Kyselina močová je u človeka biologicky aktívny koncový produkt metabolizmu purínov. Zvýšené koncentrácie kyseliny močovej sprevádzajú metabolický syndróm a jeho komponenty už u detí a mladistvých. K hyperurikémii prispieva konzumácia nápojov prisládzaných fruktózou, nakoľko je jediným sacharidom, pri ktorého metabolizme vzniká kyselina močová. Súčasné vedomosti neumožňujú odlíšiť, či je hyperurikémia kauzálnou príčinou komponentov metabolického syndrómu, alebo len sprievodným znakom naznačujúcim zvýšené riziko vzniku kardiovaskulárnych ochorení. V súčasnosti neexistuje konsenzus či a ako liečiť asymptomatickú hyperurikémiu u detí a mladistvých. Zaujať zásadný postoj k týmto otázkam umožnia len výsledky randomizovaných kontrolovaných prospektívnych štúdií, ktoré poskytnú dôkazy o prospešných účinkoch tejto liečby na prevalenciu metabolického syndrómu a incidenciu kardiovaskulárnych chorôb., In humans, uric acid represents a biologically active end-product of purine nucleotides. Several studies in children and adolescents documented an association between hyperuricaemia and the components metabolic syndrome. High intake of fructose-sweetened beverages might increase uricaemia, since fructose is the only saccharide which metabolism results in the formation of uric acid. Current knowledge does not allow distinguishing whether hyperuricaemia is causally related to the components of metabolic syndrome, or rather represents a marker of an enhanced risk, and poor outcome. No guidelines exist whether or not to treat asymptomatic hyperuricaemia in the adolescents. Dysfunkce autonomního nervového systému patří k častým klinickým projevům roztroušené sklerózy (RS). Incidence autonomní dysfunkce (AD) u pacientů s RS se v různých studiích pohybuje mezi 16 a 80 % a jednoznačně se zvyšuje s délkou trvání RS a progredující disabilitou. Může se však projevit v kterékoli fázi demyelinizačního onemocnění a dokonce může být jeho prvním klinickým projevem. AD významně ovlivňuje kvalitu života pacientů s RS a přispívá k celkové disabilitě. Přesto postižení autonomního nervového systému patří mezi stále relativně poddiagnostikované symptomy tohoto onemocnění. Nejčastějším a nejzávažnějším typem AD u pacientů s RS je narušení funkce kardiovaskulárního a/nebo urogenitálního systému. Méně častá je dysfunkce gastrointestinální, sudomotorická či narušení pupilomotoriky. Autonomní (především kardiovaskulární) dysfunkce u pacientů s RS také úzce souvisí s rozvojem únavy. Cílem tohoto sdělení je poskytnout přehled jednotlivých typů AD u pacientů s RS a možností jejich diagnostiky., Autonomic nervous system dysfunction (AD) represents a frequent clinical presentation of multiple sclerosis (MS). According to published studies, the incidence of AD in MS patients ranges between 16 and 80% and gradually increases with the length of the demyelinating disease and with the progression of disability. However, AD can occur in any phase of MS and can even represent its first symptom in some patients. Autonomic dysfunction has a significant negative impact on the quality of life in MS patients and contributes to overall disability. Even so, autonomic nervous system dysfunction is frequently underdiagnosed in patients with multiple sclerosis. Cardiovascular and urogenital dysfunction are the most frequent types of AD in MS patients with the highest impact on their clinical status and the quality of life. Less frequently, gastrointestinal, sudomotor or pupilomotor dysfunction can be found. Autonomic (in particular cardiovascular) dysfunction in multiple sclerosis is closely related to fatigue, another frequent clinical symptom in MS patients. The aim of this paper is to sum up the spectrum of symptoms of autonomic dysfunction in multiple sclerosis and its diagnostic tools. Úvod: Existuje mnoho studií věnujících se problematice biliárních komplikací po jaterních resekcích. Přes zlepšení operační techniky a perioperační péče má incidence této komplikace spíše vzrůstající charakter. Byly analyzovány četné prediktivní faktory. Na jejich vliv na vznik biliárního leaku doposud nepanuje shoda. Za cíl analýzy jsme si stanovili zhodnotit incidenci biliárního leaku, jeho vliv na mortalitu a na dobu hospitalizace na našem pracovišti. Současně jsme provedli rozbor známých prediktivních faktorů. Metoda: Autoři retrospektivně analyzují soubor 146 nemocných, u kterých byla na Chirurgické klinice 2. LF Univerzity Karlovy a ÚVN, Praha v období 2010–2013 provedena jaterní resekce. K vyhodnocení biliárního leaku jsme užili současnou uznávanou klasifikaci ISGLS (International Study Group of Liver Surgery). Závažnost komplikace byla stanovena dle Clavien-Dindo. Statistickou významnost prediktivních faktorů jsme určili pomocí Fisherova exaktního testu a Studentova t-testu. Výsledky: Incidence biliárního leaku byla 21 %. Dle ISGLS bylo 6,5 % pacientů zařazeno do skupiny typu A, v 61,2 % se jednalo o leak B a v 32,3 % o leak typu C. Stupeň závažnosti dle Clavien-Dindo I-II, IIIa, IIIb, IV a V byl v poměru 19,3 %, 42 %, 9,7 %, 9,7 %, resp. 19,3 %. Jako statisticky významné jsme stanovili tyto faktory: operaci pro malignitu (p<0,001), velkou jaterní resekci (p=0,001), dobu operace (p<0,001), vyšší peroperační krevní ztrátu (p=0,02), konstrukci HJA (p=0,005), portální venózní embolizaci/two-stage chirurgii (p=0,009) a ASA skóre (p=0,02). Biliární leak významně prodloužil dobu hospitalizace (p<0,001). V souboru pacientů s biliárním leakem byla 23krát vyšší perioperační mortalita (p<0,001) než v souboru bez leaku. Závěr: Biliární leak je jednou z nejzávažnějších komplikací jaterních resekcí. Většina rizikových faktorů je těžko ovlivnitelných a na jejich vliv nepanuje jednoznačná shoda. Incidenci by mohly redukovat diskutované peroperační leak testy. V budoucnu bude třeba dále pracovat na zlepšení perioperačního managementu a techniky k prevenci vzniku této závažné komplikace. V léčbě se uplatňuje multidisciplinární přístup., Introduction: Many previous reports have focused on bile leakage after liver resection. Despite the improvements in surgical techniques and perioperative care the incidence of this complication rather keeps increasing. A number of predictive factors have been analyzed. There is still no consensus regarding their influence on the formation of bile leakage. The objective of our analysis was to evaluate the incidence of bile leakage, its impact on mortality and duration of hospitalization at our department. At the same time, we conducted an analysis of known predictive factors. Method: The authors present a retrospective review of the set of 146 patients who underwent liver resection at the Department of Surgery of the 2nd Faculty of Medicine of the Charles University and Central Military Hospital Prague, performed between 2010−2013. We used the current ISGLS (International Study Group of Liver Surgery) classification to evaluate the bile leakage. The severity of this complication was determined according to the Clavien-Dindo classification system. Statistical significance of the predictive factors was determined using Fisher‘s exact test and Student‘s t-test. Results: The incidence of bile leakage was 21%. According to ISGLS classification the A, B, and C rates were 6.5%, 61.2%, and 32.3%, respectively. The severity of bile leakage according to the Clavien-Dindo classification system – I-II, IIIa, IIIb, IV and V rates were 19.3%, 42%, 9.7%, 9.7%, and 19.3%, respectively. We determined the following predictive factors as statistically significant: surgery for malignancy (p<0.001), major hepatic resection (p=0.001), operative time (p<0.001), high intraoperative blood loss (p=0.02), construction of HJA (p=0.005), portal venous embolization/two-stage surgery (p=0.009) and ASA score (p=0.02). Bile leakage significantly prolonged hospitalization time (p<0.001). In the group of patients with bile leakage the perioperative mortality was 23 times higher (p<0.001) than in the group with no leakage. Conclusion: Bile leakage is one of the most serious complications of liver surgery. Most of the risk factors are not easily controllable and there is no clear consensus on their influence. Intraoperative leak tests could probably reduce the incidence of bile leakage. In the future, further studies will be required to improve the perioperative management and techniques to prevent such serious complications. Předmětem této práce je objektivně posoudit dostupná výzkumná data týkající se bezpečnosti biologické terapie v těhotenství a kojení pacientek s idiopatickými střevními záněty. Biologická léčba se zdá být v graviditě bezpečná bez zvýšeného rizika vrozených malformací. Dostupné klinické údaje naznačují, že účinnost infliximabu a adalimumabu v dosažení klinické odpovědi a udržení remise těhotných pacientek převažuje teoretické riziko ohrožení plodu těmito léky. Terapie anti-TNFα preparáty by měla být, pokud je to možné, ukončena na konci 2. trimestru pro transplacentární přestup a z toho vyplývající riziko pro plod. Užití infliximabu a adalimumabu je pravděpodobně slučitelné s kojením. If possible, anti-TNF therapy should be stopped by the end of the second trimester due to transplacental transfer and potential risk for the fetus. The use of infliximab and adalimumab is probably compatible with breastfeeding. Úvod: Radioimunoterapie patří mezi rychle se rozvíjející klinické metody. Často je nejvhodnější možností terapie u pacientů s relabujícím lymfomem, případně u starších pacientů. Vývoj nových typů imunokonjugátů patří proto v posledních letech mezi klíčová témata ve vývoji radiofarmak. Metody: Imunoglobulin G M75 byl značen metodou elektrofilní jodace radionuklidem 125I, který je optimální pro vývoj metody značení a také pro stanovení vlastností značeného preparátu (vazebná aktivita, imunospecifita). Výhody 125I jsou zejména jeho dlouhý poločas (59,408 dne) a nízká radiační zátěž. Zachování vazby aktivního místa na antigen bylo testováno metodou ELISA, koncentrace byla stanovena modifikovanou metodou podle Bradfordové. Fyziologická distribuce byla testována na myších CD1-Foxn1nu. Výsledky: Stanovená kumulace značené protilátky v nádorové tkáni byla > 7 % podané radioaktivity, vazebná aktivita protilátky byla první den po značení vyšší než 90 % ve srovnání s vazebnou aktivitou neznačené protilátky. Závěr: Připravený radioimunokonjugát IgG M75 s 125I si zachovává dostatečnou schopnost vazby antigenu, a má proto potenciál v terapii tumorů exprimujících karbonátdehydratasu. Bisphosphonates (BP) are potent inhibitors of bone resorption used mainly in the treatment of metastatic bone disease and osteoporosis. By inhibiting bone resorption, they prevent complications as pathological fracture, pain, tumor-induced hypercalcemia. Even though patient’s benefit of BP therapy is huge, various side effects may develop. Bisphosphonate-related osteonecrosis of the jaws (BRONJ) is among the most serious ones. Oncologic patients receiving high doses of BP intravenously are at high risk of BRONJ development. BPs impair bone turnover leading to compromised bone healing which may result in the exposure of necrotic bone in the oral cavity frequently following tooth extraction or trauma of the oral mucosa. Frank bone exposure may be complicated by secondary infection leading to osteomyelitis development with various symptoms and radiological findings. In the management of BRONJ, conservative therapy aiming to reduce the symptoms plays the main role. In patients with extensive bone involvement resective surgery may lead to complete recovery, provided that the procedure is correctly indicated. Since the treatment of BRONJ is difficult, prevention is the main goal. Therefore in high risk patients dental preventive measures should be taken prior to bisphosphonate administration. Cíle studie: 1. Zhodnocení buněčností likvoru (CSF, z angl. cerebrospinal fluid = mozkomíšní mok), koncentrací glukózy v CSF, hodnot glukózového kvocientu (Q glu. ), koncentrací laktátu v CSF a hodnot koeficientu energetické bilance (KEB) coby ukazatelů intenzity zánětlivého procesu v centrálním nervovém systému (CNS) ve skupinách pacientů bez postižení CNS, pacientů s mírnými serózními záněty CNS neinfekční etiologie, pacientů se serózními záněty CNS infekční etiologie a pa- cientů s purulentními záněty CNS s extracelulárními bakteriemi v patogenezi. 2. Porovnání informačního potenciálu parametrů energetického metabolismu glukózy v likvorovém kompartmentu v soubo - ru vyšetřených pacientů, tzn. koncentrací glukózy v CSF, hodnot Q glu. , koncentrací laktátu v CSF a hodnot KEB. Typ studie: Retrospektivní studie. Materiál a metody: Vyšetřili jsme 133 vzorků CSF pacientů bez postižení CNS, 227 vzorků CSF pacientů s mírnými serózními záněty CNS s intrathekální syntézou imunoglobulinů neinfekční etiologie, 208 vzorků CSF pacientů se serózními záněty CNS infekční příčiny a 140 vzorků CSF pacientů s purulentními záněty CNS s extracelulárními bakteriemi v patogenezi. Oblastí našeho zájmu byly buněčnost CSF, koncentrace glukózy v CSF, hodnoty Q glu. , koncentrace laktátu v CSF a hodnoty KEB. Pro statistické hodnocení vybraných parametrů jsme použili D’Agostinův omnibus test, Kruskalův-Wallisův test s následnou post hoc analýzou Dunnovou metodou, Spearmanův korelační koeficient a multinomiální logistickou regresní analýzu. Výsledky: V likvorovém kompartmentu pacientů bez postižení CNS jsme nezjistili žádné změny na úrovni buněčností CSF ani změny energetické. V likvorovém kompartmentu pacientů s mírnými serózními záněty CNS neinfekční etiologie byly pouze v některých případech patrny zvýšené buněčnosti CSF a jisté odchylky hodnot Q glu. , koncentrací laktátu v CSF a hod - not KEB od normálu. V likvorovém kompartmentu pacientů se serózními záněty CNS infekční etiologie byly pozorovány projevy nápadně zvýšené intenzity zánětlivého procesu v likvorovém kompartmentu především na úrovni buněčností CSF, koncentrací laktátu v CSF a hodnot KEB. Vysoká intenzita purulentního zánětlivého procesu bakteriální etiologie v CNS se výrazně projevila na úrovni všech sledovaných parametrů. Nejtěsnější vztah jsme v našem souboru pacientů zjistili mezi buněčnostmi CSF a hodnotami KEB ( ρ = -0,770), následoval vztah mezi buněčnostmi CSF a koncentracemi laktátu v CSF ( ρ = 0,734), hodnotami Q glu. ( ρ = -0,676) a koncentracemi glukózy v CSF ( ρ = -0,544). Využitelnost uvedených parametrů k predikci intenzity zánětlivého procesu v CNS jsme ověřili pomocí multinomiální logistické regresní analýzy. Nejvyšší pre - dikční potenciál měly buněčnost CSF se 71,9 % a KEB se 71,6 % správně zařazených pacientů. Následovaly koncentrace laktátu v CSF s 64,7 %, Q glu. s 58,8 % a koncentrace glukózy v CSF s 54,7 % správně klasifikovaných pacientů. Závěr: Získané výsledky podpořily vhodnost využití úrovní buněčností CSF, koncentrací glukózy v CSF, hodnot Q glu. , kon - centrací laktátu v CSF a hodnot KEB k diagnostice postižení a monitorování intenzity zánětů v CNS. Navíc umožnily vytvořit pořadí energetických parametrů v likvorovém kompartmentu pacientů našeho souboru podle úrovně jejich informačního potenciálu. Nejpříznivějších výsledků pro hodnocení intenzity zánětlivého procesu v CNS tak bylo dosaženo v případě hod - not KEB, následovaly je koncentrace laktátu v CSF a hodnoty Q glu. a nejméně příznivých výsledků jsme dosáhli v případě koncentrací glukózy v CSF., Objective: 1. Compare the information potential of the used parameters of the glucose energy metabolism in the CSF compartments in our group of the investigated patients, i.e. concentrations of glucose in the CSF, values of the Q glu. , concentrations of lactate in the CSF and values of the KEB. Design: Retrospective study. Material and Methods: We examined 133 CSF specimens in patients without CNS impairment, 227 CSF specimens in pa - tients with slight serous inflammation with intrathecal synthesis of immunoglobulins of non-infectious aetiology in the CNS, 208 CSF specimens in patients with serous inflammation of infectious aetiology in the CNS and 140 CSF specimens in patients with purulent inflammation in the CNS with extracellular bacteria in pathogenesis. The objects of our interest were numbers of cells in the CSF, concentrations of glucose in the CSF, values of the Q glu. , concentrations of lactate in the CSF and values of the KEB. The D’Agostino Omnibus test, the Kruskal-Wallis test with follow-up post hoc analysis using the Dunn’s method, the Spearman correlation coefficient and the multinomial logistic regression analysis were used for statis - tical analysis of the examined parameters. Results: We did not find any changes in the numbers of cells in the CSF and in energy ratios in the CSF compartment of patients without CNS impairment. We found raised numbers of cells in the CSF and slight alterations of the glucose quo - tients, lactate concentrations in the CSF and the values of the KEB only in some patients with slight serous inflammations of non-infectious aetiology in the CNS. We observed manifestations of conspicuously increased intensity of inflammation in the numbers of cells in the CSF, lactate concentrations in the CSF and the values of the KEB in patients with serous inflam - mations of infectious aetiology in the CNS. Very high intensity of purulent inflammation in the CNS of bacterial aetiology was well apparent in all the evaluated parameters. Concerning the relationship, either direct or indirect, between the number of cells in the CSF and the other parameters, we found the highest correlation between the number of cells in the CSF and the values of the KEB ( ρ = -0.770), followed by the lactate concentrations in the CSF ( ρ = 0.734), the Q glu. ( ρ = -0.676) and the glucose concentrations in the CSF ( ρ = -0.544). We verified the applicability of the parameters mentioned above for prediction of the intensity of inflammation in the CNS via multinomial logistic regression analysis. The number of cells and the KEB, with 71.9 % and 71.6 % respectively, has the highest prediction potential of the correctly classified patients. They were followed by the lactate concentration in the CSF with 64.7 %, the Q glu. with 58.8 % and the glucose concentration with 54.7 % of the correctly classified patients. Conclusion: Our study supports the applicability of the numbers of cells in the CSF, the glucose concentrations in the CSF, the values of the Q glu. , the lactate concentrations in the CSF and the values of the KEB for diagnosing CSF impairment and for monitoring the intensity of inflammation in the CNS. Further, the results enabled determination of the information potential of the energy parameters. The values of the KEB were most suitable for evaluation of the intensity of inflammation in the CNS. Less suitable results were achieved in case of the lactate concentrations in the CSF.
