Střelná poranění mozku jsou velmi závažná poranění a přežití závisí na včasné diagnostice a intervenci neurochirurga. Důležitým aspektem je i volba zbraně. V této kazuistice je uváděn případ pacienta hospitalizovaného na Neurochirurgické klinice ve FN Brno. Použitá zbraň je mezi replikami historických zbraní velmi raritní a statistiky poranění dlouhou předovkou neuvádějí., Missile brain injuries are very serious injuries, survival of which depends on early diagnosis and neurosurgical intervention. The choice of a weapon is an important aspect. In this case report, we describe a case treated at the Department of Neurosurgery, University Hospital Brno. The weapon causing this injury was a quite rare replica of a historical weapon and injury statistics do not report injuries caused by a longrifle. Behçet's disease is a multisystem chronic inflammatory disease generally presenting as recurrent oral-genital ulcers and uveitis. Vascular involvement is a common presentation. However, cardiac involvement is rare in this disorder. In this case report, we present a large right ventricular thrombus detected both in computed tomography and echocardiography in a 32 year-old male patient with complaints of fever, haemoptysis and weight loss. V kazuistice je popsán průběh onemocnění 55letého nemocného s neuroendokrinním tumorem a postižením jater, pankreatu, mezenteria, tenkého střeva a mediastina. Léčba spočívala v podávání analog somatostatinu a při progresi onemocnění byla přidána léčba sunitinibem., Case history of 55 years old man with neuroendocrine tumor of liver, pankreas, mesentery, small intestine and mediastinum is decribed. Úvod: Synchronní nádory pankreatu a jiné primární nádory nejsou časté. V autoptických materiálech se tato koincidence uvádí kolem 5,6 %. Současné resekce pankreatu a jiného orgánu pro synchronní malignity jsou publikovány pouze ve zcela ojedinělých souborech případů či jednotlivých kazuistikách. Autoři předkládají tři případy řešené chirurgicky v jedné době. Kazuistiky: V sestavě 400 resekcí pankreatu (2006−2014) byli tři nemocní s karcinomem pankreatu a jinou synchronní maligní duplicitou. Dva nemocní se prezentovali symptomatologií nádoru pankreatu – bolesti břicha, úbytek na váze, obstrukce žlučových cest. Druhými nádory byly asymptomatický tumor antra žaludku a tumor ledviny. Pacienti podstoupili pravostrannou pankreatoduodenektomii (PDE) a subtotální gastrektomii s lymfadenektomií, resp. levostrannou nefrektomii. Histopatologické nálezy byly vývodový adenokarcinom hlavy pankreatu pT2N1M0, G3 a adenokarcinom žaludku pT1N1M0, G2, resp. vývodový adenokarcinom hlavy pankreatu pT3N1M0, G2 a renální papilární karcinom pT1bN0M0, G3. Pooperačně u obou následovala chemoterapie gemcitabinem. První nemocný zemřel 12 měsíců od operace s generalizací do jater. Druhý nemocný zemřel 19 měsíců od operace, s nově zjištěným karcinomem prostaty. Třetí nemocný byl vyšetřován pro bolesti epigastria a při gastroskopii byl nalezen tumor antra žaludku, při CT pak asymptomatický malý tumor hlavy pankreatu. Podstoupil pravostrannou PDE a subtotální gastrektomii s lymfadenektomií. Histopatologické nálezy byly: vývodový karcinom hlavy pankreatu pT2N0M0, G1 a adenokarcinom žaludku pT2bN2M0, G3. Následovaly radioterapie a chemoterapie 5-fluorouracilem a leukovorinem. Žije téměř 5 let od operace, bez recidivy. Závěr: Přežití nemocných operovaných pro duplicitní synchronní malignitu pankreatu a jiného primárního nádoru v naší sestavě významně ovlivnily vlastnosti pankreatického tumoru. Pokud se duplicita projevila symptomy pankreatického tumoru, bylo přežití významně kratší nežli u incidentálně zjištěného tumoru pankreatu při manifestaci jiné malignity., Introduction: The occurence of synchronous pancreatic cancer and other primary cancer is not frequent and reaches about 5.6% as reported in autoptic studies. Double resections of the pancreas with another organ due to synchronous malignancies have been published only in quite sporadic sets of cases or individual case reports. The authors present three cases of synchronous pancreatic malignancies and stomach or renal cancers treated with surgery, including results and survival. Cases: Three patients with synchronous double cancer were identified in a series of 400 pancreatic resections (2006−2014). Two patients presented with symptoms of pancreatic periampullary tumors (bile duct obstruction , weight loss and abdominal pain). The second malignancies were identified as incidental during diagnostic work-up (asymptomatic cancer of the stomach, kidney). Pancreatoduodenectomies (PDE) with lymphadenectomies were performed due to ductal adenocarcinomas (pT2N1M0 G3 and pT3N1M0 G2). The second procedures included subtotal gastrectomy with lymphadenectomy (gastric adenocarcinoma pT1N1M0, G2) and nephrectomy (renal papillary carcinoma pT1bN0M0, G3). Postoperative adjuvant chemotherapy with gemcitabine was given in both patients. Survival rates were 12 and 19 months, respectively. The third patient suffered from abdominal pain and weight loss. Diagnostic work-up revealed stomach carcinoma and early pancreatic adenocarcinoma. Double resection – subtotal gastrectomy with lymphadenectomy and pancreatoduodenectomy with lymphadenectomy – was performed. Gastric adenocarcinoma pT2N2M0, G3 and pancreatic ductal papillary-mucinous adenocarcinoma pT2N0M0, G1 were found in the specimens. Adjuvant radiochemotherapy with 5-fluorouracil and leukovorine was given postoperatively. This patient is still alive nearly 5 years after the surgery, without any reccurence. Conclusion: The survival of patients with double synchronous pancreatic malignancies and other primary tumors in our set seems to be influenced by the stage and biology of pancreatic cancer. The survival was worse when the duplicity was presented with symptoms of pancreatic cancer. Toxická epidermální nekrolýza je vzácné, akutní, život ohrožující onemocnění projevující se rozsáhlým odlučováním epidermis a slizničních povrchů. Jde o závažnou nežádoucí reakci na podávané léky, nejčastěji antibiotika, antikonvulziva či nesteroidní antirevmatika. Jako první ji popsal skotský dermatolog Alan Lyell v roce 1956, proto je též nazývána Lyellův syndrom. Mortalita tohoto onemocnění je vysoká, proto je klíčová včasná diagnostika a neodkladné zahájení adekvátní péče o pacienta. Jedná se o vzácné onemocnění s velmi malou incidencí a závažnou prognózou, což značně ztěžuje provedení velkých randomizovaných klinických studií. Je tedy obtížné hodnotit efektivitu jednotlivých terapeutických možností. Jako nejperspektivnější se prozatím jeví nitrožilní podávání imunoglobulinů se zřejmým zlepšením klinického stavu pacienta, dobrou tolerancí a minimálními vedlejšími nežádoucími účinky. Nové experimentální postupy se snaží hledat cestu terapie cíleným ovlivněním granulyzinu jakožto hlavního cytotoxického mediátoru. It is a serious adverse response to administered drugs, mostly antibiotics, anticonvulsants or NSAIDs. First described by Scottish dermatologist Alan Lyell in 1956, it is also known as Lyell’s syndrome. Mortality of the disease is high, therefore its early diagnosis is crucial and immediate initiation of appropriate patient care necessary. It is a rare disease with a very low incidence and serious prognosis, which is a considerable hindrance to undertaking large randomized clinical studies. It is therefore difficult to evaluate the effectiveness of various therapeutic options. As the most promising so far appears the administration of intravenous immunoglobulins, apparently reaching improvement in the clinical condition of the patient, with a good tolerances and minimal side adverse effects. New experimental techniques endeavour to seek a way to the therapy through targeted influencing of granulysin as the major cytotoxic mediator. BACKGROUND: Intussusception of the large bowel in adults is a very rare pathological condition. However, it has its clinical importance because intussusception is very often associated with an intraluminal lesion. CASE REPORT: We report two cases of the large bowel intussusception, ileocolic and colorectal. Both intussusceptions were associated with a malignant tumor. However, the clinical presentation was different. One of the intussusceptions was of non-transient character, while the second one resolved spontaneously before operation. Both patients underwent surgery and malignant tumors were found and removed. RESULTS: The purpose of the article is to draw attention to intussusception and emphasize that intussusception, either transient or non-transient, should be further examined. Cíl práce: V kazuistice prezentujeme raritní nález primárního plazmablastického lymfomu penisu, který klinicky imitoval karcinom glans penis. Jedná se o druhý publikovaný případ tohoto typu lymfomu penisu ve světě. Kazuistika: Pacient, narozen 1929, byl odeslán na naše pracoviště ošetřujícím urologem pro nově zjištěný exofytický tumor glans penis, indikovali jsme parciální amputaci penisu. Histologické vyšetření amputátu prokázalo primární plazmablastický lymfom. Následná restagingová vyšetření neprokázala systémové onemocnění, pacient je nyní 22 měsíců po výkonu bez známek onemocnění a zůstává v hematologické dispenzární péči. Závěr: Primární plazmablastický lymfom penisu představuje extrémně raritní diagnózu, ve světové literatuře je popsán pouze jediný případ. Jedná se o agresivní typ lymfomu, který obvykle vede v krátké době ke smrti pacienta. Ačkoli léčba těchto nádorů je převážně založena na chemoterapii, u našeho pacienta vedl radikální operační výkon k úplnému vyléčení., We report a case of an elderly man who has presented with tumorous affection involving the uncircumcised penile glans and prepuce. Primary clinical examination gave strong suspicion for primary carcinoma of penis, therefore a suprapubic cystostomy and partial amputation of penis were performed. Histopatological examination revealed uncommon diagnosis of plasmablastic lymphoma involving glans penis and prepuce. The vast majority of penile tumours are squamous cell carcinomas, primary malignant lymphoma of penis is extremely rare. As staging results were negative, no adjuvant systemic chemotherapy was neccesary. Sometimes intraorbital foreign bodies lead to unexpected findings. A 16-year old boy was referred due to ocular surface trauma. A conjunctival laceration was detected at the level of the left caruncle with associated left exotropia, reduced adduction as well as a preretinal hemorrhage along the nasal periphery of the fundus. A blow-out fracture of the medial orbital wall was suspected and a CT scan of the orbits was scheduled which revealed the presence of a large intraorbital foreign body. The removal of the intraorbital foreign body (which proved to be a bullet) and precautionary laser photocoagulation along the nasal periphery of the left eye were performed. Variant branching pattern of the cords of brachial plexus coupled with erroneous communications has been an area of concern for surgeons opting to explore this region. Anaesthetic blocks and surgical approaches are the highlights of these interventions, where a keen familiarization of the anatomy of this region is mandatory. The present case description reports a unilateral variant branching pattern of the posterior cord coexistent with a neural communication between lateral and medial cords in an adult male cadaver. This intercordal neural communication between lateral and medial cords was oriented obliquely and measured 2.2 cm in length. Furthermore, the posterior cord revealed a variant branching pattern. It branched into three upper subscapular nerves and a common trunk for the thoracodorsal and lower subscapular nerves. The lowest of the three upper subscapular nerves gave a communicating twig to the thoracodorsal nerve.
