Brown tumors are bony lesions caused by rapid osteoclastic activity, which rare involved jaws. Renal osteodystrophy (ROD) is associated with different pathogenetic mechanisms--disorder of calcium-phosphate metabolism, impaired metabolism of vitamin D, increased parathyroid activity that lead to extreme concentrations of parathormone. The authors report two cases of jaw enlargement in patients received haemodialysis with excessive increase values of alkaline phosphatase and parathormone in serum. The patients were treated surgically with corrective procedures in maxillo-facial area. ROD of the jaws could be severe complication in dialysis patients with end stage of CKD if no appropriate care aimed at correction or prevention of parathyroid hyperfunction was applied to them. and PF. Castlemanova choroba je vzácnou nosologickou jednotkou, která byla v literatuře poprvé popsána v roce 1954 Castlemanem a Lownovou. Jde o onemocnění primárně postihující lymfatické uzliny, u kterého se rozlišují čtyři histomorfologické podtypy a dva klinické obrazy (uni a multicentrický). Nejčastěji se klinicky projeví jako lymfadenopatie mediastina, krku či retroperitonea. Příznaky nemoci jsou výrazně heterogenní a záleží na daném histologickém podtypu. Taktéž prognóza se liší podle typu nemoci, kdy může jít jak o zcela benigní nálezy (zejména forma unicentrická), tak o potenciálně maligní formu, při které nemoc progreduje až do stadia maligní lymfoproliferace (multicentrická forma). Autoři prezentují případ 29letého muže s klinicky manifestním (ikterus, poruchy střevní pasáže) objemným tumorem retroperitonea lokalizovaným subhepatálně, který byl úspěšně radikálně exstirpován. Pooperační průběh byl nekomplikovaný. Definitivní histopatologická diagnóza potvrdila hyalinně-vaskulární typ Castlemanovy nemoci. Vzhledem k tomu, že se jednalo o multicentrickou formu onemocnění, bude následovat adjuvantní onkologická léčba. It is rare sickness which primary affects lymfatic nodes. There are four histo-morfologic subtypes and two clinic manifestations (uni and mulicentric). Comon clinical picture is simple lymfadenopathy of neck, medistinal a retroperitonal lymfatic nodes. Signs of this disease are heterogenic and depend on histological subtype. Also prognosis depends on type of illness. It should take place as benign diagnosis (unicentric form) but also can be potencionally malignant form which shlould exacerbate to malignant lymfoproliferation (multicentric form). Authors present the case of 29 years old men with clinical manifestation (jundice, intermitent bowel obstruction) of lagre retroperitoneal tumor in subhepatal localization. The treatment was radical surgical extirpation, surgery was without complications. Definitive histological diagnosis was confirmed as hyaline-vascular type of Castleman disease. Nádory hlavy a krku patří celosvětově mezi karcinomy se vzrůstající incidencí. Hlavním etiologickým faktorem podmiňujícím jejich vznik je kouření, abúzus alkoholu a v poslední době potvrzený HPV. Dvě třetiny pacientů přicházejí v pokročilém stadiu a až třetina pacientů má četné komorbidity. V jejich terapii je nejdůležitější multidiciplinární přístup. Standardem léčby je dle NCCN konkomitantní chemoradioterapie s platinovým derivátem (kategorie 1), nebo radioterapie konkomitantně s biologickou léčbou cetuximabem, nebo chirurgický přístup a dále dle histologie adjuvantní radioterapie či chemoradioterapie (1). V poslední době je zvažována v indikaci léčby také pozitivita HPV. Jedná se o pacienty s lepší prognózou, ale zatím nebyla stanovena standardní terapie. Cetuximab je monoklonální chimerická IgG1 protilátka, která cíleně blokuje extracelulární část EGFR. V České republice je zaregistrován mimo jiné pro léčbu pacientů s lokálně pokročilými nádory hlavy a krku konkomitantně s radioterapií dle Bonnerovy studie fáze III publikované v r. 2006 v N Engl J Med (2), která prokázala pro kombinaci radioterapie a cetuximabu proti radioterapii samotné signifikantní navýšení lokoregionální kontroly 24 měsíců versus 14,9 měsíců, s mediánem OS 54,0 měsíců versus 29,3 měsíců, bez signifikantního navýšení morbidity a toxicity., Worldwide, head and neck tumours are among cancers with an increasing incidence. The main aetiological factors determining their development are smoking, alcohol abuse, and recently detected HPV. Two thirds of patients present in an advanced stage and up to a third of them have numerous comorbidities. A multidisciplinary approach is of utmost importance in the treatment. The standard treatment according to NCCN is concomitant chemoradiotherapy with a platinum derivative (category 1) or radiotherapy concomitantly with biological therapy with cetuximab, or surgical approach, followed by, according to histology, adjuvant radiotherapy or chemoradiotherapy (1). Recently, HPV positivity has also been taken into consideration concerning indications for treatment. This applies to patients with a better prognosis, but no standard treatment has been determined so far. Cetuximab is a chimeric IgG1 monoclonal antibody that targets the extracellular domain of the EGFR. Nádory oblasti hlavy a krku představují v Evropě u mužů čtvrté nejčastější nádory. Nejvyšší výskyt je pozorován v zemích okolo Středozemního moře. Incidence nádorů orofaryngu v České republice je 0,91/100 000 obyvatel s mortalitou 0,65/100 000 obyvatel, nejvíce zastoupeným klinickým stadiem nádoru je klinické stadium IV (58,9 %). U nádorů patra je incidence 0,34/100 000 obyvatel s mortalitou 0,15/100 000 obyvatel. Nejvíce zastoupeným klinickým stadiem je také stadium IV (26,7 %). Postižení žen je výrazně nižší než u mužů, v ČR je poměr mužů k ženám přibližně 4:1. Z hlediska rizikových faktorů hraje rozhodující vliv kouření a konzumace alkoholu. Průměrný věk pacientů se pohybuje mezi 50 a 60 lety. V kazuistikách bude představen pacient s nádorem orofaryngu a pacient s nádorem měkkého a tvrdého patra a patrové tonzily. V obou případech šlo o pokročilé lokální nálezy bez vzdálených metastáz u „typických“ pacientů mužského pohlaví s dlouholetým abúzem alkoholu a tabakizmem., Head and neck tumours are the fourth most common cancer among men in Europe. The highest incidence is in the Mediterranean countries. The incidence rate of oropharyngeal cancer in the Czech Republic is 0.91/100 000 inhabitants with a mortality rate of 0.65/100 000 inhabitants; the most prevalent clinical stage of the tumour is clinical stage IV (58.9%). In palatine cancer, the incidence rate is 0.34/100 000 inhabitants with a mortality rate of 0.15/100 000 inhabitants. The most prevalent clinical stage is also clinical stage IV (26.7%). Women are significantly less affected than men, with the ratio of women to men being 4:1 in the Czech Republic. In terms of risk factors, smoking and alcohol consumption play a crucial role. Chemodektom, paragangliom glomus caroticum neboli tumor karotického glomu se vyskytuje v populaci s incidencí 1 : 30 000. Tumor je makroskopicky, mikroskopicky i biologicky velmi podobný adrenálnímu feochromocytomu. Významným znakem těchto tumorů je jejich bohatá vaskularizace. Patří mezi nejvíce prokrvené tumory vůbec. Histologicky se popisuje alveolární uspořádání nádorových buněk, na rozdíl od feochromocytomu, ostře od sebe ohraničených jemnými pruhy vaziva se širokými kapilárami a buňkami podobnými buňkám Schwannovým. Cytoplazma nádorových buněk hlavních je slabě eozinofilní a jemně granulovaná. Jádra buněk mohou být pleomorfní a hyperchromní, ale nutno konstatovat, že jaderná atypie není jednoznačnou známkou malignity. Biologicky se jedná o tumory převážně benigní. Klinicky se jedná o pomalu rostoucí, nebolestivý, tuhý a kulovitý útvar za úhlem dolní čelisti. Přestože chemodektom je všeobecně považován za radiosenzitivní, hlavní terapeutický postup je chirurgická exstirpace tumoru., Chemodectoma, paraganglioma of glomus caroticum or carotid glomus tumor occurs in the population with an incidence of 1:30 000. Autoři uvádějí kazuistiku pacienta s epiteloidním hemangioendoteliomem jater (HEHE), který byl léčen chemoembolizací (TACE). Při dlouhodobém sledování 9 let od stanovení diagnózy je klinicky bez obtíží a bez progrese onemocnění při vyšetření zobrazovacími metodami. Clear cell odontogenic carcinoma (CCOC) is a rare odontogenic tumor associated with aggressive clinical behavior, metastasis and low survival. We report a case of CCOC affecting the maxilla of 62 year-old woman. It was first described as a clinicopathological entity in 1985 and to date only 67 cases were described in the English literature. We are understanding of the behavior of this carcinomas was depend on limited case reports. V kazuistike opisujeme prípad 49-ročného muža od detstva morbídne obézneho (BMI 40 kg/m2), s artériovou hypertenziou (asi od 15 rokov, liečená od roku 2004), dyslipidémiou (od roku 2006), diabetes mellitus 2. typu (od roku 2006, na inzulinoterapii od roku 2008) a fajčením (do roku 2011, 20 cigariet denne). Liečba: 16 druhov liekov, 8 na hypertenziu, statín, liečba diabetu, aspirín, alopurinol. V roku 2010 (ako 45-ročný) hospitalizovaný na našej klinike pre dyspnoe a bolesti na hrudníku s hodnotou krvného tlaku 180/110 mm Hg (zistené srdcové zlyhávanie s ejekčnou frakciou ĽK 33 %, vo funkčnej triede NYHA II, echokardiograficky: ľavá predsieň: 46 mm, rozmer ľavej komory v diastole: 70 mm, interventrikulárne septum: 12 mm, hypokinéza septa, dopplerovská ultrasonografia artérií dolných končatín (kalcifikácie, difúzne aterosklerotické zmeny, neprítomná stenóza), CT koronárna angiografia (signifikantná stenóza ľavej koronárnej artérie). Zahájená liečba perorálny furosemid 40 mg denne. V máji roku 2011 hospitalizovaný pre akútny koronárny syndróm: akútny NSTEMI spodnej steny (koronarografia: 2-cievne postihnutie, realizovaná PKI, implantovaný DES – ramus circumflexus, paroxyzmus fibrilácie predsiení, funkčná trieda NYHA III, ľavokomorová ejekčná frakcia: 30 %, pľúcna hypertenzia). V roku 2012 realizovaná renálna denervácia pre rezistentnú hypertenziu, implantovaný karotický stent pre stenózu karotickej artérie, prítomná diabetická nefropatia (KDOQI 3. stupňa, GF 40 ml/min). V auguste roku 2014 hospitalizovaný na našej klinike pre pľúcny edém, kardiogénny šok, akútnu ischémiu pravého predkolenia pri periférnej embolizácii, prítomný atriálny flutter, zhoršenie renálnych parametrov, echokardiograficky: ľavá predsieň: 55 mm, rozmer ľavej komory: 75 mm, akinéza septa a zadnej steny, oklúzia artérií pravej dolnej končatiny (vzhľadom na vážny stav pacienta kontraindikovaná angiochirurgická intervencia, zvažovaná amputácia končatiny z vitálnej indikácie), pacient zomrel po 4 dňoch hospitalizácie na intenzívnej jednotke po neúspešnej liečbe. Kombinácia ochorení diabetes, hypertenzia a ischemická choroba srdca je častá a prognosticky vážna. Diabetes zvyšuje kardiovaskulárnu morbiditu a mortalitu, a preto by sme mali hľadať diabetes u všetkých kardiovaskulárnych pacientov., The case study describes a case of 49-year-old man with morbid obesity since childhood (BMI 40 kg/m2), arterial hypertension (approx. since aged 15, treated since 2004), dyslipidemia (since 2006), type 2 diabetes mellitus (since 2006, on insulin therapy since 2008) and smoking (until 2011, 20 cigarettes a day). Treatment: 16 types of medication, 8 for hypertension, statin, therapy for diabetes, aspirin, allopurinol. In 2010 (when aged 45) hospitalized in our clinic with dyspnoea and chest pain with a high pressure reading of 180/110 mm Hg (identified symptoms of heart failure with LV ejection fraction of 33 %, in NYHA II functional class, echocardiographically: left atrium: 46 mm, left ventricular chamber size in diastole: 70 mm, interventricular septum: 12 mm, septal hypokinesis, Doppler ultrasonography of lower limb arteries (calcification, diffuse atherosclerotic changes, absent stenosis), CT coronary angiography (significant stenosis of the left coronary artery). Treatment started with 40 mg oral dose of furosemide daily. In May 2011 he was hospitalized with an acute coronary syndrome: acute NSTEMI of the inferior wall (coronarography: 2-vascular problems, implemented PKI, implanted DES – ramus circumflexus, paroxysmal atrial fibrillation, NYHA III functional class, left ventricular ejection fraction: 30 %, pulmonary hypertension). In 2012 renal denervation for resistant hypertension was carried out, carotid stent implanted for stenosis of the carotid artery, presence of diabetic nephropathy (KDOQI stage 3, GF 40 ml/min). In August 2014 admitted to our clinic with pulmonary oedema, cardiogenic shock, acute ischemia of the right calf with peripheral embolisation, presence of atrial flutter, impairment of renal parameters, echocardiographically: left atrium: 55 mm, left ventricle size: 75 mm, akinesis of the septum and posterior wall, occlusion of the right leg arteries (given the patient‘s serious state angio-surgical intervention was contraindicated, vitally indicated leg amputation considered), the patient died after 4 days of hospitalization in an intensive care unit after unsuccessful treatment. A combination of diabetes, hypertension and ischemic heart disease is frequent and prognostically serious.
