. Iressa (gefitinib) byla autorizována pro terapii NSCLC 1.1.2009 na podkladě studií INTEREST a IPASS. Je indikována v 1. linii léčby lokálně pokročilého nebo generalizovaného nemalobuněčného bronchogenního karcinomu (NSCLC) u pacientů s pozitivní EGFR mutací (mutace zvyšující citlivost k léčbě v exonech 18 a 19, bodové mutace na exonu 21). Jde o perorální formu léčby s podáváním 1x denně 250 mg. Jedná se o první cílenou monoterapii, která prokázala lepší účinnost v porovnání s kombinovanou chemoterapií v 1. linii léčby pokročilého NSCLC s aktivní mutací EGFR-TK., Lung cancer is one of the most frequent malignant tumors. Its incidence in men in the Czech Republic, although slightly decreasing, in 2009, was 87.45 cases per 100000 inhabitants, in women there has been a continual increase in incidence, which, in 2009, reached 35.75 cases per 1000000 inhabitants. Almost 75 % of patients are diagnosed with locally advanced or metastatic disease (st.IIIB, IV). In the past, patients were treated with systemic chemotherapy, in recent years biologic treatment has been introduced into clinical practice which focuses on cellular signaling pathways and their influence. These therapeutic products are marked under the name of „targeted therapy“, because they react with precisely specified molecular target. The number of cells of solid tumors of epithelial origin has been reported to increase activation of the receptor for epidermal growth factor (EGFR), at which a targeted therapy of low molecular weight inhibitors of tyrosine kinases (gefitinib, erlotinib) is aimed. After binding to the protein they inhibit phosphorylation of the tyrosine kinase part of the receptor and lead to blocking subsequent signaling pathways. As a result of this process, mitotic activity of cells is reduced, sensitivity to apoptosis increases and the growth of the tumor slows down or, eventually, stops. Iressa (gefinitib) was authorized for the treatment of NSCLC on 1st January 2009, on the basis of INTEREST and IPASS trials. It is indicated in the first line treatment of locally advanced or metastatic non-small cell lung cancer (NSCLC) patients with positive EGFR mutation (mutations in exon 18 and 19, point mutation in exon 21). It is a form of treatment with oral administration of 250 mg once a day. Východiska: Monoklonální anti-EGFR protilátka cetuximab představuje cílenou biologickou léčbu, která se stává standardem personalizované medicíny nemocných s metastazujícím kolorektálním karcinomem. Stupeň kožního postižení je nejvýznamnějším prediktivním faktorem účinnosti cetuximabu. Formou klinické kazuistiky je prezentována možná léčba kožní toxicity cetuximabu. Klinická kazuistika: Autoři popisují případ 57leté pacientky s metastazujícím kolorektálním karcinomem, která byla léčena paliativní biochemoterapií ve schématu cetuximab+FOLFIRI. Šest týdnů od zahájení terapie došlo k progresi kožního nálezu a rozvoji papulopustulózního exantému III. stupně. Včasné zahájení lokální léčby na bázi aplikace krému s 0,1% vitaminem K1 v kombinaci se systémovou antibiotickou léčbou a antihistaminiky umožnila nemocné pokračovat v biologické léčbě. Závěr: Včasná dermatologická léčba nemocných s projevy kožní toxicity při terapii monoklonální protilátkou cetuximab umožní pokračování onkologické léčby bez přerušení tak, aby pacienti mohli mít z této léčby co největší užitek., Background: Cetuximab, a monoclonal anti-EGFR antibody, represents aimed biological treatment that becomes a standard of personalized medicine with metastatic colorectal carcinoma patients. Level of skin toxicity is the most significant predictive factor of cetuximab effectiveness. Possible dermal toxicity treatment with cetuximab application is presented in the form of a clinical case report. Case: Authors describe a case of a 57-year-old woman-patient suffering from metastatic colorectal carcinoma who was treated through palliative bio-chemotherapy in the schedule: cetuximab+FOLFIRI. Progression of the dermal disease and papulopustulous progress of the grade III exanthema were detected in a six-week-long period from the therapy initiation. Timely initiation of local treatment on the basis of cream with 0.1 % vitamin K1 application in combination with systematic antibiotic treatment and antihistamines application enabled the patient to continue with biological treatment. Plicní karcinomy jsou z onkologických onemocnění nejčastější příčinou smrti, nemalobuněčný plicní karcinom je nejčastějším histologickým typem plicního tumoru. Gefitinib je prostředkem biologické léčby pokročilého a metastatického nemalobuněčného plicního karcinomu u pacientů s aktivační mutací receptoru pro epidermální růstový faktor v první linii léčby, tzn. u chemonaivních pacientů. Kazuistika se zabývá dvěma případy pacientů léčených gefitinibem s podobnými vstupními parametry a odlišným efektem léčby., Lung carcinoma is the most common cause of death among cancer patients. Východiska: Americká společnost klinické onkologie (ASCO) vytvořila panel expertů a provedla systematický přehled dostupné literatury. Panel vytvořil konsenzuální doporučení expertů. Cílem naší práce je ve srovnání s tímto doporučením zhodnotit použitý léčebný postup. Klinická kazuistika: Uvádíme kazuistiku 32leté pacientky s karcinomem prsu, hormonálně dependentním, HER2 pozitivním, u které v průběhu adjuvantní léčby došlo ke generalizaci onemocnění do CNS (centrální nervový systém). Popsán průběh onemocnění, během kterého byly kombinovány různé modality lokální i systémové léčby, které vedly k přežití v délce 4 let od diseminace onemocnění. Léčebný postup byl ve shodě s ASCO doporučením, kombinoval při opakovaných recidivách uvedené možnosti lokální terapie, stereotaktickou radiochirurgii (SRS), ozáření celého krania (WBRT) a chirurgický výkon. U prezentované pacientky jsme použili systémovou léčbu, která byla v dané době dostupná, tedy trastuzumab, lapatinib, taxany, kapecitabin, vinorelbin a hormonální léčbu. V současnosti zavedení nových preparátů výrazně rozšířilo možnosti systémové léčby. Závěr: Provedený léčebný postup kombinace SRS a WBRT je ve shodě s doporučením pro danou velikost solitární metastázy do mozku. V oblasti systémové léčby se současné možnosti značně rozšířily díky zavedení nových preparátů HER2 terapie a provedená terapie odpovídá době její aplikace. Doporučení, která v roce 2014 byla prezentována v Journal of Clinical Oncology (JCO) považujeme za vysoce přínosná., Background: The American Society of Clinical Oncology (ASCO) has developed a panel of experts which conducted a systematic review of the available literature. Panel of experts developed consensus recommendations. The aim of our work is to compare guidelines and evaluate the used therapeutic procedure. Case: We present a case of 32 year old female with breast cancer, hormone dependent, HER2 positive. During adjuvant therapy occured metastatic lesion in the CNS (central nervous system). Course of the disease is presented, during which the various modalities were combined, local and systemic treatments were used, which led to the survival period of 4 years of disease from the time of dissemination. The treatment procedures were in accordance with ASCO recommendations, repeated relapses were treated with a variety of methods of local therapy (SRS WBRT and surgery). Currently the introduction of new drugs substantially expanded options for systemic therapy. Conclusions: Used treatment with a combination of SRS and WBRT is in line with the recommendation for a given size of solitary metastasis to the brain. In the area of systemic therapy the current options greatly expanded by the introduction of new drugs. Used antiHER2 therapy corresponds to the time of its application.
