A hybrid approach to elephant trunk technique for treatment of thoracic aortic aneurysms combines a conventional surgical and endovascular therapy. Compared to surgery alone, there is a presumption that mortality and morbidity is reduced. We present a case report of a 42-year-old man with a giant aneurysm of the entire thoracic aorta, significant aortic and tricuspid regurgitation and ventricular septum defect. Obrovskobuněčné arteritida je řazena mezi vaskulitidy velkých cév. Jejím prvním projevem může být horečnatý stav s elevací markem zánětu (FW a CRP), v rámci vyšetřování takových nemocných lze využít PET (PET/CT) vyšetření. Cíl. Ověření, zda vysoká akumulace FDG ve stěnách velkých tepen odpovídá obrovskobuněčné arteritidě. Metoda. Zařazeno bylo 39 nemocných s podezřením na vaskulitidu velkých tepen na základě PET (25 pacientů) a PET/CT (14 pacientů), jednalo se o 30 žen a devět mužů ve věku 45-81 let, průměr 64,9 let, medián 65 let. K potvrzení byl zvolen přímý průkaz histologickým vyšetřením z excize tepny (4 pacienti) nebo nepřímý - terapeutický test poklesem FW a CRP při kortikoterapii v imunosupresivních dávkách (39 pacientů). Akumulace FDG byla měřena v sedmi cévních oblastech, pokud přesahovala akumulaci referenčního jaterního parenchymu, byla oblast hodnocena jako pozitivní. Kontrolním souborem byl vzorek 100 pacientů bez vaskulitidy. Porovnávány byly výsledky dalších zobrazovacích vyšetření. Výsledky. Obrovskobuněčné arteritis byla ověřena u tří ze čtyř histologických vyšetření. Nepřímým testem byla potvrzena vaskulitida u všech 39 pacientů. U pacientů s vaskulitidou před nasazením kortikoterapie byla nalézána pozitivita alespoň tří cévních oblastí, zatímco v kontrolním souboru byla nalézána pozitivita 0-2 cévních oblastí. Další zobrazovací vyšetření jasné známky vaskulitidy nedetekovala, mohla být hodnocena jako abnormní. Závěr. Nález generalizované vysoké akumulace FDG ve velkých tepnách u nemocných se známkami aktivního zánětlivého onemocnění svědčil pro vaskulitidu velkých tepen. PET (PET/CT) vyšetření detekovalo onemocnění v časné fázi a určovalo i jeho rozsah. Přímé zobrazení postižení spánkových tepen se zdařilo až za použití PET/CT skeneru., Aim. To verify if high FDG uptake in the walls of large vessels is corresponding with giant cell arteritis. Method. Based on PET (25 pts) and PET/ CT (14 pts) examinations, 39 patients with suspicion of large-vessel vasculitis formed a cohort in which were 30 males and 9 females aged 45-81 with a mean age of 64.9 and median of 65 years. For verifying, a direct proof using histologie examination of a vessel excision (4 pts) or an indirect one - a therapeutic test for ESR and CRP decreases during corticotherapy in immunosuppressive doses (39 pts) were opted. FDG uptake was measured in 7 vessel areas with an area evaluated as positive when it surpassed an uptake of the liver parenchyma. As a control group a sample of 100 patients free from vasculitis was used. For comparison, results from further imaging examinations were used. Results. Giant cell arteritis was proved in 3 from 4 histologie examinations. By indirect testing, vasculitis in all 39 patients was confirmed. In patients with vasculitis before the use of corticotherapy, positivity of at least 3 vessel areas was found, whereas in the control group positivity was observed in 0-2 vessel areas. Further imaging examinations did not detect clear symptoms of vasculitis, it could be evaluated as abnormal. Conclusion. The finding of generalized high FDG uptake in large vessels in patients with symptoms of active inflammatory disease gave evidence for large-vessel vasculitis. The PET (PET/CT) examinations detected the disease in its early phase and also determined the extent thereof. Alzheimerova choroba (ACH) je nejčastější forma demence. Jedná se o degenerativní, nevyléčitelné, terminální onemocnění. Postihuje až 75 % nemocných na celém světě postižených demencí. Prevalence tohoto onemocnění je vysoká, etiologie stále není známá. Je sledována řada rizikových faktorů. Pilotní studie projektu Epidemiologie a genetika Alzheimerovy choroby s 334 případy a 102 kontrolami z psychiatrických léčeben přináší následující výsledky z dotazníkového šetření a stěru z bukální sliznice. Dotazník se skládá z otázek zaměřených na základní identifikační údaje, vzdělání, životní styl, vaskulární rizikové faktory, zájmy, zaměstnání, výskyt nemoci v rodině, výsledky Minimental State Examination (MMSE). Dále jsou uvedeny výsledky analýzy polymorfizmů genu pro angiotenzinkonvertázu (ACE). Ve výsledcích je naznačeno, že u pacientů s ACH se v anamnéze častěji vyskytuje kardiovaskulární onemocnění a úraz hlavy. Cíle studie: 1. Zhodnocení buněčností likvoru (CSF, z angl. cerebrospinal fluid = mozkomíšní mok), koncentrací glukózy v CSF, hodnot glukózového kvocientu (Q glu. ), koncentrací laktátu v CSF a hodnot koeficientu energetické bilance (KEB) coby ukazatelů intenzity zánětlivého procesu v centrálním nervovém systému (CNS) ve skupinách pacientů bez postižení CNS, pacientů s mírnými serózními záněty CNS neinfekční etiologie, pacientů se serózními záněty CNS infekční etiologie a pa- cientů s purulentními záněty CNS s extracelulárními bakteriemi v patogenezi. 2. Porovnání informačního potenciálu parametrů energetického metabolismu glukózy v likvorovém kompartmentu v soubo - ru vyšetřených pacientů, tzn. koncentrací glukózy v CSF, hodnot Q glu. , koncentrací laktátu v CSF a hodnot KEB. Typ studie: Retrospektivní studie. Materiál a metody: Vyšetřili jsme 133 vzorků CSF pacientů bez postižení CNS, 227 vzorků CSF pacientů s mírnými serózními záněty CNS s intrathekální syntézou imunoglobulinů neinfekční etiologie, 208 vzorků CSF pacientů se serózními záněty CNS infekční příčiny a 140 vzorků CSF pacientů s purulentními záněty CNS s extracelulárními bakteriemi v patogenezi. Oblastí našeho zájmu byly buněčnost CSF, koncentrace glukózy v CSF, hodnoty Q glu. , koncentrace laktátu v CSF a hodnoty KEB. Pro statistické hodnocení vybraných parametrů jsme použili D’Agostinův omnibus test, Kruskalův-Wallisův test s následnou post hoc analýzou Dunnovou metodou, Spearmanův korelační koeficient a multinomiální logistickou regresní analýzu. Výsledky: V likvorovém kompartmentu pacientů bez postižení CNS jsme nezjistili žádné změny na úrovni buněčností CSF ani změny energetické. V likvorovém kompartmentu pacientů s mírnými serózními záněty CNS neinfekční etiologie byly pouze v některých případech patrny zvýšené buněčnosti CSF a jisté odchylky hodnot Q glu. , koncentrací laktátu v CSF a hod - not KEB od normálu. V likvorovém kompartmentu pacientů se serózními záněty CNS infekční etiologie byly pozorovány projevy nápadně zvýšené intenzity zánětlivého procesu v likvorovém kompartmentu především na úrovni buněčností CSF, koncentrací laktátu v CSF a hodnot KEB. Vysoká intenzita purulentního zánětlivého procesu bakteriální etiologie v CNS se výrazně projevila na úrovni všech sledovaných parametrů. Nejtěsnější vztah jsme v našem souboru pacientů zjistili mezi buněčnostmi CSF a hodnotami KEB ( ρ = -0,770), následoval vztah mezi buněčnostmi CSF a koncentracemi laktátu v CSF ( ρ = 0,734), hodnotami Q glu. ( ρ = -0,676) a koncentracemi glukózy v CSF ( ρ = -0,544). Využitelnost uvedených parametrů k predikci intenzity zánětlivého procesu v CNS jsme ověřili pomocí multinomiální logistické regresní analýzy. Nejvyšší pre - dikční potenciál měly buněčnost CSF se 71,9 % a KEB se 71,6 % správně zařazených pacientů. Následovaly koncentrace laktátu v CSF s 64,7 %, Q glu. s 58,8 % a koncentrace glukózy v CSF s 54,7 % správně klasifikovaných pacientů. Závěr: Získané výsledky podpořily vhodnost využití úrovní buněčností CSF, koncentrací glukózy v CSF, hodnot Q glu. , kon - centrací laktátu v CSF a hodnot KEB k diagnostice postižení a monitorování intenzity zánětů v CNS. Navíc umožnily vytvořit pořadí energetických parametrů v likvorovém kompartmentu pacientů našeho souboru podle úrovně jejich informačního potenciálu. Design: Retrospective study. Material and Methods: We examined 133 CSF specimens in patients without CNS impairment, 227 CSF specimens in pa - tients with slight serous inflammation with intrathecal synthesis of immunoglobulins of non-infectious aetiology in the CNS, 208 CSF specimens in patients with serous inflammation of infectious aetiology in the CNS and 140 CSF specimens in patients with purulent inflammation in the CNS with extracellular bacteria in pathogenesis. The objects of our interest were numbers of cells in the CSF, concentrations of glucose in the CSF, values of the Q glu. , concentrations of lactate in the CSF and values of the KEB. The D’Agostino Omnibus test, the Kruskal-Wallis test with follow-up post hoc analysis using the Dunn’s method, the Spearman correlation coefficient and the multinomial logistic regression analysis were used for statis - tical analysis of the examined parameters. Results: We did not find any changes in the numbers of cells in the CSF and in energy ratios in the CSF compartment of patients without CNS impairment. We found raised numbers of cells in the CSF and slight alterations of the glucose quo - tients, lactate concentrations in the CSF and the values of the KEB only in some patients with slight serous inflammations of non-infectious aetiology in the CNS. We observed manifestations of conspicuously increased intensity of inflammation in the numbers of cells in the CSF, lactate concentrations in the CSF and the values of the KEB in patients with serous inflam - mations of infectious aetiology in the CNS. Very high intensity of purulent inflammation in the CNS of bacterial aetiology was well apparent in all the evaluated parameters. Concerning the relationship, either direct or indirect, between the number of cells in the CSF and the other parameters, we found the highest correlation between the number of cells in the CSF and the values of the KEB ( ρ = -0.770), followed by the lactate concentrations in the CSF ( ρ = 0.734), the Q glu. ( ρ = -0.676) and the glucose concentrations in the CSF ( ρ = -0.544). We verified the applicability of the parameters mentioned above for prediction of the intensity of inflammation in the CNS via multinomial logistic regression analysis. The number of cells and the KEB, with 71.9 % and 71.6 % respectively, has the highest prediction potential of the correctly classified patients. They were followed by the lactate concentration in the CSF with 64.7 %, the Q glu. with 58.8 % and the glucose concentration with 54.7 % of the correctly classified patients. Conclusion: Our study supports the applicability of the numbers of cells in the CSF, the glucose concentrations in the CSF, the values of the Q glu. , the lactate concentrations in the CSF and the values of the KEB for diagnosing CSF impairment and for monitoring the intensity of inflammation in the CNS. Further, the results enabled determination of the information potential of the energy parameters. The values of the KEB were most suitable for evaluation of the intensity of inflammation in the CNS. Less suitable results were achieved in case of the lactate concentrations in the CSF. Chronická neinfekční onemocnění představují závažný problém soudobé společnosti. Jejich etiologie má komplexní charakter. Jsou výsledkem v čase se vyvíjející interakce genetických a vnějších faktorů, jejichž podstatná část je vázána na životní styl a chování jedince. Genetická složka může být životním stylem modifikována, do jisté míry však sama chování a životní styl determinuje. Celoživotní vliv prostředí na genom je z velké části zprostředkován epigeneticky. Epigenetické mechanismy řídí chování genů, jsou podkladem fenotypové plasticity. Expozice faktorům modifikujícím epigenom v prenatálním a časně postnatálním období formuje vnímavost ke vzniku onemocnění, v dalším životě pak modifikuje manifestaci genetické vlohy a potenciálně participuje na patogenezi. Prenatálně indukované změny epigenomu mohou být předány na další generace potomků. Významným induktorem těchto alterací jsou nutriční faktory a kouření. V souvislosti s environmentální modifikací genové exprese je studována zejména DNA metylace, post-translační modifikace histonů a regulační aktivita nekódující RNA. Potenciální reverzibilita epigenetických procesů otevírá nové možnosti pro prevenci komplexních chorob. Their etiology is complex as they result from a time-evolving interaction of genetic and environmental factors, a significant part of which are based on individual behaviour and lifestyle. Lifestyle can modify the impact of genetic load; nonetheless, behaviour and lifestyle are themselves determined by genes to a certain extent. Lifelong environmental influences on the genome are largely mediated epigenetically. Epigenetic mechanisms modify the activity of genes and form a base of the phenotype plasticity. Exposure to epigenetic modificators prenatally and in the early post-natal stage form susceptibility to diseases, modifiying manifestation of a genetic predisposition later in life and potentially participating in pathogenesis. Prenatally induced changes of the epigenome can be passed on to subsequent generations of offspring. Nutritional factors and smoking are major inducers of these epigenetic alterations. The most studied epigenetic mechanisms in relation to environmentally induced modification of gene expression are DNA methylation, post-translational histone modifications, and non-coding RNA regulatory activity. The potential reversibility of epigenetic processes opens up new possibilities for the prevention of complex diseases. Životní styl je charakterizován vzorci chování, které komplexně působí na psychické i fyzické zdraví jedince, ovlivňují vznik a rozvoj nemocí, jejich průběh, úspěšnost léčby a kvalitu života. Chování tvořící životní styl je formováno faktory environmentálními, psychosociálními i genetickými, které vzájemně interagují a mají potenciál modifikovat genetické riziko komplexních chorob ovlivněním genové exprese. Cíl: Významem naší prospektivní studie je možnost významné redukce celkového objemu kontrastní látky při CTA DK díky optimalizaci akvizičního času na základě individuálních cirkulačních parametrů. Metoda: Celkem u 68 osob s ischemickou chorobou dolních končetin bylo prospektivně provedeno CT angiografické vyšetření břišní aorty a dolních končetin (CTA DK) s použitím kontrastní látky o objemu 40 ml (31 osob) a 50 ml (37 osob) a vysokou rychlostí aplikace (6 ml/s). Akviziční čas byl individuálně vypočten na základě aorto-popliteálního (Ao-P) tranzitního času získaného stanovením denzitní bolusové křivky na dvou úrovních (aorta/popliteální tepna) po jednorázovém podání 10 ml kontrastní látky (tzv. dvojitý testovací bolus). Kvalita vyšetření vyšetření byla hodnocena subjektivním (skóre 1-3) a objektivním měřením denzity v pěti úrovních. Vyšetření byla označena jako nediagnostická, pokud byla alespoň jedna oblast při subjektivním hodnocení ohodnocena skórem 3. Ve statistickém hodnocení byla porovnána dosažená kvalita s hodnotami akvizičního času a zpoždění. Výsledky: Prokázali jsme statisticky významnou korelaci akvizičního času (21-43) a subjektivního (p = 0,0015) a objektivního (p = 0,0117) hodnocení kvality. Také v případě Ao-P tranzitního času byla prokázána statistická korelace s hodnocením kvality (0,0013, resp. 0,0191). Signifikantně vyšší akviziční čas byl použit u vyšetření s celkově subjektivně nediagnostickou kvalitou (p = 0, 073) a ve všech případech byl vyšší než 31 s. Naopak všechna vyšetření s akvizičními časy pod 30 s byla plně diagnostická. Při podání 50 ml kontrastní látky bylo dosaženo významně vyšší denzity v jednotlivých sledovaných úrovních (p = 0,076), hodnoty akvizičního času a Ao-P tranzitního času se významně mezi skupinami s různým množstvím kontrastní látky nelišily. Závěr: Naše práce prokázala možnost významné redukce objemu kontrastní látky při CTA DK za pomoci optimalizace akvizičního času na základě Ao-P tranzitního času, který byl získán po jednorázové aplikaci 10 ml kontrastní látky (dvojitý test bolus). Byla prokázána statisticky významná korelace akvizičního času a Ao-P tranzitního času na kvalitě provedených vyšetření., Aim: The goal of the prospective study is to determine feasibility of peripheral CT angiography (CTA) with adaptive scan time using low volume of contrast agent. Methods: We performed CTA in extent of abdominal aorta and lower limbs on dual-source CT in two groups of patients with different volumes of contrast agent (400 mgl/l). Volumes of 40ml (31 patients) and 50 ml (21 patients) with 6ml/s injection were used for examinations. Individual scan time was calculated from peak transit time established from double test bolus (single administration of 10 ml) measurements (proximal abdominal aorta and popliteal arteries). We analyzed subjective quality of CTA (1-3) and objective quality (intraluminal density and CNR) in 5 different levels. Examinations with subjective quality 3 (non-diagnostic) in one level were considered as overall non-diagnostic. Results: We found significant correlation between scan time (21-43 s) and subjective (p = 0.0015) and objective quality (p = 0.0117) in both groups. Significantly higher (p = 0.029) scan time was used in examinations with overall non-diagnostic quality (6 in 40 ml group and 3 in 50 ml group). The scan times in non-diagnostic examinations were completely over 31s (31.6-42.4) and all examinations with scan time below 30 s were with excellent quality. Significantly better objective quality was reached in 50 ml group (p = 0.076). Time parameters were not significantly different in both groups. Conclusion: Our results have proved feasibility of peripheral CTA with adaptive scan time and low volume of contrast agent. Cíl. Posoudit vliv iterativní rekonstrukce dat na efektivní dávky vyšetření CT s duální energií záření (DECT). Metoda. Byla zhodnocena efektivní dávka u celkem jedenácti skupin po 40 vyšetřeních provedených na dvouzdrojovém přístroji první a druhé generace za použití režimu jednozdrojového jednoenergetického a DECT s použitím iterativní rekonstrukce technikou sinogram affirmed iterative reconstruction (SAFIRE) v oblasti hrudníku a břicha včetně protokolů využívajících dvoufázové akvizice dat v režimu DECT. Výsledky. Bylo prokázáno pomocí t-testu pro nezávislé výběry, že mezi vyšetřeními na přístroji druhé generace dvouzdrojového CT s použitím iterativního algoritmu SAFIRE není statisticky významný rozdíl v dávce mezi režimem jednozdrojovým jedno-energetickým režimem DECT (p = 0,0299; 0,9337; 0, 8207), naopak významný rozdíl je mezi protokoly DECT bez iterativní rekonstrukce a s ní (p < 0,0001 u všech porovnání). Závěr. Iterativní rekonstrukce umožňuje významně snížit efektivní dávku i protokolech DECT a zachovat dávkovou neutralitu ve srovnání s vyšetřeními na jednozdrojových systémech., Aim. To evaluate the influence of the iterative reconstruction on the effective dose obtained from dual-energy CT (DECT). Method. The effective dose was calculated within the 11 groups per 40 examinations performed using first and second generation of dual-source systems. Examinations were obtained in single-source single-energy and dual-source dual-energy regimens in thoracic or abdominal regions including the protocols containing iterative data reconstruction using algorithm sonogram affirmed reconstruction (SAFIRE). Results. Using the t-test for independent data samples was confirmed, that the effective dose obtained with DECT regimen does not differ from those obtained in single-source-single-energy regimen, even if both using SAFÍRE algorithm (p = 0.0299; 0.9337; 0.8207), but on the other side the significant diference occured between DECT with and without iterative reconstruction (p < 0.0001 in all). In view of the high anti-oxidative potential oftocotrienol, the role of the tocotrienol-rich fraction (TRF) of palm oil in preventing pregnancy induced hypertension (PIH) was explored in a randomized double-blind placebo-controlled clinical trial in an urban teaching hospital. Healthy primigravidae were randomized to receive either oral TRF 100 mg daily or placebo, from early second trimester until delivery. Out of 299 women, 151 were randomized into the TRF arm and 148 into the placebo arm. A total of 15 (5.0%) developed PIH. Although there was no statistically significant difference in the incidence of PIH (4/151 or 2.6% in the TRF arm vs. 11/148 or 7.4% in the placebo arm, p = 0.058) between the two arms, there was a tendency towards a lower incidence of PIH in the TRF arm compared to the placebo arm. With TRF supplementation, the relative risk (RR) of PIH was 0.36 (95% CI 0.12-1.09). In conclusion, although TRF from palm oil does not statistically significantly reduce the risk of development of PIH in the population studied, the 64% reduction in incidence of PIH is substantial. INTRODUCTION: Autologous stem cell transplantation (ASCT) became standard of care for patients with multiple myeloma (MM) under the age of 65 years. We routinely perform ASCT for newly diagnosed MM since 1996 in our department. PATIENTS AND METHODS: We retrospectively analyzed all 285 transplants in 185 patients done for MM from January 1996 till December 2010. We analyzed overall survival (OS) and progression-free survival (PFS) regarding conditioning, stage, complete or very good partial remission (CR, VGPR) achievement, renal impairment, single vs. double transplant. RESULTS: Estimated 10-years survival of the whole set of patients is 39% (median survival 95 months). Patients with renal impairment show same OS as those without (p = 0.22). Patients show similar overall survival and event free survival regardless of type of transplant. We observed better outcome in terms of overall survival in patients treated with new drugs (p = 0.0014). Reaching CR or VGPR was not translated into better OS (p = 0.30) and EFS (p = 0.10). Also stage of the disease and whether single or double transplant was used did not make any significant difference in the outcome. CONCLUSION: Stem cell transplantation greatly improved outcome of patients with MM. Poor outcome of allogeneic transplantation in our group of patients is related to high transplant related mortality (20% vs. 0%) and unexpected high relapse rate. There is a trend towards better survival, when new drugs are incorporated at any time in the course of the disease.
