V současnosti umírá v České republice ročně asi 110 000 osob. Ve více než 80 % úmrtí se jedná o osoby v geriatrickém věku. Proto je nezbytné se seriózně zabývat problematikou paliativní a terminálni péče v geriatrii. V přehledném článku autorka definuje obsah a náplň geriatrické paliativní péče a její hlavní charakteristiky. Na vlastních výsledcích demonstruje vysoký výskyt obtěžujících symptomů a jejich nedostatečné zvládnutí u geriatrických pacientů s terminálni prognózou. V závěru se zamýšU nad možnostmi zlepšení současné situace. Je třeba identifikovat bariéry v českém systému zdravotní a sociální péče a cílevědomě usilovat o jejich odstraňování na úrovni systémově, jednodivých poskytovatelů péče i konkrétních pečovatelů., Currently, the yearly death rate reaches 110 000 inhabitants in the Czech republic. More than 80 % of deaths occurs in geriatric age. Therefore it seems unavoidable to deal seriously with the palliative and end-of-life care m geriatrics. In this review article the author defines content of geriatric palliative care and its main characteristics. Based on her own results she demonstrates high occurrence of distressing symptoms and its inadequate management in geriatric patients with terminal prognosis. In conclusion she is rethinking possibilities of improvement of the current situation. Chronická bolest ve stáří má svoje specifika, související s involucí a degenerativními změnami. Nádorová i nenádorová bolest je stále podceňována, léčbě je věnována nedostatečná pozornost, což vede k jejímu trvání nebo k častým komplikacím z nesprávného a nesystémového léčení chronického bolestivého stavu. Farmakoterapie je základem komplexní, multidisciplinární léčby bolesti. Volba analgetik se řídí intenzitou bolesti, její patofyziologií a fyzickým stavem nemocného. Základním lékem mezi neopioidními analgetiky je paracetamol. Nesteroidní antirevmatika (NSA) jsou spojena s vyšším rizikem nežádoucích účinků, zvláště v kombinaci s kortikoidy nebo antikoagulancii. Postižení gastrointestinálního traktu z NSA je u starých pacientů významně častější než v mladším věku. Bezpečnější jsou selektivní antirevmatika, koxiby (celekoxib, rofekoxib, valdekoxib). Pro silnější bolesjsou indikované slabé opioidy, zastoupené kodeinem, resp. dihydrokodeinem a tramadolem. Pokud jsou silné opioidy typu morfinu nebo fentanylu správně indikované a dávkování přizpůsobeno stavu pacienta, pak jsou bezpecné i ve stáří. Neuropatická bolest a doprovodná úzkost či deprese vyžadují použití koanalgetik, adjuvancií, jako jsou antidepresiva a antikonvulziva. Významnou roh ve spojení s bolestí ve stáří hrají psychosociální faktory., Chronical pain in elder age has its specific features, related to the involution and to the degenerative changes. The tumour pain as well as non-tumour pain are still underestimated, an insufficient attention is paid to the therapy. which leads to its duration and frequent complications related to incorrect and non-systematic treatment of chronical pain condition. Pharmacotherapy is a basis of complex, multidisciplinary therapy of pain. The choice of analgesics is lead by the pain intensity, its pathophysiology and the physical condition of the patient. The basic drug among the nonopioid analgesics is paracetamol. Nonsteroid antireumatics (NSA) are connected with a higher risk of adverse effects, especially in combination with corticoids or anticoagulatives. The frequency of occurence of gastrointestinal tract affection caused by NSA is signihcantly higher in elder patients than in younger age. Selective antíreumaúcs, coxibs (celecoxib, rofecoxib) are safer. Weak opioids, represented by codein or dihydrocodein and tramadol are indicated for stronger pain. If strong opioids morphine or phentanyl type are indicated correctly and the dosing is adjusted to the patient's condition, then they are safe also in elder age. Neuropatic pain and associated anxiety or depression require the use of coanalgesics, adjuvantive therapy as antidepressives and anticonvulsives.
