Their efficacy is being tested in numerous clinical trials worldwide. Eribulin je netaxanový inhibitor dynamiky mikrotubulů. Je syntetickým analogem halichondrinu B. Působením na mikrotubuly eribulin účinkuje antimitoticky v G2/M fázi buněčného cyklu, ničí mitotické vřeténko a způsobuje apoptózu. Specificky inhibuje růstovou fázi mikrotubulů a sekvestruje tubulin v neproduktivní agregáty. V poslední době byly publikovány výsledky preklinického výzkumu „nemitotického“ mechanizmu účinku eribulinu (zejm. vliv na vaskularizaci nádoru, epiteliálně mezenchymální přechod, invazivitu a schopnost migrace)., Eribulin is a non-taxane microtubule dynamics inhibitor. It is a synthetic analogue of halichondrin B. By acting on microtubules, eribulin has an antimitotic effect in the G2/M phase of cell cycle, disrupting the mitotic spindle and inducing apoptosis. It specifically inhibits the growth phase of microtubules and sequesters tubulin in non-productive aggregates. Extrakty z rostlin jsou důležitým zdrojem většího počtu klinicky používaných protinádorových léčiv. Většina působí jako konvenční cytostatika modulující mikrotubuly (vinkristin, vinblastin, taxany), nebo inhibitory topoizomeráz jako např. deriváty epipodofylotoxinu a kamptotecinu (etopozid, irinotecan). Z rostlin či mořských organizmů jsou odvozeny také látky s účinkem na cílené nitrobuněčné struktury (flavopiridol, kombrestatin aj). Řada látek obsažených v ovoci a zelenině může interferovat z různými etapami kancerogeneze. Nádory skupiny Ewingova sarkomu/primitivního neuroektodermálního tumoru (ES/PNET) jsou typické pro věkovou skupinu dětí a adolescentů. Devadesát procent pacientů je mladších 20 let. Prognóza je nepříznivá, s 5letým přežitím mezi 55–70 % v případě lokalizovaného, méně než 20 % u metastatického onemocnění. S narůstajícím věkem jsou výsledky léčby ještě méně příznivé. Léčba by měla být vedena na základě rozhodnutí a spolupráce v rámci multioborového týmu. Uvádíme naše zkušenosti s léčbou ES/PNET dospělých pacientů léčených dle protokolů EURO E.W.I.N.G. 99/EWING 2008., Ewing´s Family of Tumours is most frequently observed in children and adolescent age. 90 % of patients are younger than 20 years. Prognosis is unfavourable, with 5-year overall survival from 55 to 70 % in localized, not exceeding 20 % in primarily metastatic disease. The results of the therapy are getting worse by increasing age. Multidisciplinary treatment planning is mandatory in all cases of this tumor. Léčba chronické lymfocytární leukemie (CLL) dosáhla v posledních letech mimořádných pokroků. Důležitým mezníkem v léčbě CLL bylo zavedení monoklonálních protilátek a kombinované imunochemoterapie, které vedlo k významnému zlepšení léčebných výsledků a poprvé i k prodloužení celkového přežití. Navzdory těmto léčebným úspěchům je CLL stále považována za nevyléčitelné onemocnění. Pouze alogenní transplantace má potenciálně kurativní potenciál. Ta je však proveditelná pouze u malé, selektované skupiny mladších nemocných. S rozšiřujícími se poznatky o CLL a novými léčebnými možnostmi se snažíme léčbu nemocných maximálně individualizovat s přihlédnutím k věku nemocných, biologickým faktorům, přidruženým onemocněním a našim terapeutickým cílům. Vývoj nových léků a léčebných kombinací u CLL postupuje rychle vpřed a poskytuje tak naději na další zlepšování prognózy našich pacientů., The treatment of chronic lymphocytic leukemia (CLL) has achieved extraordinary progress over the last years. An important milestone in the treatment of CLL was reached with the incorporation of monoclonal antibodies and combined chemoimmunotherapy which led to significant improvements in treatment outcomes and survival advantage for the first time. Despite these therapeutic successes, CLL is still considered to be an incurable disease. Only the allogenic transplantation is potentially curative but it is feasible only for selected group of younger patients. There is an effort to individualize the treatment of patients with regard to the age of patients, biological factors, comorbidities and our therapeutic goals. High grade gliomas are some of the deadliest human tumours. Conventional treatments such as surgery, radiotherapy and chemotherapy have only a limited effect. Nowadays, resection is the common treatment of choice and although new approaches, such as perioperative magnetic resonance imaging or fluorescent microscopy have been developed, the survival rate of diagnosed patients is still very low. The inefficacy of conventional methods has led to the development of new strategies and the significant progress of nanotechnology in recent years. These platforms can be used either as novel imaging tools or to improve anticancer drug delivery into tumours while minimizing its distribution and toxicity in healthy tissues. Amongst the new nanotechnology platforms used for delivery into the brain tissue are: polymeric nanoparticles, liposomes, dendrimers, nanoshells, carbon nanotubes, superparamagnetic nanoparticles and nucleic acid based nanoparticles (DNA, RNA interference [RNAi] and antisense oligonucleotides [ASO]). These nanoparticles have been applied in the delivery of small molecular weight drugs as well as macromolecules - proteins, peptides and genes. The unique properties of these nanoparticles, such as surface charge, particle size, composition and ability to modify their surface with tissue recognition ligands and antibodies, improve their biodistribution and pharmacokinetics. All of the above mentioned characteristics make of nanoplatforms a very suitable tool for its use in targeted, personalized medicine, where they could possibly carry large doses of therapeutic agents specifically into malignant cells while avoiding healthy cells. Karcinóm prsníka je príkladom rýchleho rozvoja vedeckých poznatkov, začínajúc od molekulárnej diagnostiky a končiac cielenou liečbou na molekulárnej úrovni. Neoadjuvantná chemoterapia je indikovaná u veľmi heterogénnej skupiny nádorov. O type liečby rozhoduje nielen biologická variabilita (stav receptorov, stav HER-2), ale tiež individuálna variabilita (PS, vek, komorbidita, rozsah ochorenia). Primárna systémová liečba (neoadjuvantná, indukčná, iniciálna) je dnes štandardná pre lokálne pokročilé nádory a niektoré vybrané operabilné typy karcinómov prsníka. Pre neuspokojivú liečebnú odpoveď sa hľadajú nové režimy s iným konceptom dávkovej intenzity, denzity, sekvencie. V neoadjuvancii je možné využiť všetky necielené aj cielené liečebné metódy na podklade výsledkov mnohých klinických štúdií., Breast cancer is a model of quickly development scientific knowledges begining from molecular diagnostic and termination with targeted therapies on the molecular level. Neoadjuvant chemotherapy is indicated for very heterogeneous group of tumours. Treatment type is decided not only by biologic variability (receptor status, HER-2 status) but with individual variability (PS, comorbidity, extent of illness). Primary systemic treatment (neoadjuvant, induction, inicials) is standard treatment for locally advanced tumor and for some eligible type of surgery breast cancer, too. If treatment response is not satisfied it is necessary to look for new treatment regimens with different concept of dose intensity, density and sequence of treatment. Výskyt jaterních metastáz kolorektálního karcinomu se odvíjí od incidence KRK, která je v České republice jedna z nejvyšších na světě a představuje cca 90 nových případů na 100 000 obyvatel/rok. Celkově se jedná ročně o 9.000 nových případů onemocnění. U 60 % z nich se vyvine generalizace onemocnění v podobě jaterních metastáz. Radikální léčbou je provedení resekce jater a podání adjuvantní chemoterapie. Autoři uvádějí typy jaterních resekcí, míru posuzované radikality, faktory vedoucí ke kontraindikaci resekce jater a dále možnosti zvýšení procenta resekability. Nejlepších výsledků v léčbě je dosahováno multimodálním přístupem s uplatňováním personalizovaného přístupu ke každému pacientovi., The occurrence of livermetastatic colorectal carcinoma is related to the incidence of colorectal cancer (CRC) which, in the Czech Republic, is one of the highest worldwide with approximately 90 new cases per 100.000 inhabitants/year. Overall, 9.000 new cases of the disease occur annually. Sixty percent of them develop generalized disease in the form of liver metastases. Radical treatment involves liver resection and administration of adjuvant chemotherapy. The authors report the types of liver resections, the degree of radicality, the factors resulting in contraindication of liver resection as well as the options of increasing the percentage of resectability. Angiomyolipom ledviny (AML) je benigní mezenchymový nádor, který se vyskytuje sporadicky nebo v rámci komplexu tuberózní sklerózy (TSC). Celková prevalence AML je 0,44 % a vyskytuje se čtyřikrát častěji u žen. Prevalence v ženské populaci je tedy 0,6 %, zatímco u mužů 0,28 %. Jeho diagnostika je založena na přítomnosti tukové složky v nádoru a lze tedy identifikovat předoperačně pomocí zobrazovacích metod. Morfologie AML je velmi variabilní a někdy tedy tuková složka chybí, což může způsobit diagnostické rozpaky. Zcela zásadní roli má aktivní sledování pacientů s jasnou diagnózou AML, jehož růst je pomalý s minimální morbiditu. Pacient však může přijít ve stadiu komplikací, které zahrnují hematurii, bolest či krvácení do retroperitonea způsobené rupturou nádoru. Mezi hlavní rizikové faktory krvácení jsou velikost nádoru, procento zastoupení cévní složky, kterou tvoří aneuryzmatické cévy a přítomnost TSC. Selektivní arteriální embolizace úspěšně řeší hemoragické komplikace a redukuje objem AML. V rámci dlouhodobých výsledků však selhává díky revaskularizaci a dalšímu růstu AML. Zde pak mají lepší výsledky chirurgické operace, kde se snažíme v maximální míře šetřit funkci ledviny. Slibné dlouhodobé výsledky má též radiofrekvenční ablace, která je ale silně limitována velikostí AML. U objemnějších AML se zdá být dobrou alternativou resekčního výkonu kryoablace, která však vyžaduje hůře dostupné speciální přístrojové vybavení. Novinkou je pak systémová léčba mTOR inhibitory (everolimus, sirolimus), které mají nezastupitelné místo u nemocných s TSC, kde jsou AML mnohočetné a recidivující. Tato terapie může pomoci redukovat celkový objem AML a umožnit případný resekční výkon., Renal angiomyolipoma (AML) is a benign mesenchymal tumor, which occurs sporadically or is associated with tuberous sclerosis complex (TSC). The overall prevalence of AML is 0.44 %, its incidence in women is 4 times higher than in men. Prevalence in the female population is thus 0.6 %, but male rates 0.28 %. Diagnosis is based on the presence of fat in the tumor. In most cases AML can be identified preoperatively. AML morphology is highly variable and in cases where fat is missing, preoperative diagnostic based on imaging methods is not possible. Active monitoring of patients with AML is crucial, because AML grows slowly and with minimal morbidity. Patient can present with haematuria, pain or retroperitoneal bleeding by tumors rupture. The main risk factors for bleeding are tumors volume, percentage of the vascular component in the tumor and the presence of TSC. Selective arterial embolization resolves hemorrhagic complications and reduces the volume of the tumor. The long-term outcomes of embolisation are poor due to tumor revascularization and growth. Nephron sparring surgery is associated with better long term outcomes. Radiofrequency ablation is another option, but strongly restricted by the volume tumor. An additional option is cryoablation, which has better results in cases where tumor is large, but this requires special equipment. Systematic treatment using mTOR inhibitors (everolimus, sirolimus, has recently been introduced, which are essential in patients with TSC, where the disease is often complicated by multiple and recurrent lesion. Kolorektální karcinom má v České republice nejvyšší četnost výskytu a zaujímá jednu z čelních příček incidence na světě. Pouze nemocní s resekovatelnými vzdálenými metastázami mají naději na vyléčení. Hlavním cílem onkologické léčby tedy zůstává prodloužení přežití a zlepšení kvality života. Možnosti klasické chemoterapie se v současnosti zdají být vyčerpány. Další prodloužení přežití pacientů s metastazujícím kolorektálním karcinomem (mKRK) přinesla cílená biologická léčba. Klíčové je nalezení vhodných prediktivních biomarkerů, které dovolí identifikaci cílové populace pacientů. V přehledovém článku je analyzován stav mutace RAS vzhledem k možné sekvenci cílené biologické léčby u nemocných s mKRK s ohledem na nová data, která byla prezentována na ASCO 2014., Colorectal cancer has the highest incidence rate in the Czech Republic and occupies one of the leading positions in incidence worldwide. Only patients with resectable distant metastases have hope for a cure. Prolongation of survival and improvement in the quality of life thus remain the main goal of cancer treatment. The possibilities of classic chemotherapy appear to be currently exhausted. Further prolongation of survival of patients with metastatic colorectal cancer (mCRC) has been provided by targeted biological therapy. It is crucial to find suitable predictive biomarkers that will allow identification of a target population of patients.
