Cíl: Cílem je zhodnotit zkušenosti s léčbou aneurysmatu podkolenní tepny stentgraftem. Metodika: V období od června 2011 do října 2014 jsme na našem pracovišti léčili 18 aneurysmat podkolenní tepny u 14 pacientů pomocí stentgraftu. Jednalo se o 13 mužů a jednu ženu ve věku od 50 do 90 let. Věkový průměr byl 70 let, medián 69,5 let. Léčili jsme tři aneurysmata o průměru menším než 2 cm, 12 aneurysmat o průměru 2-5 cm a tři výdutě o průměru větším než 5 cm. Výsledky: Pacienti byli sledováni 2-35 měsíců, průměrná doba sledování byla 13, 4 měsíce, medián 10 měsíců. Technická úspěšnost byla 100 %. Primární průchodnost byla 78 %, sekundární průchodnost byla 94 %. Podle Kaplanovy-Maierovy analýzy byly pravděpodobnost primární průchodnosti za 6 a 12 měsíců po implantaci stentgraftu stejná, a to 75,8 % (95 % CI: 54,6-97,0 %), pravděpodobnost sekundární průchodnosti za 6 a 12 měsíců 94,4 % (95 % CI: 83, 8-100 %). Závěr: Endovaskulární léčba stentgraftem je možnou alternativou chirurgické léčby aneurysmatu podkolenní tepny. Endovaskulární léčba je jednoduchá, bezpečná a účinná., Aim: To evaluate the experience in the treatment of popliteal artery aneurysm with stentgraft. Method: From June 2011 to October 2014, we were treated 18 aneurysms of the popliteal artery in 14 patients using the stentgraft at our department. There were 13 men and one woman aged from 50 to 90 years. The mean age was 70 years. We treated three aneurysms with a diameter of less than 2 cm, 12 aneurysms 2-5 cm in diameter and 3 aneurysms with a diameter greater than 5 cm. Cíl: Stále více literárních údajů přináší přesvědčivé informace o tom, že EVAR může být indikována i u nemocných s rupturou aneurysmatu abdominální aorty (RAAA). Výhodou EVAR při léčbě RAAA je, že umožňuje vyhnout se laparotomii, a předchází tak výraznému snížení krevního tlaku vedoucímu až k cirkulačnímu kolapsu při uvolnění retroperitonea, snižuje kardiorespirační zátěž a minimalizuje ztráty krve. Cílem práce je zhodnotit vlastní zkušenosti s akutní endovaskulární léčbou rupturovaných aneurysmat abdominální aorty či aneurysmat společné kyčelní tepny. Metodika: Od roku 2011 do konce roku 2014 jsme léčili celkem deset nemocných s RAAA nebo rupturou aneurysmatu arteria iliaca communis (RAAIC). Průměrný věk nemocných zahrnutých do souboru byl 69,3 let v rozmezí od 61 do 79 let (medián věku nemocných - 68 let). Sedm nemocných bylo léčeno pro RAAA, tři nemocní pro RAAIC. Výsledky: Celkem u osmi nemocných jsme použili bifurkační stentgraft - u všech sedmi nemocných s RAAA a jednoho nemocného s RAAIC U jednoho ze sedmi nemocných s RAAA byla pro délku proximálního krčku 5 mm použita chimney technika. U dvou nemocných bylo možné řešit RAAIC pouze pomocí tubulárního stentgraftu. Primární technická úspěšnost dosáhla v našem souboru 100%, perioperační letalita 20 %. Průměrná doba dalšího sledování nemocných v době zpracování souboru byla 13,5 měsíců, v rozmezí od 10 do 38 měsíců. Déle než 1 rok bylo sledováno sedm nemocných. Z pozdních komplikací jsme u jednoho nemocného zaznamenali endoleak typu II bez zvětšování vaku aneurysmatu. Jedenkrát jsme zaznamenali uzávěr raménka bifurkačního stentgraftu. Závěr: Na základě našich zkušeností a literárních výsledků jsou na našem pracovišti v současné době hemodynamicky stabilní pacienti s RAAA a RAAIC a vhodnou morfologií indikováni k endovaskulární léčbě., Aim: In comparison with open surgery the endovascular treatment of ruptured abdominal aortic aneurysm can avoid laparotomy with subsequent significant blood pressure drop leading to the circulation collapse while releasing of the retroperitoneum, can reduce cardiopulmonary complications and minimize blood loss. The aim of the study is to evaluate our own experience with acute endovascular treatment of ruptured abdominal aortic aneurysm and common iliac artery aneurysm. Methods: From the year 2011 to December 2014 10 patients with average age of 69.3 years (range 61-79 years, median - 68 years) were treated for ruptured abdominal aortic aneurysm and common iliac artery aneurysm (RCIAA). Seven patients were treated for ruptured abdominal aortic aneurysm and three patients for ruptured common iliac artery aneurysm. Úvod: Je uváděno, že prevalence neurodegenerativních onemocnění v malých evropských komunitách izolovaných od okolí je vyšší ve srovnání s běžnou populací. Tentýž jev jsme pozorovali v malém, relativně izolovaném regionu jihovýchodní Moravy. Cíl: Zjištění prevalence neurodegenerativního parkinsonizmu v izolovaném regionu jihovýchodní Moravy. Metodika: Byla použita metoda třífázového vyšetření. V první fázi byly do ordinací praktických lékařů distribuovány dotazníky, které byly vyplněny všemi pacienty, kteří se z jakéhokoliv důvodu dostavili v průběhu tří měsíců do ordinace praktického lékaře. Ve druhé fázi byly u pozitivních respondentů příznaky parkinsonizmu objektivizovány vyšetřením edukovaným ambulantním neurologem. Ve třetím stupni byli respondenti, u kterých byly při skríningovém vyšetření příznaky parkinsonizmu objektivizovány, přijati k detailnímu vyšetření za hospitalizace v terciárním centru pro diagnostiku a léčbu neurodegenerativních onemocnění. Výsledky: Celková prevalence parkinsonizmu v populaci starší 50 let byla 2,8 % (95% CI: 2,2-3,4); prevalence v populaci 50-64 let byla 1,9 % (95% CI: 1,2-2,5); a v populaci nad 65 let byla 4,06 % (95% CI: 2,9-5,1). Byly získány tři velké rodokmeny s patrnou autozomálně dominantní dědičností parkinsonizmu. Závěr: Zjištěná prevalence byla překvapivě vysoká a podstatně odlišná od hodnot prevalence publikovaných v jiných evropských zemích. Vzhledem k charakteristice regionu je předpokládán vliv genetických faktorů. Současný výzkum je zaměřen na genetické pozadí a DNA analýzu probandů z rodokmenů, ve kterých byla identifikována autozomálně dominantní dědičnost parkinsonizmu. We recently observed this phenomenon in a small specific region of south-eastern Moravia. Objective: To assess the prevalence of neurodegenerative parkinsonism in an isolated region with a rural population in south-eastern Moravia. Methods: A three-stage method of data collection was used. In the first phase, originally designed questionnaires were distributed to general practitioners and completed by all patients who visited them for any reason during a three-month period. In the second phase, positive responders were examined by trained primary care neurologists. Finally, the diagnosis was confirmed or excluded by a movement disorders specialist. Results: The overall prevalence in the population older than 50 years of age was 2.8% (95% CI: 2.2-3.4); the prevalence in the population from 50 to 64 years was 1.9% (95% CI: 1.2-2.5), and it was 4.06% (95% CI: 2.9-5.1) in the population over 65 years of age. Three large families with an autosomal-dominant inheritance patterns of parkinsonism were identified. Conclusions: The prevalence rates were surprisingly high; they substantially differed from the published prevalence rates in other European countries. Due to the characteristics of the region, we expected a particular impact of genetic factors, most probably the autosomal-dominant inheritance of parkinsonism. V současné době je věnována familiární hypercholesterolemii (FH) po letech opět zvýšená pozornost. Je tomu tak především proto, že se toto geneticky podmíněné onemocnění stává cílem velmi účinných nových hypolipidemik, PCSK-9 inhibitorů, lomitapidu a mipomersenu. Předložené sdělení se zabývá popisem 2 souborů nemocných (vždy ve své době z největších na světě), před 50 lety a nyní. I když přímé statistické srovnání není možné, lze přeci jen najít v klinickém obrazu FH jisté vývojové změny. Faktem však je, že základní charakteristika FH se zásadně nezměnila. V biochemickém obrazu dominuje těžká izolovaná hypercholesterolemie s hodnotami celkového cholesterolu 9–10 mmol/l, s hodnotami LDL-cholesterolu 7–8 mmol/l a s normálními hodnotami triglyceridů. Je zajímavé, že právě triglyceridy se při srovnání s dobou před 50 lety významně zvyšují a dosahují hraničních hodnot. Hladiny HDL-cholesterolu jsou normální. Manifestace ICHS ve věku od 40 let u mužů a od 50 let u žen není výjimkou (ojediněle se vyskytují i případy infarktu myokardu již ve 3. dekádě života). Klasickou klinickou manifestací FH je xantomatóza. Pravděpodobně v důsledku časné detekce nemocných s FH a účinné agresivní léčby se se šlachovými xantomy, s xanthelasma palpebrarum a arcus lipoides setkáváme méně často než před desítkami let. Obezita, diabetes mellitus (DM) ani hypertenze nepatří mezi typické klinické příznaky FH., Currently, the familial hypercholesterolemia (FH) rises the interest. The reason is that this genetic disorder is targeted by newly emerged and highly effective hypolipidemic agents, PCSK-9 inhibitors, lomitapid and mipomersen. Present paper discusses 2 patient study groups, before 50 years and nowadays. Although direct statistical analysis is impossible some changes in clinical features of FH might be found over the course of the time. In fact, the basic FH characteristic has not changed dramatically. Severe isolated hypercholesterolemia with total cholesterol 9–10 mmol/l, LDL-cholesterol 7–8 mmol/l and normal values of triglycerides dominates in laboratory analysis. Interestingly, the values of triglycerides increase and almost reach the pathological range in comparison to the values from the period 50 years ago. The values of HDL-cholesterol are normal. Manifestation of CHD in male patients over 40 years of age and in female patients over 50 years of age is not exceptional (rarely occur cases of myocardial infarction in third decade of age). Typical clinical manifestation of FH is xanthomatosis. The early detection and aggressive treatment in FH patients cause that xanthoma tendinosum, xanthelesma and arcus lipoides are less frequent as decades ago. Úvod: Terapie ran kontrolovaným podtlakem (NPWT) je efektivní metodou lokální terapie širokého spektra nehojících se ran. V současnosti je využívána především u hospitalizovaných pacientů, pro systematické použití v ambulantním režimu nejsou v České republice dostupná klinická data. Metody: V prospektivní nerandomizované studii autor porovnává klinickou a nákladovou efektivitu léčby ulcerací na dolní končetině u diabetiků kontrolovaným podtlakem (NPWT) ambulantně versus za hospitalizace. Cílem sledování je stanovit účinnost NPWT v ambulantním režimu na základě hodnocení vývoje spodiny rány, kvantifikovat předpokládanou redukci celkových nákladů na terapii a porovnat efektivitu obou režimů navzájem. Do sledování bylo zařazeno 36 hospitalizovaných (NPWT systémy RENASYS GO nebo PICO) a 28 ambulantních pacientů (PICO). NPWT byla ukončena v momentě, kdy byla spodina rány zcela pokryta granulační tkání nebo při vzniku vážných komplikací léčby. Hodnotili jsme velikost ulcerace, charakter její spodiny, počet NPWT výměn, její celkovou délku, intenzitu bolesti, přítomnost komplikací a náklady na terapii. Výsledky: Sledování prokázalo porovnatelnou klinickou účinnost NPWT v ambulantním režimu (n=28) versus za hospitalizace (n=36, statisticky nesignifikantní rozdíly v hojení ulcerací verifikované na základě vývoje spodiny rány a redukce plochy ulcerací v čase), stejné riziko komplikací spojených s NPWT (statisticky nesignifikantní rozdíly ve funkčnosti obou technologií a ve výskytu komplikací vzniklých v průběhu sledování) i obdobné dlouhodobé výsledky (statisticky nesignifikantní rozdíly v počtu amputací). Celkové náklady na ambulantní terapii ran byly statisticky signifikantně nižší (16 700 Kč versus 36 000 Kč, p 0,001), stejně tak průměrné náklady na 1 den podtlakové terapie (817 Kč versus 3400 Kč, p 0,001). Závěr: Analýza klinických výsledků jednoznačně prokázala, že ambulantní podtlaková terapie ulcerací syndromu diabetické nohy (metoda PICO) má stejnou efektivitu jako podtlaková terapie za hospitalizace. V ambulantním provozu je NPWT levnější, převazy jsou vnímány méně bolestivě a NPWT je pacienty lépe snášena., Introduction: The negative pressure wound therapy (NPWT) is an effective local treatment method of many non-healing wounds. NPWT is routinely used for inpatient treatment in the Czech Republic; however, no clinical data is available for systematic outpatient treatment. Methods: The prospective non-randomized study was used to compare the clinical impact and cost-effectiveness of diabetic leg-ulcer and foot-ulcer negative pressure treatment in outpatient and inpatient settings. The aim of the study was to assess the effectiveness of outpatient NPWT on the basis of wound bed evaluation, to detect and quantify pre-expected reductions in the total costs of the therapy, and to compare the efficacy of both treatment options. Enrolled subjects included inpatients (n=36, NPWT systems RENASYS GO or PICO) and outpatients (n=28, PICO system). NPWT was concluded when the wound bed had been totally covered by granulation tissue, or when serious health problems occurred. We monitored the ulcer area, wound bed trait, number of NPWT dressing changes, length of NPWT, intensity of pain (VAS scale), presence of complications, and treatment costs. Results: We found comparable clinical efficacy of outpatient (n=28)/inpatient (n=36; statistically non-significant differences in healing of ulcers verified based on wound bed development and reduction of ulcer areas in time) NPWT, the same risk of complications associated with NPWT (statistically non-significant differences in the function of both technologies and in complications developed during follow-up) as well as similar long-term results (statistically non-significant differences in the count of amputations). The total costs for outpatient wound treatment were statistically significantly lower (EUR 600 versus EUR 1300, p=0.001), and so were the average one-day-NPWT costs (EUR 30 versus EUR 120, p=0.001). Conclusion: We demonstrated that outpatient diabetic-foot-ulcer NPWT provided the same clinical efficiency as inpatient NPWT, while outpatient NPWT was less expensive, less painful and better tolerated than inpatient NPWT., and J. INTRODUCTION: Autologous stem cell transplantation (ASCT) became standard of care for patients with multiple myeloma (MM) under the age of 65 years. We routinely perform ASCT for newly diagnosed MM since 1996 in our department. PATIENTS AND METHODS: We retrospectively analyzed all 285 transplants in 185 patients done for MM from January 1996 till December 2010. We analyzed overall survival (OS) and progression-free survival (PFS) regarding conditioning, stage, complete or very good partial remission (CR, VGPR) achievement, renal impairment, single vs. double transplant. RESULTS: Estimated 10-years survival of the whole set of patients is 39% (median survival 95 months). Patients with renal impairment show same OS as those without (p = 0.22). Patients show similar overall survival and event free survival regardless of type of transplant. We observed better outcome in terms of overall survival in patients treated with new drugs (p = 0.0014). Reaching CR or VGPR was not translated into better OS (p = 0.30) and EFS (p = 0.10). Also stage of the disease and whether single or double transplant was used did not make any significant difference in the outcome. CONCLUSION: Stem cell transplantation greatly improved outcome of patients with MM. Poor outcome of allogeneic transplantation in our group of patients is related to high transplant related mortality (20% vs. 0%) and unexpected high relapse rate. There is a trend towards better survival, when new drugs are incorporated at any time in the course of the disease. Úvod: Fyloidní tumor je velmi zřídka se vyskytující nádor prsu. Vyskytuje se pouze v 1 % případů všech nádorů prsu. Diagnostika fyloidního tumoru může být obtížná vzhledem k malému počtu případů. Léčba fyloidního tumoru je vždy chirurgická. Metody: V letech 2004−2013 jsme na Chirurgické klinice FN Plzeň operovali 12 pacientek s fyloidním nádorem prsu z celkového počtu 1564 operací pro nádory prsu (0,8 %). Hodnotili jsme věk, biologické chování nádoru v závislosti na velikosti nádoru a délce trvání, vzdálené metastázy, terapii a přežití. Výsledky: Průměrný věk v době operace byl 50 let (26−84), délka trvání nemoci do operačního řešení byla 1 měsíc až 10 let. Velikost tumoru se pohybovala v rozmezí 2−29 cm, tumory menší než 5 cm byly vždy benigní. 7x pro benigní fyloidní nádor byla provedena excize tumoru. 5x byl diagnostikován maligní fyloidní tumor a byla vždy provedena mastektomie, ve 3 případech s exenterací axily, kdy uzliny byly vždy benigní. V jednom případě po mastektomii následovala radioterapie pro dosahování nádoru do okraje resekátu, ostatní pacientky byly pouze dispenzarizovány. 2 pacientky měly generalizaci do plic diagnostikovanou 5 a 10 měsíců po operaci prsu. 1 pacientka s generalizací zemřela, ostatní žijí bez lokální recidivy onemocnění. Medián přežití je 52 měsíců, interval bez nemoci je 50 měsíců. Závěr: Z našeho souboru vyplývá, že pokud je fyloidní nádor diagnostikován včas, jedná se téměř výhradně o benigní variantu, při déle trvající anamnéze a s rostoucí velikostí tumoru nádory malignizují. Chirurgická léčba je dostačující i v případě maligních forem. Výkon na prsu není třeba doplňovat o exenteraci axily., Introduction: Phyllodes tumour is a breast tumour occurring very rarely. It accounts for only in 1% of all cases of breast tumour. The diagnosis of phyllodes tumours can be difficult in consideration of the small number of cases. Treatment of phyllodes tumours is always surgical. Methods: In 2004–2013, we operated on twelve female patients with phyllodes tumours out of the total number of 1564 surgeries for breast tumours (0.8%) at the Department of Surgery at Teaching Hospital in Pilsen. We evaluated the age, the biological behaviour of the tumour depending on the tumour size and duration, the distant metastases, therapy and survival. Results: The average age at the time of surgery was fifty years (26−84), the duration of disease to the surgical solution ranged from one month to ten years. Tumour size was in the range of two to twenty-nine centimetres, tumours measuring less than five centimetres were always benign. Tumour excision for benign phyllodes tumour was performed seven times. Malignant phyllodes tumour was diagnosed five times with mastectomy performed in each case, and the axilla was exenterated in three cases where nodes were benign in each of them. In one case, mastectomy was followed by radiotherapy because the tumour reached the edge of the resected part; the other patients were only monitored. In two patients, tumour spreading into the lungs was diagnosed at five to ten months after breast surgery. One patient with generalized disease died, the other ones live with no local recurrence of this disease. Median survival is fifty-two months; the disease-free interval is fifty months. Conclusion: The results show that if phyllodes tumour is diagnosed in time, it is almost exclusively benign. If the case history is longer and the tumour is growing, the likelihood of malignancy increases. Surgical treatment is also sufficient in the case of malignant forms. Cíl studie: Na základě soudobých studií podat přehled o hlavních biologických funkcích lipoproteinů o vysoké hustotě (HDL), zejména ve vztahu k reverznímu transportu cholesterolu a k jejich protizánětlivému působení. Popsat pravděpodobné mechanismy vzniku dysfunkčních prozánětlivých HDL a uvést některé parametry asociované s tímto procesem. Podat přehled o laboratorních metodách stanovení kvantitativních, strukturních a funkčních vlastností HDL, včetně moderních metod založených na hmotové spektrometrii. Typ studie: přehledový článek Závěr: V případě systémového zánětu a/nebo oxidačního stresu dochází ke vzniku dysfunkčních HDL akumulujících oxidanty. Probíhá strukturní modifikace apolipoproteinu AI, což vede mimo jiné k inhibici reverzního transportu cholesterolu a tvorbě prozánětlivých HDL. Na ztrátě příznivých vlastností HDL se podílí modifikace proteinové i lipidové složky HDL. Izolované měření HDL cholesterolu nemusí u některých syndromů bezvýhradně korelovat s mírou kardiovaskulárního rizika a v některých situacích je vhodné stanovit markery mapující strukturu a funkci HDL., Objective: On the basis of recent studies to make an overview concerning the crucial biological functions of high density lipoproteins (HDL), with emphasis on the role in reverse cholesterol transport and their antiinflammatory traits. The aim was to describe probable mechanisms of dysfunctional proinflammatory HDL formation with introduction some of associated parameters. To present laboratory methods for determination of quantitative, structural and functional qualities of HDL, including advanced mass spectrometry techniques. Study design: review Conclusion: In the case of systemic inflammation and/or oxidative stress, the formation of dysfunctional HDL accumulating oxidants takes place, including apo AI structural modifications. This process can lead to the inhibition of reverse cholesterol transport and proinflammatory HDL generation, among others. Loss of beneficial qualities results from modification of both lipid and protein components of HDL. BACKGROUND: In the cholesteatoma surgery ossicles can be replaced to reconstruct middle ear function. It is important that these ossicles are free of squamous epithelium, to prevent residual disease. This study focuses on the histological findings of the malleus and incus harvested during cholesteatoma surgery. MATERIALS AND METHODS: Eighty middle ears ossicles were examined in vivo and histologically to consider the relationship of cholesteatoma to ossicles, grade of bone destruction and invasion of cholesteatoma to deeper layers of bone. RESULTS: Serious ossicular destruction was observed more frequently in incus compared to malleus (p=0.0065). Difference of ossicles destruction between children and adults was not significant (p=0.3032). Deep invasion of cholesteatoma into the vascular spaces or inner core of the bone was not observed. CONCLUSIONS: Autograft ossicles from cholesteatomatous ears should not necessarily be rejected for reconstruction of the ossicular chain. Cíl:Cílem naší studie bylo kvantitativně vyhodnotit poruchy písma u pacientů s Parkinsonovou nemoci (PN) ve srovnání s věkově a pohlavím vázanými zdravými kontrolami (ZK) pomocí digitalizačního tabletu. Soubor a metoda: Prospektivně jsme zařadili 40 pacientů s PN (průměrný věk 68,6 ? 11,36 let, délka trvání nemoci 8,02 ? 4,79 let) a 40 věkem a pohlavím vázaných ZK (průměrný věk 62,55 ? 11,22 let). Všichni jedinci byli praváci bez přítomnosti deprese či demence. Každý subjekt podstoupil sedm cvičení pro vyšetření písma a kresbu Archimédovy spirály a elips s pomocí digitalizačního tabletu. Byly hodnoceny rychlostní parametry mikrografie a kresby při pohybu pera po tabletu i nad tabletem. Pro statistickou analýzu dat jsme použili Mann‑Whitneyho U test a Spearmanovy korelace s korekcí na opakovaná měření (Benjamini‑Hochbergova metoda). Výsledky: U PN ve srovnání se ZK jsme při psaní na tabletu zjistili statisticky významné snížení v parametrech: okamžitá rychlost, okamžité zrychlení, okamžitá změna zrychlení v čase. Změny se zvýrazňovaly s délkou psaného segmentu. Ještě významnější byly rozdíly mezi oběma skupinami při hodnocení pohybu pera nad tabletem, tj. před vlastním zahájením psaní, při přípravě na pohyb. Zaznamenali jsme pokles sledovaných hodnot až o 20 % ve srovnání se ZK. Závěr: U pacientů s PN jsme prokázali specifické změny nejen při vlastním psaní, ale i ve fázi přípravy na psaní, které lze kvantifikovat pomocí digitalizačního tabletu. Výsledky studie mohou mít přímý klinický dopad: umožní nám studovat mikrografii jako možný časný klinický marker rozvoje PN., Aim: The aim of this study was to assess micrographia in patients with Parkinson’s disease (PD) as compared to healthy controls (HC) using a digitizing tablet. Methods: We included 40 PD (mean 68.6 ? 11.36 years, duration of illness 8.02 ? 4.79 years) and 40 age- and sex-matched HC (mean 62.55 ? 11. 22 years). All subjects were right-handed, without the presence of depression or dementia. Each subject underwent seven exercises for writing and drawing of Archimedes spiral and ellipses using a digitizing tablet. The speed parameters of micrographia and drawing during the movement of a pen in the air and on the tablet were evaluated. The Mann-Whitney U test, Spearman correlation and Benjamini-Hochbergs method were used for statistical data analysis. Results: A statistically significant reduction in parameters of velocity, acceleration, and jerk was found when comparing both groups during writing. Changes were more pronounced with increased length of the written segment. The differences between the two groups were more pronounced when the in-air movements were assessed, i.e. during movement preparation. The values decreased up to 20% compared to HC. Conclusion: PD-specific changes assessed with a digitizing tablet were demonstrated not only during writing but also during preparation for writing.
