Cíl: Cílem práce je retrospektivní zhodnocení nefarmakologické léčby nejčastěji se vyskytujících pravidelných supraventrikulárních tachykardií s důrazem na reálnou bezpečnost a efektivitu zákroků. Metodika: Do sledování jsme zahrnuli všechny konsekutivní pacienty s diagnózou přítomnosti přídatné dráhy (AP) a/nebo AV reentry tachykardie (AVRT), AV nodální reentry tachykardie (AVNRT) a typického flutteru síní (FLS), kteří byli v letech 2008–2012 léčeni na našem pracovišti pomocí radiofrekvenční katetrové ablace (RFA). Výsledky: Celkem jsme v tomto období provedli 1 168 ablací, z toho 141 zákroků pro AVRT/AP, 383 zákroků pro AVNRT a 644 zákroků pro FLS. Akutní úspěšnost nefarmakologické léčby v našem souboru u AVRT/AP byla 98,1 %, u AVNRT 98,6 % a 99,8 % u FLS. Výskyt komplikací souhrnně za všechny tři diagnostické jednotky byl 1,03 %. Data o úspěšnosti a výskytu komplikací jsou v našem souboru zcela ve shodě a často i příznivější v porovnání s literárně uváděnými údaji. Závěr: U indikovaných nemocných se supraventrikulární tachykardií je nefarmakologická léčba RF katetrovou ablací vysoce efektivní a bezpečnou metodou volby., Aim: The aim of our study is a retrospective review of non‑pharmacological treatment of the most commonly occurring regular supraventricular tachycardias with an emphasis on safety and effectiveness. Methods: We included the data of all our consecutive patients diagnosed with an accessory pathway (AP) and/or AV reentry tachycardia (AVRT), AV nodal reentry tachycardia (AVNRT) and typical atrial flutter (AFL), who were treated in our centre using radiofrequency catheter ablation (RFA) between 2008 and 2012. Results: In total, we conducted 1,168 ablations, comprising 141 procedures for AVRT/AP, 383 procedures for AVNRT and 644 procedures for AFL. The acute success rate was 98.1% for AVRT/AP, 98.6% for AVNRT and 99.8% for AFL. The incidence of complications was 1.03% in all procedures. Success and complication rates in our centre are consistent, and often more favourable in comparison with the literature. V retrospektivní pozorovací studii hodnotíme míru úspěšnosti operační techniky rekonstrukce strikturybulbární části uretry (za období 1988–2013) v italském centru s velkým objemem případů. Do studie byli zařazeni pacienti vyžadující uretroplastiku bulbární části močové trubice. Ze studie byli vyloučeni pacienti se strikturou penilní uretry a pacienti s neúplnými zdravotními záznamy. Primární cíl studie spočíval v hodnocení celkové míry úspěšnosti operační rekonstrukce striktury bulbární části močové trubice.Sekundární výsledné parametry zahrnovaly hodnocení úspěšnosti jednotlivých operačních technik. Pro účely studie jsme užili deskriptivní statistické metody. Do studie bylo zařazeno celkem 614 mužů ve věku 39 let (medián). Medián sledování činil 82 měsíců (rozmezí 12–302 měsíců). Z celkem 614 pacientů podstoupilo 338 (55 %) mužů uretroplastiku technikou ventrálního onlay graftu, 208 (33,9 %) mužů anastomózu konec ke konci, 44 (7,2 %) mužů uretroplastiku technikou dorzálního onlay graftu a 24 (3,9 %) mužů anastomózu s augmentací. Jako úspěšných bylo klasifikováno 520 (84,7 %) výkonů a v 94 (15,3 %) případech došlo k selhání. Technika anastomózy konec ke konci byla úspěšná u 87 % pacientů, uretroplastika technikou ventrálního onlay graftu u 85,5 % pacientů, technika dorzálního onlay graftu u 72,7 % pacientů a anastomóza s augmentací u 75 % pacientů. Rekonstrukce striktury bulbární uretry s využitím jedné ze čtyř jednodobých operačních technik dosáhla uspokojivých výsledků u 84,7 % pacientů., We performed a retrospective, observational study to evaluate the success rate of surgical techniques used to repair bulbar urethral strictures in an Italian high‑volume centre from 1988 to 2013. Patients requiring bulbar urethroplasty were included. Patients with penile stricture or incomplete clinical records were excluded from the study. The primary outcome of the study was to evaluate the overall success rate of surgical repair of bulbar urethral strictures. The secondary outcome of the study was to investigate the success rate of the single surgical techniques. Descriptive statistical methods were applied. Six hundred fourteen male patients with a median age of 39 years were included in the study. The median followup was 82 months (range 12–302 months). Out of 614 patients, 338 (55%) underwent ventral onlay graft urethroplasty, 208 (33.9%) end‑to‑end anastomosis, 44 (7.2%) dorsal onlay graft urethroplasty and 24 (3.9%) augmented anastomotic repair. Out of 614, 520 (84.7%) were classified as success and 94 (15.3%) as failures. End‑to‑end anastomosis provided 87% success rate, ventral onlay graft technique, 85.5%, dorsal onlay graft technique 72.7% and augmented anastomotic repair 75%.
