- Cíl: Na základě existujících cizojazyčných testů sestavit a validovat vlastní, v českém jazyce dosud chybějící test funkcionální komunikace u pacientů s afázií. Metodika: Byl vytvořen Dotazník funkcionální komunikace (DFK), který hodnotí stav komunikace v reálných situacích pomocí 20 položek v oblastech: I. bazální komunikace, II. sociální komunikace, III. čtení a psaní a IV. čísla a orientace; každá položka je hodnocena šestistupňovou škálou (0–5 bodů). Maximální možná hodnota Indexu funkcionální komunikace (Index FK) dosahuje 100 bodů. DFK byl validován na souborech zdravých dobrovolníků (n = 110, medián věku 63 let), pacientů s afázií (n = 38, medián věku 62 let) a s Alzheimerovou demencí (n = 8, medián věku 81,5 let). Výsledky: Hodnoty Indexu FK korelovaly významně s věkem (Spearman r = –0,354; p = 0,000148), proto byly stanoveny normativní hodnoty Indexu FK pro věk: 50–59 let: > 90; 60–69 let: > 85; 70 a více let: > 76. Pomocí ROC analýzy byla potvrzena vysoká diagnostická validita DFK v diskriminaci mezi normálními jedinci a pacienty s afázií (senzitivita při cut‑off hodnotě Indexu FK 86,5 je 89,5 % a specificita je 93,6 %; AUC = 0,974; p < 0,001). Hodnoty Indexu FK u pacientů s afázií významně korelovaly se stupněm jazykového deficitu vyjádřeného testem MASTcz a nezjistili jsme rozdíl oproti hodnotám Indexu FK u pacientů s poruchami komunikace u Alzheimerovy demence. Potvrdili jsme test‑retest reliabilitu opakovaným vyšetřením 10 pacientů s afázií. Byl vytvořen terapeutický materiál, který navazuje na položky DFK. Závěr: DFK je nový validovaný test v českém jazyce, který doplňuje paletu diagnostických nástrojů pro pacienty s afázií. Výzkum naznačil jeho výborné psychometrické vlastnosti, jež je třeba ověřit na větších souborech., Aim: To develop and validate a new, in the Czech language still missing test of functional communication for patients with aphasia. Methods: Functional Communication Questionnaire (FCQ) comprises 20 items that evaluate communication in four areas of real situations: I. Basal communication, II. Social communication, III. Reading and writing, and IV. Calculation and orientation. Every item is evaluated on 6–degree scale (0–5 points), and the sum (Functional Communication Index – FCI) of 100 points represents maximum possible value. FCQ was validated in groups of healthy volunteers (n = 110, median age 63 years), patients with aphasia (n = 38, median age 62 years), and patients with Alzheimer dementia (AD) (n = 8, median age 81.5 years). Results: Values of FCI correlated significantly with age (Spearman r = –0.354, p = 0.000148); and different normal limits of FCI were established for decades: 50–59 years: > 90; 60–69 years: > 85; 70–79 years: > 76. Using ROC analysis we confirmed high diagnostic validity of FCQ in discrimination between healthy controls and patients with aphasia (sensitivity of 89.5% and specificity of 93.6% for cut‑off value 86.5; AUC = 0.974; p < 0.001). Degree of functional communication impairment quantified with FCI in patients with aphasia correlated with the degree of language deficit quantified with MASTcz, while there was no significant difference in FCI values between patients with aphasia and AD. Repeated evaluation in 10 patients with aphasia proved very high test‑retest reliability of FCQ. New therapeutic material based on FCQ was finally introduced into clinical practice. Conclusion: FCQ extends the repertoire of diagnostic tests for patients with aphasia available in the Czech language. Our results showed very good psychometric characteristics of FCQ that should be confirmed by further research. V hormonální substituční terapii volíme progestin dle jeho metabolické neutrálnosti a bezpečnosti vzhledem k možnému riziku onemocnění karcinomem prsu. Mikronizovaný progesteron a dydrogesteron mají pravděpodobně lepší bezpečnostní profil než syntetické progestiny. Také v suplementaci gestagenů v těhotenství dáváme přednost přirozeným gestagenům před syntetickými progestiny. The best progestogen for the hormone replacement therapy could be metabolic neutral and safe for breast tissue. Micronised progesterone and dydrogesterone have better safety profile than synthetic progestagens. Also during pregnancy we prefer natural gestagens to synthetic ones. - Úvod: Léčba intratékálním baklofenem (ITB) kontinuálně pomocí pumpových systémů je důležitou léčebnou modalitou u nemocných s těžkou spasticitou, která již nezabírá na perorální léky nebo když se objeví výrazné nežádoucí účinky. Tato metoda je relativně velmi bezpečná, přesto se ale mohou vyskytnout komplikace spojené s touto léčbou. Nejčastěji se objevují komplikace spojené s implantací systému. Jde zejména o problémy s katétrem. Rotace pumpy je extrémně vzácnou komplikací a její výskyt je méně než 0,3 %. Kazuistika: U 74leté ženy s těžkou spasticitou po chronickém míšním poranění ve výši Th9 byla úspěšné provedena implantace baklofenové pumpy (Medtronic, Synchromed II). Pacienka měla bezprostřední úlevu se snížením spasticity již na dávce baklofenu 100 ?g/den. Na první ambulantní kontrole se opakovaně nezdařilo doplnit rezervoár pumpy. Bylo vysloveno podezření na její rotaci. Pacientka byla okamžitě odeslána na rentgenologické vyšetření, kde bylo potvrzeno přetočení pumpy o 180 stupňů. Tentýž den byla pacienta v lokální anestezii operována, pumpa byla otočena a důkladně zafixována stehy. Vzhledem k rychlé revizi se u pacientky nerozvinul syndrom z náhlého odnětí baklofenu při prázdném rezervoáru pumpy. Závěr: Rotace pumpy je vzácná komplikace léčby ITB. Je nutné okamžité vyřešení tohoto problému, neboť hrozí rozvoj abstinenčního syndromu z náhlého odnětí baklofenu. Léčbu ITB je třeba provádět v centrech se specializovanou péčí o tyto pacienty. Despite generally favorable outcomes and relative safety of this method, complications associated with ITB can be observed. The most common problems include surgery‑related complications resulting from system implantation, including catheter failure. Pump rotation is a rare complication with incidence less than 0.3%. Case report: Seventy‑four‑years‑old female suffered from spasticity after chronic spinal cord injury at T9 level. e underwent successful implantation of ITB pump (Medtronic, Synchromed II) with prompt spasticity relief on baclofen dose of 100 ?g/day. During the first outpatient appointment, it was difficult to puncture and fill the reservoir with baclofen. Plain x‑ray revealed rotation of the pump with reservoir in downright position. On the same day, the patient underwent emergency surgical revision with correction of the pump position and refill of the reservoir. No baclofen withdrawal syndrome or other complications occurred and the patient recovered well. Conclusion: Pump rotation is a very unusual and rare complication of ITB treatment. - BACKGROUND: A few reports demonstrate the occurrence of egg allergy in adolescent and adult patients suffering from atopic dermatitis and the association of this allergy to other food and aeroallergens. AIMS AND OBJECTIVES: The aim of this study is to evaluate the occurrence of egg allergy in patients suffering from atopic dermatitis at the age 14 years and older and to evaluate the relationship between egg allergy or egg sensitisation and the sensitisation to dust, mites, feather, and animal dander. MATERIALS AND METHODS: Complete dermatological and allergological examination was performed. These parameters were examined: food allergy and food sensitisation to egg white and yolk, to mites, animal dander (mixture), feather and dust. The statistical evaluation of the relations among egg allergy, egg sensitisation and sensitisation to mites, animal dander (mixture), feather and dust was performed. Two hundred and eighty eight patients were included in the study (90 men, 198 women, with the average age 25.2). RESULTS AND CONCLUSION: Egg allergy was recorded in 5% and egg sensitisation in 20% of patients; sensitisation to dust is recorded more often in patients with positive results in sIgE for egg white and/or yolk. and J. - Empagliflozin je nový lék používaný ke snížení hyperglykemie u pacientů s diabetem 2. typu. Patří do nejnovější třídy antidiabetik, tzv. gliflozinů, které zabraňují zpětnému vstřebávání glukózy prostřednictvím inhibice sodíko-glukózového transportéru SGLT2. Způsobují tak terapeutickou glykosurii, díky níž dojde ke ztrátě přibližně 70 g glukózy denně. Tím dochází nejen k poklesu glykemie, ale i k poklesu tělesné hmotnosti, protože tato vyloučená glukóza nemůže být použita jako energetický substrát. Ve studiích fáze 3 byla prokázána terapeutická účinnost empagliflozinu v monoterapii, kombinaci s ostatními perorálními antidiabetiky i inzulinem. Dalším příznivým účinkem empagliflozinu je mírný pokles krevního tlaku. Empagliflozin byl v Evropské unii schválen k léčbě pacientů s diabetem 2. typu v květnu 2014, v ČR je dostupný od října 2014 a od 1. února 2015 je empagliflozin (pod obchodním názvem Jardiance?) částečně hrazen ze zdravotního pojištění pro léčbu pacientů s diabetem 2. typu buď v kombinaci s metforminem, nebo v kombinaci s inzulinem (s nebo bez metforminu)., Empagliflozin is a new medicine used to reduce hyperglycemia in patients with type 2 diabetes. It belongs to the most advanced class of antidiabetic drugs, known as gliflozins, which prevent reabsorption of glucose through inhibiting SGLT2 sodium-glucose cotransporter. Thereby they cause therapeutic glycosuria, thanks to which a loss of approximately 70 g of glucose per day occurs. This not only effects the decrease in glycemia, but also the loss of body mass, since this excreted glucose cannot be used as an energetic substrate. The studies within phase 3 have proven the therapeutic efficacy of empagliflozin in monotherapy, in combination with the other oral antidiabec drugs and insulin. Another favourable effect of empagliflozin is a slight decrease in blood pressure. In May 2014, empagliflozin was approved for the treatment of patients with type 2 diabetes within the European Union 2014, since October 2014 it has been available in the Czech Republic and since 1 February 2015 empagliflozin (under the trade name Jardiance?) has been partially covered by the health insurance for the treatment of patients with type 2 diabetes either in combination with metformin or in combination with insulin (with or without metformin)., and Martin Prázný, Jiří Slíva

110. Encephalitis with prolonged but reversible splenial lesion

- INTRODUCTION: The splenium of the corpus callosum has a specific structure of blood supply with a tendency towards blood-brain barrier breakdown, intramyelinic edema, and damage due to hypoxia or toxins. Signs and symptoms of reversible syndrome of the splenium of the corpus callosum typically include disorientation, confusion, impaired consciousness, and epileptic seizures. CASE REPORT: A previously healthy 32-year-old man suffered from weakness, headache, and fever. Subsequently, he developed apathy, ataxia, and inability to walk, and therefore was admitted to the hospital. Cerebrospinal fluid showed protein elevation (0.9 g/l) and pleocytosis (232/1 ul). A brain MRI showed hyperintense lesions in the middle of the corpus callosum. The patient was treated with antibiotics, and subsequently, in combination with steroids. Two months later, the hyperintense lesions in the splenium and the basal ganglia had disappeared. Almost seven months since his hospitalization in the Department of Neurology, the patient has returned to his previous employment. He now does not exhibit any mental changes, an optic edema and urological problems have improved. In addition, he is now actively engaged in sports. CONCLUSION: We have described a case of a 32-year-old man with confusion, ataxia, and inability to stand and walk. The man developed a febrile meningeal syndrome and a hyperintense lesion of the splenium, which lasted for two months. Neurological changes, optic nerve edema, and urinary retention have resolved over the course of seven months.
