Brown tumors are bony lesions caused by rapid osteoclastic activity, which rare involved jaws. Renal osteodystrophy (ROD) is associated with different pathogenetic mechanisms--disorder of calcium-phosphate metabolism, impaired metabolism of vitamin D, increased parathyroid activity that lead to extreme concentrations of parathormone. The authors report two cases of jaw enlargement in patients received haemodialysis with excessive increase values of alkaline phosphatase and parathormone in serum. The patients were treated surgically with corrective procedures in maxillo-facial area. ROD of the jaws could be severe complication in dialysis patients with end stage of CKD if no appropriate care aimed at correction or prevention of parathyroid hyperfunction was applied to them. and PF. Nádory oblasti hlavy a krku představují v Evropě u mužů čtvrté nejčastější nádory. Nejvyšší výskyt je pozorován v zemích okolo Středozemního moře. Incidence nádorů orofaryngu v České republice je 0,91/100 000 obyvatel s mortalitou 0,65/100 000 obyvatel, nejvíce zastoupeným klinickým stadiem nádoru je klinické stadium IV (58,9 %). U nádorů patra je incidence 0,34/100 000 obyvatel s mortalitou 0,15/100 000 obyvatel. Nejvíce zastoupeným klinickým stadiem je také stadium IV (26,7 %). Postižení žen je výrazně nižší než u mužů, v ČR je poměr mužů k ženám přibližně 4:1. Z hlediska rizikových faktorů hraje rozhodující vliv kouření a konzumace alkoholu. Průměrný věk pacientů se pohybuje mezi 50 a 60 lety. V kazuistikách bude představen pacient s nádorem orofaryngu a pacient s nádorem měkkého a tvrdého patra a patrové tonzily. V obou případech šlo o pokročilé lokální nálezy bez vzdálených metastáz u „typických“ pacientů mužského pohlaví s dlouholetým abúzem alkoholu a tabakizmem., Head and neck tumours are the fourth most common cancer among men in Europe. The highest incidence is in the Mediterranean countries. The incidence rate of oropharyngeal cancer in the Czech Republic is 0.91/100 000 inhabitants with a mortality rate of 0.65/100 000 inhabitants; the most prevalent clinical stage of the tumour is clinical stage IV (58.9%). Autoři uvádějí kazuistiku pacienta s epiteloidním hemangioendoteliomem jater (HEHE), který byl léčen chemoembolizací (TACE). Při dlouhodobém sledování 9 let od stanovení diagnózy je klinicky bez obtíží a bez progrese onemocnění při vyšetření zobrazovacími metodami. Součástí práce je shrnutí charakteristiky HEHE jako vzácného maligního nádoru, jeho diagnostika i možnosti léčby., Authors present a case report of a patient with epithelioid hemangioendothelioma (HEHE) treated by chemoembolization (TACE). Clear cell odontogenic carcinoma (CCOC) is a rare odontogenic tumor associated with aggressive clinical behavior, metastasis and low survival. We report a case of CCOC affecting the maxilla of 62 year-old woman. It was first described as a clinicopathological entity in 1985 and to date only 67 cases were described in the English literature. We are understanding of the behavior of this carcinomas was depend on limited case reports. V kazuistike opisujeme prípad 49-ročného muža od detstva morbídne obézneho (BMI 40 kg/m2), s artériovou hypertenziou (asi od 15 rokov, liečená od roku 2004), dyslipidémiou (od roku 2006), diabetes mellitus 2. typu (od roku 2006, na inzulinoterapii od roku 2008) a fajčením (do roku 2011, 20 cigariet denne). Liečba: 16 druhov liekov, 8 na hypertenziu, statín, liečba diabetu, aspirín, alopurinol. V roku 2010 (ako 45-ročný) hospitalizovaný na našej klinike pre dyspnoe a bolesti na hrudníku s hodnotou krvného tlaku 180/110 mm Hg (zistené srdcové zlyhávanie s ejekčnou frakciou ĽK 33 %, vo funkčnej triede NYHA II, echokardiograficky: ľavá predsieň: 46 mm, rozmer ľavej komory v diastole: 70 mm, interventrikulárne septum: 12 mm, hypokinéza septa, dopplerovská ultrasonografia artérií dolných končatín (kalcifikácie, difúzne aterosklerotické zmeny, neprítomná stenóza), CT koronárna angiografia (signifikantná stenóza ľavej koronárnej artérie). Zahájená liečba perorálny furosemid 40 mg denne. V máji roku 2011 hospitalizovaný pre akútny koronárny syndróm: akútny NSTEMI spodnej steny (koronarografia: 2-cievne postihnutie, realizovaná PKI, implantovaný DES – ramus circumflexus, paroxyzmus fibrilácie predsiení, funkčná trieda NYHA III, ľavokomorová ejekčná frakcia: 30 %, pľúcna hypertenzia). V roku 2012 realizovaná renálna denervácia pre rezistentnú hypertenziu, implantovaný karotický stent pre stenózu karotickej artérie, prítomná diabetická nefropatia (KDOQI 3. stupňa, GF 40 ml/min). V auguste roku 2014 hospitalizovaný na našej klinike pre pľúcny edém, kardiogénny šok, akútnu ischémiu pravého predkolenia pri periférnej embolizácii, prítomný atriálny flutter, zhoršenie renálnych parametrov, echokardiograficky: ľavá predsieň: 55 mm, rozmer ľavej komory: 75 mm, akinéza septa a zadnej steny, oklúzia artérií pravej dolnej končatiny (vzhľadom na vážny stav pacienta kontraindikovaná angiochirurgická intervencia, zvažovaná amputácia končatiny z vitálnej indikácie), pacient zomrel po 4 dňoch hospitalizácie na intenzívnej jednotke po neúspešnej liečbe. Kombinácia ochorení diabetes, hypertenzia a ischemická choroba srdca je častá a prognosticky vážna. Diabetes zvyšuje kardiovaskulárnu morbiditu a mortalitu, a preto by sme mali hľadať diabetes u všetkých kardiovaskulárnych pacientov., The case study describes a case of 49-year-old man with morbid obesity since childhood (BMI 40 kg/m2), arterial hypertension (approx. since aged 15, treated since 2004), dyslipidemia (since 2006), type 2 diabetes mellitus (since 2006, on insulin therapy since 2008) and smoking (until 2011, 20 cigarettes a day). Treatment: 16 types of medication, 8 for hypertension, statin, therapy for diabetes, aspirin, allopurinol. In 2010 (when aged 45) hospitalized in our clinic with dyspnoea and chest pain with a high pressure reading of 180/110 mm Hg (identified symptoms of heart failure with LV ejection fraction of 33 %, in NYHA II functional class, echocardiographically: left atrium: 46 mm, left ventricular chamber size in diastole: 70 mm, interventricular septum: 12 mm, septal hypokinesis, Doppler ultrasonography of lower limb arteries (calcification, diffuse atherosclerotic changes, absent stenosis), CT coronary angiography (significant stenosis of the left coronary artery). Treatment started with 40 mg oral dose of furosemide daily. In May 2011 he was hospitalized with an acute coronary syndrome: acute NSTEMI of the inferior wall (coronarography: 2-vascular problems, implemented PKI, implanted DES – ramus circumflexus, paroxysmal atrial fibrillation, NYHA III functional class, left ventricular ejection fraction: 30 %, pulmonary hypertension). In 2012 renal denervation for resistant hypertension was carried out, carotid stent implanted for stenosis of the carotid artery, presence of diabetic nephropathy (KDOQI stage 3, GF 40 ml/min). In August 2014 admitted to our clinic with pulmonary oedema, cardiogenic shock, acute ischemia of the right calf with peripheral embolisation, presence of atrial flutter, impairment of renal parameters, echocardiographically: left atrium: 55 mm, left ventricle size: 75 mm, akinesis of the septum and posterior wall, occlusion of the right leg arteries (given the patient‘s serious state angio-surgical intervention was contraindicated, vitally indicated leg amputation considered), the patient died after 4 days of hospitalization in an intensive care unit after unsuccessful treatment. A combination of diabetes, hypertension and ischemic heart disease is frequent and prognostically serious. Tuberkulózní postižení střev je v rozvinutých zemích vzácné onemocnění, které postihuje zejména imigranty a imunokompromitované pacienty. Klinický obraz může imitovat nitrobřišní infekce, nádorová onemocnění či Crohnovu chorobu. Diferenciální diagnostika Crohnovy choroby a břišní tuberkulózy může být pro klinika a patologa svízelná, v obou případech se jedná o chronický granulomatózní zánět s podobnou symptomatologií. Prezentujeme případ 53letého pacienta s perforační peritonitidou při tuberkulóze střeva, u kterého byla původně mylně diagnostikována Crohnova choroba. Průběh choroby, rozsah a povaha nálezu byly v každé fázi léčby příčinou kritických komplikací. S touto zkušeností předáváme i doporučení: břišní tuberkulózu zvažovat v diferenciálně diagnostické rozvaze každého chronického střevního onemocnění. Clinical presentation may mimic many other abdominal diseases such as intra-abdominal infections, tumors, and Crohn´s disease. Differential diagnosis of Crohn´s disease and intestinal tuberculosis poses a dilemma to clinicians and pathologists as both are chronic granulomatous disorders with similar clinical features. We report the case of a 53 year-old man presenting with perforated intestinal tuberculosis in which the initial diagnostic work-up suggested Crohn´s disease. The severity of the findings resulted in critical complications during the course of treatment. This case points out the need for awareness of intestinal tuberculosis in the differential diagnosis of chronic intestinal disease. Autor v článku popisuje šest případů emfyzematózní pyelonefritidy. Průměrný věk nemocných byl 63,5 roku, onemocnění se vyskytovalo častěji u mužů a ani v jednom případě nebyl příčinou konkrement. Dvě třetiny nemocných byli diabetici a nejčastěji byla prokázána jako etiologické agens Escherichia coli a v jednom případě Klebsiella pneumonie. Autor uvádí způsoby léčby jednotlivých případů onemocnění. Pouze v jednom případě vedla emfyzematózní pyelonefritida ke smrti pacienta., The author of this communication describes six cases of emphysematous pyelonephritis. The mean age of patients was 63.5, with the disease seen most frequently in men. None of the cases were associated with stone disease. Two thirds of patients had diabetes mellitus. Escherichia coli was confi rmed in the urine most frequently, and in one case, Klebsiella pneumoniae was identifi ed. The author presents the methods of treatment used in each case. Pro hemodialyzované pacienty, u kterých byl diagnostikován diferencovaný karcinom štítné žlázy, může mít zásadní význam léčba 131I. Diagnóza nádorového onemocnění tyto pacienty vyřazuje z registru čekatelů pro transplantaci ledviny do doby, než bude prokázána 3-5letá remise. Jelikož je 131I vylučován z těla převážně ledvinami, je efektivní poločas 131I u pacientů s renálním selháním prodloužen. To je důvod, proč by měla být redukována podávaná léčebná aktivita, aby došlo ke snížení radiační zátěže především pro krvetvornou kostní dřeň. Prezentujeme kazuistiku 48letého muže v terminálním renálním selhání, léčeného 131I pro papilární karcinom štítné žlázy. Terapeutické aktivity jsme snížili o 30 %. Hemodialýza byla prováděna po podání léčebné aktivity 131I na lůžkové části kliniky nukleární medicíny. 131I je dialyzovatelný a po každé dialýze klesla zbytková aktivita o 33-67 %. Radiační zátěž personálu hemodialyzačního centra byla minimální. V léčbě uveálních melanomů se v některých případech používá radiochirurgie CyberKnifem. Histologickými a imunohistologickými změnami v nádorové tkáni po terapii se zabývá poměrně málo autorů. V tomto sdělení uvádíme výsledky vyšetření dvou takových případů. U obou pacientů byla provedena enukleace bulbu pro bolestivý glaukom. U prvého pacienta šlo o recidivující melanom, který byl ošetřen gama nožem před deseti lety. Pro lehkou progresi ložiska melanomu bylo provedeno ošetření CyberKnifem celkovými dávkami 45Gy ve 3 frakcích. Po necelých pěti měsících po ozáření bulbu došlo k regresi recidivujícího nádoru z původní prominence 1,4 mm v ploše 6 mm × 0,6 mm na prominenci 0,3 mm v ploše 3 mm × 0,3 mm. Histologicky byla zaznamenána rezidua řídce celulárního ložiska regresivně změněných nádorových buněk a přítomnost reaktivních změn optického nervu. Druhý pacient byl měsíc po radioterapii CyberKnifem s následnou komplikovanou operací katarakty a s enukleací bulbu bez regrese – tedy zániku nádorových buněk. Velikost tumoru 11,1 mm × 15mm s prominencí 9,3mm přetrvávala i po ozáření dávkou 40Gy/1Fr. V histologickém vyšetření byly nádorové buňky bez patologických změn. V imunohistochemickém vyšetření jsme pozorovali u prvého pacienta vysokou difuzní expresi proteinu bcl-2 až do výše 75 % a méně než 1% pozitivitu ve vyšetření s proliferačním markerem Ki-67. U druhého případu byla situace odlišná, s výraznou až 75% pozitivní expresí bcl-2 proteinu v nádorových buňkách v povrchových vrstvách tumoru a s klesajícím počtem pozitivní exprese v hlubokých vrstvách nádoru, kde dosahovala maxima do 25 %. Proliferační aktivita s pozitivní jadernou expresí v Ki-67 vzrůstala od povrchu, kde se pohybovala v rozmezí 1–3 % do hloubky nádoru a k jeho bázi, kde dosahovala až 5 %. Naše pozorování ukazuje na výraznou redistribuci genetické aktivity uvnitř nádoru. Ta se projevuje „overexpresí“ bcl-2 proteinu a u druhého případu je patrná rozdílná pozitivita bcl-2 proteinu v různých vrstvách nádoru. Vztah tohoto fenomenu k radioterapii není dosud jasný., Radiosurgery with CyberKnife is used in the treatment of uveal melanomas in some cases. Histological and immunohistological changes in tumour tissue following treatment have been addressed by relatively few authors. This paper presents the results of investigations of two such cases. Enucleation of the eye was performed for painful glaucoma in both patients. Patient 1 had a recurrent melanoma treated with a Gamma Knife ten years ago. Due to slight progression in the melanoma site, treatment with CyberKnife was used with total doses of 45 Gy in 3 fractions. Some five months following eye irradiation, there was regression of the recurrent tumour from the original height of 1.4 mm in an area of 6 mm x 0.6 mm to that of 0.3 mm in an area of 3 mm x 0.3 mm. Histologically, residua of a sparsely cellular locus of regressively altered tumour cells and the presence of reactive changes in the optic nerve were noted. Patient 2 underwent radiotherapy with CyberKnife one month prior, with subsequent complicated cataract surgery and enucleation of the eye without regression, i.e. destruction of tumour cells. The size of the tumour of 11.1 mm x 15 mm with a height of 9.3 mm remained unchanged even after irradiation with a dose of 40 Gy/1 fr. No pathological changes in the tumour cells were seen on histological examination. In patient 1, immunohistochemistry showed high diffusion expression of the bcl-2 protein of up to 75 % and less than 1 % positivity on examination with the proliferation marker Ki-67. In patient 2, the situation was different, with significant, as high as 75 % positive expression of the bcl-2 protein in tumour cells in the superficial tumour layers and a decreasing rate of positive expression in the deep tumour layers where it reached no more than 25 %. The proliferation activity with positive nuclear expression of Ki-67 increased from the surface, where it ranged from 1 % to 3 %, to the depth of the tumour and toward its base, where it reached up to 5 %. Our observation suggests significant redistribution of genetic activity within the tumour. It is manifested by overexpression of the bcl-2 protein and, in patient 2, different bcl-2 protein positivity in the various tumour layers is apparent.
