The authors demonstrate the possibility of improving surgical results by the reduction of perioperative bleeding in thoracic surgery associated with extended resection procedures. We focused on patients in whom the expected perioperative blood loss was greater than 500 ml. The first group consisted of patients with lung cancer stage III A after neoadjuvant chemotherapy had been indicated to extend the resection procedure. The second group consisted of patients with chest wall and mediastinum tumors of various etiologies. The third group consisted of patients with post-inflammatory thoracic complications in whom combined decortication and pleurectomy was necessary. Tuberkulózní postižení střev je v rozvinutých zemích vzácné onemocnění, které postihuje zejména imigranty a imunokompromitované pacienty. Klinický obraz může imitovat nitrobřišní infekce, nádorová onemocnění či Crohnovu chorobu. Diferenciální diagnostika Crohnovy choroby a břišní tuberkulózy může být pro klinika a patologa svízelná, v obou případech se jedná o chronický granulomatózní zánět s podobnou symptomatologií. Prezentujeme případ 53letého pacienta s perforační peritonitidou při tuberkulóze střeva, u kterého byla původně mylně diagnostikována Crohnova choroba. Průběh choroby, rozsah a povaha nálezu byly v každé fázi léčby příčinou kritických komplikací. S touto zkušeností předáváme i doporučení: břišní tuberkulózu zvažovat v diferenciálně diagnostické rozvaze každého chronického střevního onemocnění. Clinical presentation may mimic many other abdominal diseases such as intra-abdominal infections, tumors, and Crohn´s disease. Differential diagnosis of Crohn´s disease and intestinal tuberculosis poses a dilemma to clinicians and pathologists as both are chronic granulomatous disorders with similar clinical features. We report the case of a 53 year-old man presenting with perforated intestinal tuberculosis in which the initial diagnostic work-up suggested Crohn´s disease. The severity of the findings resulted in critical complications during the course of treatment. This case points out the need for awareness of intestinal tuberculosis in the differential diagnosis of chronic intestinal disease. Cysta choledochu je vzácné onemocnění, výrazně vyšší výskyt zaznamenáváme u asijské populace. Etiologie není zcela známa, avšak uvažuje se o spojení s anomáliemi v anatomii žlučových cest. Přestože je svojí povahou benigní, jedná se o prekancerózu s nebezpečím zvratu v karcinom žlučových cest. Jediným kurativním řešením je radikální chirurgické odstranění s rekonstrukcí žlučových cest. Autoři prezentují kazuistiku nemocné s cystou ductus hepatocholedochus typu I dle Todaniho klasifikace., Choledochal cyst is a rare disease with a considerably higher incidence found in the Asian population. Although its etiology is not completely known, the disease is believed to be associated with anomalies in the anatomy of the biliary tract. While being a benign unit, it is considered as a precancerosis with the risk of conversion to the biliary tract carcinoma. Úvod: Česká republika zaujímá v incidenci kolorektálního karcinomu (KRK) čelní místo ve světě. Radikální odstranění nádoru představuje nejúčinnější část v rámci současné multimodální terapie. Autoři prezentují průběžné výsledky roboticky asistované léčby u nemocných s karcinomem rekta (KR). Metoda: Vlastní soubor obsahuje 61 roboticky operovaných pacientů pro KR. U posledních 31 pacientů byla data získávána prospektivně. Analyzovány byly epidemiologické údaje, perioperační ukazatele, komplikace a onkologické parametry. Výsledky: Roboticky asistovanou operační léčbu pro karcinom rekta na našem pracovišti podstoupilo 61 nemocných: 34 mužů a 27 žen průměrného věku 62 let (33–80 let). Neoadjuvantní onkologická léčba byla indikována u 46 % pacientů. Průměrná krevní ztráta činila 187 ml, u tří nemocných byly podány transfuzní přípravky. Šest operací jsme byli nuceni konvertovat, u 16 pacientů jsme zaznamenali komplikace. Insuficience anastomózy se vyskytla u 10 % pacientů, u 4 nemocných si stav vyžádal operační léčbu. Žádný nemocný nezemřel. Rekurenci maligního onemocnění jsme diagnostikovali u 3 (5 %) pacientů. U 27 nemocných byla (od roku 2013) stanovena kvalita mezorektální excize (ME) a metrické stanovení cirkumferentního resekčního okraje – (y)pCRO. Ve dvou případech byla potvrzena pozitivita (y)pCRO a přibližně u čtvrtiny operantů nebyla ME kompletní. Závěr: Chirurgická léčba je v interdisciplinárním terapeutickém procesu u KR klíčová. Naše průběžné výsledky jsou srovnatelné se závěry některých literárně uveřejněných sdělení. Da Vinci systém je bezpečným manipulátorem v léčbě KR a operatérovi přináší nesporné výhody při preparaci ve stísněném pánevním prostoru. V současnosti chybí zhodnocení přínosu robotické operativy pro pacienta v oblasti břišní chirurgie z dlouhodobého hlediska a s dostatečným počtem operantů, resp. vysokým stupněm evidence based medicine (EBM). Vysoká pořizovací cena přístroje, jednotlivých instrumentů s vybavením a nesystémová úhrada představují významnou překážku v širším využití robotického systému v léčbě KR i jiných abdominálních malignit v ČR., Introduction: The incidence of colorectal cancer (CRC) in the Czech Republic is reported to be one of the highest on the global scale. Radical tumor removal has been observed to be the most effective part in the context of current multimodal therapy. The authors present their preliminary results of robotic assisted treatment of rectal carcinoma (RC). Method: The observed group includes 61 patients who underwent robotic assisted treatment for rectal cancer. The data were collected prospectively in the last 31 patients. Analyses were conducted on epidemiological data, perioperative outcomes, complications and oncological results. Results: Robotic assisted treatment of RC was performed in 61 patients: 34 men and 27 women, mean age of 62 years (33–80). Neoadjuvant oncological treatment was indicated in 46% of the patients. Average blood loss was 187 ml, transfusions were administered in three cases. Conversion to open procedure was performed 6 times, and 16 patients had postoperative complications. Anastomotic leak was observed in 10% of the patients, and 4 patients undewent surgical treatment. No patient died. Local recurrence of the cancer was diagnosed in 3 (5%) patients. The quality of mesorectal excision (ME) and the circumferential resection margin [(y)pCRM] have been determined in 27 patients since 2013. Positive (y)pCRM was recorded in two cases and incomplete ME was observed in 25.8% of the patients. Conclusion: Surgical treatment for RC is pivotal in multimodal therapy. Our preliminary results are similar to the conclusions in other published studies. The da Vinci robotic system is a safe manipulator in the treatment of RC and provides indisputable benefits to the surgeon when operating in the narrow pelvic space. However, the benefits of robotic treatment in abdominal surgery are yet to be evaluated in patients (with respect to long-term results, sufficient number of patients or a high EBM level of evidence). The high purchase price of the robotic device, individual instruments with equipment and non-systemic compensation constitute a significant hindrance that prevents wider use of the robotic system in the treatment of RC and other abdominal malignancies in the Czech Republic., and D. Retrospektivní studie u žen do 49 let analyzovala novotvary následující po primárních nádorech cervixu (CC), dělohy (CU), vaječníků (CO) a primární novotvary před těmito gynekologickými nádory (GC), hlášené v Národním onkologickém registru ČR v letech 1976–2010. Celkem 3 727 pacientek těchto tří diagnóz představovalo 12,1 % z 30 794 nově evidovaných GC, spojených s 4 160 dalšími novotvary. Jejich vývoj se týkal a) 1 339 žen s primárním CC, 715 s CU, 664 s CO (s následnými 1 509 a 831, respektive 758 dalšími novotvary), b) 263 žen s následným CC, 212 s CU, 534 s CO (s předcházejícími 274 a 223, respektive 565 primárními novotvary). Průměrný interval vzniku následných novotvarů byl 14,2 roku po CC, 13,6 roku po CU a 10,2 roku po CO. Nejvíce z 3 098 následných nádorů bylo 21 % trávicího traktu, 16 % prsů, 13 % kůže, 11,7 % rodidel, 9,7 % dýchacích a 7,8 % močových cest; z nich během prvního roku po primárním GC bylo nejvíce nádorů vaječníků, endometria, cervixu a kolorekta. Nejvíce z 1 063 primárních novotvarů před GC bylo 26,7 % rodidel, 24,3 % prsů a 14,8 % trávicího traktu. Během 35 let počet žen s primárními GC klesal, zejména v zastoupení jejich časných klinických stadií, žen s následnými GC rostl, zejména jejich pokročilých stadií. Časná stadia byla evidována u 70,4 % žen s primárními GC a 54 % s následnými GC, pokročilá stadia u 29,5 % žen s následnými GC a 13,3 % s primárními GC, neznámá stadia zahrnovala asi 16,5 %. Ze studie se odhaduje asi 1100 případů pokročilých stadií u žen do 49 let přežívajících s GC v letech 1976–2010. Většina z hodnocených kritérií byla nepříznivá u žen s karcinomy ovarií. Zejména těmto případům v pokročilých stadiích by měly zabránit dispenzární opatření a preventivní intervence. Omezené finanční zdroje a přetrvávající rizika životního stylu neumožňují zpomalit v blízké budoucnosti nárůst primárních a následných novotvarů u onkologicky přežívajících žen nejen v gynekologii., The neoplasms following primary cancers of cervix (CC), uterus (CU), ovary (CO), and primary neoplasms before these gynaecological cancers (GC), were analyzed in a retrospective study in survived females aged 0–49 years, notified in the National Czech Cancer Registry between 1976 and 2010. A total 3,727 patients of these three diagnoses, presented 12.1 % of 30,794 newly registered GC, were associated with 4,160 other neoplasms. Their development was concerned with a) 1,339 females with primary CC, 715 with CU and 664 with CO (followed by 1,509 and 831, respectively 758 other neoplasms) and b) subsequent 263 CC, 212 CU and 534 CO (preceded by 274 and 223, respectively 565 primary neoplasms). The average interval of occurrence of subsequent neoplasms was 14.2 years after CC, 13.6 years after CU and 10.2 years after CO. The most frequent of 3.098 subsequent neoplasms were 21 % of digestive tract, 16 % breast, 13 % skin, 11.7 % female genital, 9.7 % respiratory and 7.8 % urinary organs; the most frequent during the first year after primary GC were those of the ovarial, endometrial, cervical and colorectal cancers. The most frequent of 1,062 primary neoplasms before GC were 26.7 % cancers of female genital organs, 24.3 % breast and 14.8 % digestive tract. During 35 years the number of females with primary GC has decreased, especially of their representation at the early clinical stages, the females with subsequent GC has increased, especially at the advanced stages. There were recorded early stages in 70.4 % females with primary GC and 54 % with subsequent GC, advanced stages in 29.5 % females with subsequent GC and 13.3 % with primary GC, the unknown stages included about 16.5 %. In our study are estimated about 1100 cases of advanced stages in females aged 49 years survived with GC in 1976–2010. Most of the evaluated criteria were unfavorable among females with ovarian cancers. Mainly the cases at advanced stages must be prevented by the dispensary guidelines and preventive interventions. INTRODUCTION: Cellulitis remains a very serious disease even today. Mortality, which varied between 10-40%, has been reduced owing to the standard securing of airway patency and use of an appropriate surgical treatment approach. MATERIALS AND METHODS: A total of 195 patients were hospitalised for cellulitis at the University Hospital in Hradec Králové during 2007-2011. The following parameters were evaluated: age, gender, dependence of incidence of the disease on the season of the year, frequency of attacks of the particular areas and their clinical characteristics, aetiology of the inflammation, types of patient complaints, prevalence of current systemic diseases, results of microbiological and selected laboratory analyses, socio-economic status of the patients, and duration of patient stay at the hospital. Statistical analysis was performed by using Pearson's correlation coefficient, the statistical significance level was p < 0.05. RESULTS: The mean age of the patients was 39.8 years. The group of 195 patients included 108 (55%) males and 87 (45%) females. The mean time between the first symptoms of the disease and admission to the Department was 5 days. From among the 195 patients, 116 (59.5%) were working persons, 79 (40.5%) were non-working (children, students, unemployed persons, women on maternity leave, retired people). The odontogenic origin of the disease was verified in 173 (88.7%) patients. In total, 65 (33.3%) patients had no coinciding complicating systemic disease, 22 (11.3%) patients had diabetes mellitus. The most frequent symptom of cellulitis was painful swelling, found in 194 (99.5%) patients, followed by jaw contracture, found in 153 (78.5%) patients. CONCLUSION: The results are largely very similar to those of previous studies performed in other countries, except that we found no correlation between the prevalence of cellulitis and the socio-economic status, nor have we confirmed Klebsiella pneumoniae sp. as the cause of cellulitis in patients with diabetes mellitus. and H. Spánek se významně podílí na celkovém zdraví a kvalitě života člověka, je důležitý nejen pro duševní a fyzickou regeneraci, ale také pro normální funkci metabolických pochodů, endokrinního a imunitního systému. V moderní společnosti dochází k postupnému zkracování délky spánku v důsledku celé řady faktorů. Epidemiologické studie v posledním desetiletí potvrdily, že spánek kratší než 7 hodin představuje rizikový faktor pro rozvoj obezity, diabetu mellitu 2. typu, hypertenze, kardiovaskulárních onemocnění, depresí a zvyšuje riziko mortality. Cílem této práce bylo zhodnotit délku spánku a posoudit vztah mezi délkou spánku a BMI u dospělé populace (n=478, 18–65 let). Metoda sběru dat zahrnovala 7denní záznam o délce spánku za den a antropometrická měření. Z výsledků vyplynulo, že průměrná délka spánku v souboru činila 7,45 (±0,91) hodin za den. 28 % dospělých osob přesto spalo méně než doporučených 7 hodin denně. Statisticky nejméně spali jedinci ve věku 40–49 let (průměr 7,2±0,80 hodin). Riziko obezity definované jako BMI ≥30 kg/m2 v souboru stoupalo se zkracující se délkou spánku: pro kategorie spánku [<6,3 h], [≥6,3 až <8,3 h] a [≥8,3 h] bylo riziko obezity na základě relativních četností vyjádřeno poměrem 2 : 1,5 : 1,2. Jedinci spící průměrně <6,3 hodiny vykazovali 1,3krát vyšší riziko obezity než ostatní jedinci spící déle i při zohlednění faktoru věku, tento závěr však není statisticky významný (OR=1,256; 95% IS: 0,631; 2,422). Z hlediska prevence obezity se dostatečný a kvalitní spánek jeví jako významný ochranný faktor, který by zejména v současné době rychlého životního tempa a při stoupajícím počtu obézních jedinců neměl být, vedle zdravé stravy a dostatečné pohybové aktivity, opomíjen., Sleep plays a significant role in overall health and in the quality of human life. It is not only important for mental and physical regeneration, but also for the normal functioning of metabolic processes, for the endocrine and immune systems. In modern society there is a gradual shortening of the length of sleep due to a number of factors. In the last decade a number of epidemiological studies suggest that sleeping less than 7 hours a day increases the risk of developing obesity, type 2 diabetes mellitus, hypertension, cardiovascular diseases, and depression and is associated with premature mortality. The aim of this study was to evaluate sleep duration and to assess the relationship between sleep duration and BMI in the adult population (n=478, 18 to 65 years). The method of data collection consisted of a 7-day record of the length of the sleep per day and anthropometric measurements. The results showed that the average sleep duration was 7.45 (±0.91) hours per day. However, 28% of adults slept less than a recommended 7 hours a day. The individuals aged 40–49 years slept statistically the shortest time (mean 7.2±0.80 hours). The risk of obesity increased indirectly with duration of sleep in this population, for sleep categories [<6.3 h], [≥6.3 to <8.3 h] and [≥8.3 h] the risk of obesity defined as BMI ≥30 kg/m2 was 2 : 1.5 : 1.2 based on the relative frequencies of obese and non-obese participants. Individuals sleeping on average <6.3 hours were at 1.3 times higher risk of obesity than other individuals sleeping longer after adjusting for age, this conclusion was not statistically significant (OR=1.256, 95% CI: 0.631; 2.422). BACKGROUND: Human immunodeficiency virus (HIV) continues to be a serious health issue and one of the world most devastating epidemics. An estimated 1.5 million people died from AIDS-related illnesses in 2013, and an estimated 37 million people with AIDS have died worldwide since the epidemic has begun. HIV infection is known for its oral manifestations which causes discomfort and pain for infected individuals. The objective of this study was to document oral conditions of HIV positive patients and the pattern and frequency of oral and dental lesions. METHODS: All patients with confirmed HIV infection who were treated at the Department of Dentistry, University Hospital in Hradec Králové, were examined. RESULTS: During the study period, 29 HIV positive patients were examined and treated--19 men, 10 women, with mean age of 32.9 years (range 22-58 years). 72.41% patients received ART. In total, all patients underwent 186 visits. The most frequent treatments were associated with teeth and periodontal lesions (71.80%), oral mucosal lesions were diagnosed and treated only in 3.96% cases. BACKGROUND: The goal of this prospective study was to determine the frequency of micrometastases in patients with squamous cell carcinoma (SCC) of the oral cavity, pharynx and larynx in whom elective neck dissection was indicated (cN0). PATIENTS AND METHODS: A total of 12 patients (10 males and 2 females) were enrolled in the study. The age ranged 42-73 years (median 62 years). Elective neck dissection was performed in all patients (8 ipsilateral, 4 bilateral) and a total of 256 lymph nodes were removed and sent for microscopic examination. RESULTS: The presence of tumor cells in cervical lymph nodes was found in 5/12 (42%) patients. Micrometastases of SCC were found in two patients and isolated tumor cells (ITC) in two other patients. In the remaining one patient with oropharyngeal SCC, a micrometastasis of papillary thyroid carcinoma (PTC) was detected. Positive lymph nodes were localized in level II in three patients with SCC of larynx, hypopharynx and tongue base, respectively, in level I in one patient with SCC of oral tongue and in level III in one patient with PTC. CONCLUSION: Our results indicate that SCC of head and neck has a high potential for creating micrometastases which frequency is higher compared to clinically detected macrometastases. Cíl: Prezentovat zajímavý případ 44leté ženy s metastázou karcinomu prsu v axilární lymfatické uzlině vpravo s negativní mamografií, biopsií a ultrasonografií prsů, kdy 18F-FDG PET/CT odhalil multifokální karcinom prsu vpravo. Materiál a metoda: Pacientka vyhledala lékaře pro hmatnou rezistenci v pravé axile. Následovala exstirpace paketu lymfatických uzlin z pravé axily, histologické vyšetření prokázalo metastázu karcinomu prsu. Mamografie a ultrasonografie obou prsů byla s negativním výsledkem. Vzhledem k pozitivitě metastáz v lymfatických uzlinách v pravé axile následovala ještě biopsie z pravého prsu, také s negativním nálezem. V rámci pátrání po primárním ložisku karcinomu prsu pacientka absolvovala celotělové PET/CT zobrazení v rozsahu od baze lební do proximální třetiny stehen, a to 60 minut po intravenózní aplikaci radiofarmaka 18F-FDG o aktivitě 377 MBq (5 MBq/kg). Akviziční čas PET skenu byl 2,5 minuty na 15 cm projekci, celkem 7 projekcí. Kontrastní (i.v. + per os kontrast) CT sken byl proveden ve venózní fázi. Výsledky: PET/CT odhalil multiložiskový viabilní tumor v laterální části pravého prsu a viabilní metastázy v lymfatických uzlinách v pravé axile a infraklavikulárně vpravo. Závěr: 18F-FDG PET/CT správně detekoval multifokální karcinom prsu u pacientky s pozitivní metastázou tohoto karcinomu v lymfatické uzlině a neznámým primárním ložiskem, čímž zkrátil dobu diagnostického procesu a umožnil zvolit správnou léčbu., Aim: To present a rare case report of the multifocal breast cancer finding on 18F-FDG PET/CT in a 44-year-old woman with the positive right axillary breast metastases and negative mammography, ultrasound and right breast biopsy. Material and Method: The patient underwent 18F-FDG PET/CT for the positive right axillary lymph node breast cancer metastases and negative mammography, ultrasound a right breast biopsy. The whole body PET/CT was obtained 60 minutes after intravenous injection of 18F-FDG (377 MBq – 5 MBq/kg). The PET scan acquisition time was 2.5 minutes per bed position for whole body imaging from base of the skull to mid femur and seven bed positions were necessary. Contrast-enhanced CT (venous phase) provided both full CT evaluation (including intravenous and oral contrast) and PET attenuation correction. Results: The examination revealed PET/CT positive multifocal tumour in the right breast and regional metastases in the right axilla and infraclavicular region without distant metastases. The patient underwent the subsequent neoadjuvant chemotherapy, surgery and adjuvant radiotherapy and the complete remission was induced.
