Úvod: Zhodnocení možnosti miniinvazivního odběru žilního graftu (Endoscopic Vein Harvest = EVH) za účelem cévně-chirurgické revaskularizace infrainguinálního tepenného systému končetiny. Zhodnocení našich časných výsledků a zkušeností s uvedenou metodou. Metoda: V časovém období duben 2012 až březen 2015 podstoupilo 16 pacientů endoskopický odběr velké safény (VSM) v rámci implantace infrainguinálního bypassu. Sedm (43,7 %) pacientů bylo indikováno k operaci pro obraz kritické končetinové ischemie (Rutherford kategorie 5). 9 (56,3 %) pacientů bylo indikováno k výkonu pro těžké intermitentní klaudikace (Rutherford kategorie 3). Průměrný věk pacientů byl 59,9 roku, mužů bylo 14 (87,5 %), ženy 2 (12,5 %). Selekce pacientů k endoskopickému odběru VSM probíhala na základě klinického a sonografického zhodnocení její vhodnosti. U pacientů byla sledována jejich předoperační anamnestická data, klinické nálezy, peroperační a pooperační průběh včetně komplikací. Pacienti byli prospektivně sledováni v časových intervalech 3, 6, 12, 18, 24 měsíců a následně ? 1 rok od operačního výkonu. Byla vyhodnocena průchodnost rekonstrukcí pomocí Kaplanovy-Meierovy metody. Statistická analýza souboru byla provedena s použitím softwaru IBM SPSS Statistics 21.0. Výsledky: U všech pacientů byla odebraná VSM použita k provedení proximálního (10; 62,5 %) nebo distálního (6; 37,5 %) femoropopliteálního bypassu (FPB). Jeden pacient, v časné fázi zavádění metody podstoupil po endoskopické disekci VSM konverzi na otevřený odběr. U 2 pacientů (12,5 %) pooperačně vznikla chirurgická ranná infekce (SSI) Szilagyi gr. II (1 pacient s dokončeným EVH – lokalizace tříslo, 1 pacientka po konverzi EVH – lokalizace tříslo a odběrová rána VSM). Průměrná doba sledování pacientů v souboru činila 10,2 měsíce. Primární, primární asistovaná a sekundární průchodnost po 1. roce sledování byla 82,0 %, 93,8 % a 100,0 %; po 2. roce sledování byla rovněž 82,0 %, 93,8 % a 100,0 %. Amputation-free survival po 1. i 2. roce sledování byl 100,0 %. Žádný pacient v průběhu studie nezemřel – letalita 0,0 %. Závěr: Endoskopický odběr VSM představuje miniinvazivní alternativu ke klasickému otevřenému odběru VSM u pacientů, kteří podstupují infrainguinální bypass. Naše časné výsledky femoropopliteálních bypassů s použitím metody EVH vykazují srovnatelné výsledky průchodností oproti rekonstrukcím s tradičním otevřeným odběrem VSM. Je současně patrná nízká incidence chirurgických ranných infekcí u takto provedených bypassů., Introduction: This study was undertaken to determine the feasibility of endoscopic vein harvest (EVH) for infrainguinal arterial bypass surgery. We describe our initial experience and early results of bypasses done using this minimally invasive approach. Method: From April 2012 to March 2015, 16 patients underwent 16 femoropopliteal bypass operations with great saphenous vein (GSV) being harvested by endoscopic technique. The indication for intervention was critical limb ischemia (Rutherford category “5”) in 7 patients (43.7%) and severe intermittent claudication (Rutherford category “3”) in 9 patients (56.3%). There were 14 male (87.5%) and 2 female (12.5%) patients, with a mean age of 59.9 years. Selection of patients for EVH was based on clinical and duplex ultrasound appearance of GSV. Only patients with adequate GSV were considered for EVH. We collected data regarding patients’ demographics, history, clinical findings, operative procedures and postoperative recovery including complications. Patients were followed at 3, 6, 12, 18 and 24 months postoperatively and yearly thereafter. Patencies were analyzed by Kaplan-Meier method. Statistical analysis was performed using IBM SPSS Statistics 21.0 software (IBM Corp, Armonk, NY). Results: Endoscopically harvested GSV was utilised for formation of proximal (10; 62.5%) or distal (6; 37.5%) femoropopliteal bypass. One patient underwent conversion to open harvest after endoscopic dissection of the vein. It occurred early in our experience. All other GSV harvests were accomplished endoscopically. 2 patients (12.5%) developed postoperative surgical site infection (SSI) Szilagyi gr. II (1 patient after successful EVH – location: groin; 1 patient after conversion of EVH to open vein harvest – location: groin and vein harvest incision). Mean follow-up was 10.2 months (range 0.3 to 27.0 months). At 1 and 2 years, primary patency was 82.0% and 82.0%, assisted primary patency was 93.8% and 93.8%, and secondary patency was 100.0% and 100.0%. At 1 and 2 years, amputation-free survival was 100.0% and 100.0%. No patient died within the study period (mortality 0.0%). Conclusions: Endoscopic harvest of GSV is a minimally invasive alternative to a standard open harvest of GSV. It is a feasible option for patients undergoing infrainguinal arterial bypass. In our early experience, patencies of EVH femoropopliteal bypasses are comparable to those achieved using traditional open vein harvest technique. Úvod: Adenokarcinom pankreatu (PDAC) je jedním z nejagresivnějších maligních onemocnění. Jeho špatná prognóza je dána kombinací různých faktorů, mezi něž patří agresivní biologie onemocnění, nespecifické či chybějící příznaky v časných stadiích, jejich nedocenění, protrahovaná diagnostika a zpoždění v zahájení léčby. Většina pacientů je diagnostikována již v pokročilém stadiu onemocnění. Medián přežití u těchto pacientů se pohybuje mezi 2 a 11 měsíci. Nejčastějšími důsledky lokálně pokročilého onemocnění, které si vynutí intervenci, jsou obstrukce žlučových cest a duodena. Cílem práce byla analýza přežívání pacientů s radikálně inoperabilním PDAC, kteří podstoupili paliativní chirurgický výkon či exploraci s biopsií, stanovení vlivu faktorů pacienta, nádoru a léčebné modality na délku přežití. Metody: Jedná se o retrospektivní analýzu souboru pacientů s inoperabilním PDAC, kteří podstoupili neradikální chirurgickou intervenci v letech 2006–2014. Předmětem analýzy byly věk, histopatologické nálezy, chirurgická a onkologická léčba a doba přežití. Výsledky byly statisticky zpracovány a vyhodnoceny pomocí softwaru IBM SPSS Statistics verze 22 (USA). Výsledky: Soubor tvořilo celkem 184 pacientů (105 mužů, 79 žen) s radikálně inoperabilním PDAC. Průměrný věk byl 64 let a převažovali pacienti s onemocněním ve stadiu IV. Průměrná délka přežití byla 7,04 měsíce, medián 4,7 měsíce. Závěr: Byl zjištěn statisticky signifikantní vliv pohlaví, stadia onemocnění, přítomnosti vzdálených metastáz v době operace a podání onkologické léčby na přežívání těchto pacientů. Průměrná délka i medián přežití pacientů s radikálně inoperabilním tumorem v našem souboru koresponduje s výsledky podobných studií., Introduction: Pancreatic cancer (PDAC) is one of the most aggressive malignancies. Its poor prognosis is due to a combination of various factors, mainly aggressive biology of the tumour, non-specific symptoms in early stages, their underestimation, prolonged time to diagnosis and late onset of treatment. The majority of patients are diagnosed in an advanced stage of the disease. Median survival of these patients ranges from 2−11 months. The most common consequences of locally advanced disease that require intervention include obstruction of the duodenum and biliary obstruction. The purpose of our study was to analyze the survival of patients with radically inoperable PDAC undergoing palliative surgery or exploration with biopsy, and to evaluate the influence of patient and tumour factors and treatment modalities on survival. Methods: In our retrospective study we included all patients with radically inoperable PDAC undergoing a non-radical surgical intervention between 01 January 2006 and 31 December 2014. Patient age, histopathological findings, surgical and oncological treatment and survival were included in the analysis. The results were statistically processed and evaluated using IBM SPSS Statistics software version 22 (USA). Results: 184 patients with radically inoperable PDAC, 105 males and 79 females, were included in our study. Mean age of the patients was 64 years and most patients presented with stage IV of the disease. Mean survival time was 7.04 months and median 4.7 months. Conclusion: We determined a statistically significant influence of the following factors on patient survival: sex, stage, presence of distant metastases at the time of surgery and oncological treatment administration. Spinální epidurální lipomatóza (SEL) je stav, při kterém dochází k patologickému nahromadění tuku v epidurálním prostoru páteřního kanálu. Za možnou příčinu bývá považováno užívání kortikosteroidů, obezita, endokrinologické onemocnění (nejčastěji endogenní nadprodukce kortizolu), u části případů však zůstává příčina neobjasněna. SEL se stává symptomatickou zřídka, a to ve chvíli, kdy způsobí kompresi míchy nebo nervových kořenů. Klinická manifestace závisí na úrovni postižení páteřního kanálu. Pro terapii SEL je určující závažnost komprese nervových struktur a tomu odpovídající tíže klinického syndromu. Při hodnocení šíře epidurálního tuku na zobrazovacích vyšetřeních, při volbě terapeutického postupu a při stanovení prognózy mohou být nápomocné nové klasifikace, které vytvořili Borré (pro bederní páteř) a Quint (pro hrudní páteř). Terapeutické možnosti zahrnují konzervativní terapii (redukce tělesné hmotnosti, redukce dávky kortikoidů, léčba endokrinologického onemocnění, analgetika, rehabilitace) a chirurgickou dekompresi. Samotná SEL je však velmi vzácná. U pacientů se často vyskytuje v koincidenci s jinými, hlavně degenerativními změnami páteře. V naší práci uvádíme tři kazuistiky, na kterých demonstrujeme možnou příčinu, průběh a terapii tohoto onemocnění. U prvního pacienta se lipomatóza manifestovala syndromem kaudy, v druhém případě radikulárním syndromem a u třetího pacienta SEL způsobila kompresi hrudní míchy., Spinal epidural lipomatosis (SEL) is a condition associated with pathological fat accumulation in the epidural area of the spinal canal. The disorder is likely caused by the use of corticosteroids, obesity, endocrinal disorders (especially endogenous overproduction of cortisol), although, in some cases, no cause is immediately evident. SEL rarely becomes symptomatic unless it leads to compression of the spinal cord or nerve roots. Clinical manifestation depends on the level at which the spinal canal is affected. Severity of compression of nerve structures and the corresponding intensity of clinical syndrome are the most important factors in the choice of SEL therapy. Recent classifications for evaluation of epidural fat layer on radiographic images by Borré (for lumbar spine) and Quint (for thoracic spine) can help in guiding the diagnostic and treatment approaches. Therapeutic options include conservative therapy (reduction of body weight, reduction of any corticoid dosage, treatment of the endocrinal disorder, analgesics, rehabilitation) and surgical decompression. However, SEL itself is rare and is frequently found together with other (e.g. degenerative) spinal changes. We report three cases from our own patient base through which we demonstrate possible causes, course and therapy of this disorder. Lipomatosis manifested as cauda equina syndrome in the first patient, as radicular syndrome in the second, and SEL led to compression of the thoracic spinal cord in the third. Úvod: Cílem tohoto článku je prezentovat naše zkušenosti s transanální endoskopickou mikrochirurgií (TEM) jako jedním ze základních operačních přístupů k selektovaným pacientům s nádory konečníku. Metody: Retrospektivně jsme vyhodnocovali soubor 80 pacientů, u kterých byla pro nádor konečníku provedena operační léčba pomocí TEM. U těchto pacientů jsme sledovali a statisticky vyhodnocovali konkrétní předoperační a pooperační ukazatele, které bychom chtěli prezentovat: předoperační bioptické vyšetření a pooperační výsledek histologického vyšetření, velikost nádoru a jeho uložení v konečníku, hloubku resekce a hodnocení základních demografických ukazatelů. Výsledky: Z našich zkušeností u zmíněných pacientů vyplývá, že není významný rozdíl mezi věkem operovaných mužů a žen ani při rozlišení na benigní a maligní nádory. Nejčastěji operovanými pacienty byli muži, průměrně ve věku 64 let, kteří byli primárně vyšetřeni pro krvácení z konečníku. Nejčastějším nálezem byl low-grade adenom, který byl potvrzen i v definitivním resekátu. Celkem u 17 pacientů byl nalezen v definitivním histologickém vyšetření maligní tumor, který nebyl zachycen předoperačně. Závěr: V rukách zkušeného chirurga představuje TEM reálnou možnost provedení radikální resekce i maligního, lokálně nepokročilého nádoru konečníku, pokud je v dosahu operačního rektoskopu. TEM je metodou bezpečnou, bez negativního vlivu na funkce análních svěračů. Vzhledem k možnosti falešně negativního předoperačního histologického vyšetření preferujeme radikální odstranění nádoru s celou šířkou stěny konečníku. I když optimální lokalizace pro použití operačního rektoskopu je na zadní straně konečníku, téměř u poloviny našich pacientů se tyto nádory vyskytovaly na stěně přední. Vzhledem k technické proveditelnosti sutury stěny rekta je vhodné, aby báze tumoru nepřesahovala polovinu obvodu konečníku. Technika TEM je vhodná zejména pro nízce uložené, nestenotizující, lokálně nepokročilé nádory konečníku. Naše dobré výsledky jsou dány především přísnou selekcí pacientů, technickým zvládnutím operačního postupu a letitými zkušenostmi., Introduction: The purpose of this paper is to present our experience with transanal endoscopic microsurgery (TEM) as an operative approach in a selected group of patients with rectal tumours. Methods: We present a retrospective study of 80 patients operated for rectal tumour using TEM. Specific preoperative and postoperative indicators were gathered and statistically evaluated. We assessed histological examination before the surgery and definitive histology of the resected tumour, resection depth and basic demographic data of our patients. Results: Our experience suggests that there is no significant difference between the age of patients according to sex, even if malignant and benign tumours are assessed separately. The majority of our operated patients were male patients, mean age of 64 years who were primarily referred to our department because of bleeding low-grade adenomas of the rectum. In 17 patients a malignant tumour was diagnosed based on final histopathologic examination. None of them had signs of malignancy before the surgery. Conclusion: In the hands of an experienced surgeon, TEM is a feasible option of radical removal of rectal, locally not advanced malignancies if within reach of TEM rectoscope. TEM is a safe procedure that does not result in any alteration of rectal sphincter functions. Possible false negative preoperative benign diagnosis leads us to approach radical removal of the tumour with full wall thickness resection. Although the posterior aspect of the rectum is an optimal location for the rectoscope, anterior tumours were found in almost one half of our patients. Considering technical feasibility of the rectal wall suture, the base of the tumour should not extend beyond one half of the rectal circumference. Low non-stenotising noninvasive tumours of the rectum are suitable for TEM.
