V kazuistike opisujeme prípad 49-ročného muža od detstva morbídne obézneho (BMI 40 kg/m2), s artériovou hypertenziou (asi od 15 rokov, liečená od roku 2004), dyslipidémiou (od roku 2006), diabetes mellitus 2. typu (od roku 2006, na inzulinoterapii od roku 2008) a fajčením (do roku 2011, 20 cigariet denne). Liečba: 16 druhov liekov, 8 na hypertenziu, statín, liečba diabetu, aspirín, alopurinol. V roku 2010 (ako 45-ročný) hospitalizovaný na našej klinike pre dyspnoe a bolesti na hrudníku s hodnotou krvného tlaku 180/110 mm Hg (zistené srdcové zlyhávanie s ejekčnou frakciou ĽK 33 %, vo funkčnej triede NYHA II, echokardiograficky: ľavá predsieň: 46 mm, rozmer ľavej komory v diastole: 70 mm, interventrikulárne septum: 12 mm, hypokinéza septa, dopplerovská ultrasonografia artérií dolných končatín (kalcifikácie, difúzne aterosklerotické zmeny, neprítomná stenóza), CT koronárna angiografia (signifikantná stenóza ľavej koronárnej artérie). Zahájená liečba perorálny furosemid 40 mg denne. V máji roku 2011 hospitalizovaný pre akútny koronárny syndróm: akútny NSTEMI spodnej steny (koronarografia: 2-cievne postihnutie, realizovaná PKI, implantovaný DES – ramus circumflexus, paroxyzmus fibrilácie predsiení, funkčná trieda NYHA III, ľavokomorová ejekčná frakcia: 30 %, pľúcna hypertenzia). V roku 2012 realizovaná renálna denervácia pre rezistentnú hypertenziu, implantovaný karotický stent pre stenózu karotickej artérie, prítomná diabetická nefropatia (KDOQI 3. stupňa, GF 40 ml/min). V auguste roku 2014 hospitalizovaný na našej klinike pre pľúcny edém, kardiogénny šok, akútnu ischémiu pravého predkolenia pri periférnej embolizácii, prítomný atriálny flutter, zhoršenie renálnych parametrov, echokardiograficky: ľavá predsieň: 55 mm, rozmer ľavej komory: 75 mm, akinéza septa a zadnej steny, oklúzia artérií pravej dolnej končatiny (vzhľadom na vážny stav pacienta kontraindikovaná angiochirurgická intervencia, zvažovaná amputácia končatiny z vitálnej indikácie), pacient zomrel po 4 dňoch hospitalizácie na intenzívnej jednotke po neúspešnej liečbe. Kombinácia ochorení diabetes, hypertenzia a ischemická choroba srdca je častá a prognosticky vážna. Diabetes zvyšuje kardiovaskulárnu morbiditu a mortalitu, a preto by sme mali hľadať diabetes u všetkých kardiovaskulárnych pacientov., The case study describes a case of 49-year-old man with morbid obesity since childhood (BMI 40 kg/m2), arterial hypertension (approx. since aged 15, treated since 2004), dyslipidemia (since 2006), type 2 diabetes mellitus (since 2006, on insulin therapy since 2008) and smoking (until 2011, 20 cigarettes a day). Treatment: 16 types of medication, 8 for hypertension, statin, therapy for diabetes, aspirin, allopurinol. In 2010 (when aged 45) hospitalized in our clinic with dyspnoea and chest pain with a high pressure reading of 180/110 mm Hg (identified symptoms of heart failure with LV ejection fraction of 33 %, in NYHA II functional class, echocardiographically: left atrium: 46 mm, left ventricular chamber size in diastole: 70 mm, interventricular septum: 12 mm, septal hypokinesis, Doppler ultrasonography of lower limb arteries (calcification, diffuse atherosclerotic changes, absent stenosis), CT coronary angiography (significant stenosis of the left coronary artery). Treatment started with 40 mg oral dose of furosemide daily. In May 2011 he was hospitalized with an acute coronary syndrome: acute NSTEMI of the inferior wall (coronarography: 2-vascular problems, implemented PKI, implanted DES – ramus circumflexus, paroxysmal atrial fibrillation, NYHA III functional class, left ventricular ejection fraction: 30 %, pulmonary hypertension). In 2012 renal denervation for resistant hypertension was carried out, carotid stent implanted for stenosis of the carotid artery, presence of diabetic nephropathy (KDOQI stage 3, GF 40 ml/min). In August 2014 admitted to our clinic with pulmonary oedema, cardiogenic shock, acute ischemia of the right calf with peripheral embolisation, presence of atrial flutter, impairment of renal parameters, echocardiographically: left atrium: 55 mm, left ventricle size: 75 mm, akinesis of the septum and posterior wall, occlusion of the right leg arteries (given the patient‘s serious state angio-surgical intervention was contraindicated, vitally indicated leg amputation considered), the patient died after 4 days of hospitalization in an intensive care unit after unsuccessful treatment. A combination of diabetes, hypertension and ischemic heart disease is frequent and prognostically serious. Úvod: PARADIGM HF srovnávala inhibitor angiotenzinového receptoru a inhibitor neprilysinu LCZ696 s enalaprilem u nemocných se srdečním selháním a sníženou ejekční frakcí. Metodika: Jednalo se o dvojitě slepou studii u 8 442 nemocných se srdečním selháním NYHA klasifikace II, III a IV a ejekční frakcí pod 40 %, kteří dostávali LCZ696 (2 × 200 mg) nebo enalapril (2 × 10 mg) přidané ke standardní medikaci. Primární cíl byl smíšený – kardiovaskulární mortalita a hospitalizace pro první srdeční selhání. Studie byla naplánována tak, aby odhalila rozdíl v kardiovaskulární mortalitě. Výsledky: Studie byla ukončena předčasně podle předepsaných pravidel při průměrné době sledování 27 měsíců pro jasný prospěch z léčby LCZ696. V době ukončení se primární cíl vyskytl u 914 nemocných (21,8 %) ve LCZ696 skupině a u 1 117 nemocných (26,5 %) ve skupině léčené enalaprilem (HR LCZ696 skupina, 0,80; 95% CI, 0,73–0,87; p < 0,001). Celkem 711 nemocných (17,0 %) léčených LCZ696 a 835 nemocných (19,8 %) léčených enalaprilem zemřelo (HR pro úmrtí z jakékoli příčiny 0,84; 95% CI, 0,76–0,93; p < 0,001); z těchto nemocných 558 (13,3 %) a 693 (16,5 %) zemřelo z kardiovaskulární příčiny (HR 0,80; 95% CI, 0,71–0,89; p < 0,001). LCZ696 ve srovnání s enalaprilem snižoval riziko hospitalizace pro srdeční selhání o 21 % (p < 0,001) a snižoval symptomy srdečního selhání (p = 0,001). Ve skupině léčené LCZ696 bylo více hypotenzí a nezávažných angioedémů, ale menší výskyt renálního selhání, hyperkalemie a kašle než ve skupině léčené enalaprilem. Závěr: LCZ696 byl účinnější než enalapril ve snížení rizika kardiovaskulárního úmrtí a hospitalizací pro srdeční selhání. Methods: In this double‑blind trial, 8442 patients with class II, III, or IV heart failure and an ejection fraction of 40% or less were assigned to receive either LCZ696 (at a dose of 200 mg twice daily) or enalapril (at a dose of 10 mg twice daily), in addition to the standard therapy. There were two primary outcomes – mortality from cardiovascular causes and hospitalisation for heart failure, but the trial was designed to detect the differences in the rates of mortality from cardiovascular causes. Results: In line with the pre‑specified rules, the trial was stopped early, after a median follow‑up of 27 months, because the boundary indicating a clear benefit achieved with LCZ696 had been exceeded. At the time of study closure, the primary outcome had occurred in 914 patients (21.8%) in the LCZ696 group and 1117 patients (26.5%) in the enalapril group (hazard ratio in the LCZ696 group, 0.80; 95% confidence interval [CI], 0.73 to 0.87; P < 0.001). Conclusions: LCZ696 was more effective than enalapril in reducing the risk of death and of hospitalisation for heart failure. V listopadu roku 2014 se konal v Paříži mítink evropských expertů v kardiovaskulárních (KV) onemocněních a lipidech, na kterém se diskutoval závažný problém preventivní kardiologie – reziduální vaskulární riziko dané aterogenní dyslipidemií (AD), tj. zvýšenými triglyceridy a sníženým HDL-cholesterolem. Experti přijali na základě diskuse konsenzus o vlivu AD na KV riziko a o dosud existujících důkazech o snížení KV rizika kombinovanou léčbou staninem a fenofibrátem. Aterogenní dyslpidemie by měla být sekundárním cílem léčby dyslipidemií, protože se podílí na makrovaskulárním i mikrovaskulárním reziduálním riziku. Sekundárním cílem léčby je non-HDL-cholesterol jako nejlevnější a nejdostupnější marker reziduálního vaskulárního rizika. Kombinovaná terapie statinem a fibrátem je bezpečná, dobře tolerovaná, snižuje aterogenitu plazmy a výskyt KV příhod u pacientů s AD a vysokým KV rizikem, diabetem 2. typu nebo metabolickým syndromem. Klíčová slova: aterogenní dyslipidemie – fenofibrát – kardiovaskulární riziko – kombinovaná terapie – statiny – triglyceridy, A meeting of European experts in cardiovascular (CV) disease and lipids was convened in Paris, November 2014, where an important problem of preventive cardiology – residual vascular risk done by atherogenic dyslipidemia (AD) – was discussed. On the basis of discussion have the experts summarised a consensus concerning AD, its CV risk and up to date evidence of combined therapy with statin and fenofibrate on CV risk. Atherogenic dyslipidemia should be the secondary aim of dyslipidemia treatment, because it is a reason of residual vascular risk. Non-HDL-cholesterol should be the secondary target of AD treatment, which is a most unexpensive and easiest marker of residual vascular risk. Combined therapy with statin and fenofibrate is safe, well tolerated and reduces plasma atherogenity and CV events in patients with AD and high CV risk, type 2 diabetes or metabolic syndrome. Hlavní příčiny morbidity a mortality u polycythemie vera (PV) a esenciální trombocytemie (ET) jsou trombóza a krvácení, progrese do myelofibrózy a přechod do akutní leukemie. Myelosupresivní léčba, především pak hydroxyurea, sice může četnost cévních komplikací omezit, ale u tohoto přípravku existují jisté obavy stran zvýšeného výskytu leukemické transformace. Léčba těchto poruch proto představuje náročný problém a abychom se vyhnuli neadekvátní expozici cytotoxickým léčivům na straně jedné či suboptimální léčbě na straně druhé, měli bychom dodržovat „risk-oriented” léčebnou strategii. Mezi známé rizikové faktory kardiovaskulárních příhod patří vyšší věk a předchozí trombóza, zatímco vliv nových biologických faktorů včetně leukocytózy a mutačního statutu, popř. mutační zátěže JAK2V617F je předmětem bádání. Nízkorizikoví pacienti s PV by měli být léčeni pouze flebotomií a aspirinem, zatímco vysoce rizikoví pacienti by měli rovněž podstoupit cytotoxickou léčbu. Co se týká léčby ET, u nízkorizikových pacientů jednoznačné indikace pro intervenci neexistují, nicméně vysoce rizikoví pacienti by měli podstoupit chemoterapii. U vybraných pacientů včetně pacientů s rezistencí nebo intolerancí na hydroxyureu se mohou uplatnit i jiné léčebné možnosti, jako např. interferon alfa či anagrelid., Major causes of morbidity and mortality in polycythemia vera (PV) and essential thrombocythemia (ET) are represented by thrombosis and bleeding, progression to myelofibrosis and transformation to acute leukemia. Myelosuppressive therapy, preferentially with hydroxyurea, can reduce the rate of vascular complications, but there is some concern about an increased rate of leukemic transformation with this agent. Therefore, the management of these disorders poses a significant challenge, and a risk-oriented therapeutic approach should be followed to avoid inappropriate exposure to cytotoxic drugs on one side or suboptimal treatment on the other. Established risk factors for cardiovascular events are represented by older age and previous thrombosis, while the impact of novel biological factors, including leukocytosis and JAK2V617F mutational status and/or mutational burden, is under investigation. Low-risk PV patients should be managed only with phlebotomy and aspirin, while high-risk patients should also receive cytotoxic therapy. Regarding the management of ET, there is no clear indication for intervention in low-risk patients, while high-risk patients should be managed with chemotherapy. Tato práce pojednává o některých klíčových aspektech diagnostických kritérií pro MPN podle klasifikace Světové zdravotnické organizace (WHO) z roku 2008, přičemž většina poznámek se omezuje na onemocnění polycythaemia vera (PV). Při analýze hlavních kritérií poznamenáváme, že existují pacienti s PV, jejichž erytrocytóza může být dokumentována pouze měřením objemu erytrocytární masy a nikoli měřením koncentrace hemoglobinu nebo hematokritu. Probíráme reprodukovatelnost histopatologie pro diagnostiku různých stadií MPN, existenci normálních hladin EPO u pacientů s PV, specifitu růstu endogenních erytroidních kolonií (EEC) pro diagnózu PV a naše současné znalosti týkající se cytogenetiky a molekulární biologie PV a jiných MPN a jejich patofyziologie. Rovněž analyzujeme různé prognostické ukazatele morbidity (trombóza) a mortality u PV – vysokou hladinu hematokritu, leukocytózu a procentuální zastoupení buněk s mutací JAK2V617F. V případě esenciální trombocytemie (ET) komentujeme rozlišení pravé ET a “časné PMF”. V závěru stručně shrnujeme obecně přijímaná doporučení European LeukemiaNet (ELN) pro léčbu Ph-negativní MPN na základě nové stratifikace rizika, na důkazech založené riziko kancerogeneze u hydroxyurey a racionální doporučení pro výběr konkrétních léků., In this paper, we discuss some critical parts of the World Health Organization (WHO) 2008 criteria for classification of MPNs classification, with most of the remarks limited to polycythemia vera (PV). When analyzing the major criteria, we note that there are PV patients whose erythrocytosis can only be documented by red cell mass measurements and not by hemoglobin concentration or hematocrit measurements. We discuss the reproducibility of histopathology for the diagnosis of different stages of MPNs, the existence of normal EPO levels in PV patients, the specificity of Endogenous erythroid colony (EEC) growth for PV diagnosis and our current knowledge of the cytogenetics and molecular biology of PV and other MPNs and their pathophysiology. We also analyze various prognostic morbidity (thrombosis) and mortality markers in PV – high hematocrit level, leukocytosis and percentage of cells with the JAK2V617F mutation. In essential thrombocythemia (ET), we comment on the distinction of true ET and “early PMF”.
