Třicetiletá pacientka byla opakovaně léčena radiojodem pro papilární karcinom štítné žlázy, s okrsky patologické akumulace 131I v kalvě. V průběhu terapie nemocná podstoupila neurochirurgický zákrok s odstraněním těchto ložisek, které byly patologem hodnoceny primárně jako hemangiomy. Vzhledem následnému poklesu tyreoglobulinu až k negativním hodnotám bylo iniciováno opakované čtení histologického nálezu, který nakonec potvrdil přítomnost metastázy karcinomu štítnice. Pudendální neuralgie je špatně snesitelný bolestivý stav způsobený postižením (většinou kompresí) pudendálního nervu. Klinicky je charakterizován progresivní bolestí v pánvi, která se horší sezením, bývá spojena se sexuální dysfunkcí, potížemi s močením a defekací. Ke stanovení jisté diagnózy jsou používána nanteská kritéria. V této práci prezentujeme soubor šesti pacientů, kteří byli od roku 2009 léčeni pro toto onemocnění ve FN Brno, a popisujeme současné terapeutické možnosti a hodnotíme jejich efekt., Pudendal neuralgia is a very painful condition caused by compression or entrapment of the pudendal nerve. Clinical characteristics include progressive pelvic pain, worsened by sitting, sexual dysfunction and difficulty with urination and defecation. Nantes criteria are used to confirm pudendal neuralgia. We present a group of six patients treated for pudendal neuralgie at the University Hospital in Brno since 2009 and discuss the options for the treatment of this syndrome. Je prezentován případ 29leté pacientky, u které bylo zjištěno vzácné nádorové onemocnění – primární leiomyosarkom ledviny. Diagnóza byla stanovena u pacientky ve 32. týdnu gravidity pomocí ultrazvuku, nádor se projevoval výraznou bolestivostí levého boku, kvůli které byla pacientka odeslána na naši ambulanci. Při vyšetření byl zjištěn objemný tumor levé ledviny, který byl po porodu, indukovaném v dřívějším termínu, potvrzen i při vyšetření počítačovou tomografií (CT). Nádor byl úspěšně odoperován, histologicky byl verifikován nález leiomyosarkomu. Nyní je pacientka 19 měsíců od operačního výkonu bez nálezu progrese onemocnění či metastáz., We present a case of 29-year-old patient with diagnosis of a rare cancer: primary leiomyosarcoma of the kidney. The diagnosis was determined in the patient in 32th week of pregnancy using ultrasound. Tumour manifested with severe pain on left side, for which the patient was referred to our clinic. During examination an extensive tumour of the left kidney was detected, which was confirmed by CT scan after an early delivery. The tumour was successfully surgically removed. Histologic examination proved diagnosis of leiomyosarcoma. Nineteen months after the surgery the patient has no evidence of disease progression or metastasis., and Wasserbauer R., Ivičič J.
V článku popisujeme kazuistiky dvou pacientů s maligním a benigním primárním tumorem retroperitonea, které byly diagnostikovány na základě symptomatologie z progredujícího narůstaní objemu tumoru. U jednoho pacienta byla provedena punkční biopsie se suspekcí na low grade liposarkom. Pacient byl indikován k operační revizi. S odstupem sedmi měsíců od operace došlo k recidivě onemocnění a byla provedena exstirpace recidivy tumoru s adjuvantní chemoterapií. Po ukončení systémové léčby byl pacient pro druhou recidivu opět operován s neradikálním odstraněním tumoru a byl indikován k paliativní systémové léčbě. V druhém případě byla na základě zobrazovacích metod indikovaná adrenalektomie pro suspektní tumor nadledviny. Definitivní histologie vyloučila afekci nadledviny a popsala ganglioneurom retroperitonea., We decribe a case reports of two patients with benign and malignant primary retroperitoneal tumors that presented with symptoms due to of the progressive tumor growth. The recurrence of the tumor occurs in seventh months post-surgery, at which point the tumor was excised and followed by postoperative chemotherapy. Upon completion the patient underwent an additional surgery for a second recurrence. Incomplete removal of the tumor was performed, and palliative systemic therapy followed. In the second case, a patient was indicated to adrenalectomy for suspicion of adrenal tumor based on finding from imaging methods. Low-grade myofibroblastic sarcoma (LGMS) is a very rare, atypical myofibroblastic tumor with fibromatosis-like features with predilection mostly in head and neck region. LGMS occurs primarily in adult patients with a slight male predominance. Only few cases of LGMS affecting the larynx have been reported in literature to this date. We describe a case of low-grade myofibroblastic sarcoma of the larynx in a 40-year-old male patient. Congenital malformations such as lumbosacral transitional vertebrae and spina bifida occulta constitute unrare anomalies and could affect the symptomatology of low back pain. A transitional vertebra is characterized by elongation of one or both transverse processes, leading to the appearance ofa sacralized fifth lumbar vertebra or a lumbarized first sacral vertebra. Furthermore, sacral spina bifida occulta is a developmental anomaly that corresponds to the incomplete closure of the vertebral column. In the present case report, we describe a case of a dried sacrum presenting a partially sacralized fifth lumbar vertebra and total spina bifida, extended from first to fifth sacral vertebra. A pseudoarthrosis is formed on the left side and the specimen could be incorporated in Castellvi's type IIa. Maligní mezoteliom je vzácný nádor, jehož incidence je 0,5−3 případy na milion obyvatel. Pouze přibližně ve 20 % případů se vyskytuje na peritoneu. Autoři na příkladu kazuistiky nemocného s výskytem maligního mezoteliomu v peritoneální dutině dokladují obtížnost předoperační diagnostiky, kdy dle předoperačních vyšetření bylo nejprve pomýšleno na primární tumor jater a kdy teprve definitivní histologické vyšetření stanovilo diagnózu. V diskuzi se pak věnují epidemiologii, diagnostice a léčbě tohoto raritního onemocnění a zdůrazňují nezastupitelnou roli chirurgického řešení při jeho léčbě. Only about 20% of cases occur in the peritoneum. The authors use the case report of a male patient with malignant mesothelioma occurring in the peritoneal cavity to demonstrate the difficulty of preoperative diagnosis: preoperative examinations made the authors consider a primary liver tumour and only definitive histological examination revealed the diagnosis. The discussion deals with the epidemiology, diagnosis and treatment of this rare disease and underlines the unique role of surgical therapy in its management. Závažný defekt koagulačného faktora XII (FXII) je veľmi zriedkavou, záhadnou a málo známou vrodenou poruchou. Na rozdiel od defektu ostatných koagulačných faktorov je úplne asymptomatický. Prekvapivo nevedie k abnormálnemu krvácaniu, dokonca ani pri rozsiahlejších chirurgických výkonoch. Vysvetlenie pre chýbanie krvácavých prejavov nie je známe. Predpokladá sa, ale nie je to dokázané, že pacienti nie sú dostatočne chránení pred trombózou. Defekt FXII sa obyčajne odhalí náhodou rutinným hemokoagulačným vyšetrením uskutočneným pred operáciou. Keďže FXII zohráva dôležitú úlohu pri tvorbe koagula in vitro, jeho defekt spôsobuje pri laboratórnom vyšetrení výrazné predĺženie aktivovaného parciálneho tromboplastínového času. Hlavným problémom spojeným s defektom FXII sú potom ďalšie zbytočné vyšetrenia, strata času pri poskytovaní zdravotnej starostlivosti a obavy z chirurgických výkonov, ktoré môžu vyplývať z abnormálneho laboratórneho výsledku. Kľúčové slová: aktivovaný parciálny tromboplastínový čas – faktor XII – hemokoagulácia – krvácanie, Severe coagulation factor XII (FXII) deficiency is a very rare, mysterious and not well known inherited condition. Unlike other coagulation factor deficiencies, it is totally asymptomatic. Surprisingly, it does not lead to abnormal bleeding, even with major surgical procedures. The explanation for the lack of bleeding manifestations is unknown. It is suggested, but unproven, that patients are not sufficiently protected from thrombosis. FXII deficiency is usually discovered by accident through a routine coagulation testing done prior to surgery. Since FXII plays an important role in clot formation during in vitro measurements, its deficiency causes a marked prolongation of the activated partial thromboplastin time in the laboratory examination. The main concern related to FXII deficiency is the unnecessary testing, delay in health care and worry of surgical interventions that may be prompted by the abnormal laboratory result. Krvácení do trávicího traktu je jednou z náhlých příhod břišních, se kterou se setkáváme relativně často. Většina těchto příhod je v současné době řešitelná endoskopicky, urgentní chirurgickou revizi si žádají spíše výjimečně. Přibližně 5 % krvácení do GIT je skrytých v jejunoileální oblasti. Předložená kazuistika masivního krvácení z tenkého střeva přináší současný názor na diagnostiku a léčebné možnosti z pohledu každodenní klinické praxe službu konajících chirurgů. Most cases can nowadays be managed endoscopically, surgery is rarely required. Approximately 5% of gastrointestinal bleeding cases are cases of so-called obscure gastrointestinal bleeding. The presented massive gastrointestinal bleed case report provides a current view on diagnostic and therapeutic modalities in the context of everyday clinical practice. OBJECTIVES: Osseous dysplasias are the most common subtype of fibro-osseous lesions of the maxillofacial bones. They are benign and often present as incidental asymptomatic lesions. Diagnosis can be made with clinical and radiographic examination. CASE REPORT: This article reports the case of a 47-year-old man with a huge cementifying osseous dysplasia of the maxilla that presented with secondary infection after teeth extraction and repair of oroantral fistula. A subtotal maxillectomy had to be done after conservative treatment failed to resolve the infection. DISCUSSION/CONCLUSION: Biopsy, tooth extraction or surgical trauma to the affected bones of osseous dysplasia could easily lead to severe secondary infection, the treatment of which may be very difficult, and thus should be avoided if possible. and S.
