Karcinom prsu v diseminovaném stadiu je nevyléčitelné onemocnění s definovanou dobou přežití, která v případě viscerálních metastáz dosahuje jen několika měsíců. U pacientek s viscerálním postižením, symptomatických a hormonálně nedependentních je základem léčby chemoterapie. U pacientek HER2 negativních (resp. triple negativních) má své místo antiangiogenní léčba bevacizumabem, která je schválena v I. linii léčby metastatického HER2 negativního karcinomu prsu v kombinaci s paclitaxelem, a to na základě významného prodloužení PFS ve studii III. fáze E2100 (1). Kazuistika: 53letá pacientka onemocněla triple negativním karcinomem prsu v roce 2008. Absolvovala neoadjuvantní chemoterapii, radikální mastektomii s exenterací axilly s pooperačním stagingem ypT1a ypN1. Následovala adjuvantní chemoterapie, ozáření. V květnu 2012 prokázány metastázy v plicích a v mediastinálních uzlinách dle CT. Provedena bronchoskopie a histologicky verifikovány metastázy duktálního karcinomu, triple negativního, dle FISH nebyla nalezena amplifikace genu HER2. Indikována ke kombinované léčbě paclitaxel + bevacizumab. Došlo ke kompletní remisi onemocnění. Léčba samotným bevacizumabem pokračuje dosud. Pacientka přežívá od zjištění generalizace dosud 43 měsíců! Kvalita života je výborná., Metastatic breast cancer is an incurable disease with a defined time of survival. This time is limited to only a few months when visceral metastases are present. Chemotherapy is the crucial treatment for patients with visceral involvement, symptomatic and hormone-independent disease. There is a role as well for an antiangiogenic therapy with bevacizumab in HER2-negative disease. Bevacizumab is approved for the treatment of metastatic HER2-negative breast cancer in combination with paclitaxel. This approval is based on significant improvement in PFS reported in phase III trial E2100. Case report: 53-years old patient, female, was diagnosed with triple-negative breast cancer in 2008. Chemotherapy was delivered in neoadjuvant setting, followed by radical mastectomy with dissection of axilla. Pathological staging was ypT1a ypN1. Adjuvant chemotherapy was applied as well as radiation therapy. In May 2012 the evidence of metastatic disease in lung and mediastinal lymph nodes was proved by CT scan. Triple negative breast cancer was verified from bronchology exam. FISH analysis of HER2 was also negative. Combination treatment with paclitaxel + bevacizumab was indicated. Jako Bouveretův syndrom je označována obstrukce v oblasti duodena impaktovaným žlučovým kamenem, který vycestuje cholecystoduodenální či cholecystogastrickou píštělí. Onemocnění je vzácné, vyskytuje se zejména u starších žen s četnými komorbiditami a vysokým operačním rizikem. K diagnóze se zpravidla dospěje endoskopicky nebo pomocí zobrazovacích metod. Pokud stav nelze řešit endoskopicky, je indikováno operační řešení. Autoři prezentují případ Bouveretova syndromu u 79leté pacientky, u níž se zdařilo zdokumentovat všechny fáze vyšetřovacího a léčebného procesu. Součástí sdělení je přehled literatury týkající se diagnostiky a terapie tohoto vzácného syndromu., Bouveret syndrome is a gastric outlet obstruction caused by impaction of a gallstone that passes through a cholecystoduodenal or cholecystogastric fistula. It is a rare disease, most common in elderly women with multiple comorbidities and high surgical risk. The diagnosis can be made either radiologically or endoscopically. Endoscopic extraction is the preferred therapeutic option. Surgical intervention is indicated when endoscopic methods fail. We describe a case of Bouveret syndrome in a 79 years old woman. Brown tumors are bony lesions caused by rapid osteoclastic activity, which rare involved jaws. Renal osteodystrophy (ROD) is associated with different pathogenetic mechanisms--disorder of calcium-phosphate metabolism, impaired metabolism of vitamin D, increased parathyroid activity that lead to extreme concentrations of parathormone. The authors report two cases of jaw enlargement in patients received haemodialysis with excessive increase values of alkaline phosphatase and parathormone in serum. The patients were treated surgically with corrective procedures in maxillo-facial area. ROD of the jaws could be severe complication in dialysis patients with end stage of CKD if no appropriate care aimed at correction or prevention of parathyroid hyperfunction was applied to them. and PF. The authors present the case of a young woman with newly diagnosed Takayasu’s arteritis. This woman, with arterial hypertension, was investigated for the unspecific symptoms at the beginning. Afterwards, the transthoracic echocardiography showed dysfunction of the left ventricle and the abdominal sonography showed a stenosis of the right renal artery. PET/CT scan showed chronic modification after inflammatory processes on the wall of the thoracic and abdominal aorta. Ruptured pregnancy in the rudimentary horn of women who have had a vaginal delivery is rare and unpredictable. However, when undiagnosed, this condition could lead to maternal morbidity and mortality. We report a pregnancy at 19 weeks gestation presented with acute abdomen and hypovolemic shock. She was initially thought to have an intrauterine pregnancy with the provisional diagnosis of a ruptured uterus. Kolorektální karcinom patří ve vyspělých zemích mezi nejčastější nádorová onemocnění a Česká republika je celosvětově na špici v jeho výskytu. Roční incidence u nás přesahuje 7 000 osob ročně. Primárně je 20 % pacientů diagnostikováno ve stadiu IV a téměř u poloviny pacientů se v průběhu onemocnění vyskytnou metastázy. V posledních 15 letech onkologická léčba metastického onemocnění prodělala významný vývoj. Standardem se stala terapie fluorouracilem, jehož modulace leucovorinem vedla k prodloužení mediánu přežití u infúzních režimů až k 15 měsícům (symptomatická terapie 6–8 měsíců). Kombinace fluorouracilu s oxaliplatinou nebo irinotecanem vede k dalšímu prodloužení k 20 měsícům. Následné přidání biologické léčby (bevacizumab, cetuximab, panitumumab) posouvá medián nad 20 měsíců. Cetuximab je monoklonální chimérická IgG1 protilátka, která cíleně blokuje extracelulární část EGFR. V České republice je zaregistrován pro léčbu pacientů s generalizovaným kolorektálním karcinomem s EGFR pozitivitou a K-ras wild type v monoterapii nebo v kombinaci s chemoterapií po selhání terapie irinotecanem a oxaliplatinou. Ve 2 registračních studiích (EMR 62 202–007-BOND 1, IMCL CP02–9 923) vedl ke klinickému benefitu u více jak poloviny nemocných. Ve studii publikované Cunninghamem a spolupracovníky byli pacienti zařazeni do ramene s cetuximabem a irinotecanem anebo do ramene s cetuximabem samotným (PR byla dokumentována u 23 % pacientů, SD u 33 % léčených pacientů). Medián přežití byl prodloužen z 6,9 měsíců na 8,6 měsíců. Evropská léková agentura (EMEA) doporučila cetuximab k léčbě metastického kolorektálního karcinomu (s expresí EGFR, K-ras wild type) v kombinaci s chemoterapií do I. linie léčby na základě studie CRYSTAL. Letos v dubnu byly publikovány v JCO výsledky této studie, která potvrdila efekt cetuximabu v I. linii léčby K-ras wild type karcinomů kolorekta s efektem prodloužení přežití (medián OS = 23,5 versus 20,0 měsíců; hazard ratio (HR) = 0,796; P = 0,0093), PFS (medián PFS = 9,9 versus 8,4 měsíců; HR = 0,696; P = 0,0012) a četnost léčebných odpovědí (RR) (RR = 57,3 % versus 39,7 %; odds ratio = 2,069; P < 0,001) ve srovnání se samotným FOLFIRI., Colorectal cancer in third-line treatment Colorectal cancer is among the most common malignancies in the developed countries and the Czech Republic is the leading country in terms of its incidence worldwide. The annual incidence in the Czech Republic exceeds 7 000 persons. Primarily, 20 % of patients are diagnosed with stage IV and nearly half of the patients develop metastases during the course of the disease. In the past 15 years, oncological treatment for metastatic disease has made significant progress. Treatment with fluorouracil has become a standard; its modulation with leucovorin has resulted in a prolonged median survival in infusion regimens of up to 15 months (6–8 months in symptomatic treatment). A combination of fluorouracil with oxaliplatin or irinotecan leads to a further extension up to 20 months. A subsequent addition of biological therapy (bevacizumab, cetuximab, panitumumab) pushes the median beyond 20 months. Cetuximab is a chimeric IgG1 monoclonal antibody which targets the extracellular domain of the EGFR. In the Czech Republic, it is registered for the treatment of patients with generalized colorectal cancer with EGFR positivity and K-ras wild type after a failed treatment with irinotecan. In two registration studies, it resulted in a clinical benefit in more than half of the patients (PR was documented in 23 % patients; SD in 33 % of the patients treated). The median survival increased from 6.9 months to 8.6 months. Cíl práce: Upozornit na možnosti laparoskopického chirurgického řešení náhlé příhody břišní v graviditě. Typ práce: Kazuistika. Vlastní pozorování: Formou kazuistiky prezentujeme případ těhotné ve 23. týdnu gravidity s náhlou příhodou břišní neznámé etiologie a její laparoskopické řešení. Závěr: Laparoskopie v rukou zkušeného operatéra se jeví jako velmi bezpečná metoda v případech nejasné bolesti podbříšku, a to i v graviditě v závislosti na velikosti dělohy, tedy stáří gravidity. V tomto případě byla příčinou torze fibromu tuby, která byla řešena salpingektomií., The aim of the study: Draw attention to possibilities of LPSC surgical solutions of acute abdominal evens in pregnancy. Type of paper: Case study. Own observation: We are presenting a case of a pregnant woman at 23 weeks who suffered an acute abdominal event of unknown etiology and its laparoscopic solution. Conclusion: Laparoscopy in the hands of an experienced surgeon appears as a very safe method in cases of lower abdominal pain of unclear origin also in pregnancy, depending on the size of the uterus, i.e. the stage of pregnancy. Prezentujeme případ mladé ženy, jež prodělala v dospělém věku operaci vrozené cysty pravého hepatiku, kterou vzhledem k těsnému kontaktu s velkými cévami nebylo možné kompletně resekovat. Byla provedena parciální resekce a cysto-jejuno-anastomóza (CJA) kdrenáži žluči. Pacientka byla 4 roky bez obtíží. Posléze se dostavily bolesti břicha, subikterus a laboratorní známky cholestázy Kvůli těmto potížím a opakovaným cholangoitidám prodělala řadu vyšetření a intervencí, které měly zajistit ozřejmění příčiny obstrukce a obnovení odtoku žluči. Použité zobrazovací metody neprokázaly jednoznačnou příčinu obtíží, proto bylo přistoupeno k operační revizi, při níž byl peroperační ultrasonografií prokázán tumor v oblasti CJA. Nález byl vzhledem k lokalizaci inoperabilní. Pacientka měla následně zjištěnu diseminaci malignity v játrech i v uzlinách a zemřela 5 měsíců od zjištění maligní diagnózy na hepatorenální selhání. Tento poměrně raritní případ dokládá zvýšené riziko maligního zvratu vrozených malformací žlučových cest především v případě jejich diagnózy a léčby v dospělém věku. V dostupné literatuře existuje jen málo dokumentovaných případů metachronního karcinomu po resekci žlučových cest., We present a case of a young woman, who underwent surgery for congenital bile duct cyst in adulthood. Due to location of the cysts it was impossible to completely remove it. The cyst was partially resected and it was performed cyst-jejunal anastomosis (CJA) to drain of bile. After operation the patient was 4 years without any problems. After this interval appeared pain in the right upper quadrant and subicterus with laboratory evidence of cholestasis. The patient suffered from recurrent cholangitis due to which a number of diagnostic tests and interventions were performed. Used imaging metods did not demonstrate an unequivocal cause of problems, therefore subsequently surgical revision was performed and the in-traoperative ultrasonography demonstrated tumor in anastomosis. The finding was inoperable due to inappropriate location. In the following period liver and lymphnode metastases appeared and patient died five months after diagnosis of malignancy. This relatively rare case illustrates an increased risk of malignant transformation of congenital malformations of the biliary tract. Chylózní ascites představuje zřídka se vyskytující komplikaci radikálních onkogynekologických operačních výkonů v retroperitoneu, především jako následek systematické pánevní a zejména paraaortální lymfadenektomie. Terapeutický přístup je vysoce individuální, primárně konzervativní s důrazem na zavedení dietních omezení a vede k výsledkům za 4–8 týdnů.
