Nádory oblasti hlavy a krku představují v Evropě u mužů čtvrté nejčastější nádory. Nejvyšší výskyt je pozorován v zemích okolo Středozemního moře. Incidence nádorů orofaryngu v České republice je 0,91/100 000 obyvatel s mortalitou 0,65/100 000 obyvatel, nejvíce zastoupeným klinickým stadiem nádoru je klinické stadium IV (58,9 %). U nádorů patra je incidence 0,34/100 000 obyvatel s mortalitou 0,15/100 000 obyvatel. Nejvíce zastoupeným klinickým stadiem je také stadium IV (26,7 %). Postižení žen je výrazně nižší než u mužů, v ČR je poměr mužů k ženám přibližně 4:1. Z hlediska rizikových faktorů hraje rozhodující vliv kouření a konzumace alkoholu. Průměrný věk pacientů se pohybuje mezi 50 a 60 lety. V kazuistikách bude představen pacient s nádorem orofaryngu a pacient s nádorem měkkého a tvrdého patra a patrové tonzily. V obou případech šlo o pokročilé lokální nálezy bez vzdálených metastáz u „typických“ pacientů mužského pohlaví s dlouholetým abúzem alkoholu a tabakizmem., Head and neck tumours are the fourth most common cancer among men in Europe. The highest incidence is in the Mediterranean countries. The incidence rate of oropharyngeal cancer in the Czech Republic is 0.91/100 000 inhabitants with a mortality rate of 0.65/100 000 inhabitants; the most prevalent clinical stage of the tumour is clinical stage IV (58.9%). Clear cell odontogenic carcinoma (CCOC) is a rare odontogenic tumor associated with aggressive clinical behavior, metastasis and low survival. We report a case of CCOC affecting the maxilla of 62 year-old woman. It was first described as a clinicopathological entity in 1985 and to date only 67 cases were described in the English literature. We are understanding of the behavior of this carcinomas was depend on limited case reports. The authors demonstrate the possibility of improving surgical results by the reduction of perioperative bleeding in thoracic surgery associated with extended resection procedures. We focused on patients in whom the expected perioperative blood loss was greater than 500 ml. The first group consisted of patients with lung cancer stage III A after neoadjuvant chemotherapy had been indicated to extend the resection procedure. The second group consisted of patients with chest wall and mediastinum tumors of various etiologies. The third group consisted of patients with post-inflammatory thoracic complications in whom combined decortication and pleurectomy was necessary. Cíl studie: Metabolismus železa musí být velmi pečlivě regulován. Klíčovým systémovým regulátorem je peptidový hormon hepcidin, který způsobuje “uzamčení” železa uvnitř buňky; to pak nemůže být utilizováno. Dialyzovaní pacienti často trpí anémií, která je způsobena mnoha faktory, mj. stavem mikrozánětu a také zřejmě zvýšenou retencí hepcidinu v důsledku snížené glomerulární filtrace. Naším cílem bylo popsat vztahy mezi hepcidinem a dalšími parametry metabolismu železa, zánětu a erytropoezy u těchto pacientů. Typ studie: Observační Název a sídlo pracoviště: Ústav klinické biochemie a hematologie LF UK a FN v Plzni, Alej Svobody 80, 304 60 Plzeň Materiál a metody: Do studie bylo zařazeno 164 chronicky dialyzovaných pacientů (věk 66 ± 13, 25 - 92 let; 101 mužů a 63 žen) a 37 zdravých kontrol (věk 55 ± 20, 21 - 92 let; 16 mužů a 21 žen), u kterých byl stanoven jejich kompletní krevní obraz, parametry metabolismu železa, zánětu a výživy. Výsledky: Koncentrace železa, transferrinu a hemoglobinu byla významně nižší u dialyzovaných pacientů (p<0,0001), naopak ferritin (p<0,0001), solubilní transferrinové receptory(p<0,05), hepcidin (p=0,0003), CRP a IL-6 (p<0,0001) byly u těchto pacientů významně vyšší než u zdravých kontrol. Hepcidin slabě koreloval pozitivně s CRP a ferritinem a negativně s transferrinem u dialyzovaných pacientů, u zdravých kontrol nebyla nalezena žádná korelace hepcidinu s ostatními parametry. Závěr: Zdá se, že vliv zánětu na hladiny hepcidinu není u hemodialyzovaných pacientů zcela zásadní a na jeho koncentraci se podílí i jiné faktory, např. jeho retence., Objective: Peptide hormone hepcidin is a key systemic regulator of the iron metabolism. Hepcidin binds to the iron cell exporter ferroportin so iron is kept in the cells unavailable for erythropoiesis. The synthesis of hepcidin is up-regulated by high iron stores and inflammation. Dialyzed patients have very often impaired iron management – they suffer from anemia, which is caused by many factors including the state of micro inflammation and hepcidin retention due to decreased glomerular filtration rate. Our aim was to describe the relationship of hepcidin and other parameters of iron metabolism, erythropoiesis and inflammation. Design: Observation Settings: Institute of Clinical Biochemistry and Hematology, Charles University Hospital in Pilsen, Alej Svobody 80, 304 60 Plzeň Material and Methods: Complete blood cell count, iron, ferritin, transferrin, CRP, albumin, creatinine, hepcidin, soluble transferrin receptors (sTfR) and IL-6 were measured in samples from 164 patients included in chronic hemodialysis program (age 66 ± 13, 25 - 92 years), 63 women and 101 men and 37 control healthy volunteers (age 55 ± 20, 21 - 92 years), 21 women and 16 men. Results: Iron, transferrin and hemoglobin were significantly lower in the patients group (p<0.0001) while ferritin (p<0.0001), sTfR (p<0.05), hepcidin (p=0.0003), CRP and IL-6 (p<0.0001) were significantly higher in the patients group. Hepcidin weakly positively correlated with CRP and ferritin and weakly negatively correlated with transferrin in hemodialyzed patients. No correlation of hepcidin with other biochemical parameters in controls was found. Conclusion: It seems that the influence of inflammation on hepcidin levels in hemodialyzed patients is not crucial and other factors including its retention in end-stage renal disease participate., T. Pokus používat víno k léčbě začíná v dávnověku, avšak teprve v současné době byl prokázán jeho příznivý účinek v prevenci infarktu myokardu, cévní mozkové příhody, diabetes mellitus a hypertenze. Pití velmi dobře působí také v pokročilém věku. Nejlepší účinky má každodenní pití při večeři, u mužů 20–40 g alkoholu, u žen polovina této dávky., We have been drinking wine at least for 10 000 years or longer, in our region already from Celtic and Germanic times. Favorable effects on prevention of myocardial infarction, stroke, diabetes and hypertension were demonstrated recently, following attempts to use alcohol for treatment in ancient times. Favorable effects of wine drinking could be seen also in aging population. The best results for men are reached by daily drinking of 20–40 g alcohol at dinner. Paraneoplastická limbická encefalitida je vzácné onemocnění náležící do skupiny paraneoplastických neurologických syndromů. V diagnostice hraje zásadní roli zobrazení mozku magnetickou rezonancí. Předkládáme kazuistiku 57letého muže, u něhož vyšetření magnetickou rezonancí jistě přispělo ke stanovení této diagnózy. The diagnostic role of magnetic resonance imaging (MRI) is of utmost importance. We report a case of a 57-year-old man in whom MRI significantly contributed to establishing of proper diagnosis. Other important diagnostic test includes identification of specific, so called antineural auto antibodies. Only a few cases of pneumocystis pneumonia (PCP) in Cushing’s syndrome have been published in the literature so far. In the majority of these patients, the pneumonia occurred after reduction of the hypercortisolism with medicamentous treatment. We report two cases of PCP during conservative treatment of hypercortisolism. We describe clinical, imaging and laboratory findings in two patients and review published cases of pneumocystits pneumonia in Cushing’s syndrome. A 60-year-old woman and 20-year-old man with Cushing’s syndrome due to ectopic ACTH syndrome were treated at our department. Both developed pneumocystis pneumonia early after treatment with ketoconazole and ethomidate bromide had been introduced and the levels of cortisol rapidly decreased. PCP prophylaxis in patients with high cortisolemia should be started before treatment of hypercortisolism in current practice. Cíl. Pseudomyxom peritonea je velmi vzácné semimaligní onemocnění. Záměrem našeho sdělení je pojednání o typickém nálezu, možnostech léčby a komplikacích. Metoda. Nedávné případy z našeho pracoviště ilustrují patognomické znaky nemoci v CT obrazu, obvyklý klinický nález, průběh onemocnění i svízelnost jeho léčby. Ve dvou našich kazuistikách je příčinou pseudomyxomu peritonea mucinózní cystadenom appendixu, v jednom appendikální low grade cystadenokarcinom, v posledním případě mucinózní cystadenokarcinom neznámého původu. U prvního pacienta se navíc objevila raritní komplikace - metastatická ložiska intrathorakálně. Závěr. Pseudomyxom peritonea je klinický syndrom, který je způsoben excesivní produkcí gelatinózního ascitu buňkami mucinózního cystadenomu či cystadenokarcinomu různého původu, nejčastěji se však jedná o tumory appendixu. Typický morfologický nález v zobrazovacích metodách určuje jejich významné místo v diagnostice, stagingu a monitorování výsledků léčby pseudomyxomu., Aim. Pseudomyxoma peritonei is an uncommon semimalignous disease. The aim of our statement is the discussion about the typical finding, the possibilities of treatment and complications. Method. The recent findings from our workplace illustrate pathological signs of the disease on CT-scan, usual clinical finding, the progression of the disease and the arduous of its treatment. In two of our cases is the cause of pseudomyxoma peritonei mucinous cystadenoma of the appendix, in one low grade mucinous cystadenocarcinoma and in the last case the mucinuos cystadenoma of unknown origin. There has appeared by the first patient a rare complication - intrathoracic implants lesions. Conclusion. Pseudomyxoma peritonei is a clinical syndrome that is caused by the excessive production of gelatinous ascites cells mucinous cystadenoma or cystadeno-karcinoma of different origin, but mostly these are the cases of appendix tumors. Stiff-person syndrome (SPS) is a rare disorder characterized by muscle stiffness and painful spasms. Misdiagnosis may occur due to the fact that the clinical picture of SPS is often atypical. The main pathophysiologic mechanism underlying the development of SPS is insufficient inhibition at the cortical and spinal levels. There is good evidence for a primary autoimmune etiology. A 61-year-old man was admitted to a neurological department due to muscle hypertonia with episodic attacks of painful spasms predominantly affecting axial muscles. The symptoms developed shortly after tickborne meningoencephalitis. Electromyography (EMG) revealed signs of continuous motor unit activity. Antibodies against glutamate decarboxylase (anti-GAD) were highly elevated. We present a case of a man who developed clinically severe anti-GAD positive SPS, provoked by tick-borne encephalitis. After therapeutic plasma exchange (TPE) a rapid, temporary improvement of the clinical and neurophysiological findings was noted. Only after being placed on long-term immunosuppression did the patient achieve stable recovery.
