Účel studie: Hlavním cílem studie bylo odhadnout dosažení patologické kompletní odpovědi nemocných trpících lokálně pokročilým adenokarcinomem rekta po předoperační (neoadjuvantní) chemoradioterapii (CHRT) s využitím individuálních farmakokinetických parametrů 5-fluorouracilu (5-FU). Sekundárním cílem bylo hodnocení bezpečnosti a snášenlivosti léčby. Tato otevřená prospektivní studie podporovaná grantem IGA NS 9693–04/2008 zahrnula 34 dospělých s lokálně pokročilým karcinomem konečníku ozařovaných do celkové dávky 50,4 Gy ve 28 frakcích po 1,8 Gy 10–15 MV paprsky v režimu 5 + 2 (5 dní radioterapie, 2 dny volno) s kontinuálně podávaným 5-FU v i. v. infuzi 200–1000 mg/m2 po dobu 4–5 týdnů kontinuálně 7denní infuzí. Chirurgická resekce následovala 4–6 týdnů po ukončení CHRT a po klinickém restagingu. Hodnocena byla klinická a patologická odpověď na chemoradioterapii pomocí zobrazovacího (MR) a histopatologického vyšetření, vyjádřená v % jako reziduální choroba. Výsledky a závěr: Výsledek dokládá vztah mezi velikostí kumulativní dávky 5-FU a kumulativní AUC 5-FU (r = 0,61, p < 0,001). Podobný vztah byl prokázán mezi kumulativní AUC 5-FU a metabolickým poměrem (poměr dosažených plazmatických koncentrací neaktivního metabolitu dihydrofluorouracilu (5-FUH2) ku koncentracím 5-FU, r = -0,80, p < 0,0001). Kumulativní AUC korelovala se stupněm odpovědi na léčbu (r = – 0,53, p < 0,005) a určovala také stupeň toxicity léčby. Pokud bychom chtěli dosáhnout kompletní patologickou odpověď, pak by denní dávka 5-FU měla být u středně rychlého metabolizéra >350 mg/m2 a kumulativní AUC1–39 dní > 50 mg/L*h. U žádného z nemocných nebyla nalezena mutace v genu pro dihydropyrimidindehydrogenázu (DPD) a multidrug resistance-1 protein (MDR-1), přesto byla nalezena vysoká interindividuální variabilita v dosažených plazmatických koncentracích 5-FU, a to i s ohledem na cirkadiální rytmus kinetiky léčiva., Background and Purpose: The main goal of the present study was to estimate the early patients´response following neoadjuvant chemoradiotherapy (CHRT) based on 5-fluorouracil (5-FU) with curative aim in relation to plasma concentrations and pharmacokinetic parameters of 5-FU. Secondary objectives included evaluation of the safety and tolerability of the regimen. Patients and Methods: This open prospective study enrolles 34 adult patients with locally advanced rectal cancer, who received 5-FU 200 -1000 mg/m2 administered as a continuous i. v. infusion over 4–5 week and radiotherapy delivered with 10–15 MV photon beams at 1.8 Gy/fraction up o 50.4 Gy in 28 daily fractions for 5 days a week. Surgical resection with curative aim followed 4–6 weeks after the completion of CHRT and clinical restaging. Pathologic response evaluation and the rate of tumor regression was evaluated using tumor downstaging by MR, histopathological staging, and expressed as residual disease (%). Results and Conclusion: The outcome evidenced the correlation between the cumulative 5-FU dose and cumulative AUC of 5-FU (r = 0.61; p < 0.001). The similar relationship was demonstrated between the cumulative AUC and metabolic ratio (the plasma concentration od inactive metabolite dihydrofluotouracile 5-FUH2 to 5-FU; r = -0.80; p < 0.0001). The cumulative AUC was correlated with tumor regression rate (r = -0.53; p < 0.005) and determined toxicity grade. To reach pCR, the daily dose of 5-FU in patient with average metabolic ratio of 5-FUH2/5-FU should be >350 mg/m2 and the cumulative AUC1–39days > 50 mg/L*h. INTRODUCTION: Autologous stem cell transplantation (ASCT) became standard of care for patients with multiple myeloma (MM) under the age of 65 years. We routinely perform ASCT for newly diagnosed MM since 1996 in our department. PATIENTS AND METHODS: We retrospectively analyzed all 285 transplants in 185 patients done for MM from January 1996 till December 2010. We analyzed overall survival (OS) and progression-free survival (PFS) regarding conditioning, stage, complete or very good partial remission (CR, VGPR) achievement, renal impairment, single vs. double transplant. RESULTS: Estimated 10-years survival of the whole set of patients is 39% (median survival 95 months). Patients with renal impairment show same OS as those without (p = 0.22). Patients show similar overall survival and event free survival regardless of type of transplant. We observed better outcome in terms of overall survival in patients treated with new drugs (p = 0.0014). Reaching CR or VGPR was not translated into better OS (p = 0.30) and EFS (p = 0.10). Also stage of the disease and whether single or double transplant was used did not make any significant difference in the outcome. CONCLUSION: Stem cell transplantation greatly improved outcome of patients with MM. Poor outcome of allogeneic transplantation in our group of patients is related to high transplant related mortality (20% vs. 0%) and unexpected high relapse rate. There is a trend towards better survival, when new drugs are incorporated at any time in the course of the disease. Poruchy spánku patria k najčastejším non‑motorickým prejavom u pacientov s Parkinsonovou chorobou (PCh), v populačných štúdiách je uvádzaný ich výskyt až u 60 % pacientov. Štúdie tiež uvádzajú rozdielnosť prejavov PCh medzi mužmi a ženami. Cieľ: Zistiť, či sú u pacientov s PCh na Slovensku prítomné medzi pohlaviami rozdiely v klinických prejavoch PCh a výskyte porúch spánku. Materiál a metódy: Zisťovanie bolo robené dotazníkovou metódou s účasťou pacientov a neurológov z jednotlivých regiónov Slovenskej republiky. Dotazníky boli vyplňované počas rutinných kontrolných návštev. Boli zisťované socioekonomické premenné ako pohlavie, vek, vek pri začiatku ochorenia, stupeň poškodenia hodnotený škálou Hoehna a Yahrovej, dominujúci fenotyp ochorenia a používaná medikácia. Pri hodnotení porúch spánku bola použitá slovenská verzia dotazníka PDSS. Výsledky: Získali sme dáta od 1 067 pacientov. Pri porovnávacích analýzach neboli zistené podstatné rozdiely vo väčšine hodnotených parametrov. U mužov bol o niečo vyšší podiel pacientov s hodnotou škály Hoehna a Yahrovej 4 a boli zistené signifikantne vyššie dávky levodopy v monoterapii (p < 0,01). Významné rozdiely boli zistené v skóre škály PDSS pre hodnotenie rušivých halucinácií počas noci – skóre u mužov bolo 8,22, u žien 8,48 (p < 0,05). Podobný výsledok sme zaznamenali v skóre hodnotiacom nutnosť močenia počas noci – u mužov bolo skóre 5,90, u žien 6,53 (p < 0,01). Redakční rada potvrzuje, že rukopis práce splnil ICMJE kritéria pro publikace zasílané do biomedicínských časopisů., Sleep disturbances are one of the most common non-motor symptoms in patient with Parkinson’s disease (PD) with community‑based studies reporting prevalence data of 60%. Differences in symptoms between men and women have been reported. Aim: In the present study, we assessed whether there are gender differences in clinical presentation of PD and prevalence of sleep disturbances in individuals diagnosed in the Slovak Republic. Material and method: Questionnaires were distributed to participating neurologists and patients in outpatient practices across the Slovak Republic. Sociodemographic variables – gender, age, age at onset, disease severity according to Hoehn and Yahr stage, phenotype of the dominating symptom of Parkinson’s disease and type of medication – were collected. The Slovak language version of the PDSS was used in a questionnaire to test sleep disturbances. Results: Data from 1,067 outpatients with PD were collected. Comparative analyses showed males and females not to be significantly different on the majority of the demographic and medical characteristics collected. Males had a slightly higher proportion of individuals with Hoehn and Yahr score 4 and, among those taking levodopa medication as monotherapy, males took significantly higher levodopa dose than females (p < 0.01). A significant difference in the distribution of PDSS subscores between males and females was observed on item 7 (distressing hallucinations at night) – score for males 8.22, for females 8.48, p < 0.05. Gliomy limbického a paralimbického systému představují anatomicky a tím pádem i operačními přístupy unikátní skupinu mozkových nádorů. Mikrochirurgie v kombinaci s moderními perioperačními postupy zvyšuje šance na radikální odstranění těchto gliomů. Z hlediska lokalizace je dělíme do tří základních skupin: 1. gliomy inzuly, 2. amygdalohippokampálního komplexu a 3. cingula. V minimonografii se věnujeme anatomii i funkci limbického a paralimbického systému a v návaznosti na ni představujeme ucelený pohled na jednotlivé neurochirurgické přístupy k těmto oblastem. Maximální resekce za zachování funkční integrity mozku je předpokladem následné smysluplné onkologické léčby. Taktéž prezentujeme výsledky operací našeho pracoviště ve smyslu morbidity a radikality za posledních sedm let., Limbic and paralimbic gliomas represent a unique group of brain tumors both from an anatomic and surgical point of view. Microsurgery, together with modern perioperative procedures, enables a wider extent of resection of these gliomas. Based on localization, we divide gliomas into three basic groups: 1. insular, 2. amygdalohippocampal complex and 3. cingulum gliomas. In our review, we describe the anatomy and function of the limbic and paralimbic system and in relation to that, we present a description of the specific neurosurgical approaches to these regions. Maximal resection with brain function preservation represents the basis for subsequent oncologic therapy. We also present the surgical results of our department in the last seven years with regard to morbidity and radicality. Cíl studie: Na základě soudobých studií podat přehled o hlavních biologických funkcích lipoproteinů o vysoké hustotě (HDL), zejména ve vztahu k reverznímu transportu cholesterolu a k jejich protizánětlivému působení. Popsat pravděpodobné mechanismy vzniku dysfunkčních prozánětlivých HDL a uvést některé parametry asociované s tímto procesem. Podat přehled o laboratorních metodách stanovení kvantitativních, strukturních a funkčních vlastností HDL, včetně moderních metod založených na hmotové spektrometrii. Typ studie: přehledový článek Závěr: V případě systémového zánětu a/nebo oxidačního stresu dochází ke vzniku dysfunkčních HDL akumulujících oxidanty. Probíhá strukturní modifikace apolipoproteinu AI, což vede mimo jiné k inhibici reverzního transportu cholesterolu a tvorbě prozánětlivých HDL. Na ztrátě příznivých vlastností HDL se podílí modifikace proteinové i lipidové složky HDL. Izolované měření HDL cholesterolu nemusí u některých syndromů bezvýhradně korelovat s mírou kardiovaskulárního rizika a v některých situacích je vhodné stanovit markery mapující strukturu a funkci HDL., Objective: On the basis of recent studies to make an overview concerning the crucial biological functions of high density lipoproteins (HDL), with emphasis on the role in reverse cholesterol transport and their antiinflammatory traits. The aim was to describe probable mechanisms of dysfunctional proinflammatory HDL formation with introduction some of associated parameters. To present laboratory methods for determination of quantitative, structural and functional qualities of HDL, including advanced mass spectrometry techniques. Study design: review Conclusion: In the case of systemic inflammation and/or oxidative stress, the formation of dysfunctional HDL accumulating oxidants takes place, including apo AI structural modifications. This process can lead to the inhibition of reverse cholesterol transport and proinflammatory HDL generation, among others. Loss of beneficial qualities results from modification of both lipid and protein components of HDL. Cíl studie: Metabolismus železa musí být velmi pečlivě regulován. Klíčovým systémovým regulátorem je peptidový hormon hepcidin, který způsobuje “uzamčení” železa uvnitř buňky; to pak nemůže být utilizováno. Dialyzovaní pacienti často trpí anémií, která je způsobena mnoha faktory, mj. stavem mikrozánětu a také zřejmě zvýšenou retencí hepcidinu v důsledku snížené glomerulární filtrace. Naším cílem bylo popsat vztahy mezi hepcidinem a dalšími parametry metabolismu železa, zánětu a erytropoezy u těchto pacientů. Typ studie: Observační Název a sídlo pracoviště: Ústav klinické biochemie a hematologie LF UK a FN v Plzni, Alej Svobody 80, 304 60 Plzeň Materiál a metody: Do studie bylo zařazeno 164 chronicky dialyzovaných pacientů (věk 66 ± 13, 25 - 92 let; 101 mužů a 63 žen) a 37 zdravých kontrol (věk 55 ± 20, 21 - 92 let; 16 mužů a 21 žen), u kterých byl stanoven jejich kompletní krevní obraz, parametry metabolismu železa, zánětu a výživy. Výsledky: Koncentrace železa, transferrinu a hemoglobinu byla významně nižší u dialyzovaných pacientů (p<0,0001), naopak ferritin (p<0,0001), solubilní transferrinové receptory(p<0,05), hepcidin (p=0,0003), CRP a IL-6 (p<0,0001) byly u těchto pacientů významně vyšší než u zdravých kontrol. Hepcidin slabě koreloval pozitivně s CRP a ferritinem a negativně s transferrinem u dialyzovaných pacientů, u zdravých kontrol nebyla nalezena žádná korelace hepcidinu s ostatními parametry. Závěr: Zdá se, že vliv zánětu na hladiny hepcidinu není u hemodialyzovaných pacientů zcela zásadní a na jeho koncentraci se podílí i jiné faktory, např. jeho retence., Objective: Peptide hormone hepcidin is a key systemic regulator of the iron metabolism. Hepcidin binds to the iron cell exporter ferroportin so iron is kept in the cells unavailable for erythropoiesis. The synthesis of hepcidin is up-regulated by high iron stores and inflammation. Dialyzed patients have very often impaired iron management – they suffer from anemia, which is caused by many factors including the state of micro inflammation and hepcidin retention due to decreased glomerular filtration rate. Our aim was to describe the relationship of hepcidin and other parameters of iron metabolism, erythropoiesis and inflammation. Design: Observation Settings: Institute of Clinical Biochemistry and Hematology, Charles University Hospital in Pilsen, Alej Svobody 80, 304 60 Plzeň Material and Methods: Complete blood cell count, iron, ferritin, transferrin, CRP, albumin, creatinine, hepcidin, soluble transferrin receptors (sTfR) and IL-6 were measured in samples from 164 patients included in chronic hemodialysis program (age 66 ± 13, 25 - 92 years), 63 women and 101 men and 37 control healthy volunteers (age 55 ± 20, 21 - 92 years), 21 women and 16 men. Results: Iron, transferrin and hemoglobin were significantly lower in the patients group (p<0.0001) while ferritin (p<0.0001), sTfR (p<0.05), hepcidin (p=0.0003), CRP and IL-6 (p<0.0001) were significantly higher in the patients group. Hepcidin weakly positively correlated with CRP and ferritin and weakly negatively correlated with transferrin in hemodialyzed patients. No correlation of hepcidin with other biochemical parameters in controls was found. Conclusion: It seems that the influence of inflammation on hepcidin levels in hemodialyzed patients is not crucial and other factors including its retention in end-stage renal disease participate., T. BACKGROUND: In the cholesteatoma surgery ossicles can be replaced to reconstruct middle ear function. It is important that these ossicles are free of squamous epithelium, to prevent residual disease. This study focuses on the histological findings of the malleus and incus harvested during cholesteatoma surgery. MATERIALS AND METHODS: Eighty middle ears ossicles were examined in vivo and histologically to consider the relationship of cholesteatoma to ossicles, grade of bone destruction and invasion of cholesteatoma to deeper layers of bone. RESULTS: Serious ossicular destruction was observed more frequently in incus compared to malleus (p=0.0065). Difference of ossicles destruction between children and adults was not significant (p=0.3032). Deep invasion of cholesteatoma into the vascular spaces or inner core of the bone was not observed. CONCLUSIONS: Autograft ossicles from cholesteatomatous ears should not necessarily be rejected for reconstruction of the ossicular chain. Cíl: Určení vztahu vybraných ukazatelů ke střednímu tlaku v plicnici (mPAP). Posouzení přítomnosti plicní hypertenze (PH) na základě RTG a HRCT obrazů. Metodika: 53 nemocných s chronickým plicním onemocněním podstoupilo pravostrannou srdeční katetrizaci, RTG hrudníku a HRCT plic. U 30 nemocných byla PH potvrzena. Skiagramy i HRCT byly hodnoceny třemi radiology, kteří se kromě jednotlivých nálezů vyjadřovali k přítomnosti PH. Byl vyhodnocen vztah relevantních parametrů k mPAP a shoda radiologů s objektivní přítomností PH. Výsledky: Nejsilnější vazba k mPAP byla na RTG hrudníku nalezena u poměru TI/ LHB (r = 0,256650, p = 0,02048) a šíře truncus intermedius (r = 0,188897, p = 0,023362). U HRCT vyšetření jsme nalezli významnou vazbu mezi hodnotou mPAP a průměrem kmene plicnice (r = 0,396894, p = 0,000001) a indexem PA/AA (r = 0, 382652, p = 0,000002). Hodnota rozhodovací meze pro PH u průměru kmene plicnice byla 31,2 mm se specificitou 80,769 % a senzitivitou 68,182 %. Shoda radiologů s mPAP při hodnocení RTG byla 58,49 %, 54,72 % a 64,15 %, při hodnocení HRCT byla 70,83 %, 60,42 % a 62,50 %. Závěr: Existuje vztah mezi šíří centrálních úseků plicních tepen a mPAP. Při pozitivitě několika specifických nálezů lze vyjádřit podezření na PH, při jejich absenci lze PH vyloučit s velkou pravděpodobností., Aim: To evaluate relationship between selected parameters and mean pulmonary artery pressure (mPAP). To assess prediction of pulmonary hypertension (PH) on chest radiographs (CXR) and High-resolution CT (HRCT) of lungs. Methods: 53 individuals with chronic lung disease underwent right heart cathetrisation, CXR and HRCT. 30 patients had PH confirmed. CXRs and HRCTs were reviewed by three radiologist who besides evaluation of findings stated whether PH is present. Relation between relevant parameters and mPAP and agreement between radiologists and real presence of PH were assessed. Results: The strongest correlation on CXR was found between mPAP and ratio of right descending pulmonary artery width and the left main bronchus (r = 0.256650, p = 0.02048) and right descending branch of pulmonary artery width (r = 0.188897, p = 0.023362). The strongest correlation on CXR was found between mPAP and pulmonary artery diameter (r = 0.396894, p = 0.000001) and PA/AA ratio (r = 0.382652, p = 0.000002. The cut-off value for PH for PA width was 31,2 mm with specificity 80.769% and sensitivity 68,182%. Agreement between radiologists and value of mPAP was in CXRS 58.49%, 54.72% and 64.15%, in HRCT of the lungs 70.83%, 60.42% and 62.50%. Conclusion: Width of pulmonary arteries and mPAP are related. Imunostimulace polybakteriálním lyzátem (Urivac®) v prevenci recidivujících infekcí dolních močových cest. Infekce močových cest (IMC) zaujímají v evropských zemích i u nás dlouhodobě druhé místo ve výskytu infekčních onemocnění v populaci a představují tak významný socioekonomický problém. Spolu s respiračními infekty se rozhodující měrou podílejí na preskripci antimikrobiálních přípravků. Extenzivní antimikrobiální léčba je jednou z nejvýznamnějších příčin nárůstu bakteriální rezistence. Určitou možnost prevence recidiv infekcí dolních močových cest a snížení spotřeby antimikrobiálních preparátů představuje imunostimulace. Prospektivní multicentrická studie hodnotila klinický účinek, snášenlivost a přínos k léčbě recidivujících zánětů při podávání imunostimulačního přípravku Urivac®. Počet atak infekcí dolních močových cest se po imunostimulaci významně snížil jak v jejím průběhu, tak po jejím ukončení (p = 2,2x10-16). V průběhu imunostimulace bylo bez příznaků 85 % nemocných a při dalším sledování 70,8 %. Snížil se také počet aplikovaných antimikrobiálních přípravků. Subjektivní zlepšení přetrvávalo ve více než 57 % i po ukončení léčby. Mikrobiologická odpověď dosáhla 77 %, v odstupu od ukončení léčby se snižuje. U exacerbací byla uropatogenní flóra detekována v 61 %. Nežádoucí účinky u dvou nemocných vedly k předčasnému ukončení terapeutického režimu., In European countries, including the Czech Republic, urinary tract infections (UTIs) have long occupied the second place in the incidence of infectious diseases in the population, thus representing a major socioeconomic problem. Along with respiratory infections, they are the major contributors to the prescription of antimicrobial agents. Extensive antimicrobial therapy is one of the most important causes of increased bacterial resistance. Immunostimulation is a possible way of preventing recurrent lower urinary tract infections and reducing the consumption of antimicrobial agents. A prospective multicentre study evaluated the clinical effi cacy, tolerability, and benefi t to treatment of recurrent infections with the immunostimulatory drug Urivac®. The number of attacks of lower urinary tract infections following immunostimulation reduced signifi cantly both during its course and after its cessation (p = 2.2 x 10–16). During immunostimulation, 85 % of patients were symptom-free, and during subsequent follow-up it was 70.8 %. There was associated with a reduction in the number of the antimicrobial agents administered. Subjective improvement persisted in more than 57 % even after treatment cessation. Microbiological response reached 77 %; it decreases with increased time from treatment cessation. In the case of exacerbations, uropathogenic fl ora was detected in 61 %. Cíl: Zjistit průměrnou incidenci infekční endokarditidy (IE) v okrese Tábor za sledované období a srovnat získané údaje s dříve publikovanou celorepublikovou studií. Metodika: Retrospektivní monocentrická observační studie sledující výskyt a charakteristiky infekční endokarditidy v definované spádové oblasti za roky 2009–2013. Zařazeni byli pouze pacienti s prokázanou a léčenou infekční endokarditidou a pacienti, u nichž byla diagnóza stanovena až pitvou. Výsledky: Ve sledovaném období bylo zachyceno celkem 39 případů infekční endokarditidy, což odpovídá průměrné incidenci 7,8 případů na 100 000 obyvatel ročně. Medián věku pacientů v době diagnózy byl 67 let. V souboru nemocných výrazně převažovali muži, kteří byli postiženi 3krát častěji než ženy. Přibližně stejně četně byla postižena mitrální (36 %) a aortální (33 %) chlopeň a elektroda kardiostimulačního systému (31 %). Ve srovnání s referenční prací bylo zachyceno více infekcí u pacientů s kardiostimulátory. Dvě třetiny infekcí byly způsobeny stafylokoky, což je proti pilotní studii výrazný nárůst. Naopak mortalita se podstatně neměnila. Závěr: Charakteristiky infekční endokarditidy se vyvíjejí, hospitalizační mortalita pacientů s IE je vysoká, kolem 30 %. Samo onemocnění IE je z hlediska roční úmrtnosti špatným prognostickým faktorem. Klíčová slova: incidence – infekční endokarditida, Aim: To find out an average incidence of infective endocarditis (IE) in Tabor district in a monitored period of time and compare the obtained data with previously published national study. Methods: Retrospective monocentric observational study monitoring occurence and characteristics of infective endocarditis in a defined region in years 2009–2013. Only patients with proved and treated infective endocarditis and patients whose diagnosis was revealed by autopsy were involved. Results: In the monitored time 39 cases of infective endocarditis occured, which means that the average incidence was 7.8 cases on 100 000 people per year. Median age in the time of diagnosis was 67. The group of patients was strongly dominated by men, who were affected three times more often than women. Almost the same rates of infective endocarditis were localized on mitral (36 %) and aortic (33 %) valve and electrode of a pacemaker (31 %). In comparison with the reference study there were more pacemaker-related infections detected. Two thirds of infections were caused by staphylococci. That means a prominent growth when compared to the pilot study. On the contrary, there was no significant change in mortality. Conclusion: Characteristic features of infective endocarditis are evolving, patients’ in-hospital mortality rate is high, about 30 %. The disease by itself is a poor prognostic factor in a one-year mortality rate. Key words: incidence infective – infective endocarditis, and Rudolf Koubek
