Primární nádory mozku jsou heterogenním onemocněním. Nejzávažnější diagnózu představuje glioblastoma multiforme, který tvoří 75 % z high grade gliomů. Jedná se o agresivní nádory s omezenými terapeutickými možnostmi a špatnou prognózou. Medián přežití od stanovení diagnózy je bez léčby 4,6 měsíce, s léčbou kolem 12 měsíců. Zlatým standardem terapie je v dnešní době radikální operace, chemoradioterapie a následně adjuvantní chemoterapie temozolomidem. V naší práci jsme se zaměřili na přínos přidání temozolomidu ke standardní radioterapii v porovnání s jeho toxicitou., Primary brain tumors are a heterogeneous disease. The most important is glioblastoma multiforme, which constitutes 75 % of the high grade gliomas. These are aggressive tumors with limited therapeutic options and poor prognosis. INTRODUCTION: Annually more than 27,000 persons die of cancer in the Czech Republic and the overall incidence of malignancies is still increasing. These data shows the need for affordable and good follow-up care especially for patients without any cancer treatment due to irreversible progression of tumor. Currently the outpatient palliative cancer care gets more into the forefront. Prerequisite for a well working outpatient palliative care is cooperation with general practitioners and home health care agencies. The purpose of the so called program of palliative cancer care is to guide a patient in palliative cancer care and to improve the cooperation among health care providers. METHODS: During the period from January 2008 to October 2010 we evaluated in patient without any oncology treatment due to irreversible progression of tumor. RESULTS: In palliative outpatient clinic we treated 446 patients, 119 of them received home care services with average length of 27.8 days. 77 patients died at home, 51 in health facilities and 41 in inpatient hospice care. CONCLUSION: We present pilot study focusing on outpatient palliative cancer care which shows the real benefit from early indication of palliative cancer care. Periprosthetic fractures are the third most common reason for revision total hip arthroplasty. Surgical treatment of periprosthetic fractures belongs to the most difficult procedures due to the extensive surgery, elderly polymorbid patients and the high frequency of other complications. The aim of this study was to evaluate the results of operatively treated periprosthetic femoral fractures after total hip arthroplasty. We evaluated 47 periprosthetic fractures in 40 patients (18 men and 22 women) operated on between January 2004 and December 2010. The mean follow-up period was 27 months (within a range of 12-45 months). For the clinical evaluation, we used modified Merle d'Aubigné scoring system. In group of Vancouver A fractures, 3 patients were treated with a mean score of 15.7 points (good result). We recorded a mean score of 14.2 points (fair result) in 6 patients with Vancouver B1 fractures, 12.4 points (fair result) in 24 patients with Vancouver B2 fractures and 12.7 points (fair result) in 7 patients with Vancouver B3 fractures. In group of Vancouver C fractures, we found a mean score of 16.2 points (good result) in 7 patients. Therapeutic algorithm based on the Vancouver classification system is, in our opinion, satisfactory. Accurate differentiation of B1 and B2 type of fractures is essential. Preoperative radiographic images may not be reliable. If in doubt, checking the stability of the prosthesis fixation during surgery should be performed. and M. Korbel, P. Sponer, T. Kucera, E. BACKGROUND: The male-female ratio at birth (M/F: male births divided by total births), which is anticipated to approximate 0.515, has been shown to exhibit latitude gradients and secular trends. METHODS: Annual national data for male and female live births for the 15 countries that comprise the former Soviet Union were obtained from the World Health Organisation for the period 1980-2009 (115,167,569 total live births) and analysed with contingency tables. Spearman correlation was also carried out to compare percentage annual gross domestic product growth (GDP%--downloaded from the World Bank) and M/F. In this context, GDP% is used as a measure for economic hardship or wellbeing within the populace. RESULTS: There have been overall highly significant secular increases in M/F (p < 0.0001) in the countries and regions investigated. M/F is significantly lower in the three more northern regions (Russian Federation, Baltic States and Central Asia. M/F 0.51324, 0.51335-0.51314) than the two more southern regions (Southern Caucasus and Eastern Europe. M/F 0.51654, 0.51635-0.51672). There was a male excess of 113,818 live births.There was a significant positive correlation between GDP% and M/F for Armenia, Azerbaijan and Uzbekistan. There was a significant negative correlation in Estonia. CONCLUSION: Previous studies have shown that improving socioeconomic conditions increase M/F, and the converse has also been demonstrated. This is a potential influence in this geographical area since this region has relatively recently emerged from communist rule and experienced an overall economic upturn, but is only partially supported using GDP%. Another factor may be the selective termination of female pregnancies. The latitude gradient parallels that of neighbouring Europe but no theory has been put forward to convincingly explain this finding to date. and V. Grech
BACKGROUND: Latitude gradients and secular trends in Europe and North America have been found in the male-female ratio at birth (M/F: male births divided by total births) which approximates 0.515. METHODS: Annual national data for Yugoslavia and the post-Yugoslavia States for male and female live births were obtained from the World Health Organisation and analysed with contingency tables. RESULTS: This study analysed 22,020,729 live births. There was a increasing trend in M/F prior to the breakup of the former Yugoslavia (1950-1990, p = 0.002), followed by a decreasing trend after 1990 (p = 0.02). A latitude gradient was also noted, with more males being born in southern, warmer latitudes (p < 0.0001). There was an overall excess of 42,753 male births based on an anticipated M/F of 0.515. CONCLUSION: M/F is decreasing in this region, similar to the rest of Europe and North America. A latitude gradient is also present with more males being born in warmer (more Southern) latitudes (p < 0.0001), even in this small region and over the short time-frame studied. and V. Grech
Jedním z významných faktorů ovlivňujících stravu je obliba jednotlivých potravin, potravinové preference. Základní chuťové preference jsou vrozené, typicky preference pro sladkou a slanou chuť a naopak odmítání hořké a kyselé chuti. Vrozené preference spolu s konkrétními zkušenostmi a dalšími vlivy utváří míru obliby jednotlivých potravin. Obecně se má za to, že preference patří mezi silné determinanty stravy, že lidé, a obzvláště děti, jedí především to, co jim chutná. Cílem předložené studie bylo zjistit, do jaké míry a jakým způsobem se potravinové preference promítají do skutečné konzumace u rozsáhlého souboru adolescentů. Výzkum byl proveden u 1655 dospívajících ve věku 15 let v rámci studie ELSPAC v oblasti města Brna. U celkem 56 položek v osmi potravinových a jedné nápojové skupině byly dotazníkovými nástroji zjišťovány jednak preference, jednak frekvence konzumace a byl analyzován jejich vzájemný vztah. Výsledky ukázaly, že preference a skutečná konzumace se výrazně liší. Preference se tedy do konzumace promítají jen částečně. Z deseti nejpreferovanějších položek byly pouze dvě mezi deseti nejkonzumovanějšími. Z deseti nejméně preferovaných se pět položek objevilo mezi deseti nejméně konzumovanými, "negativní preference" se tedy s konzumací shodují poněkud více. Konzumace byla celkově výrazně zdravější, než odpovídalo preferencím. Bližší analýza ukázala, že lze identifikovat několik typů souvislosti mezi preferencemi a konzumací, podle různých tvarů regresních křivek. Celkem nejvíce položek (27) vykazovalo závislost tvaru "J", ale například položky ve skupině sladkosti vykazovaly výhradně závislost "U", zatímco skupina masa vykazovala převážně lineární závislost. Praktické důsledky nelineárních závislostí spočívají zejména v tom, že některé potraviny jsou konzumovány výrazně více (nebo naopak méně), než odpovídá jejich preferencím. Jsou diskutována možná vysvětlení, spočívající v různých externích vlivech. Hlavním závěrem je, že preference nejsou rozhodujícím faktorem pro konzumaci, což dává jasný signál, že strava adolescentů je výrazně ovlivnitelná vnějšími vlivy, v optimálním případě příznivým směrem, cílenými preventivními aktivitami., Food preferences belong to significant factors influencing diet. Basic taste preferences are inherited, typically the preferences for sweet and salty tastes or dislike of bitter and sour tastes. Inherited taste preferences together with experience and other influences form the extent of preference of particular foods. Generally it is believed that food preferences belong to strong determinants of food choice, and that people, especially children, eat what they like. The objective of the study was to investigate to what extent and how food preferences are reflected in consumption in a large sample of adolescents. The research involved 1,655 adolescents aged 15 years, and was part of the ELSPAC study in the Brno district. Both food preferences (liking) and consumption frequency were investigated by questionnaire survey for 56 items in 8 food groups and 1 beverage group, and the relationship between preferences and consumption was analysed. The results show that preferences and consumption differ substantially and thus preferences are only partially reflected by consumption. From 10 most preferred items only 2 were among the 10 most consumed. From 10 least preferred items, 5 were classed among the 10 least consumed, thus "negative preferences" coincide with consumption to a greater extent. Consumption was generally healthier than could be assumed according to preferences. Detailed analyse showed several types of association between preferences and consumption, as demonstrated by different shapes of regression curves. Most items, 27 altogether, showed the "J" type of association, but items in the sweets group showed solely "U" type, while items in the meat group largely had a linear association. The main practical consequences of nonlinear associations are that some foods are consumed considerably more (or less) than preferences would allow. The possible explanations, based on various external influences, are discussed. The main conclusion is that preferences are not the most important determinates for consumption, which gives a clear signal that the diet of adolescents is highly influenceable by external factors, optimally in a favourable manner, by targeted preventive activities., Jindřich Fiala, Lubomír Kukla, and Literatura
Cíl: Stanovit, jak často je SPECT/CT potřebné ke specifikaci abnormálního nálezu na planárním WB kostním skenu, zda je užitečné porovnat scintigrafický nález s předcházejícím vyšetřením, v kolika případech SPECT/CT určí definitivní diagnózu a kolik ložisek abnormální aktivity nacházíme na kostním skenu u benigních a maligních afekcí. Pacienti a metoda: Retrospektivně jsme prostudovali 471 WB skenů u 450 pacientů s karcinomem prsu, plic a prostaty, kteří v roce 2008 podstoupili kostní scintigrafii. SPECT/CT nebo CT jsme použili v 11,9 % vyšetření u 12,4 % pacientů. Opakovaná scintigrafie (319 pacientů po kostní scintigrafii a 36 pacientů po SPECT/CT) sloužila jako referenční vyšetření. Výsledky: Srovnání s předchozí scintigrafií bylo užitečné u pacientů s karcinomem prsu před indikací SPECT/CT. S použitím SPECT/CT jsme mohli specifikovat nejisté scintigrafické léze u 90,9 % vyšetření. Pro detekci kostních metastáz dosáhlo CT ve SPECT/CT vyšetření senzitivitu 72,8 %, specificitu 96 %, pozitivní a negativní prediktivní hodnotu 88,9 %. V pravděpodobnosti rozvoje kostních metastáz do 2 let od vyšetření mezi pacienty s jedním suspektním ložiskem ve skeletu a pacienty bez suspektního ložiska jsme nezaznamenali signifikantní rozdíl (p = 0,4272), ovšem riziko vzrůstalo, jestliže jsme nalezli suspektních ložisek více (p ≤ 0,0001). Závěr: SPECT/CT zvyšuje specificitu kostní scintigrafie. Po porovnání scintigrafického nálezu s předchozím vyšetřením se zdá rozumné uvažovat o použití SPECT/CT v 10-20 % vyšetření. Ledvinné angiomyolipomy (AML) jsou mezenchymové benigní tumory, řadí se mezi hamartomy mohou se vyskytovat osamoceně nebo se sdružují s komplexem tuberózní sklerózy (TSC). Pacienti s AML mají riziko vzniku spontánního, v některých případech až život ohrožujícího krvácení. V tomto sdělení jsou popsány tři případy spontánního krvácení AML: první u 69leté pacientky s prokázanou angiomyolipomatosou ledvin oboustranně, druhý u 60leté pacientky s dlouhodobě sledovaným solitárním AML a třetí u mladého 28letého pacienta s diagnózou tuberózní sklerózy., Renal angiomyolipoma is the mesenchymal benign tumor which belongs to the group of hammartomas. It can occur sporadically or in association with tuberous sclerosis complex. The possible morbidity associated with angiomyolipoma is spontaneous life-threatening haemorrhage. In this report we describe three AML cases. The first one, 69-years old female with diagnosis angiomy-olipomatosis in both kidneys.
