A hybrid approach to elephant trunk technique for treatment of thoracic aortic aneurysms combines a conventional surgical and endovascular therapy. Compared to surgery alone, there is a presumption that mortality and morbidity is reduced. We present a case report of a 42-year-old man with a giant aneurysm of the entire thoracic aorta, significant aortic and tricuspid regurgitation and ventricular septum defect. Cíl práce: Prezentujeme kazuistiku pacienta, u kterého došlo k rozvoji oboustranného seminomu na podkladě intratubulární germinální neoplazie – neklasifikovatelný typ (IGCNU – intratubular germ cell neoplasia unclassified type). Kazuistika: Pacient s anamnézou seminomu levého varlete klinického stadia I a IGCNU po radikální orchiektomii a adjuvantní radioterapii byl odeslán k urologickému vyšetření pro suspektní tumor solitárního pravého varlete dle sonografického vyšetření. Indikovali jsme 3biopsii varlete, která prokázala přítomnost IGCNU. Následovala radikální orchiektomie, kde byl histologicky verifikován klasický seminom. Závěr: Intratubulární germinální neoplazie představuje vysoké riziko rozvoje invazivního germinálního tumoru varlete. Léčba zahrnuje radioterapii či chemoterapii cisplatinou. Alternativou je pečlivá dispenzarizace a pravidelné sonografické kontroly, zejména u pacientů se solitárním varletem, kteří chtějí zachovat fertilitu a endokrinní funkci. Biopsie kontralaterálního varlete u pacientů s germinálním tumorem k vyloučení IGCNU by měla být provedena v případě kryptorchizmu, poruchy spermiogeneze a atrofií varlete s objemem pod 12 ml, vhodná je rovněž u pacientů s mikrolitiázou varlete a předchozím výskytem IGCNU., Objective: We are presenting a case report of young man who developed an asynchronous bilateral seminoma based on an intratubular germ cell neoplasia of unclassified type (IGCNU). Case report: A patient, with history of a stage I seminoma of the left testis, based on an IGCNU of the left testis, after a radical orchidectomy and adjuvant radiotherapy, was referred to the urology clinic with a suspected tumor of the right testis. A testicular biopsy was made and the histological examination showed an IGCNU. A radical orchidectomy was performed and a seminoma was found. Conclusion: IGCNU is a precursor lesion of germ cell testicular tumors. Treatment of the IGCNU includes radiotherapy or chemotherapy with cisplatin. Surveillance remains an option for those with a solitary testicle who wish to preserve fertility and endocrine function. A contralateral testicular biopsy, in patients with a history of germ cell tumors, should be made in cases of cryptorchidism, poor spermatogenesis, and testicular atrophy with a testicular volume of less than 12 ml. Unilateral otosclerosis combined with avascular necrosis of stapes crura is a rare entity. It should be considered in a case of high grade otosclerosis. Symptoms are the same as in patients who suffer from common otosclerosis. Patients complain on progressive hearing loss and tinnitus. The diagnosis is made clinically by conventional audiologic evaluation and radiologically by x-ray mastoid Schuller's view and CT scan. HRCT scan makes visible all parts of ossicular chain and gives surgeon some information about ossicular chain damage. Castlemanova choroba je vzácnou nosologickou jednotkou, která byla v literatuře poprvé popsána v roce 1954 Castlemanem a Lownovou. Jde o onemocnění primárně postihující lymfatické uzliny, u kterého se rozlišují čtyři histomorfologické podtypy a dva klinické obrazy (uni a multicentrický). Nejčastěji se klinicky projeví jako lymfadenopatie mediastina, krku či retroperitonea. Příznaky nemoci jsou výrazně heterogenní a záleží na daném histologickém podtypu. Taktéž prognóza se liší podle typu nemoci, kdy může jít jak o zcela benigní nálezy (zejména forma unicentrická), tak o potenciálně maligní formu, při které nemoc progreduje až do stadia maligní lymfoproliferace (multicentrická forma). Autoři prezentují případ 29letého muže s klinicky manifestním (ikterus, poruchy střevní pasáže) objemným tumorem retroperitonea lokalizovaným subhepatálně, který byl úspěšně radikálně exstirpován. Pooperační průběh byl nekomplikovaný. Definitivní histopatologická diagnóza potvrdila hyalinně-vaskulární typ Castlemanovy nemoci. Vzhledem k tomu, že se jednalo o multicentrickou formu onemocnění, bude následovat adjuvantní onkologická léčba. It is rare sickness which primary affects lymfatic nodes. There are four histo-morfologic subtypes and two clinic manifestations (uni and mulicentric). Comon clinical picture is simple lymfadenopathy of neck, medistinal a retroperitonal lymfatic nodes. Signs of this disease are heterogenic and depend on histological subtype. Also prognosis depends on type of illness. It should take place as benign diagnosis (unicentric form) but also can be potencionally malignant form which shlould exacerbate to malignant lymfoproliferation (multicentric form). Authors present the case of 29 years old men with clinical manifestation (jundice, intermitent bowel obstruction) of lagre retroperitoneal tumor in subhepatal localization. The treatment was radical surgical extirpation, surgery was without complications. Definitive histological diagnosis was confirmed as hyaline-vascular type of Castleman disease. Nádory oblasti hlavy a krku představují v Evropě u mužů čtvrté nejčastější nádory. Nejvyšší výskyt je pozorován v zemích okolo Středozemního moře. Incidence nádorů orofaryngu v České republice je 0,91/100 000 obyvatel s mortalitou 0,65/100 000 obyvatel, nejvíce zastoupeným klinickým stadiem nádoru je klinické stadium IV (58,9 %). U nádorů patra je incidence 0,34/100 000 obyvatel s mortalitou 0,15/100 000 obyvatel. Nejvíce zastoupeným klinickým stadiem je také stadium IV (26,7 %). Postižení žen je výrazně nižší než u mužů, v ČR je poměr mužů k ženám přibližně 4:1. Z hlediska rizikových faktorů hraje rozhodující vliv kouření a konzumace alkoholu. Průměrný věk pacientů se pohybuje mezi 50 a 60 lety. V kazuistikách bude představen pacient s nádorem orofaryngu a pacient s nádorem měkkého a tvrdého patra a patrové tonzily. V obou případech šlo o pokročilé lokální nálezy bez vzdálených metastáz u „typických“ pacientů mužského pohlaví s dlouholetým abúzem alkoholu a tabakizmem., Head and neck tumours are the fourth most common cancer among men in Europe. The highest incidence is in the Mediterranean countries. The incidence rate of oropharyngeal cancer in the Czech Republic is 0.91/100 000 inhabitants with a mortality rate of 0.65/100 000 inhabitants; the most prevalent clinical stage of the tumour is clinical stage IV (58.9%). Chemodektom, paragangliom glomus caroticum neboli tumor karotického glomu se vyskytuje v populaci s incidencí 1 : 30 000. Tumor je makroskopicky, mikroskopicky i biologicky velmi podobný adrenálnímu feochromocytomu. Významným znakem těchto tumorů je jejich bohatá vaskularizace. Patří mezi nejvíce prokrvené tumory vůbec. Histologicky se popisuje alveolární uspořádání nádorových buněk, na rozdíl od feochromocytomu, ostře od sebe ohraničených jemnými pruhy vaziva se širokými kapilárami a buňkami podobnými buňkám Schwannovým. Cytoplazma nádorových buněk hlavních je slabě eozinofilní a jemně granulovaná. Jádra buněk mohou být pleomorfní a hyperchromní, ale nutno konstatovat, že jaderná atypie není jednoznačnou známkou malignity. Biologicky se jedná o tumory převážně benigní. Klinicky se jedná o pomalu rostoucí, nebolestivý, tuhý a kulovitý útvar za úhlem dolní čelisti. Přestože chemodektom je všeobecně považován za radiosenzitivní, hlavní terapeutický postup je chirurgická exstirpace tumoru., Chemodectoma, paraganglioma of glomus caroticum or carotid glomus tumor occurs in the population with an incidence of 1:30 000. INTRODUCTION: The splenium of the corpus callosum has a specific structure of blood supply with a tendency towards blood-brain barrier breakdown, intramyelinic edema, and damage due to hypoxia or toxins. Signs and symptoms of reversible syndrome of the splenium of the corpus callosum typically include disorientation, confusion, impaired consciousness, and epileptic seizures. CASE REPORT: A previously healthy 32-year-old man suffered from weakness, headache, and fever. Subsequently, he developed apathy, ataxia, and inability to walk, and therefore was admitted to the hospital. Cerebrospinal fluid showed protein elevation (0.9 g/l) and pleocytosis (232/1 ul). A brain MRI showed hyperintense lesions in the middle of the corpus callosum. The patient was treated with antibiotics, and subsequently, in combination with steroids. Two months later, the hyperintense lesions in the splenium and the basal ganglia had disappeared. Almost seven months since his hospitalization in the Department of Neurology, the patient has returned to his previous employment. He now does not exhibit any mental changes, an optic edema and urological problems have improved. In addition, he is now actively engaged in sports. CONCLUSION: We have described a case of a 32-year-old man with confusion, ataxia, and inability to stand and walk. The man developed a febrile meningeal syndrome and a hyperintense lesion of the splenium, which lasted for two months. Neurological changes, optic nerve edema, and urinary retention have resolved over the course of seven months.
