Přehledové sdělení se věnuje diagnostickému zobrazení akutních aortálních syndromů s důrazem na multidetektorovou výpočetní tomografii a na její význam pro volbu léčby. Diskutovány jsou jednak techniky CT zobrazení včetně CT s EKG synchronizací a dále diagnostické nálezy u disekce aorty, intramurálního hematomu a penetrujícího vředu aorty. CT s vysokým rozlíšením (HRCT) sa stalo metódou voľby v zobrazovaní a diagnostike ochorení stredného ucha a prevodovej poruchy sluchu. Pomocou HRCT dokážeme detailne popísať anatómiu spánkovej kosti, patológiu stredného ucha (otitis média chronica, cholesteatóm a ich komplikácie) a traumatické zmeny. Magnetická rezonancia má naopak vysoké rozlíšenie mäkkých tkanív, preto ju využívame k diferenciálnej diagnostike mäkkotkanivovej masy stredného ucha zobrazenej na CT, k odlíšeniu reziduálneho ochorenia alebo recidívy v teréne pooperačných zmien a k zobrazeniu intrakraniálnych komplikácií akútnej a chronickej otitídy CT a MR sú komplementárnymi zobrazovacími metódami, pomáhajú stanoviť správnu diagnózu, ktorá je pre klinika dôležitá z hľadiska stratégie liečby. V publikácii uvádzame spektrum patológií stredného ucha, s ktorými sa pri zobrazení spánkovej kosti môžeme stretnúť a na ktoré treba myslieť., High resolution computed tomography (HRCT) is a modality of choice in diagnostic imaging of middle ear pathology and conductive hearing loss. It enables to image a detail anatomy of temporal bone, pathology of middle ear (e.g. chronic otitis média, cholesteatoma and their complications) and traumatic changes in this area. On the other hand, magnetic resonance imaging (MRI) enables a high resolution of soft tissue structures. It is used in imaging of soft tissue masses in middle ear cavity previously diagnosed on CT, differentiating residual disease or recurrence in postoperative ear, and in imaging of intracranial complications of acute and chronic otitis. CT and MRI are complementary imaging modalities in helping the clinicians to determine the proper strategy for patient treatment. Zobrazenie sluchového nervu a vnútorného ucha ako aj vyšších centier sluchovej dráhy pred kochleárnou implantáciou pomocou MR a/alebo CT je v dnešnej dobe štandardom. Stav vnútorného ucha má zásadný vplyv na výsledok kochleárnej implantácie (Kí). U väčšiny kandidátov na Kí je hluchota spôsobená poruchou funkcie Cortiho orgánu v inak anatomicky diferencovanom uchu so zachovaným počtom gangliových buniek a vlákien sluchového nervu. Úlohou zobrazovacích metód je prispieť k určeniu príčiny hluchoty a identifikovať anatomické podmienky pre samotný chirurgický výkon. V článku sme sa zamerali na spektrum patologických nálezov spánkovej kosti, vnútorného ucha a sluchového nervu, s ktorými sa môžeme stretnúť a na ktoré pred kochleárnou implantáciou treba upozorniť. Predoperačné zistenie abnormalít na úrovni vnútorného a prípadne stredného ucha, dáva chirurgovi možnosť implantovať vhodné ucho, naplánovať chirurgický postup a vybrať správny typ elektródy na implantáciu., CT and/or MR imaging of acoustic nerve, inner ear and superior centers of acoustic pathway is a gold štandard before planned cochlear implantation. Condition of inner ear plays an important role in determining the outcome of cochlear implantation (CI). Most of the candidates for CI lost their hearing by impairment of the Corti organ, in otherwise anatomically normal ear with normál number of ganglion cells and fibers of acoustic nerve. Role of imaging is help to find the cause of deafness and identify anatómie conditions before surgical intervention. In this article we are trying to review different pathologic conditions of temporal bone, inner ear and acoustic nerve that are important to identify before cochlear implantation. Uroteliální karcinomy vyžadují komplexní přístup ke stagingu, jelikož jejich typickou vlastností je multifokalita - možnost lokálně agresivního chování i vzdálené diseminace. V komplexním zobrazení je možné využít MDCT, MRI i 18F-FDG-PET/CT. Při hodnocení nádorů je zásadní odlišit invazi nádoru do okolní tukové tkáně nebo do parenchymu ledviny, eventuálně do okolních orgánů. Lokální šíření je možné posoudit poměrně spolehlivě pomocí MDCT i MRI. Pro posouzení vzdálené diseminace onemocnění je optimální metodou 18F-FDG-PET/CT, která nabízí odhalení metastáz v uzlinách ve skeletu, játrech i plicním parenchymu. Při předoperačním plánování je využíváno předností trojrozměrného modelování cévního zásobení i vývodních cest močových., Urothelial carcinomas need complex approach during staging, because of they may be typically multi-focal, locally aggressive behavior and potential remote dissemination. The possibilities of complex imaging cover MDCT, MRI and 18F-FDG-PET/CT. In the assessment of the tumors, it is most important to distinguish the invasive spread of the tumor to the surrounding fatty tissue or into renal parenchyma or into neighboring organs respectively. Local invasion could be evaluated by MDCT or MRI with the relatively high accuracy. 18F-FDG-PET/CT is the optimal method able to search on the remote metastases. It offers depiction of the metastases in lymph nodes, in the skeleton, liver or lung parenchyma. Renální karcinom jako nejvýznamnější solidní nádor ledvin dosahuje v České republice nejvyšší incidence v celosvětovém měřítku. Odlišit jej mezi ostatními solidními a cystoidními lézemi je nejdůležitější úlohou zobrazovacích metod. Základní metodou je ultrasonografie, kterou je možné rozšířit pomocí aplikace kontrastní látky na metodu vysoce spolehlivou v diferenciální diagnostice solidních útvarů. K předoperačnímu zobrazení nádorů ledvin je nezbytné zobrazení cévního zásobení ledviny i vlastního tumoru pomocí CT angiografie nebo MR angiografie. Magnetickou rezonancí lze blíže specifikovat především hypovaskularizované nádory, u kterých se difuzní vážení ukazuje jako velmi důležité při odlišení papilárních renálních karcinomů. PET/CT s podáním fluorodeoxyglukózy je významnou metodou umožňující na základě míry akumulace fluorodeoxyglukózy posoudit agresivity renálního karcinomu., Renal cell carcinoma reaches the highest incidence in the Czech Republic world wide. The most important role of the imaging methods is to distinguish it in between the all solid and cyst-like renal lesions. Even the ultrasound (sometimes including application of contrast material) remains the basic imaging method, CT-angiography and MR-angiography is the most important recent methods to display its own tumorous tissue including the vessels supplying the tumor and the whole kidney. Magnetic resonance is able to improve the differential diagnosis in hypo-vascular (especially papillary renal cell carcinoma) when the diffusion weighted imaging is used.
