Cíl: Přezkoumání benefitů metody klokánkování pro nedonošené novorozence, to znamená najít dostatek důkazů pro opodstatnění používání metody klokánkování na Jednotce intenzivní péče pro novorozence. Metodika: Vyhledávání validních indikátorů pozitivního vlivu metody klokánkování prostřednictvím metody praxe založené na důkazech. Pro získání relevantních zdrojů byly využity licencované databáze a volně přístupné databáze - Medline, Bibliomedica, Nursing: Best Evidence for Nursing care, Google Scholar. Výsledky: Analýza výsledků prokázala jednoznačně pozitivní vliv metody klokánkování na nedonošené novorozence umístěné na Jednotce intenzivní péče, a to zejména v oblasti vnímání bolesti u invazivních výkonů, efektivního kojení, zvýšení váhového přírůstku, delšího a klidnějšího spánku, pozitivnější interakce matka – dítě, udržení nebo zvýšení tělesné teploty, zkrácení doby hospitalizace, růstu do délky, růstu obvodu hlavičky a aktivace centrální nervové soustavy. Závěry: Výzkumné studie (n = 34) a jeden systematický přehled ukazují pozitivní vliv metody klokánkování na nedonošeného novorozence umístěného na Jednotce intenzivní péče v oblastech biopsychosociálních. V podmínkách českého zdravotnictví chybí vypracované platné standardy pro využívání metody v praxi., Objective: To review benefits of kangaroo care method for premature babies, it means finding enough evidence to justify using the kangaroo care method in the Intensive care unit for newborns. Methods: Searching for valid indicators of a positive influence method of kangaroo care through method evidence-based practice. To obtain relevant sources were used licensed databases and freely available databases - Medline, Bibliomedica, Nursing: Best Evidence for Nursing care, Google Scholar. Results: Analysis of results showed a clearly positive impact of the kangaroo care methods on premature infants placed in intensive care, especially in the perception of pain in invasive procedures, effective breastfeeding, increased weight gain, longer and quieter sleep, more positive interactions of mother - child, maintaining or increasing body temperature, reduced hospitalization, growth in length, head circumference growth and activation of the CNS. Conclusions: The research studies (n = 34) and one systematic review have shown a positive influence of the kangaroo care methods on premature newborns placed in intensive care in the areas of bio-psycho-social. Úvod: Existuje mnoho studií věnujících se problematice biliárních komplikací po jaterních resekcích. Přes zlepšení operační techniky a perioperační péče má incidence této komplikace spíše vzrůstající charakter. Byly analyzovány četné prediktivní faktory. Na jejich vliv na vznik biliárního leaku doposud nepanuje shoda. Za cíl analýzy jsme si stanovili zhodnotit incidenci biliárního leaku, jeho vliv na mortalitu a na dobu hospitalizace na našem pracovišti. Současně jsme provedli rozbor známých prediktivních faktorů. Metoda: Autoři retrospektivně analyzují soubor 146 nemocných, u kterých byla na Chirurgické klinice 2. LF Univerzity Karlovy a ÚVN, Praha v období 2010–2013 provedena jaterní resekce. K vyhodnocení biliárního leaku jsme užili současnou uznávanou klasifikaci ISGLS (International Study Group of Liver Surgery). Závažnost komplikace byla stanovena dle Clavien-Dindo. Statistickou významnost prediktivních faktorů jsme určili pomocí Fisherova exaktního testu a Studentova t-testu. Výsledky: Incidence biliárního leaku byla 21 %. Dle ISGLS bylo 6,5 % pacientů zařazeno do skupiny typu A, v 61,2 % se jednalo o leak B a v 32,3 % o leak typu C. Stupeň závažnosti dle Clavien-Dindo I-II, IIIa, IIIb, IV a V byl v poměru 19,3 %, 42 %, 9,7 %, 9,7 %, resp. 19,3 %. Jako statisticky významné jsme stanovili tyto faktory: operaci pro malignitu (p<0,001), velkou jaterní resekci (p=0,001), dobu operace (p<0,001), vyšší peroperační krevní ztrátu (p=0,02), konstrukci HJA (p=0,005), portální venózní embolizaci/two-stage chirurgii (p=0,009) a ASA skóre (p=0,02). Biliární leak významně prodloužil dobu hospitalizace (p<0,001). V souboru pacientů s biliárním leakem byla 23krát vyšší perioperační mortalita (p<0,001) než v souboru bez leaku. Závěr: Biliární leak je jednou z nejzávažnějších komplikací jaterních resekcí. Většina rizikových faktorů je těžko ovlivnitelných a na jejich vliv nepanuje jednoznačná shoda. Incidenci by mohly redukovat diskutované peroperační leak testy. V budoucnu bude třeba dále pracovat na zlepšení perioperačního managementu a techniky k prevenci vzniku této závažné komplikace. V léčbě se uplatňuje multidisciplinární přístup., Introduction: Many previous reports have focused on bile leakage after liver resection. Despite the improvements in surgical techniques and perioperative care the incidence of this complication rather keeps increasing. A number of predictive factors have been analyzed. There is still no consensus regarding their influence on the formation of bile leakage. The objective of our analysis was to evaluate the incidence of bile leakage, its impact on mortality and duration of hospitalization at our department. At the same time, we conducted an analysis of known predictive factors. Method: The authors present a retrospective review of the set of 146 patients who underwent liver resection at the Department of Surgery of the 2nd Faculty of Medicine of the Charles University and Central Military Hospital Prague, performed between 2010−2013. We used the current ISGLS (International Study Group of Liver Surgery) classification to evaluate the bile leakage. The severity of this complication was determined according to the Clavien-Dindo classification system. Statistical significance of the predictive factors was determined using Fisher‘s exact test and Student‘s t-test. Results: The incidence of bile leakage was 21%. According to ISGLS classification the A, B, and C rates were 6.5%, 61.2%, and 32.3%, respectively. The severity of bile leakage according to the Clavien-Dindo classification system – I-II, IIIa, IIIb, IV and V rates were 19.3%, 42%, 9.7%, 9.7%, and 19.3%, respectively. We determined the following predictive factors as statistically significant: surgery for malignancy (p<0.001), major hepatic resection (p=0.001), operative time (p<0.001), high intraoperative blood loss (p=0.02), construction of HJA (p=0.005), portal venous embolization/two-stage surgery (p=0.009) and ASA score (p=0.02). Bile leakage significantly prolonged hospitalization time (p<0.001). In the group of patients with bile leakage the perioperative mortality was 23 times higher (p<0.001) than in the group with no leakage. Conclusion: Bile leakage is one of the most serious complications of liver surgery. Most of the risk factors are not easily controllable and there is no clear consensus on their influence. Intraoperative leak tests could probably reduce the incidence of bile leakage. In the future, further studies will be required to improve the perioperative management and techniques to prevent such serious complications. Cieľ: Cieľom štúdie bolo posúdiť bolesť, funkčný stav, úzkosť, depresiu, sociálne aktivity a sociálnu oporu u seniorov s reumatoidnou artritídou (RA) a ich vzájomné vzťahy, posúdiť možné rozdiely vzhľadom na pohlavie, rodinný stav, vek, trvanie ochorenia a komorbiditu. Metódy: U 60 seniorov s RA bola posudzovaná intenzita bolesti vizuálnou analógovou škálou (VAS), funkčná disabilita Stanfordským dotazníkom hodnotiacim zdravie indexom disability (HAQ-DI), úzkosť Beckovým inventárom úzkosti (BAI), depresia Zungovým sebahodnotiacim dotazníkom depresie (SDS) a sociálna interakcia škálou sociálnych aktivít (SA) a sociálnej opory (SO) nástroja Škály merania vplyvu chorôb kĺbov 2 (AIMS2). Výsledky: U seniorov s RA bolo zistené pociťovanie priemerne silnej bolesti, stredná až ťažká funkčná disabilita, prežívanie stredne silnej úzkosti a depresie, obmedzovanie v spoločenských aktivitách. Vo vnímaní sociálnej opory bolo zistené najlepšie skóre. Medzi sledovanými premennými boli Pearsonovou koreláciou zistené väčšinou signifikantné (P ≤ 0,05) kladné slabé až silné vzťahy. Medzi pohlaviami neboli zaznamenané rozdiely. Horšie skóre (P ≤ 0,05) bolo zistené v oblasti prežívania úzkosti a vo vnímanej sociálnej opore u osamelo žijúcich seniorov oproti seniorom žijúcim v manželskom zväzku. Štatisticky významný pozitívny vzťah (P ≤ 0,05) sa potvrdil medzi vekom a prežívaním úzkosti, medzi trvaním ochorenia a depresiou, medzi komorbiditou a funkčným stavom a depresiou. Záver: Ako naznačujú výsledky, prioritou v starostlivosti o túto skupinu seniorov by nemalo byť iba zmierňovanie bolesti a zlepšovanie/udržiavanie funkčných schopností, ale aj redukcia úzkosti a depresie, a to o to viac, čím sú seniori starší, čím u nich trvá ochorenie dlhšie, s komorbiditou a tiež u osamelo žijúcich., Aim: The aim of this study was to assess pain, functional status, anxiety, depression, social activity, and social support and their interactions in older adults with rheumatoid arthritis (RA), and to assess possible differences by gender, marital status, age, duration of disease, and co-morbidity. Methods: Pain intensity was assessed by the Visual Analogue Scale (VAS), functional disability by the Stanford Health Assessment Questionnaire – Disability Index (HAQ-DI), anxiety by the Beck Anxiety Inventory (BAI), depression by the Zung Self-rating Depression Scale (SDS), social activity by the Arthritis Impact Measurement Scales 2 (AIMS2) with scale of Social Activity (SA), and social support (SO) by AIMS2 with scale Support from family and friend in 60 elderly with RA. Results: There were found average strong pain, moderate to severe functional disability, moderate anxiety and moderate-to-marked depression, and limitation in social activity in elderly with RA. The best score was found in scale of social support. Significant (p ≤ 0.05) mild to severe relations were confirmed between explored objectives by Pearson’s correlations. Between genders no significant differences were recorded. Worse score (p ≤ 0.05) was detected in anxiety and social support in elderly living alone compared to married elderly. Between age and anxiety, duration of disease and depression, co-morbidity and functional disability, and depression were confirmed significant (p ≤ 0.05) positive relationships. Cíl: Cílem práce bylo shromáždit dostupná klinická, radiologická a patologická data o kalcifikujícím pseudotumoru nervové osy (CAPNON). Soubor: Soubor je tvořen kazuistikou a současně přehledem všech pacientů publikovaných v anglické literatuře, vyhledaných v databázi PubMed pomocí klíčových slov: calcifying pseudoneoplasm, calcifying pseudotumour, brain stone, cerebral calculi a fibro‑osseous lesion. Zahrnuty jsou taktéž všechny citace případů v nalezených publikacích. Výsledky: Včetně naší pacientky jsme v literatuře nalezli celkem 72 pacientů. Intrakraniálně bylo lokalizováno 62 % těchto lézí, v páteřním kanále 31 % a v kranio‑cervikálním přechodu zbylých 7 %. Průměrný věk výskytu je 45,6 (rozmezí 2–83) let. U intrakraniální lokalizace byla tato léze nalezena supratentoriálně v 74 % a infratentoriálně v 26 %. Z nejčastějším projevů intrakraniálních CAPNON jsme nalezli epilepsii v 33,3 %, bolesti hlavy v 24 %, nebo některý z příznaků zadní jámy v 16,7 %. U pacientů s epilepsii byl CAPNON nalezen v temporálním laloku v 50 %. U CAPNON nalezených v páteřním kanále byla dominantním projevem bolest v 82 %, následovaná poruchou chůze v 27 %. V kranio‑cervikálním přechodu byla dominantním příznakem bolest v 80 %. Závěr: CAPNON je velmi vzácnou, benigní, pomalu rostoucí lézí centrální nervové soustavy. Ve většině případů ji nalezneme intrakraniálně s lehkou převahou u mužského pohlaví. Původ této léze není zcela jasný. Diagnóza CAPNON by měla být zvažována setkáme‑li se s nálezem solidně kalcifikované léze při vyšetření CT mozku a hypodenzní léze v T1 a T2 obraze při vyšetření MR. Léčebnou modalitou je radikální odstranění této léze. Incidentální léze musí být monitorovány, jelikož mohou růst a stát se symptomatickými. Redakční rada potvrzuje, že rukopis práce splnil ICMJE kritéria pro publikace zasílané do biomedicínských časopisů., Aim: The aim of this study was to summarize clinical, radiological and pathological data on calcifying pseudoneoplasm of the neural axis (CAPNON). Methods: A case report of a female patient with CAPNON is presented. In addition, all CAPNON patient case histories published in English language are included. Articles were identified via PubMed searches using the following key words: calcifying pseudoneoplasm, calcifying pseudotumour, brain stone, cerebral calculi and fibro‑osseous lesion. All cases referenced within the identified publications are also included in this review. Results: Seventy two cases were identified, including our case report. Of these, 62% were intracranial, 31% spinal and 7% were located in the cranio‑cervical junction (CCJ). The mean age of the CAPNON patients was 45.6 (range 2–83) years. 74% of intracranial CAPNON were supratentorial and 26% were infratentorial. Symptoms of intracranial CAPNON included epilepsy in 33.3%, headaches in 24% and posterior fossa symptoms in 16.7%. The majority of patients with epileptic seizures had lesions in the temporal lobe (50%). Pain was the dominant symptom in 82% and gait disorder in 27% of spinal CAPNON cases. Pain was the dominant symptom (80%) in CCJ CAPNON. Conclusion: CAPNON is a rare, benign, slowly‑growing lesion of the central nervous system. In the majority of cases, location is intracranial and there is a slightly higher prevalence in men. The origin of CAPNON remains unclear. CAPNON should be considered whenever a CT scan reveals a calcified lesion combined with hypointensity on T1 and T2-weighted MRI. Radical removal is the treatment of choice. V terapii meningeomů na prvním místě přetrvává chirurgická léčba. Cílem chirurgické léčby je úplné odstranění nádoru, které často znamená úplné vyléčení pacienta. Prognózu pacienta určuje především radikalita resekce, dále také anatomické umístění a histologický typ nádoru dle WHO klasifikace. Některé meningeomy, zejm. meningeomy baze lební, které často utiskují nebo obrůstají velké cévy a hlavové nervy, a dále potom meningeomy, které histologicky odpovídají WHO st. III, jsou velmi obtížně léčitelné. Nezanedbatelným faktorem je i celkový stav a interní komorbidity. Autoři uvádějí nejčastější typy obtížně chirurgicky řešitelných meningeomů a popisují zkušenosti svého pracoviště s chirurgickým odstraněním, další léčbou a sledováním pacientů s těmito nádory., Surgical treatment is still considered as the method of choice for the treatment of meningiomas. The goal of treatment is to completely surgically remove the tumor; complete excision often results in a permanent cure of the patient. The patient prognosis is mainly determined by the completeness of the resection but anatomical location and histological type of the tumor according to the WHO classification also play an important role. Some meningiomas, especially the skull base meningiomas, often involving large vessels and cranial nerves, as well as WHO grade III meningiomas are very difficult to treat. Patient general health status and internal comorbidities represent another very important factor. The authors describe the most common types of surgically difficult-to-treat meningiomas and describe their experience with surgical removal and additional treatment and surveillance of patients with these tumors. Assaultive acts committed by people with a mental illness is a major public health issue that affects patients with their families, law enforcement authorities, and the public at large. Failure to provide treatment is in fact a major predictor of assaultive acts in patients with schizophrenia living in the community. Considering that the indigenous ethnic groups of Central Asia have similar sociocultural characteristics, these factors may be reflected in individuals with schizophrenia who have committed serious assaultive acts in Uzbekistan. Objectives: The aim of the work was to identify the sociocultural and clinical characteristics of schizophrenic representatives of indigenous ethnic groups of Central Asia who have committed violent crimes in Uzbekistan and have been found insane in regard to their offence, and to compare these subjects to ones belonging to the other ethnic groups. Material and methods: The data were collected in 2010–2013 in the Tashkent High Security Psychiatric Hospital via face-to-face interviews and also from the patients’ charts and from forensic psychiatric examination statements. Results: The sample consisted of 201 individuals. The sample was 90.1 percent (n = 181) male, with a predominance of the paranoid schizophrenia subtype according to the ICD-10 criteria. Of the subjects, 174 ones (86.6%) were representatives of the indigenous ethnic groups of Central Asia, and 27 ones (13.4%) were representatives of other ethnic groups. The duration of illness among the subjects belonging to the indigenous ethnic groups of Central Asia was less than in the other group; the individuals were rarely referred to psychiatric care because of the popularity of alternative medicine and the stigma attached to mental illness. A positive correlation between violence and various psychotic symptoms, such as delusions, hallucinations, and thought disorder, has also been demonstrated in this group. The aim of our study was to determine the significance of lymphocyte-platelet adhesion (LPA), interleukins, transforming included 139 pregnant women aged between 17 and 27 years (21.3±4.22 years). GH was diagnosed in 119 women after 20 weeks of pregnancy. 20 patients (control group) were with physiological course of pregnancy. The distribution of patients by groups was carried out according to the level of blood pressure (BP) in accordance with ICD-10 (Geneva, WHO, 2002). The survey was conducted at the moment of detection pregnancy from 7 to 10 weeks and in dynamics of I, II and III trimesters of gestation. In the dynamics of gestation, were studied the number of desquamated endothelial cells circulating in the systemic circulation (CECs), nitrates levels, the adhesion of platelets by estimation their ability to form co-aggregates with lymphocytes by determining the percentage of lymphocytes aggregates with thrombocytes (lymphocyte-platelet plugs), at risk of hypertensive disorders, especially after 20-22 weeks of gestation and later, lymphocytes ability to platelets adhesion is rose, the concentrations of pro-inflammatory cytokines and NO level are increased. Comparative assessment of clinical efficiency of omeprazole and rabeprazole in treating gastroesophageal reflux disease (GERD). 65 patients with a verified diagnosis GERD were examined. Comparative clinical investigations of using of omeprazole and rabeprazole have revealed effectiveness of both drugs in the therapy of GERD. Retrospektivní studie u žen do 49 let analyzovala novotvary následující po primárních nádorech cervixu (CC), dělohy (CU), vaječníků (CO) a primární novotvary před těmito gynekologickými nádory (GC), hlášené v Národním onkologickém registru ČR v letech 1976–2010. Celkem 3 727 pacientek těchto tří diagnóz představovalo 12,1 % z 30 794 nově evidovaných GC, spojených s 4 160 dalšími novotvary. Jejich vývoj se týkal a) 1 339 žen s primárním CC, 715 s CU, 664 s CO (s následnými 1 509 a 831, respektive 758 dalšími novotvary), b) 263 žen s následným CC, 212 s CU, 534 s CO (s předcházejícími 274 a 223, respektive 565 primárními novotvary). Průměrný interval vzniku následných novotvarů byl 14,2 roku po CC, 13,6 roku po CU a 10,2 roku po CO. Nejvíce z 3 098 následných nádorů bylo 21 % trávicího traktu, 16 % prsů, 13 % kůže, 11,7 % rodidel, 9,7 % dýchacích a 7,8 % močových cest; z nich během prvního roku po primárním GC bylo nejvíce nádorů vaječníků, endometria, cervixu a kolorekta. Nejvíce z 1 063 primárních novotvarů před GC bylo 26,7 % rodidel, 24,3 % prsů a 14,8 % trávicího traktu. Během 35 let počet žen s primárními GC klesal, zejména v zastoupení jejich časných klinických stadií, žen s následnými GC rostl, zejména jejich pokročilých stadií. Časná stadia byla evidována u 70,4 % žen s primárními GC a 54 % s následnými GC, pokročilá stadia u 29,5 % žen s následnými GC a 13,3 % s primárními GC, neznámá stadia zahrnovala asi 16,5 %. Ze studie se odhaduje asi 1100 případů pokročilých stadií u žen do 49 let přežívajících s GC v letech 1976–2010. Většina z hodnocených kritérií byla nepříznivá u žen s karcinomy ovarií. Zejména těmto případům v pokročilých stadiích by měly zabránit dispenzární opatření a preventivní intervence. Omezené finanční zdroje a přetrvávající rizika životního stylu neumožňují zpomalit v blízké budoucnosti nárůst primárních a následných novotvarů u onkologicky přežívajících žen nejen v gynekologii., The neoplasms following primary cancers of cervix (CC), uterus (CU), ovary (CO), and primary neoplasms before these gynaecological cancers (GC), were analyzed in a retrospective study in survived females aged 0–49 years, notified in the National Czech Cancer Registry between 1976 and 2010. A total 3,727 patients of these three diagnoses, presented 12.1 % of 30,794 newly registered GC, were associated with 4,160 other neoplasms. Their development was concerned with a) 1,339 females with primary CC, 715 with CU and 664 with CO (followed by 1,509 and 831, respectively 758 other neoplasms) and b) subsequent 263 CC, 212 CU and 534 CO (preceded by 274 and 223, respectively 565 primary neoplasms). The average interval of occurrence of subsequent neoplasms was 14.2 years after CC, 13.6 years after CU and 10.2 years after CO. The most frequent of 3.098 subsequent neoplasms were 21 % of digestive tract, 16 % breast, 13 % skin, 11.7 % female genital, 9.7 % respiratory and 7.8 % urinary organs; the most frequent during the first year after primary GC were those of the ovarial, endometrial, cervical and colorectal cancers. The most frequent of 1,062 primary neoplasms before GC were 26.7 % cancers of female genital organs, 24.3 % breast and 14.8 % digestive tract. During 35 years the number of females with primary GC has decreased, especially of their representation at the early clinical stages, the females with subsequent GC has increased, especially at the advanced stages. There were recorded early stages in 70.4 % females with primary GC and 54 % with subsequent GC, advanced stages in 29.5 % females with subsequent GC and 13.3 % with primary GC, the unknown stages included about 16.5 %. In our study are estimated about 1100 cases of advanced stages in females aged 49 years survived with GC in 1976–2010. Most of the evaluated criteria were unfavorable among females with ovarian cancers. Mainly the cases at advanced stages must be prevented by the dispensary guidelines and preventive interventions. V moderní společnosti dochází k postupnému zkracování délky spánku v důsledku celé řady faktorů. Epidemiologické studie v posledním desetiletí potvrdily, že spánek kratší než 7 hodin představuje rizikový faktor pro rozvoj obezity, diabetu mellitu 2. typu, hypertenze, kardiovaskulárních onemocnění, depresí a zvyšuje riziko mortality. Cílem této práce bylo zhodnotit délku spánku a posoudit vztah mezi délkou spánku a BMI u dospělé populace (n=478, 18–65 let). Metoda sběru dat zahrnovala 7denní záznam o délce spánku za den a antropometrická měření. Z výsledků vyplynulo, že průměrná délka spánku v souboru činila 7,45 (±0,91) hodin za den. 28 % dospělých osob přesto spalo méně než doporučených 7 hodin denně. Statisticky nejméně spali jedinci ve věku 40–49 let (průměr 7,2±0,80 hodin). Riziko obezity definované jako BMI ≥30 kg/m2 v souboru stoupalo se zkracující se délkou spánku: pro kategorie spánku [<6,3 h], [≥6,3 až <8,3 h] a [≥8,3 h] bylo riziko obezity na základě relativních četností vyjádřeno poměrem 2 : 1,5 : 1,2. Jedinci spící průměrně <6,3 hodiny vykazovali 1,3krát vyšší riziko obezity než ostatní jedinci spící déle i při zohlednění faktoru věku, tento závěr však není statisticky významný (OR=1,256; 95% IS: 0,631; 2,422). In modern society there is a gradual shortening of the length of sleep due to a number of factors. In the last decade a number of epidemiological studies suggest that sleeping less than 7 hours a day increases the risk of developing obesity, type 2 diabetes mellitus, hypertension, cardiovascular diseases, and depression and is associated with premature mortality. The aim of this study was to evaluate sleep duration and to assess the relationship between sleep duration and BMI in the adult population (n=478, 18 to 65 years). The method of data collection consisted of a 7-day record of the length of the sleep per day and anthropometric measurements. The results showed that the average sleep duration was 7.45 (±0.91) hours per day. However, 28% of adults slept less than a recommended 7 hours a day. The individuals aged 40–49 years slept statistically the shortest time (mean 7.2±0.80 hours). The risk of obesity increased indirectly with duration of sleep in this population, for sleep categories [<6.3 h], [≥6.3 to <8.3 h] and [≥8.3 h] the risk of obesity defined as BMI ≥30 kg/m2 was 2 : 1.5 : 1.2 based on the relative frequencies of obese and non-obese participants. Individuals sleeping on average <6.3 hours were at 1.3 times higher risk of obesity than other individuals sleeping longer after adjusting for age, this conclusion was not statistically significant (OR=1.256, 95% CI: 0.631; 2.422).
