Úvod: Rozsah nádoru a kvalita jeho chirurgického odstranění patří k hlavním prognostickým faktorům v léčbě karcinomu rekta. Počet pooperačních lokálních recidiv závisí na úplnosti odstranění mezorekta – TME (total mesorectal excision) – a dosažení bezpečných resekčních okrajů bez nádoru. Provedení a kvalita operace záleží na předoperační diagnostice a detekci rizikových míst na rektu a mezorektu, na chirurgické schopnosti výkon správně provést a na patologickém vyšetření, hodnotícím úplnost odstranění nádoru v parametrech resekčních okrajů včetně cirkumferentního (CRO) a kompletnost mezorektální excize. Cílem práce bylo zavést a standardizovat metodu hodnocení kvality chirurgického výkonu − mezorektální excize – v léčbě karcinomu rekta hodnocením resekčních okrajů včetně CRO a kompletnosti excize. Metody: Studie měla dvě části. V první, retrospektivní části multicentrické studie byly v sestavě 288 pacientů analyzovány jednotlivé dílčí parametry diagnostiky, operací a histologického vyšetření karcinomu rekta, identifikována kritická místa a připraven jednotný protokol sledování. Závěr: Zavedení parametrického sledování do rutinní praxe zvýšilo kvalitu předléčebné a předoperační diagnostiky, vyšetření resekátu patologem a vlivem zpětnovazebné informace také kvalitu chirurgického výkonu., Introduction: Tumour size and the quality of its complete surgical removal are the main prognostic factors in rectal cancer treatment. The number of postoperative local recurrences depends on whether the mesorectum has been completely removed – total mesorectal excision (TME) – and whether tumour-free resection margins have been achieved. The surgery itself and its quality depend on the accuracy of preoperative diagnosis and detection of risk areas in the rectum and mesorectum, on the surgeon’s skills, and finally on pathological assessment evaluating whether complete tumour excision has been accomplished including circumferential margins of the tumour, and whether mesorectal excision is complete. The aim of our study was to implement and standardize a new method of evaluation of the quality of the surgical procedure – TME – in rectal cancer treatment using an assessment of its circumferential margins (CRO) and completeness of the excision. Methods: The study consisted of two parts. The first, multi-centre retrospective phase with 288 patients analysed individual partial parameters of the diagnosis, operations and histological examinations of the rectal cancer. Critical points were identified and a unified follow-up protocol was prepared. In the second, prospective part of this study 600 patients were monitored parametrically focusing on the quality of the TME and its effect on the oncological treatment results. Results: The proportion of patients with restaging following neoadjuvant therapy increased from 60.0% to 81.7% based on preoperative diagnosis. The number of specimens missing an assessment of the mesorectal excision quality decreased from 52.9% in the retrospective part of to the study to 22.8% in the prospective part. The proportion of actually complete TMEs rose from 22.6% to 26.0%, and that of nearly complete TMEs from 10.1% to 24.0%. Conclusion: The introduction of parametric monitoring into routine clinical practice improved the quality of pre-treatment and preoperative diagnosis, examination of the tissue specimen, and consequently improved quality of the surgical procedure was achieved., and J. Úvod: Observační, prospektivní a neintervenční projekt SOLOSTAR měl za cíl získat informace z klinické praxe v České republice o spokojenosti pacientů s používáním předplněného inzulinového pera SoloStar. Na celkovou spokojenost s perem SoloStar nemělo statisticky významný vliv žádné z demografických kritérií, a to ani zrakový handicap či omezení manuální zručnosti. Pacienti navíc pero SoloStar upřednostňovali před inzulinovými pery používanými před vstupem do projektu. Klíčová slova: diabetes mellitus – inzulinové pero – spokojenost pacientů, Introduction: The aim of the observational, prospective and non-interventional survey SOLOSTAR was to obtain, in a clinical practice setting in the Czech Republic, information about patient satisfaction with the use of the pre-filled insulin pen SoloStar. Methodology: 1 805 patients suffering from type 1 or 2 diabetes who began using the pen SoloStar were observed for 3 to 4 months. Satisfaction of patients with SoloStar and with its particular features was evaluated in the group of all patients and in subgroups defined by demographic parameters including the type of handicap. Patients also compared SoloStar with the pens used before the entry into the survey. Results: 98.3 % of patients rated overall satisfaction with the pen SoloStar as “excellent” and “good” (the highest and second highest rating on a 5-point scale). Demographic criteria, including visual handicap and manual dexterity handicap, did not have any statistically significant effect on the rating of the overall satisfaction with SoloStar. Features of SoloStar were rated as “excellent” and “good” by more than 90% of patients. Higher rating of some of the features of SoloStar was awarded mostly by patients without prior experience with insulin application. Patients who used other insulin applicators before entering into the survey were more satisfied with the pen SoloStar than with the previously used applicator and most of them rated the use of SoloStar (93.1 %) and insulin application with SoloStar (83.5 %) as “much easier” and “easier” (the highest and second highest rating on a 5-point scale) in comparison with the previously used pen. Conclusion: The use of the pen SoloStar in a clinical practice setting in the Czech Republic is associated with high levels of satisfaction of patients, including handicapped patients. In addition, patients preferred SoloStar over insulin pens used before the entry into the survey. Úvod: Tato práce referuje první zkušenosti s laparoskopicky asistovanou transanální totální excizí mezorekta u pacientů s karcinomem středního a dolního rekta. Závěr: Na základě prvních získaných zkušeností hodnotíme výkon jako realizovatelný a bezpečný v podmínkách našich chirurgických pracovišť s uspokojivými patologickými výsledky. Přesné indikace zatím nelze jednoznačně doporučit. Předpoklad je, že k transanální totální mezorektální excizi by měli být indikováni pacienti s nízce uloženým karcinomem rekta, event. s úzkou pánví, objemným mezorektem a objemnou prostatou., Introduction: First experiences with laparoscopic low anterior resection with transanal mesorectal excision in patients with low and mid rectal cancer are referred. Methods: During a 9 month period (between October 2013 and May 2014), 17 laparoscopically assisted rectal resections with transanal total mesorectal excision, hand-sewn colo-anal anastomosis and protective ileostomy were performed at two institutions (CHK FN Brno, CHO KOC Nový Jičín). There were 11 males and 6 females, with a mean age 68 years (range 49–81 years) in our cohort. The tumor was located 30–80mm from the anal verge (median 60 mm). Results: A negative distal resection margin was achieved in all patients. The circumferential resection margin was positive in two cases. The TME quality was described as complete in 8 cases and nearly complete in 9 cases. Mean operative time was 280 min (range 212–375 min) with no intra-operative complications. Anastomotic leakage was observed in two patients (12%). Median length of postoperative hospital stay was 9 days with a range of 6–30 days, in case of anastomotic and wound complications. Conclusion: According to our first experience we evaluated this method as feasible and safe with satisfactory pathological outcomes. We cannot recommend exact indications yet. It is assumed that the transanal approach for total mesorectal excision should be indicated in obese patients with a narrow pelvis and voluminous prostate., and V. Procházka, Z. Kala, M. Škrovina, T. Tato pilotní studie je zaměřena na parametry CT perfuze (cerebral blood flow, cerebral blood volume a permeability surface area) a multifázické CT angiografie (lokalizace trombu, stav leptomeningeálních kolaterál a morfologii durálních splavů) v predikci rozvoje maligního mozkového edému u pacientů s akutním uzávěrem v přední mozkové cirkulaci. Analyzováni byli pacienti z prospektivní multicentrické studie PROVE-IT (Precise and Rapid assessment of collaterals using multiphase CTA in the triage of patients with acute ischemic stroke for IV or IA Therapy; n = 200). Hodnocení perfuzních parametrů v kompletním teritoriu ischemie neprokázalo signifikantní rozdíl., We explore the predictive value of CT perfusion parameters (permeability surface area, cerebral blood flow, cerebral blood volume) and multiphase CT angiography (clot localization, leptomeningeal collaterals and dural sinuses morphology) in the development of malignant brain oedema in patients with acute proximal occlusion of the middle cerebral artery. Patients from the multicentre prospective study PROVE-IT (Precise and Rapid assessment of collaterals using multi-phase CTA in the triage of patients with acute ischemic stroke for IV or IA Therapy; n = 200) were analysed. Primary outcome was evaluated by midline shift ≥ 5 mm on a follow-up CT/MRI. A CBV threshold of 1.76 ml/100 g had the highest sensitivity and specificity of for the midline shift. There was no significant difference between the two groups in analysed CT perfusion parameters calculated for the entire ischemic region.
