The clinical outcome of 153 Graves' disease patients treated with a wide dose range of radioactive iodine-131 (RAI) was analyzed retrospectively. Six to nine months after the first dose of RAI 60 patients (39%) were hypothyroid (or rather thyroxine-substituted) and 26 (17%) were euthyroid, while 67 patients (44%) did not respond properly: in 32 (21%) their antithyroid drug (ATD) dose could be reduced but not withdrawn (partial response) and 35 (23%) remained hyperthyroid or the same dose of ATD was necessary (no response). The outcome did not correspond significantly to the administered activity of RAI (medians 259, 259, 222, and 259 MBq for hypothyroid, euthyroid, partial, and no response subgroups, respectively), or the activity retained in the gland at 24 h (medians 127, 105, 143, and 152 MBq). The effect was, however, clearly, and in a stepwise pattern, dependent on initial thyroid volume (17, 26, 33 and 35 ml, P < 0.001) or activity per gram tissue retained at 24 h (6.02, 4.95, 4.75, and 4.44 MBq/g, P = 0.002). Also, higher residual level of thyrotoxicosis at the time of RAI treatment was connected with worse outcome. The dose-dependency of outcome was further analyzed. When our sample was divided into tertiles, according to the adjusted dose, the same modest success rates (47%) were seen in the lower and middle tertiles. However, doses higher than 5.88 MBq/g (the upper tertile) resulted in success rate of 75%. Finer division into decils has shown a threshold-like increase in cure rate between the 7th and the 8th decil. In the first 7 decils (doses ≤ 6 MBq/g) the complete response rate was 45 to 50%, in the 8th decil (6.0 to 7.8~MBq/g) it rose to 80% and was not further increased with increasing dose. Direct comparison of higher (> 6 MBq/g, cure rate 80%) and lower (≤ 6 MBq/g, cure rate 46%) doses gave highly significant difference (P < 0.001). With our dosing range we found a dose-dependent clinical outcome that suggests an optimum delivered dose near 6.5 MBq/g, resulting in successful treatment of ca 80% patients. and J. Čepková, J. Úvod: Cílem práce je identifikovat pacienty operované pro vývodový karcinom pankreatu, kteří přežili více než 5 let a analyzovat jejich charakteristiky a faktory s vlivem na přežití. Zkušenosti a výsledky terciárního centra. Metoda: Prospektivně vedená databáze 155 pankreatických resekcí v období leden 2006 až červen 2010 byla prozkoumána se záměrem identifikace pacientů operovaných kurativně pro vývodový karcinom pankreatu. Klinická a patologická data byla analyzována retrospektivně. Kaplan-Meierova analýza byla použita pro hodnocení přežívání, Log-rank test a Coxova regresní analýza pro analýzu faktorů s vlivem na přežití. Testy byly dělány na hladině signifikance 0,05. Jako nezávisle proměnné v Coxově regresní analýze byly vybrány faktory, u nichž byl pomocí Kaplan-Meierovy analýzy prokázán signifikantní vliv na přežívání. Výsledky: V sestavě 155 resekcí pankreatu s kurativním záměrem bylo identifikováno 73 nemocných operovaných pro vývodový karcinom pankreatu, u nichž bylo možné hodnotit 5leté přežití. Více než 5 let přežilo 15 nemocných (20,5 %), z nichž 13 dosud žije. Průměrná doba přežití ve skupině přeživších 5 let je 77,1 měsíce (60–110) a medián 74 měsíce, průměrná doba přežití bez nemoci (relaps free survival) je 63,3 měsíce (14−110), medián 62 měsíce. Ze sledovaných faktorů statisticky významný vliv na přežití měly: nepřítomnost infiltrace lymfatických uzlin (p=0,031), nekomplikovaný pooperační průběh (p=0,025), nepřítomnost vaskulární invaze v tumoru (p=0,017), léčba bez nutnosti podání transfuze krve (p=0,015) a pooperační onkologické léčba – převážně chemoterapie (p=0,009). Významnými nezávislými prediktory přežívání jsou: přítomnost angioinvaze HR=2,239 (95%CI: 1,093−4,590; p=0,028), pooperační chemoterapie HR=2,587 (95%CI: 1,301−5,145; p=0,007) a nutnost podání transfuze HR=2,080 (95%CI: 1,027−4,212; p=0,042). Přítomnost angioinvaze zvyšuje riziko úmrtí pacienta 2,2x, podání transfuze zvyšuje riziko úmrtí 2,1x a nepodání chemoterapie zvyšuje riziko úmrtí 2,6x. Závěr: Vliv na dlouhodobé přežití u nemocných operovaných pro karcinom hlavy pankreatu byl prokázán u těchto faktorů: nepřítomnost infiltrace lymfatických uzlin, nekomplikovaný pooperační průběh, nepřítomnost vaskulární invaze v tumoru, léčba bez nutnosti podání transfuze krve a pooperační onkologická léčba – chemoterapie. Významnými nezávislými prediktory přežívání jsou: vaskulární invaze, chemoterapie a nutnost podání transfuze., Introduction: The purpose was to identify 5-year survivors among a group of radically resected patients with pancreatic cancer and analyse the characteristics and factors associated with their 5-year survival. Single tertiary centre experience. Method: A prospectively maintained database of 155 pancreatic resections from January 2006 to June 2010 was scanned to identify patients after curative radical resections for pancreatic ductal adenocarcinoma. The clinical and pathological data was analysed retrospectively. The outcomes of the PDAC group were evaluated using Kaplan-Meier analysis (survival) with the Log-rank test and Cox regression analysis (evaluation of prognostic factors). Characteristics of the survivors were discussed. Significance level of 0.05 was used. Those factors were used as independent variables for Cox regression analysis whose significant effect on survival was shown based on Kaplan-Meier analysis. Results: Among 155 patients undergoing a curative pancreatic resection, 73 had a pancreatic ductal adenocarcinoma. Fifteen patients (20.5%) after radical surgery survived over 5 years, 13 of whom are still alive. In the group of the survivors, the mean overall survival was 77.1 months (60−110) and the median survival was 74 months. The mean relapse-free interval in the group of the survivors was 63.3 months (14−110) with the median of 65 months. Factors associated with a longer survival included the absence of lymph node infiltration (p=0.031), uncomplicated postoperative course (p=0.025), absence of vascular invasion (p=0.017), no blood transfusions (p=0.015) and the use of postoperative therapy – predominantly chemotherapy (p=0.009). Significant independent predictors of survival included vascular invasion HR=2.239 (95%CI: 1.093−4.590; p=0.028), postoperative chemotherapy HR=2.587 (95%CI: 1.301−5.145; p=0.007) and blood transfusion HR=2.080 (95%CI: 1.027−4.212; p=0.042). The risk of death was increased 2.2 times in patients with vascular invasion, 2.1 times in patients with transfusions, and finally 2.6 times in those with no chemotherapy. Conclusion: Factors associated with an improved overall survival included: the absence of lymph node infiltration, an uncomplicated postoperative course, absence of vascular invasion, no need of blood transfusions, and finally the use of postoperative chemotherapy. Periprosthetic fractures are the third most common reason for revision total hip arthroplasty. Surgical treatment of periprosthetic fractures belongs to the most difficult procedures due to the extensive surgery, elderly polymorbid patients and the high frequency of other complications. The aim of this study was to evaluate the results of operatively treated periprosthetic femoral fractures after total hip arthroplasty. We evaluated 47 periprosthetic fractures in 40 patients (18 men and 22 women) operated on between January 2004 and December 2010. The mean follow-up period was 27 months (within a range of 12-45 months). For the clinical evaluation, we used modified Merle d'Aubigné scoring system. In group of Vancouver A fractures, 3 patients were treated with a mean score of 15.7 points (good result). We recorded a mean score of 14.2 points (fair result) in 6 patients with Vancouver B1 fractures, 12.4 points (fair result) in 24 patients with Vancouver B2 fractures and 12.7 points (fair result) in 7 patients with Vancouver B3 fractures. In group of Vancouver C fractures, we found a mean score of 16.2 points (good result) in 7 patients. Therapeutic algorithm based on the Vancouver classification system is, in our opinion, satisfactory. Accurate differentiation of B1 and B2 type of fractures is essential. Preoperative radiographic images may not be reliable. If in doubt, checking the stability of the prosthesis fixation during surgery should be performed. and M. Korbel, P. Sponer, T. Kucera, E. Účel studie: Retrospektivně jsme zpracovali výsledky léčby mozkových nádorů stereotaktickou radioterapií a radiochirurgií. Použité metody: Pacienti podstoupili plánovací CT a MR vyšetření. Poté bylo provedeno plánování cílového objemu a ozařovací techniky v BrainLab systému a pacienti byli ozářeni na X-noži Varian s vícelamelovým kolimátorem BrainLab. Radiochirurgie byla aplikována jednorázově s fixací ve stereotaktickém rámu. Pacienti, kteří podstoupili frakcionovanou stereotaktickou radioterapii, byli fixováni pomocí speciální stereotaktické masky. Statistické zpracování bylo provedeno ve spolupráci s Ústavem matematiky a statistiky PřF MU Brno. Výsledky: Celkem bylo ozářeno 101 pacientů s primárními mozkovými tumory. Nejčastěji se jednalo o meningeomy a high-grade gliomy. Medián sledování byl 22,4 měsíců. Pacienti, kteří podstoupili radiochirurgii, měli medián dávky 18 Gy. Pacienti, ozařovaní frakcionovaně, měli medián dávky 25 Gy. Toxicita léčby byla nízká. Stabilizace onemocnění byla zaznamenána u 68 % pacientů, parciální remisi dosáhlo 10 % a jeden pacient měl kompletní remisi. Závěr: Výsledky naší léčby jsou srovnatelné s publikovanými studiemi. Stereotaktické ozařovací metody mají své důležité místo v léčbě primárních tumorů mozku., We analysed retrospectively our treatment of primary brain tumors using stereotactic radiotherapy and radiosurgery. Methods: Patients underwent CT and MR examinations. Target volume and technique were planned in BrainLab system. Patients were irradiated on X knife. Radiosurgery was apllied with fixation using stereotactic frame. Stereotactic mask was used in cases of stereotactic radiotherapy. Statistical processing was performed at Department of mathematics and statistics, Faculty of Science, Masaryk university Brno. Results: 101 paients were included in our study. In the most of cases there were diagnosis of meningeoma and high-grade glioma. Median follow up was 22,4 months. Median of dose of radiosurgery was 18 Gy. Patients, whose underwent stereotactic radiotherapy, were median of doses 25 Gy. Treatment toxicity was low. Stable disease was recorded in 68 % of cases, partial remission achieved in 10 % patients and one patient had the complete remission. Conclusion: Results of our study are comparable with results other publicated studies. The pilot study deals with the clinical results of the regenerative therapeutical method in intraosseous periodontal defects done by surgical augmentation procedure using a material of a new generation prepared on the basis of a bioactive glass. A group of 10 individuals were treated properly for chronic periodontitis. Some of treated infraalveolar periodontal lesions, both periodontal pockets and interradicular defects, persisted or recurred in posterior teeth. In order to eliminate them they were indicated for advanced periodontal surgery or regenerative surgical therapy stimulating healing processes of supportive tissues, mostly alveolar bone around treated teeth. Relevant clinical parameters, i. e. values of the pocket depth, gingival recession, and loss of attachment were evaluated before and after the periodontal surgery and compared with each other. Cíl: Cílem práce bylo zjistit vývoj renálních parametrů u neselektované populace nemocných s akutní dekompenzací pokročilého chronického srdečního selhání, kteří byli během hospitalizace léčeni levosimendanem. Soubor nemocných a metodika: Vývoj parametrů renálních funkcí byl hodnocen u souboru 91 pacientů, průměrná EF LK byla 23 %, průměrná koncentrace urey 12,12 mmol/l, koncentrace kreatininu byla 147,1 mmol/l a odhadovaná glomerulární filtrace (eGFR) 42,6 ml/min/1,73m2. Krevní odběry se stanovením koncentrace urey, kreatininu a odhad glomerulární filtrace podle MDRD byly provedeny před podáním infuze a před propuštěním/úmrtím pacienta. Výsledky: Interval mezi prvním a druhým hodnocením renálních parametrů byl průměrně 11 dnů (? 7,8). Mezi prvním a druhým hodnocením došlo k poklesu koncentrace urey z 12,12 na 11,37 mmol/l (p = 0,27), kreatininu z 147,1 na 142,5 (p = 0,33) a vzestupu eGFR z 42,6 na 45,8 ml/min/1,73m2 (p = 0,067). Zastoupení počtu případů, kdy došlo ke snížení koncentrace urey a kreatininu, nebylo významné, statisticky významný byl rozdíl v počtu případů zvýšení než snížení eGFR (68 % vs 32 %; p < 0,001). Závěr: Přes nevýznamný pokles koncentrace urey a kreatininu došlo u neselektované populace pacientů s pokročilým srdečním selháním léčených levosimendanem u většího počtu nemocných ke zvýšení než ke snížení eGFR., The aim of the study was to evaluate the change of renal function parameters during hospital stay in unselected acute decompensated severe HF population treated with levosimendan. Patient population and methods: The change of renal parameters was assessed in 91 consecutive patients, the mean LV EF was 23%, mean urea level was 12.12 mmol/l, mean creatinine level was 147.1 mmol/l and estimated glomerular filtration rate (eGFR) was 42.6 ml/min/1.73m2. Results: Mean interval between the first (V1) and the second (V2) biochemical evaluation was 11 days (? 7.8). Mean urea level decreased (from 12.12 to 11.37 mmol/l, p = 0.27), as well as mean creatinine level decreased (from 147.1 to 142.5, p = 0.33), and mean eGFR increased (from 42.6 to 45.8 ml/min/1.73m2, p = 0.067) in the interval between V1 and V2. Urea and creatinine levels decreased in majority of the cases (not significant) and eGFR level significantly increased in majority of the cases (68% vs 32%, p < 0.001). Úvod: Kompletní resekce je obecně uznávaný standard léčby GII ependymomů IV. komory u dospělých, zatímco role adjuvantní radioterapie zůstává nejistá. Účelem předkládané studie je zhodnotit lokální kontrolu onemocnění a klinický stav pacientů s tímto onemocněním po chirurgické terapii. Materiál a metody: Retrospektivně jsme zhodnotili 15 pacientů (sedm mužů a osm žen) s histologicky verifikovaným GII ependymomem IV. komory v časovém období leden 2001–prosinec 2011. Výsledky: Kompletní resekce tumoru byla dosažena u 12 pacientů, u žádného z nich nebyla indikována adjuvantní radioterapie. Medián dalšího sledování byl 51 měsíců (min.–max.; 11–123 měsíců). Ke grafické recidivě došlo u jednoho pacienta, ten byl reoperován s časovým odstupem 51 měsíců od prvního zákroku. Nový neurologický deficit po operaci byl zaznamenán ve dvou případech. U dvou ze tří pacientů po subtotální resekci došlo ke grafické progresi rezidua po 11 a 16 měsících od první operace, jeden z nich byl reoperován s odstupem 27 měsíců. Adjuvantní radioterapie byla indikována u obou pacientů po reoperaci. Závěr: Dle našich dat je kompletní resekce efektivní a bezpečná léčba GII ependymomů IV. komory u dospělých. Následná radioterapie by měla být rezervována pro případy subtotální resekce s následnou progresí rezidua tumoru a reoperace., Introduction: Surgical resection is a generally accepted standard treatment modality in GII posterior fossa ependymomas in adults, while the role of adjuvant radiotherapy remains unclear. The purpose of this study was to evaluate local control of tumor growth and clinical outcome of patients after surgical treatment of GII fourth ventricle ependymomas. Material and methods: We conducted a retrospective study of 15 adult patients (seven males and eight females) with histologically confirmed GII fourth ventricular ependymoma between January 2001 and December 2011. Results: Gross total resection was achieved in 12 patients, none of them received adjuvant treatment after the initial surgery. Median follow-up after GTR was 51 months (min. – max.; 11–123 months). One patient showed radiographic signs of tumor recurrence and was re-operated 51 months after the initial surgery. Remaining patients were without any signs of tumor recurrence. After the surgery, a new neurological deficit occurred in two patients. Two of the three patients after subtotal tumor resection showed residual mass progression after 11 and 16 months postoperatively, one of them was re-operated 27 months after the initial surgery. Adjuvant radiotherapy was indicated in both re-operated patients. Conclusion: According to our data, gross total resection is effective and safe treatment of GII fourth ventricle ependymomas. Adjuvant radiotherapy should be reserved for the cases where complete removal of tumor mass is not possible and residual tumor is growing, or after repeated resection.
