Úvod: I přes medicínský pokrok studie z řady zemí ukazují na přetrvávající zvýšené riziko perinatální morbidity i mortality potomků diabetických matek. V České republice nebyla publikována data o osudu gravidit žen s diabetes mellitus 1. a 2. typu (pregestační DM). Cílem práce bylo zhodnotit osud gravidit žen s pregestačním DM a jejich potomků a zjistit, zda se zlepšil s odstupem 10 let. Metodika: Retrospektivní zhodnocení osudu gravidit žen s pregestačním DM sledovaných ve FN Plzeň v období let 2000–2009 (skupina A, n = 107) a porovnání s obdobím let 1990–1997 (skupina B, n = 39). Ze statistických testů byl použitý Wilcoxonův test nepárový, kontingenční tabulky, logistická regrese – metoda step-wise a lineární mnohočetná regrese metoda step-wise. Výsledky: Data jsou uvedena jako medián (1., 3. kvartil). Ženy ve skupině A jsou starší 28 (25, 31) vs 25 (22, 27) let, p = 0,01. Jinak se skupiny statisticky významně neliší dobou trvání diabetu, BMI, zastoupením žen s DM 2. typu. V A skupině byla dosažena lepší kompenzace DM (dle HbA1c mmol/mol) ve všech trimestrech – 1. trimestr: 59 (47, 67) vs 66 (56, 76), 2. trimestr: 46 (40, 52) vs 54 (48, 59) a 3. trimestr: 46 (40, 51) vs 53 (47, 60), p = 0,01. Klesl počet porodů sekcí 65,2 % vs 87,5 %, p < 0,05, po adjustaci na věk a dobu trvání diabetu i výskyt respiratory distress syndromu 8,9 % vs 18,2 %, p < 0,05. Klesající trend byl zaznamenán i ve výskytu porodů před 34. gestačním týdnem (1,1 % vs 6,3 %) a neonatální mortalitě (1,1 % vs 2,9 %), ale rozdíly nejsou statisticky významné. Závěr: Dosažená lepší kompenzace diabetu vedla pouze k částečnému zlepšení průběhu gravidity a osudu potomků diabetických matek., Introduction: In spite of progress in medicine, studies from a number of countries indicate steadily increased risk of perinatal morbidity and mortality in the offspring of diabetic mothers. No data regarding the pregnancy outcome in women with diabetes mellitus type 1 and 2 (pregestational DM) have been published in the Czech Republic. The aim of the study was to evaluate the pregnancy course of women with pregestational DM and outcome of their offspring and to assess whether it has improved in ten years. Methods: A retrospective evaluation of pregnancy outcome of pregestational DM women followed up in the University Hospital Pilsen in years 2000–2009 (Group A, n = 107) and comparison with the period 1990–1997 (Group B, n = 39) were performed. Wilcoxon non-paired test, contingency tables, step-wise logistic regression and step-wise linear multiple regression methods were used for statistical analyses. Results: Data is presented as median (interquartile range). Women from the Group A were older 28 (25, 31) vs 25 (22, 27) years, p = 0.01. Otherwise, the groups did not statistically significantly differ in diabetes duration, BMI, and representation of women with type 2 diabetes. A better glycemic control (HbA1c, mmol/mol) was achieved in the Group A in all trimesters – 1st trimester: 59 (47, 67) vs 66 (56, 76), 2nd trimester: 46 (40, 52) vs 54 (48, 59) and 3rd trimester: 46 (40, 51) vs 53 (47, 60), p = 0.01. The caesarean section rate decreased (65.2 % vs 87.5 %, p < 0.05). The incidence of the respiratory distress syndrome after adjustment for age and diabetes duration also decreased (8.9 % vs 18.2 %, p < 0.05). A decreasing trend in the rate of premature delivery before 34th week of gestation (1.1 % vs 6.3 %) and neonatal mortality (1.1 % vs 2.9 %) was observed, however, the differences were not statistically significant. Cíl: Součástí diagnostiky febrilní infekce močových cest (IMC) je ultrazvukové vyšetření a mikční cystouretrografie (MCU), které slouží k odhalení případných komplikací infekce a anomálií močových cest. Tak, jak se vyvíjí porozumění vzájemným vztahů mezi infekcí močových cest, jizvením a přítomností vezikoureterálního refluxu (VUR), mění se i doporučené vyšetřovací algoritmy. Materiál a metoda: Do studie bylo zahrnuto 184 pacientů, kteří podstoupili mikční cystouretrografii po první či opakované atace febrilní infekce močových cest. Byl sledován výskyt vezikoureterálního refluxu u obou skupin pacientů v korelaci s ultrazvukovým nálezem. Pacienti s vezikoureterálním refiuxem byli rozděleni do skupiny s nediktujícím (VUR I.-II. stupně) a dilatujícím (VUR III.-V. stupně) refiuxem. Výsledky: Celkově byl VUR zaznamenán u 39/184 (21 %) pacientů. Ve skupině pacientů po první atace febrilní IMC byl VUR zachycen u 23/156 (15 %) pacientů, ve srovnání s 16/28 (57 %) pacienty po opakovaných febrilních IMC (statisticky významný rozdíl, p < 0,0001). U 8/156 (5 %) pacientů po první atace febrilní IMC byl zjištěn dilatující VUR ve srovnání s 11/28 (39 %) pacienty s opakovanými febrilními infekcemi močových cest (statisticky významný rozdíl, p < 0, 0001). Závěr: V souladu s aktuálním doporučením American Academy of Paediatrics poukázaly výsledky naší studie na nízký výskyt VUR včetně dilatujícího VUR u pacientů po první atace febrilní IMC. Na základě našich zjištění nedoporučujeme provádět MCU plošně u všech pacientů ve věku 2-24 měsíců po první atace IMC., Aim: Ultrasound investigation and voiding cystouretrography (VCUG) are part of the diagnostic algorithm in children with febrile urinary tract infection (UTI) with the aim of depicting any complication of the infection and underlying anomalies of the urinary tract. As the understanding of the relationship between UTI, renal scarring and vesicoureteral reflux (VUR) evolves, new recommended imaging algorithms are being introduced. Methods: 184 patients that underwent VCUG after the first or recurrent febrile UTI were included in the study. The incidence of VUR in both groups have been evaluated and compared with ultrasound findings. Patients with VUR were divided into those with non-dilating (VUR grade I-II) and dilating (VUR grade III-V) reflux. Results: Overall VUR was diagnosed in 39/184 (21%) patients. VUR was seen in 23/156 (15%) patients in the group of patients after the first fibrile UTI, in comparison to 16/28 (57%) patients with recurrent fibrile UTI (statistically significant difference, p < 0,0001). 8/156 (5%) patients after the first febrile UTI had dilating VUR, compared to 11/28 (39%) patients after recurrent febrile UTI (statistically significant difference, p< 0,0001). Conclusion. In compliance with current recommendations of the American Academy of Paediatrics the results of this study have shown low incidence of VUR and dilating VUR in patients with a history of single febrile UTI.
