- Cíl: Cílem výzkumu bylo zjistit subjektivní proţívání ţen v těhotenství po perinatální ztrátě, identifikovat jejich potřeby a způsoby zvládání zátěţe v období právě probíhajícího těhotenství. Metodika: Výzkum byl proveden kvalitativní metodou. Metodou sběru dat byl polostrukturovaný rozhovor. Výzkumu se zúčastnilo 6 těhotných ţen ve třetím trimestru po perinatální ztrátě v předchozím těhotenství. K analýze dat bylo vyuţito některých postupů zakotvené teorie (otevřené a axiální kódování a vytváření kategorií). Výsledky: Na základě analýzy dat byly stanoveny kategorie: 1. Zkušenost perinatální ztráty, 2. Proţívání nového těhotenství, 3. Pocity, 4. Zvládání, 5. Potřeby. Analýzou rozhovorů bylo zjištěno, ţe těhotenství po perinatální ztrátě je obdobím zvýšené emocionální zátěţe, ţeny mají specifické potřeby a pouţívají různé způsoby zvládání zátěţe. Závěr: Těhotné ţeny po perinatální ztrátě vyţadují specifický přístup poskytovatelů péče. Významná je kontinuita péče počínající obdobím perinatální ztráty, pokračující přípravou na těhotenství, porod a šestinedělí. Pochopení pocitů ţen můţe pomoci ve snaze o efektivnější poskytování ošetřovatelské péče. Komunitní porodní asistentky mohou být vhodnou podporou pro tuto skupinu ţen a pomoci jim překonat náročné období těhotenství po perinatální ztrátě., Aim: The aim of this research was to determine the subjective experience of women in pregnancy after perinatal loss, identify their needs and ways of coping with the stress during an ongoing pregnancy. Methods: The study was carried out using a qualitative method. The data collection method used was a semi-structured interview. Six pregnant women in the third trimester after perinatal loss in previous pregnancies participated in the research. Some procedures of grounded theory (open and axial coding and creating categories) were used for data analysis. Results: Based on the data analysis categories and subcategories were defined: 1. The experience of perinatal loss, 2. Experiencing new pregnancy, 3. Feelings, 4. Coping, 5. Needs. Analyzing the interviews revealed that pregnancy after perinatal loss is a period of higher emotional stress, when women have specific needs and use different ways of coping with stress. Conclusion: Pregnant women after perinatal loss require a specific approach of care providers. The continuity of the care is important, starting from the period after perinatal loss, continuing in the preparation for pregnancy, childbirth and the postpartum period. Understanding the feelings of women may help in the effective provision of nursing care. - Pseudomyxoma peritonei je vzácný klinický syndrom; nádorový proces, pro který je charakteristické hromadění mucinózního ascitu a peritoneálních implantátů v břišní dutině. Primárním zdrojem je téměř vždy low grade mucinózní tumor apendixu. Jedná se o pomalu progredující, nízce agresivní, ale smrtící onemocnění s častými recidivami. Optimálním léčebným postupem je kompletní chirurgická cytoredukce (CRS) v kombinaci s peroperačně aplikovanou intraperitoneální hypertermickou chemoterapií (HIPEC) – tzv. Sugarbakerova metoda., Pseudomyxoma peritonei (PMP) is an uncommon clinical syndrome characterized by the slow and progressive accumulation of peritoneal implants and mucinous ascites, intraabdominal gelatinous collection (jelly belly). PMP is reported to originate from the low grade appendiceal mucinous neoplasm. Pseudomyxoma peritonei often recurs after treatment and may eventually cause death by abdominal visceral dysfunction via compression with mucinous ascites. Cytoreductive surgery (CRS) and perioperative intraperitoneal chemotherapy comprising hyperthermic intraperitoneal chemotherapy (HIPEC) with or without postoperative intraperitoneal chemotherapy (EPIC) has been regarded as the standard of care in specialized centers. It is often referred to as the „Sugerbaker procedure“. Conventionally, PMP is considered resistant to systemic chemotherapy. Even though complete cytoreduction is associated with prolonged overall survival, recurrence of disease is common and multiple operations are frequently required. Patients may enjoy sustained period of remission, free of symptoms, but long – term disease free survival is distinctly uncommon. The pathologic subtype remains the dominant factor in survival. - Cieľ: Práca má design prierezovej, deskriptívnej štúdie, ktorej cieľom bolo zhodnotiť, aký je vzťah medzi psychickou záťažou, výskytom depresívnej symptomatológie a subjektívnou pohodou u sestier. Metodika: Vzorku respondentov tvorilo 78 sestier pracujúcich v NsP v Levoči a Spišskej Novej Vsi. Na meranie psychickej záťaže bol použitý Meisterov dotazník neuropsychickej záťaže, na meranie výskytu depresie skríningový nástroj Patient Health Questionnaire 9 (PHQ 9) a na meranie subjektívnej pohody Personal Well-Being Index (PWI) pre zdravú dospelú populáciu. Výsledky: Bol potvrdený významný vzťah medzi psychickou záťažou, výskytom depresívnej symptomatológie a subjektívnou pohodou u sestier. Vyššia frekvencia výskytu depresívnej symptomatológie sa u nich spájala s vyššou psychickou záťažou a nižšou subjektívnou pohodou. Významným a zároveň alarmujúcim zistením bolo, že len 28 % sestier uvádzalo nevýznamnú depresívnu symptomatológiu. Až u 50 % sestier bola podľa PHQ 9 prítomná ľahká depresia. Štatisticky významné korelácie boli identifikované medzi všetkými tromi faktormi Meisterovho dotazníka, ako aj jeho hrubým skóre a PHQ 9. Najsilnejšie korelácie boli medzi Faktorom I (preťaženie) a PHQ 9. Záver: Výsledky poukazujú na skutočnosť, ţe práve aspekty pracovného preťaţenia (časová tieseň, problémy a konflikty, vysoká zodpovednosť) môžu u sestier viesť ku častejšiemu prežívaniu jednotlivých symptómov depresie (predovšetkým k únave a zníženému záujmu). V ďalšom výskume navrhujeme sa zamerať na sledovanie vzťahu troch premenných: depresia, burnout a psychická záťaž sestier., Aim: The study has a design of descriptive cross-sectional survey. The aim of this study was to investigate relationship between psychological load, depressive symptomatology and subjective well-being of nurses. Methods: Sample of respondents consisted of 78 nurses from two public hospitals (Levoča and Spišská Nová Ves). To measure the psychological load was used the Meister questionnaire of neuro-psychological load. To measure the incidence of depression was used the screening tool Patient Health Questionnaire 9 (PHQ 9). To measure subjective well-being was used Personal Well-Being Index (PWI) for a healthy adult population. Results: The results indicated that there is a significant correlation between psychological load, depressive symptomatology and subjective well-being of nurses. The higher frequency of depressive symptomatology was associated with higher level of psychological load and lower subjective well-being level. The depressive symptomatology has a significant relation with neuropsychological strain and subjective well-being. Important and at the same time alarming finding was that only 28% of nurses reported insignificant depressive symptomatology. According to PHQ 9 was present mild depression in up to 50% of nurses. Statistically significant correlations were identified between all three factors of Meister questionnaire, as well as his gross score and the PHQ 9. The strongest correlations were between Factor I (overload) a PHQ 9. Conclusion: The results show that mainly aspects of work overload (time constraints, problems and conflicts, high accountability) can lead in nurses to more frequent survival of individual symptoms of depression (especially the fatigue and reduced interests). - V naší kazuistice prezentujeme případ 11leté pacientky s akutní bolestí bederní páteře, pánve a proximální části levého stehna. Třífázová scintigrafie skeletu s nálezem zvýšené akumulace 99mTc-hydroxydifosfonátu v levém sakroiliakálním skloubení napomohla k diagnóze levostranné septické sakroileitidy. Hemokultivace byla pozitivní, etiologicky byl zjištěn Staphylococcus aureus. Parenterální antibiotická terapie vedla k poklesu zánětlivých parametrů, snížení bolestivosti. Pro zformování abscesu mezi levou lopatou kosti kyčelní a musculus iliacus byla provedena v celkové anestézii jeho evakuace. - Multiple sclerosis (MS) is the most common cause of nontraumatic neurological disability in Europe and North America. Growth factor expression could participate in the repair process of the demyelinating disease. Among growth factors, brain derived neurotrophic factors (BDNF) has been demonstrated to play an important role in neuronal and axonal survival. In the central nervous system (CNS ), neurons are the main source of BDNF. Another potential source are activated astrocytes, which are present in inflamed areas in the CNS as shown in MS. In this study, total protein concentration (TPC) and BDNF levels in the cerebrospinal fluid (CS F) samples from the patients with MS (n = 48) and control subjects (n = 53) were measured using a Bio-Rad protein assay and enzyme linked immunosorbent assay (ELISA). No significant change in the CS F TPC of patients with MS was seen as compared to normal CS F. The presence of BDNF in the CS F samples was shown by Western blot. Using ELISA , it was shown that the level of BDNF in the MS CS F is higher than in normal CS F. It is concluded that BDNF is a constant component of human CS F. - We present a 73 year old female with intractable pruritus and nonspecific cutaneous rash for a period of 9 months. She had recieved symptomatic therapy with no improvement. A complete examination revealed axillary and abdominal lymphadenopathy. A biopsy confirmed the diagnosis of Hodgkins lymphoma with Langerhans cell histiocytosis. She received 5 cycles of chemotherapy with resolution of pruritus and reduction in axillary and abdominal lymphadenopathy. The patient presented 6 months later with relapse and succumbed to the illness. Simultaneous occurrence of Langerhans cell histiocytosis and Hodgkins lymphoma may lead to misdiagnosis. - Periprosthetic fractures are the third most common reason for revision total hip arthroplasty. Surgical treatment of periprosthetic fractures belongs to the most difficult procedures due to the extensive surgery, elderly polymorbid patients and the high frequency of other complications. The aim of this study was to evaluate the results of operatively treated periprosthetic femoral fractures after total hip arthroplasty. We evaluated 47 periprosthetic fractures in 40 patients (18 men and 22 women) operated on between January 2004 and December 2010. The mean follow-up period was 27 months (within a range of 12-45 months). For the clinical evaluation, we used modified Merle d'Aubigné scoring system. In group of Vancouver A fractures, 3 patients were treated with a mean score of 15.7 points (good result). We recorded a mean score of 14.2 points (fair result) in 6 patients with Vancouver B1 fractures, 12.4 points (fair result) in 24 patients with Vancouver B2 fractures and 12.7 points (fair result) in 7 patients with Vancouver B3 fractures. In group of Vancouver C fractures, we found a mean score of 16.2 points (good result) in 7 patients. Therapeutic algorithm based on the Vancouver classification system is, in our opinion, satisfactory. Accurate differentiation of B1 and B2 type of fractures is essential. Preoperative radiographic images may not be reliable. If in doubt, checking the stability of the prosthesis fixation during surgery should be performed. and M. Korbel, P. Sponer, T. Kucera, E.
