V České republice je kolorektální karcinom jedním z nejčastějších zhoubných nádorů a až 40 % pacientů je diagnostikováno v pokročilém stadiu. Chirurgická léčba je základní léčebnou modalitou vedoucí k vyléčení, a to i v případě pokročilého onemocnění. Zavedení nových cytostatik do praxe výrazným způsobem rozšířilo léčebné možnosti i u pacientů, kteří nejsou primárně chirurgicky léčitelní. V kombinaci s biologickou léčbou pak lze dosáhnout operability i u donedávna inoperabilních stavů, a tím tak významně prodloužit život pacientů s metastatickým kolorektálním karcinomem. U pacientů s metastatickým postižením jater jak primárně operabilním, tak inoperabilním je ve většině případů indikována neoadjuvantní chemoterapie/chemobioterapie. I když dosud nebyl definován nejvhodnější režim, základním požadavkem na něj je jeho nízká toxicita a vysoká účinnost., Colorectal cancer is one of the most common malignant tumors in the Czech Republic, and up to 40 % of patients are diagnosed at an advanced stage. Surgical treatment is the primary modality of therapy leading to cure colorectal cancer even in advanced disease. The introduction of new chemotherapeutic agents in practice greatly increased treatment options in patients who are not surgically curable. In combination with biological therapy, it can be achieved operability even in recently unresectable stage and thereby significantly prolonged life of patients with metastatic colorectal carcinoma. Úvod: Incidence kolorektálního karcinomu narůstá. Nejlepší léčebné výsledky má multimodální léčba, jejíž základní součástí je snaha o R0 resekci všech nádorových ložisek. Materiál a metodika: Byla provedena literární rešerše v souvislosti z vlastními zkušenostmi. Výsledky: Spektrum chirurgických zákroků zahrnuje endoskopickou submukózní resekci, transanální endoskopickou mikrochirurgii, laparoskopické resekce, otevřené resekce a cytoredukční chirurgii s peritonektomií a hypertermickou intraperitoneální chemoterapií. Nově se kromě konceptu totální mezorektální excize začíná uplatňovat také koncept totální mezokolické excize, který vede k nižšímu počtu lokálních recidiv a zvýšenému přežívání. Závěr: Stejně jako protinádorová farmakoterapie se významně stále vyvíjí i chirurgická léčba. K určení správné pozice jednotlivých chirurgických postupů v multimodálním algoritmu léčby je nutné sledování pacientů ve vhodných databázích, případně je léčit v rámci studií., Background: The incidence of colorectal carcinoma has been increasing. Only a multimodal treatment provides the best results. Surgical R0 resection of all tumour sites is the goal of all treatment regimes. Material and methods: A survey on the literature and our experiences has been conducted. Results: Surgical treatment comprises endoscopic submucosal dissection, transanal endoscopic microsurgery, laparoscopy, open surgery and cytoreduction surgery with the hyperthermic intraperitoneal chemotherapy. Clear cell odontogenic carcinoma (CCOC) is a rare odontogenic tumor associated with aggressive clinical behavior, metastasis and low survival. We report a case of CCOC affecting the maxilla of 62 year-old woman. It was first described as a clinicopathological entity in 1985 and to date only 67 cases were described in the English literature. Wider availability of several highly effecti - ve interventions such as oral rehyd - ration therapy in cases of diarrhoea could prevent large proportion of these child deaths. Unfortunately these interventions are usually not available in African countries. Ho - wever both availability of different life-saving health interventions and child mortality varies a lot across African countries. Comparative assessment of clinical efficiency of omeprazole and rabeprazole in treating gastroesophageal reflux disease (GERD). 65 patients with a verified diagnosis GERD were examined. Comparative clinical investigations of using of omeprazole and rabeprazole have revealed effectiveness of both drugs in the therapy of GERD. The authors demonstrate the possibility of improving surgical results by the reduction of perioperative bleeding in thoracic surgery associated with extended resection procedures. We focused on patients in whom the expected perioperative blood loss was greater than 500 ml. The first group consisted of patients with lung cancer stage III A after neoadjuvant chemotherapy had been indicated to extend the resection procedure. The second group consisted of patients with chest wall and mediastinum tumors of various etiologies. The third group consisted of patients with post-inflammatory thoracic complications in whom combined decortication and pleurectomy was necessary. Článek shrnuje použití cytokinů v terapii metastatického karcinomu ledviny od doby, kdy vzhledem k radiorezistentnímu a chemorezistentnímu chování renálního karcinomu byly prakticky jedinou systémovou terapií, až k jejich užití v dnešní době, kdy ustupují do pozadí a nahrazují je nové biologické léky, jako především tyrozinkinázové inhibitory či inhibitory mTOR. Své místo si zachovává pouze interferon-? v kombinaci s bevacizumabem (monoklonální protilátka proti VEGF), jakožto jedna z možností léčby první linie terapie mRCC. Současně se článek také zabývá mechanizmy účinků cytokinů, včetně nežádoucích, které také použití této léčby omezují., The article summarizes the use of cytokines in treating metastatic renal cell carcinoma from the time when, given the radioresistant and chemoresistant properties of renal cell carcinoma, they were virtually the only systemic therapy until present day when they give way and are being replaced with new biological agents, such as tyrosine kinase inhibitors or mTOR inhibitors. It is only interferon-? in combination with bevacizumab (a monoclonal antibody against VEGF) that has retained its position as one of the therapeutic options in the first-line treatment for mRCC. Purpose: This study aimed to evaluate prostate volume changes and prostate motions during radiotherapy.Methods: In 2010, twenty-five patients were treated for prostate cancer by external beam radiotherapy with implanted fiducial markers. Coordinates of three gold markers on kilovoltage images were calculated daily. Volume changes in target structure were observed through changes in intermarker distances. Differences in patient position between laser-tattoo alignment and gold marker localization were evaluated. Intrafraction motion was assessed by measuring marker displacement on kilovoltage images acquired before and after fraction delivery. Results: Prostate shrinkage was observed in 60% of patients. The average shrinkage was 7% of the prostate’s initial volume. Corrections after laser-tattoo alignment remained mostly below 1 cm. The difference between marker centroid position on the actual images and the planning images was 2 ± 1 mm on average. The extension of intrafraction movements was 7.6 ± 0.2 mm on average. Conclusions: In our retrospective study, the possibility for prostate volume changes during radiotherapy was revealed. Úvod: Byly srovnány dvě různé metody izoelektrické fokusace v agarosovém gelu pro detekci oligoklonálních IgG pásů v likvoru a séru: komerční metoda s imunofixací (Sebia) a „home-made“ metoda s použitím přístroje Multiphor II následovaná afinitním imunoblottingem. Byla posouzena shoda mezi metodami i hodnotícími, pokud jde o přítomnost intrathékální syntézy IgG, zařazení nálezu do jednoho z pěti typů podle mezinárodní klasifikace, počet intrathékálně syntezovaných IgG pásů a počet pásů v likvoru a v séru. Výsledky: Pro posouzení shody jsme použili statistiku kappa. Pokud jde o přítomnost intrathékální syntézy IgG, byla shoda velmi dobrá (kappa 0,870 až 1,000 mezi metodami a 0,947 a 0,920 mezi hodnotícími, což představuje 0 až 5 neshodně hodnocených nálezů z celkového počtu 114). V zařazení nálezu do jednoho z pěti typů podle mezinárodní klasifikace byla shoda méně vyjádřená (kappa 0,389 až 0,596 mezi metodami); při použití „home-made“ metody byla shoda mezi hodnotícími horší (kappa 0,478) než v případě komerční imunofixační metody (kappa 0,791). Shoda v počtu intrathékálně syntezovaných IgG pásů byla jen mírná, shoda v počtu oligoklonálních IgG pásů v likvoru a v séru byla slabá. Závěry: Oběma metodami lze spolehlivě detekovat oligoklonální IgG a metody lze považovat za rovnocenné. Pro klasifikaci typu nálezu však byla shoda mezi hodnotícími lepší při použití komerční imunofixační metody. Poměrně špatná reprodukovatelnost počítání oligoklonálních IgG pásů by měla být známa klinickým lékařům, protože porovnávání počtu pásů s sebou přináší potenciální riziko zavádějících interpretací., Background: We compared two different agarose isoelectric focusing methods for detection of oligoclonal IgG bands in cerebrospinal fluid and serum: commercial method with immunofixation (Sebia) and home-made method using Multiphor II apparatus followed by affinity immunoblotting. Interobserver agreement for both methods was tested concerning the presence of intrathecal IgG synthesis, the detailed isoelectric focusing pattern type, the number of CSF-restricted oligoclonal IgG bands, and the number of oligoclonal IgG bands in CSF and in serum. Findings: Using kappa statistics for evaluation of agreement, we found there was very good agreement concerning the presence of intrathecal IgG synthesis (kappa 0.870 to 1.000 between methods, and 0.947 and 0.920 between observers, respectively, representing 0 to 5 out of 114 samples classified differently). The agreement was less pronounced when international consensus classification of isoelectric focusing patterns into 5 different types was taken into account (kappa 0.389 to 0.596 between methods); using home-made method, the interobserver agreement regarding pattern type was worse (kappa 0.478) than using commercial Sebia method (kappa 0.791). There was moderate agreement on the number of CSFrestricted oligoclonal IgG bands, and mostly poor agreement on the number of oligoclonal IgG bands in CSF and serum. Conclusions: Both methods were capable to detect oligoclonal IgG reliably, and neither method could be evaluated as superior to the other. However, better interobserver agreement regarding to the pattern type was obtained using commercial Sebia immunofixation method.
