Metastatický melanom je agresivní onemocnění. Dosud využívané léčebné možnosti jako aplikace dakarbazinu, temozolomidu, high dose interleukinu-2 byly relativně omezené. Vemurafenib je malá molekula, která má schopnost inhibovat mutovanou BRAF proteinkinázu. We assessed the intensive care unit (ICU) patients for Invasive aspergillosis (IA) with culture and non-culture based diagnostic methods from Iran. Thirty-six ICU patients with underlying predisposing conditions for IA were enrolled in the study. Sixty eight Bronchoalveolar lavage (BAL) samples were collected by bronchoscope twice a weekly. BAL samples were analyzed by microscopic examination, fungal culture and galactomannan (GM) detection. The Platelia Aspergillus GM EIA was used to quantify GM indices. Samples with a BAL GM index > or = 1 were considered as positive for GM. Patients were classified as having probable or possible IA. Out of 36 suspected patients to IA, 36.1% of cases showed IA which were categorized as: 4 cases of possible IA and 9 of probable IA. 76.2% of BAL samples were positive for GM. From 13 patients with IA, 11 (84.6%) had at least one positive BAL GM index. Of these patients, 9 (81.8%) showed probable IA. The main underlying predisposing conditions were neutropenia (53.8%) and COPD (30.8%). Our study has indicated that COPD must be considered as one of the main predisposing condition for occurrence of aspergillosis in ICU patients. Cíl. Ischemické cévní mozkové příhody (iCMP) jsou stále jednou z nejčastějších příčin mortality a morbidity postihující i mladší populaci v produktivním věku. Klíčovým faktorem ovlivňujícím prognózu po iCMP je včasná rekanalizace uzavřené mozkové tepny s obnovením perfuze v postižené mozkové tkáni. Cílem práce bylo zhodnocení bezpečnosti a efektivity kombinované revaskularizační terapie sestávající z iniciální intravenózní trombolýzy a přímo navazující mechanické trombektomie u pacientů s akutní iCMP a uzávěrem mozkové tepny. Metoda. Do souboru pacientů byli zařazeni všichni po sobě jdoucí pacienti s akutní iCMP a prokázaným uzávěrem střední mozkové tepny v úseku M1-2 nebo distální části arteria carotis interna nebo uzávěrem arteria basilaris, a to v období od června 2010 do dubna 2012. Všichni nemocní byli léčeni kombinovanou revaskularizační terapií, tzn. iniciální intravenózní trombolýzou a přímo navazující mechanickou trombektomií systémem Solitaire. Iniciální stupeň neurologického deficitu byl stanoven pomocí NIHSS skóre. Výsledný klinický stav byl zhodnocen po 90 dnech pomocí modifikované Rankinovy škály (mRS). Výsledky. Do souboru bylo podle kritérií zařazeno 50 pacientů (průměrný věk 66, 8 ? 14,6 roku) se vstupním mediánem NIHSS skóre 18,0 (8-32). Rekanalizace (TICI 2A, B a 3) bylo dosaženo u 94 % pacientů, kompletní rekanalizace (TICI 3) u 72% pacientů. Průměrný rekanalizační čas (čas od začátku symptomů do dosažení maximální rekanalizace) byl v našem souboru 244,2 ? 87,9 minut. Symptomatické intracerebrální krvácení se vyskytlo u 6 % pacientů. Medián mRS po 90 dnech byl v našem souboru 1, 60 % pacientů mělo devadesátidenní klinický výsledek dle mRS hodnocen jako dobrý (mRS 0-2). Tříměsíční mortalita byla ve sledovaném souboru 14 %. Závěr. Kombinovaná revaskularizační terapie akutní iCMP sestávající z iniciální intravenózní trombolýzy a přímo navazující mechanické trombektomie je bezpečná a efektivní. Okamžité provedení mechanické tromektomie po iniciální intravenózní trombolýze vede k výraznému zkrácení rekanalizačního času, který je rozhodující pro dobrý klinický výsledek u pacientů intervenovaných pro akutní ischemickou cévní mozkovou příhodu., Aim. Ischemic stroke is today one of the leading causes of mortality and morbidity and affects even a younger population of working age. A key factor influencing prognosis after ischemic stroke is early recanalization of close cerebral artery and reperfusion in the affected brain tissue. The aim of our study is to evaluate the safety and effectiveness of re-vascularization combined therapy consisting of initial intravenous thrombolysis and immediately after that performed mechanical thrombectomy in the treatment of patients with acute ischemic stroke. Methods. Consecutive non-selected acute ischemic stroke patients with baseline CTA or MRA documented occlusion of middle cerebral artery (MCA) (segment M1-2) or distal part of internal carotid artery (ICA) or basilar artery (BA) and treated between June 2010 and April 2012 at our stroke center were included in the study. All patiens were treated by combined therapy consisting of initial intravenous thrombolysis and immediately after that performed mechanical thrombectomy by Solitaire stent. Stroke severity was assessed using National Institutes of Health Stroke Scale (NIHSS), 90-day clinical outcome using modified Rankin Scale (mRS). Results. Fifty enrolled patients (mean age 66.8 ? 14.6 years) had baseline median NIHSS 18.0 (8-32). Overall recanalization (TICI 2A, B and 3) was achieved in 94% of patients, complete (TICI 3) in 72% of patients. The mean time from stroke onset-to-maximal recanalization was 244.2 ? 87.9 min. Symptomatic intracerebral hemorrhage occurred in 6% of patients. The median mRS score at 90 days was 1 and 60% patients had good outcome (mRS 0-2). Overall 3-month mortality was 14%. Conclusion. Combined revascularization therapy of acute ischemic stroke consisting of initial intravenous thrombolysis and immediately after that performed mechanical thrombectomy is safe and effective. Cíl studie: Klinicky afunkční adenomy (KAA) představují zhruba 25–30 % všech tumorů hypofýzy. Neurochirurgická operace nádoru bývá často neúspěšná a zůstává po ní nádorové reziduum. Účinek následné biologické léčby je závislý na přítomnosti transmembránových receptorů v nádorových buňkách. Naším cílem bylo určit expresní profil somatostatinových receptorů (SSTR1–5) a estrogenového receptoru 1 (ER1) u 69 vzorků KAA. Materiál a metody: Vzorky adenomů se uchovávaly v RNAlater Tissue Protect až do doby zpracování v laboratoři. RNA jsme izolovali Trizolem a přepsali do cDNA pomocí SuperScriptu III. Exprese jednotlivých receptorů byla stanovena metodou kvantitativní real time PCR. Získané výsledky jsme přepočítali na housekeepingový gen pro beta-glukuronidázu. Výsledky: U vyšetřovaného souboru 69 afunkčních adenomů byly kromě SSTR4 a SSTR5 exprimovány všechny zbývající vyšetřované receptory. Mediány relativních hodnot byly: 63,8 % pro SSTR1, 55,4 % pro SSTR2, 19,6 % pro SSTR3, 2,6 % pro SSTR4, 9,2 % pro SSTR5 a 75,4 % pro ER1. Závěr: Variabilní exprese jednotlivých podtypů SSTR a ER1 může ovlivnit účinnost biologické léčby KAA. Proto by určení expresního profilu sledovaných receptorů mělo předcházet zahájení biologické léčby KAA somatostatinovými analogy či modulátory estrogenových receptorů., Objective: Non-functioning pituitary adenomas (NFA) comprise about 25–30% of all pituitary tumors. Transsphenoidal neurosurgery, the treatment of choice, is often unsuccessful and frequently leaves tumor remnants. The effect of subsequent biological treatment is mediated by transmembrane receptors in the adenoma cells. Our aim was to determine the somatostatin (SSTR1–5) and estrogen receptor 1 (ER1) expression profile in 69 specimens of NFA. Material and Methods: Adenoma samples were submerged in RNAlater Tissue Protect and transported frozen to the laboratory. The RNA was isolated by Trizol Reagent and transcribed to cDNA by SuperScript III. The expression of the receptors was determined by quantitative real time PCR. Results were normalized to the beta-glucuronidase housekeeping gene. Results: With exceptions of SSTR4 and SSTR5, all other subtypes of receptors were expressed in the examined tumor specimens. The median relative quantification values were: 63.8% for SSTR1, 55.4% for SSTR2, 19.6% for SSTR3, 2.6% for SSTR4, 9.2% for SSTR5, and 75.4% for ER1. Conclusion: Variable expression of somatostatin and estrogen receptors could influence the effectiveness of the biological treatment. Nádorová tkáňová banka obsahující kryokonzerované vzorky nádorových tkání umožňuje rozvoj nových diagnostických postupů založených na metodách molekulárně-biologických a genetických analýz. Naše studie byla zahájena v červnu 2008. Prezentované výsledky představují období pěti let (follow up). Celkový počet nemocných žen s karcinomem prsu, které byly zařazeny do projektu, byl 233 pacientek, průměrný věk 65,2 roku (věkový rozdíl 27–88 let). V naší studii byly odebrány a kryokonzervovány vzorky od 156 nemocných žen. Cílem naší studie je vytvořit program evidence a hodnocení prediktivních a prognostických faktorů analyzovaných novými molekulárně-biologickými a genetickými metodami. Využívání nových technologických postupů v léčbě nádorových onemocnění jako jsou např. biotechnologie a nanotechnologie, vyžadující hledání „nových“ prediktivních a prognostických faktorů, které budou vhodně doplňovat výběr léčby., The Tumor Tissue Bank containing cryopreserved samples unables development of new diagnostic methods based on molecular biologic and genetic analyses. Our study started in June 2008. In our study were taken and samples from 156 female patients. The results reported here represent the 3-year period of follow up. 316 women with breast carcinoma, mean age 65.2 years (age range 27 to 88 years) were admitted to this project. INTRODUCTION: Annually more than 27,000 persons die of cancer in the Czech Republic and the overall incidence of malignancies is still increasing. These data shows the need for affordable and good follow-up care especially for patients without any cancer treatment due to irreversible progression of tumor. Currently the outpatient palliative cancer care gets more into the forefront. Prerequisite for a well working outpatient palliative care is cooperation with general practitioners and home health care agencies. The purpose of the so called program of palliative cancer care is to guide a patient in palliative cancer care and to improve the cooperation among health care providers. METHODS: During the period from January 2008 to October 2010 we evaluated in patient without any oncology treatment due to irreversible progression of tumor. RESULTS: In palliative outpatient clinic we treated 446 patients, 119 of them received home care services with average length of 27.8 days. 77 patients died at home, 51 in health facilities and 41 in inpatient hospice care. CONCLUSION: We present pilot study focusing on outpatient palliative cancer care which shows the real benefit from early indication of palliative cancer care. Účel studie: Retrospektivně jsme zpracovali výsledky léčby mozkových nádorů stereotaktickou radioterapií a radiochirurgií. Použité metody: Pacienti podstoupili plánovací CT a MR vyšetření. Poté bylo provedeno plánování cílového objemu a ozařovací techniky v BrainLab systému a pacienti byli ozářeni na X-noži Varian s vícelamelovým kolimátorem BrainLab. Radiochirurgie byla aplikována jednorázově s fixací ve stereotaktickém rámu. Pacienti, kteří podstoupili frakcionovanou stereotaktickou radioterapii, byli fixováni pomocí speciální stereotaktické masky. Statistické zpracování bylo provedeno ve spolupráci s Ústavem matematiky a statistiky PřF MU Brno. Výsledky: Celkem bylo ozářeno 101 pacientů s primárními mozkovými tumory. Nejčastěji se jednalo o meningeomy a high-grade gliomy. Medián sledování byl 22,4 měsíců. Pacienti, kteří podstoupili radiochirurgii, měli medián dávky 18 Gy. Pacienti, ozařovaní frakcionovaně, měli medián dávky 25 Gy. Toxicita léčby byla nízká. Stabilizace onemocnění byla zaznamenána u 68 % pacientů, parciální remisi dosáhlo 10 % a jeden pacient měl kompletní remisi. Závěr: Výsledky naší léčby jsou srovnatelné s publikovanými studiemi. Stereotaktické ozařovací metody mají své důležité místo v léčbě primárních tumorů mozku., We analysed retrospectively our treatment of primary brain tumors using stereotactic radiotherapy and radiosurgery. Methods: Patients underwent CT and MR examinations. Target volume and technique were planned in BrainLab system. Patients were irradiated on X knife. Radiosurgery was apllied with fixation using stereotactic frame. Stereotactic mask was used in cases of stereotactic radiotherapy. Statistical processing was performed at Department of mathematics and statistics, Faculty of Science, Masaryk university Brno. Results: 101 paients were included in our study. In the most of cases there were diagnosis of meningeoma and high-grade glioma. Median follow up was 22,4 months. Median of dose of radiosurgery was 18 Gy. Patients, whose underwent stereotactic radiotherapy, were median of doses 25 Gy. Treatment toxicity was low. Stable disease was recorded in 68 % of cases, partial remission achieved in 10 % patients and one patient had the complete remission. Conclusion: Results of our study are comparable with results other publicated studies. The aim of this study was to determine the prevalence of musculoskeletal problems among Czech dentists and to analyze the factors that affect these disorders. Information was gathered through questionnaire completed by 581 physicians. The questionnaire ascertained general information about physicians including their work habits and the characteristics of their work environment along with the occurrence of musculoskeletal problems as well as their intensity. In the past year the occurrence of at least mild difficulties associated with the locomotive system was reported by 96.9% of dentists surveyed (n = 557), with 66.3% of respondents (n = 381) reporting difficulties ofa moderate or major nature. Most respondents of both sexes indicated neck pain. A statistically significant correlation with the occurrence of musculoskeletal complaints of medium and major intensity was demonstrated for the following factors: sex, age, running a private practice, past injury or musculoskeletal diseases, and the perception of work as psychologically demanding. Musculoskeletal disorders in dentists in Czech Republic are relatively frequent and serious health problem. The causes of these diseases must be identified and appropriate preventive interventions undertaken that will contribute to a reduction in the incidence of these problems. and Z. Sustová, L. Melanocytární léze vykazují značnou morfologickou heterogenitu. Jejich diagnostika je náročná i přes současné využití moderních histopatologických a imunohistochemických metod. Významným přínosem v diagnostice melanocytárních lézí je zavedení molekulárně genetické metody fluorescenční in situ hybridizace s použitím čtyřbarevné sondy značící geny CCND1, RREB1, MYB a centromeru chromozomu 6. V letech 2008–2012 bylo v naší laboratoři úspěšně metodou fluorescenční in situ hybridizace vyšetřeno 160 vzorků různých typů melanocytárních lézí, 44 névů a 116 melanomů. Změny asociované s diagnózou melanomu byly prokázány u 116 vzorků (72,5 %), normální nález u 41 vzorků (25,6 %), ve dvou případech bylo rozhodnutí na hranici hodnotitelnosti metody (1,2 %) a v jednom případě se jednalo o vyšetření recidivy melanomu v jizvě, který byl svým nálezem atypický (monozomie chromozomu 6) (0,6 %). Na základě dosažených výsledků byla stanovena senzitivita metody 96,6 % a specifita metody 91%. Tyto výsledky ukazují, že fluorescenční in situ hybridizace je vhodnou metodou pro zajištění přesné a rychlé diagnostiky nejednoznačných melanocytárních lézí. Právě rychlost a správnost diagnózy je u vysoce maligních melanomů zásadní a má neoddiskutovatelný vliv na prognózu a úspěšnost léčby pacienta., Melanocytic lesions exhibit significant morphological heterogenity, making their diagnosis difficult, although both sophisticated histopathological and immunohistochemical methods are used. The introduction of molecular genetic method flourescence in situ hybridization (FISH) has contributed significantly to the diagnosing of melanocytic lesions. Between 2008 and 2012, we studied a total of 160 samples of different types of melanocytic lesions, including 44 nevi and 116 melanomas, using a four-color probe mix for CCND1, RREB1 and MYB genes and for the centromere of chromosome 6. A positive result was found in 116 melenoma samples (72.5 %) and a negative result in 41 samples (25.6 %). A borderline value was found in two samples (1.2 %), and an atypical finding (monosomy of chromosome 6) was seen in a melanoma recurrence in one case (0.6 %). Sensitivity and specificity were 96.6 % and 91 %, respectively. These results show that FISH is a suitable and relatively low-cost method for accurate, rapid diagnosis of ambiguous melanocytic lesions.
