Autoři uvádějí kazuistiku pacienta s epiteloidním hemangioendoteliomem jater (HEHE), který byl léčen chemoembolizací (TACE). Při dlouhodobém sledování 9 let od stanovení diagnózy je klinicky bez obtíží a bez progrese onemocnění při vyšetření zobrazovacími metodami. Pacienti s chirurgicky neřešitelnými metastázami kolorektálního karcinomu v játrech mají při současné systémové chemoterapii odhad přežití 1 a 2 roky 55 % a 33 %. Radiologická intervenční léčba prováděná pod kontrolou zobrazovacích metod jako je chemoembolizace (TACE), intraarteriální chemoterapie (HAI) a radiofrekvenční termoablace (RFA) má proto u těchto nemocných rostoucí význam. Využití endovaskulární a perkutánní cílené léčby jaterních metastáz má dvojí efekt. Dojde ke kontrole růstu metastáz a k redukci vedlejších účinků systémové léčby. Vývoj nových embolizačních materiálů jako DC beads (DEB), na které lze navázat různá cytostatika jako doxorubicin či irinotecan, pak umožňuje stejně jako u radiofrekvenční termoablace standardizovat protokoly a omezit rozdílné výsledky této léčby mezi pracovišti., Unresectable colorectal liver metastases have a 1- and 2-year survival of 55 % and 33 % with current systemic therapies. Cíl: Zhodnotit přežívání pacientů s hepatocelulárním karcinomem (HCC) léčených transarteriální chemoembolizací (TÁCE) s drug-eluting beads, zhodnotit korelaci délky přežití s faktory souvisejícími s primárním tumorem jater a s faktory souvisejícími s intervencí, léčbou a léčebnou odpovědí. Dalším cílem je zhodnotit reprodukovatelnost měření při hodnocení léčebné odpovědi dle kritérií RECIST, mRECIST a volumetrie. Metodika: Do retrospektivní studie bylo zahrnuto 49 pacientů s diagnózou HCC léčených ve Fakultní nemocnici Brno. Rozsah ložiska byl hodnocen na vstupním i výstupním CT/MR dvěma nezávislými operatéry. Přežívání pacientů bylo hodnoceno od data 1. podstoupené TACE, a to ve vztahu ke známým faktorům ovlivňujícím léčbu HCC. Celkové přežívání bylo hodnoceno metodikou podle Kaplan-Meiera, rozdíly v přežití log-rank testem. Míra reprodukovatelnosti měření byla stanovena pomocí Pearsonova a Spearmanova korelačního koeficientu. Výsledky: Délka přežívání nejlépe korelovala se stanovením viabilní části tumoru volumetricky a poměrem viabilní/aviabilní části tumoru. Medián přežívání od 1. chemoembolizace je 17,1 měsíce. Hodnocení odpovědi na léčbu pomocí kritérií RECIST, mRECIST a volumetrie ve všech případech prokázalo statisticky významnou reprodukovatelnost, nejlépe pak vyšla volumetrická analýza. Závěr: TACE s drug-eluting beads je moderní metoda terapie HCC. Při hodnocení léčebné odpovědi je výhodné využít volumetrické analýzy., Aim: To evaluate the survival of patients with hepatocellular carcinoma (HCC) treated by transarterial chemoembolization (TACE) with drug-eluting beads, to evaluate the correlation of the length of survival with the factors related to primary liver tumour and factors related to interventions, treatment and response to treatment. Another aim is to evaluate the interobserver variability of measurement in the evaluation response to treatment using RECIST and mRECIST criteria and volumetry. Methods: The total of 49 patients treated in the University Hospital Brno with the diagnosis of HCC were included in this retrospective study. The tumour range was evaluated on the CT/MR input and output by two independent doctors. The survival of the patients was evaluated since the date of the first TACE, in relation to well-known factors effecting the treatment of HCC. The survival was evaluated by the Kaplan-Meier method and the differences in survival by the log-rank test. The interobserver variability was determined by using Pearson and Spearman correlation coefficient. Results: The strongest correlation has been proven between the length of survival and determination of the viable part of tumour using volumetry and the ratio viable/ aviable parts of the tumour. The median of survival since the first performed TACE is 17.1 months. The evaluation of the response to treatment using RECIST and mRECIST criteria and volumetry has been proven with statistically significant small interobserver variability; however, the best result was achieved by using volumetric analysis. Conclusion: TACE with drug-eluting beads is a modern method of treatment of HCC. Radiologické metody a postupy se staly neodmyslitelnou součástí screeningu, diagnostiky, terapie a managementu onemocnění pacientů s kolorektálním karcinomem. Do popředí zájmu se dostávají CT kolonografie (virtuální kolonoskopie) v rámci screeningu, metody kontrastního ultrazvuku ke zrychlení diagnostického postupu a magnetická rezonance zejména v precizní diagnostice jaterního postižení a lokálního stagingu karcinomu rekta. V posledním desetiletí se etablovaly radiologické intervenční metody určené pro ty nemocné, u kterých standardní řešení selhává. Intervenční radiologie tak rozšiřuje možnosti léčby u pacientů s diseminovaným kolorektálním karcinomem (ve stadiu IV). Terapeutické invazivní metody při terapii metastatického postižení jater mohou být buď intravaskulární (chemoembolizace a regionální chemoterapie, předoperační embolizace portální žíly), nebo perkutánní (především radiofrekvenční termoablace). Tyto metody je možné využít jak v rámci downstagingu onemocnění se záměrem umožnění kurativní resekce, tak v rámci paliace onemocnění., Radiological methods and procedures have become an inseparable part of the screening, diagnosis, therapy and disease management of patients with colorectal cancer. Major areas of interest are CT colonography (virtual colonoscopy) during screening, contrast-enhanced ultrasound in speeding up diagnostic procedures and magnetic resonance imaging, especially in the precise diagnosis of liver disease and local staging of rectal cancer. Radiological interventional methods have been more established in the last decade, as they are suitable for those patients where a standard solution fails. Interventional radiology extends the treatment options for patients with disseminated colorectal cancer (at stage IV). Invasive therapeutic methods in the treatment of liver metastases can be either intravascular (chemoembolization and regional chemotherapy, preoperative portal vein embolization) or percutaneous (particularly radiofrequency thermoablation).
